Abstract

Objective To evaluate the intraocular pressure (IOP)-lowering efficacy and safety of 10 and 15 µg bimatoprost implant in patients with open-angle glaucoma (OAG) or ocular hypertension (OHT). Methods This randomized, 20-month, multicenter, masked, parallel-group, phase 3 trial enrolled 528 patients with OAG or OHT and an open iridocorneal angle inferiorly in the study eye. Study eyes were administered 10 or 15 µg bimatoprost implant on day 1, week 16, and week 32, or twice-daily topical timolol maleate 0.5%. Primary endpoints were IOP and IOP change from baseline through week 12. Safety measures included treatment-emergent adverse events (TEAEs) and corneal endothelial cell density (CECD). Results Both 10 and 15 µg bimatoprost implant met the primary endpoint of noninferiority to timolol in IOP lowering through 12 weeks. Mean IOP reductions from baseline ranged from 6.2–7.4, 6.5–7.8, and 6.1–6.7 mmHg through week 12 in the 10 µg implant, 15 µg implant, and timolol groups, respectively. IOP lowering was similar after the second and third implant administrations. Probabilities of requiring no IOP-lowering treatment for 1 year after the third administration were 77.5% (10 µg implant) and 79.0% (15 µg implant). The most common TEAE was conjunctival hyperemia, typically temporally associated with the administration procedure. Corneal TEAEs of interest (primarily corneal endothelial cell loss, corneal edema, and corneal touch) were more frequent with the 15 than the 10 µg implant and generally were reported after repeated administrations. Loss in mean CECD from baseline to month 20 was ~ 5% in 10 µg implant-treated eyes and ~ 1% in topical timolol-treated eyes. Visual field progression (change in the mean deviation from baseline) was reduced in the 10 µg implant group compared with the timolol group. Conclusions The results corroborated the previous phase 3 study of the bimatoprost implant. The bimatoprost implant met the primary endpoint and effectively lowered IOP. The majority of patients required no additional treatment for 12 months after the third administration. The benefit-risk assessment favored the 10 over the 15 µg implant. Studies evaluating other administration regimens with reduced risk of corneal events are ongoing. 8. 12329 Effectiveness of Surgery for Infertile Women With Advanced Stage Endometriosis: A Systematic Review and Meta-Analysis.

Abstract

To compare pregnancy rates in women with advanced stage endometriosis suffering infertility undergoing surgery vs in vitro fertilization (IVF) treatments. Systematic review and meta-analysis. Tertiary medical center. 120 reviewed abstracts. Following PRISMA guidelines, MEDLINE, Embase, and Cochrane databases were searched for studies investigating the effects of surgical treatments on fertility in patients with moderate to severe endometriosis. Comprehensive meta-analysis software was used for meta-analysis and heterogeneity assessment. The risk of bias in non-randomized studies was evaluated using the Newcastle-Ottawa Scale (NOS). Out of 120 reviewed abstracts, we screened 86 full-text articles. Five studies comparing surgical treatment to IVF met the inclusion criteria for the meta-analysis. Although the results of the meta-analysis suggested a higher likelihood of pregnancy post-surgery compared to IVF alone, statistical significance was not achieved (RR, 1.34 [95% CI, 0.91-1.57, p= 0.24). High heterogeneity among studies was observed. Four studies preferred surgery pre-IVF, while one indicated no benefit and had the lowest NOS score. In a separate analysis including 49 observational studies, spontaneous pregnancy rate post-surgery was compared with IVF treatment outcomes. The event rate for spontaneous pregnancies after surgery was 0.315 (95% CI, 0.274-0.358), whereas the event rate for IVF pregnancies after surgery was 0.264 (95% CI, 0.234-0.296). While a meta-analysis of comparative studies did not reveal statistically significant differences in pregnancy rates between surgical treatment and IVF, a meta-analysis of observational studies demonstrated higher rates of spontaneous pregnancies after surgery compared to pregnancies after IVF. Randomized Controlled Studies are needed to better understand the effect of surgery on spontaneous and IVF conception for women with advanced stage endometriosis. [ABSTRACT FROM AUTHOR]

9. Heterologous vaccination of BNT162b2 in Ad26. COV2.S-vaccinated healthcare workers elicits long-term humoral immune response

Abstract

Background. To date, there are no immunological data for the SARS-CoV-2 heterologous vaccination schedule in the South African (SA) population.Objectives. To assess and compare the immunogenicity and reactogenicity of the Jansen Ad26.COV2.S vaccine with the Pfizer/BioNTech- BNT162b2 booster following prime Ad26.COV2.S in 65 SA healthcare workers.Methods. In a prospective, quantitative, cross-sectional trial on individuals >18 years of age vaccinated with a single Ad26.COV2.S dose or single Ad26.COV2.S and a BNT162b2 single-dose/both doses booster, participants filled in a questionnaire on their demographics, type of vaccination, breakthrough infection/s (BTI/s), vaccine reactogenicity, prior SARS-CoV-2 infection and dates of vaccination. Qualitative analysis for presence/absence of anti-S (spike) immunoglobulin G (IgG) was performed using the Euroimmun anti-IgG enzyme-linked immunoassay kit, and anti-S IgG titres were quantitatively assessed using the Abbott IgG Quant II kit.Results. Between 28 October 2021 and 30 November 2021, 65 individuals were enrolled and assigned as either prime Ad26.COV2.S (n=18) or Ad26.COV2.S with a BNT162b2 supplement (n=47) at Charlotte Maxeke Johannesburg Academic Hospital, SA (mean age 45 years (95% confidence interval (CI) 29.5 - 58), 42 women (64.6%) and 23 men (35.4%)). The median IgG titre for the primed Ad26.COV2.S group was 4 272.55 (95% CI 68.40 - 10 351.40) and that for the BNT162b2 supplement group was 7 360.80 (95% CI 4 207.40 - 15 372.60). In the univariate model, the BNT162b2 supplement group showed a significant 1.99 times higher antibody titre factor (95% CI 0.045 - 5.553; p0.05) with antibody titres in both groups. However, sex (–55.381 (95% CI –76.984 - –13.498; p=0.018) in a multivariate model was found to have a statistically significant association with anti-S IgG titres observed in both groups. Participants who received their first dose of BNT162b2 9 - 10 months after their prime Ad26.COV2.S (n=44) had a higher degree of antibody response than those who received it earlier. Reactogenicity was observed to be manageable, with mild/moderate adverse effects in the study population.Conclusion. A BNT162b2 supplement given in single or two doses as booster in individuals primed with Ad26.COV2.S induced immunological response, with acceptable and manageable reactogenicity. This study provides novel evidence of the highest degree of antibody response in individuals who received a BNT162b2 first dose 9 - 10 months after prime Ad26.COV2.S, implying that a longer time gap between the two vaccines stimulates higher antibody response than a shorter gap, and that this antibody response can persist for as long as 6 months after the last BNT162b2 dose.

Abstract

NA48 is a CP violation experiment that is starting its period of data taking at the CERN SPS accelerator. The expected average data rate of a few thousand events/s (with the possibility to go up to 10 kHz), a data volume up to 100 MB/s sustained over the beam burst, and the necessity to handle different subsystems with very wide variation in the number of channels have lead to the design of a modular pipelined data acquisition system with a sampling rate of 25 ns.

Abstract

Although relatively rare compared to similar latitudes in the Pacific or Indian Oceans, massive coral colonies are present in the Tropical/Equatorial Southwestern Atlantic Ocean. However, detailed geo-chemical compositions of these corals are still largely unknown. In this work, we present growth rates, Sr/Ca, and U/Ca ratios of the coral colony (Siderastrea stellata) sampled at Rocas Atoll, off the Brazilian coast. These variables are primarily affected by sea surface temperature (SST) at seasonal scale, and by wind stress at interannual scale, these results represent a broad new finding. A lower significance at the interannual time scale between Sr/Ca and U/Ca with respect to SST is attributed to the low SST amplitude closed to Equator. An investigation on the dependence of coral growth rates with respect to the "cloud shading effect'' promoted by the Intertropical Convergence Zone (ITCZ) does not show significant influence. Additionally, rain seems to act on local geochemistry of Sr/Ca ratios and growth rate at the decadal scale.

Abstract

Cross-floor signal propagation characteristics at 434, 862, and 1705 MHz were assessed from measured data in a building similar to a hospital as a first step toward maximizing electromagnetic compatibility (EMC) between wireless radiators within a hospital and medical equipment. Certain transmitter-receiver configurations resulted in 1/[r.sup.d] signal attenuation (d = 2, and 3 [less than] d [less than] 4), while other configurations exhibited quasi-constant signal levels over increasing transmitter-receiver separations. However, little evidence exists on the association between individual concurrent comorbidities and prognosis in ovarian cancer patients.Methods: Among patients diagnosed with invasive ovarian carcinoma who participated in 23 studies included in the Ovarian Cancer Association Consortium, we explored associations between histories of endometriosis; asthma; depression; osteoporosis; and autoimmune, gallbladder, kidney, liver, and neurological diseases and overall and progression-free survival. Using Cox proportional hazards regression models adjusted for age at diagnosis, stage of disease, histology, and study site, we estimated pooled HRs and 95% confidence intervals to assess associations between each comorbidity and ovarian cancer outcomes.Results: None of the comorbidities were associated with ovarian cancer outcome in the overall sample nor in strata defined by histologic subtype, weight status, age at diagnosis, or stage of disease (local/regional vs. advanced).Conclusions: Histories of endometriosis; asthma; depression; osteoporosis; and autoimmune, gallbladder, kidney, liver, or neurologic diseases were not associated with ovarian cancer overall or progression-free survival.Impact: These previously diagnosed chronic diseases do not appear to affect ovarian cancer prognosis.
