We report a case of two stage operations for a chronic type B dissecting aortic aneurysm with aberrant right subclavian artery(ARSA) and Kommerell’s diverticulum (KD). A 62-year-old man with a type B aortic dissection from 9 years before underwent follow-up Computed Tomography (CT) revealing aortic arch with KD expanded to 53mm and aberrant right subclavian artery expanded to 23mm in diameter. We performed two stage operations. In the first, we performed ARSA division at the proximal part of vertebral artery and transposed the distal end to right common carotid artery transposition. Subsequently, we performed replacement of descending aortia and over sewing of the aneurysmal of the origin ARSA under circulatory arrest. KD with ARSA is a rare anomaly of the aortic arch, and it may develop complications such as distal embolization, subclavian-esophageal fistula, dissection and rupture. We performing in two stages, subclavian reconstruction before the intrathoracic repair would be expected to reduce the subsequent risk of distal ischemia or subclavian steal., 我々は，異所性右鎖骨下動脈（ARSA）とKommerell憩室（KD）を伴った慢性B型解離性大動脈瘤に二期的手術を行った1 例を報告する。症例は， 9 年前にB型大動脈解離を来した62歳の男性でCTによる定期検査を受け，KDを伴う53mmに拡大した大動脈弓と直径23mmに拡大した異所性右鎖骨下動脈を認めた。治療は， 2 期的手術の方針とし，初回は椎骨動脈の近位部でARSAを離断し，末端部を右総頸動脈に転位吻合した。二期目は，循環停止下で下行大動脈置換と瘤化したARSAの起始部を縫合した。ARSAを伴ったKDは稀な大動脈弓異常であり，末梢部の塞栓形成，鎖骨下動脈食道瘻，解離や破裂などの合併症を呈する可能性がある。我々が二期的に行った，大動脈置換前の鎖骨下動脈の再建は，末梢の虚血または鎖骨下動脈盗血のリスクを低下させると思われた。

Globalization of sports has rapidly advanced, leading to various challenges for conditioning during competitions and camps in other countries. This study conducted a survey to collect basic data about the conditioning of student wrestlers in Japanese and international camps. The subjects were 18 student wrestlers who were scheduled to compete in the All- Japan Student Championship Tournament and participated in both international and Japanese training camps at the Korea National Sport University (Seoul Special City, Republic of Korea, March 2014) and Agatsuma-gun, Gunma prefecture. We conducted a questionnaire-based survey at Japanese and international (Korea) camps. The results showed that body weight was significantly reduced during the last half of the Japanese camp, compared to that in the Korean camp. The dietary survey also showed that much spicy food was served during the camp in Korea, resulting in abnormal bowel movements. The results of our survey showed that it is important to provide prior education about dietary management and the use of supplements during international training camp.

我々は、川場村において「癒しと健康をもたらす」とされる、森林浴の生理学的効果を検討した。被験者は、川場村在住の19名の高齢者(男性11名、女性8名)、年齢は平均74歳(男性74±3.5歳、女性74.9±2.9歳)。8月17日の1時間森林集団散策時の天候は曇り、気温30℃から32℃、湿度58%から60%、風速0m/secから2m/sec、コントロールは、同じメンバーによる8月21日の田園地域1時間の集団散策とし、非森林浴時の天候は、森林浴時とほぼ同様であった。森林揮発性物質(フィトンチッド)、気分プローフィール(POMS)、血圧、脈拍数、空腹時の血清からNK活性細胞、カテコールアミン(アドレナリン、ノルアドレナリン、ドパミン)、コルチゾール、アディポネクチンを森林浴・非森林浴前後に測定した。川場村における森林浴調査地から森林揮発性物質が検出され、全員で、森林浴前後においてPOMSの総得点、血圧、コルチゾールおよびアドレナリンが有意に低下した。被験者中の3例に森林浴でNK細胞活性の増加を認め、女性1例のみ森林浴および非森林浴の両者でNK細胞活性の増加を認めた。川場村における森林浴研究結果から、POMS、カテコールアミンおよびコルチゾールの血中濃度低下は、森林浴が癒しと健康をもたらす効果をもつ可能性を示唆する所見と考えられた。

脂肪細胞は、摂取した余剰エネルギーを蓄えるエネルギー備蓄装置として働いているのみでなく、脂質代謝や血圧調節、血栓形成など様々な生理作用をもつ液生因子―アディポサイトカインを分泌する生体内で最大の内分泌臓器として再認識されるようになった。アディポサイトカインの一種であるアディポネクチンは、脂肪組織から特異的に分泌され、血管内皮細胞に直接働き抗動脈硬化作用をもつホルモンであることが明らかにされている。また、レプチンは、中枢神経に作用して強力な摂食抑制やエネルギー消費亢進をもたらし、脂質代謝に重要な役割を有していることが明らかにされている。我々は今回、若年女子学生における血清アディポネクチン濃度（以下、アディポネクチン）とライフスタイル関連の健康指標との関連性にっいて、アディポネクチンをELISA法によって測定し、レプチン、脂質代謝値、体力、ライフスタイルプロフィール・スコアーとの相関を検討した。対象者は、年齢20.0±1.5歳、BMI22.0±5.5kg/㎡の女子学生99名であり、アディポネクチンは5.3±2.7μ g/ml、レプチンは、7.8±4.5ng/mlであった。アディポネクチンは、HDLコレステロールと正の相関、血清レプチン、トリグリセリドおよびBMIと負の相関が認められた。BMIの高い肥満群（BMI25kg/㎡似上）は、BMI正常群（BMI22kg/㎡）に比してアディポネクチン、HDLコレステロール、ライフスタイルプロフィール・スコアーおよび体力が有意に低く、レプチン、総コレステロールおよび血清トリグリセリドが有意に高値を示した。以上の結果から、血清アディポネクチン濃度は、健康指標として役立つマーカーのひとつであり、その改善は、ライフスタイルを良好に維持するための動機付けのひとつとして重要であることが示唆された。
