Background: Breakthrough neurotechnologies have allowed for new understanding of some brain disorders; however, identification and differential diagnosis of intracranial stenotic and occlusive lesions remains challenging. Magnetic resonance imaging (MRI) with dynamic contrast enhancement (DCE) is a tool that could be used for the quantitative assessment of endothelial permeability and microvascular volume in atherosclerotic plaques (AP). Aim: To assess quantitative parameters of vascular wall abnormalities in AP area and in obviously unchanged wall of intracranial arteries with MRI DCE and high spatial resolution Т1-weighed images before and after contrast injection, with calculation of the wall enhancement index (WEI) by mathematical modelling. Methods: This was a pilot cross-sectional uncontrolled study with consecutive recruitment of 29 patients with atherosclerotic abnormalities of brachiocephalic arteries, including intracranial. The patients’ median age was 66 [57; 72] years; they were mostly men (75.9%, n = 22). For the assessment of any brain abnormalities, MRI (magnetic induction 3 Tesla, Magnetom Prisma, Siemens) was performed in patients with standard sequence (Т2, T2-FLAIR), as well as MRI DCE for the assessment of intracranial arteries, before and after intravenous contrast injection, with high spatial resolution T1-weighed imaging and suppression of the signal from bloodstream and fat, with the calculation of WEI. Results: There were significant differences in WEI in AP and in unchanged wall (0.962 [0.686; 1.387] vs. 0.111 [0.014; 0.206], p 0.001). No significant differences were found between WEI values in internal carotid arteries APs (0.722 [0.573; 1.580]), middle cerebral arteries (0.921 [0.725; 1.183]), and basilar artery (1.343 [1.002; 1.419]) (p = 0.381). We also found significant difference (p = 0.034) in the extravascular extracellular fraction volumes ve (Tofts) in AP located in the basilar artery (0.171 [0.146; 0.325]), internal carotid arteries (0.579 [0.358; 1.000]), and middle cerebral arteries (0.134 [0.101; 0.269]). Conclusion: This is the first description of quantitative parameters characterizing vascular wall abnormalities in intracranial atherosclerosis. Despite its obviously intact state, vascular walls outside the intracranial AP was shown to be abnormal as well.

Despite the large number of government programs aimed at providing quality medical care and medication to certain categories of citizens, we are faced with significant shortcomings. In this article, the authors will examine the key Russian healthcare programs funded through federal subsidies and evaluate their pros and cons. The authors explore the legal regulations that govern these programs and identify the specific categories of citizens they are designed for.

Within the framework of the presented research, the question of the manipulative potential of mass culture as a tool of political influence is raised. The methodology of the work is built based on a combination of elements of comparative, structural and descriptive analysis. The author concludes that popular culture products, even if their creators have purely commercial motives, play the role of a tool for promoting meanings and values and, due to this, are a resource for regulating the behavior and perception of the target audience. The latter turns mass culture into a product of the functioning of the orientational level of the political system. The potential of works of popular culture to develop the effect of emotional contagion contributes to the fact that the impact of broadcast content becomes as wide as possible. The high degree of dependence of the content of cultural narratives on the context and the possibility of free commentary contributes to an increase in the options for their interpretation. The result is the loss by authors and customers of works of full control over the dissemination of transmitted meanings and their interpretation.

Introduction. The technological operation of soil tillage is an energy-intensive preparatory operation. The traction resistance of tillage units has a direct impact on this indicator. Aim of the Study. The study is aimed at searching optimal operating parameters for the field cultivator tool through determining the traction resistance created by this tool. Materials and Methods. The study was carried out in the field on an experimental facility using a full factorial experiment technique with a working model based on a three-level design. Results. Through the regression analysis of experimental data, there was obtained a mathematical model that allows determining the optimal operating parameters for the field cultivator tool when performing a technological operation of soil tillage. Discussion and Conclusion. While calculating, it was found that the lowest traction resistance PТ = 0,72 kN created by the tool is for the following values: tillage depth h = 4 cm, crumbling angle of the left-hand and right-hand flat-cutting wings f = 0° and the speed of the tillage unit v = 2,61 m/s. As the optimal operating parameters for a field cultivator tool, determined using the obtained mathematical model, it is recommended to use the crumbling angle of the left-hand and right-hand flat-cutting wings of the working body f = 0°, and the speed of the tillage unit v = 2,91 m/s. Taking into account the recommended optimal operating parameters of the tool, it is possible to calculate the value of the traction resistance index at different tillage depths. The traction resistance will change within the range of 0.79–1.81 kN, when the soil tillage depth changes from 4 to 12 cm. The resulting mathematical model of the traction resistance created by the tool will allow a more accurate approach to the issue of designing agricultural machines.

Introduction. Currently, there is a tendency of steady decline in the value of ethnic identity in the self-consciousness of young people. The assessment and study of the dynamics of ethnic self-awareness are related to the need for a comprehensive approach to the problem of patriotism development of modern Russian youth. The purpose of this article is to analyze the ethnic self-consciousness and ethnic attitudes among Russian youth enrolled in humanities degree programmes at the National Research Mordovia State University. Materials and Methods. 150 junior students took part in the empirical research. The following methods were used: the 16PF questionnaire by R. Cattell (form C), the “Types of Ethnic Identity” method by G. U. Soldatova and S. V. Ryzhova, the Method for assessing the positivity and uncertainty of ethnic identity by A. N. Tatarko, N. M. Lebedeva, ethnic affiliation questionnaire by G. U. Soldatova, S. V. Ryzhova. The authors used methods of correlation and factor analysis. Results. The study revealed a high level of positive ethnic identity in the majority of Russian students. The greater part of students have an average level of ethnic identity. The results of correlation analysis showed the presence of significant relationships between types of ethnic identity and ethnic attitudes. As a result of factor analysis, five groups of factors reflecting the relationship between types of ethnic identity and personal qualities of students were identified. Discussion and Conclusion. The authorsʼ findings contribute to the study of the problem of ethnic identity and its relationship with ethnic attitudes and personality traits of Russian students. The materials of the article can be useful for specialists in the field of ethnic psychology to study and develop studentsʼ deep understanding of their own ethnic identity.

Introduction Challenges of treating the injuries of pubic symphysis in patients with combined pelvic injury require developing new techniques for stable minimally invasive osteosynthesis in the acute period of trauma. Purpose of the work was to assess the application of low invasive fixation of the pubic symphysis with a system of transpedicular screws in the patients with combined unstable pelvic injuries in the acute period of the traumatic disease. Materials and methods The results of treatment of 12 patients with polytrauma with unstable pelvic injury with rupture of the pubic symphysis who were treated at Dzhanelidze Research Institute of Emergency Medicine, St. Petersburg, in the period from 2017 to 2023, were analyzed. All the patients underwent final minimally invasive fixation of the pubic symphysis with a system of transpedicular screws in the early period of the traumatic disease. Results The overall assessment of the results in 12 patients found no complications or technical errors during the installation of transpedicular screws. Control CT scans of the pelvis showed that osteosynthesis was stable in all patients, and the position of metal was correct in all. One patient from the study group died in the clinic from massive pulmonary embolism. The long-term results in 11 patients in the period from 6 months to 3 years after trauma were evaluated as excellent and good anatomical and functional results in 10 patients, one case had fair outcome. Discussion The use of bone plates to fix ruptures of the symphysis pubis leads to disruption of its physiological mobility, and the patient’s activity in the postoperative period can cause an implant fracture. Original dynamic plates and wire cerclage help avoid this complication; however, all these techniques involve significant surgical trauma and blood loss, as well as the risk of postoperative wound suppuration. Minimally invasive methods of fixation using cannulated screws, systems such as Endobutton, Tight Rope can reduce intraoperative trauma and the risk of complications (blood loss, suppuration); however, the procedure for their installation is quite complex and lengthy, and for greater stability, external fixation of the pelvic ring is often required. The proposed minimally invasive fixation of the pubic symphysis with a system of transpedicular screws avoids a lot of shortcomings and is sufficiently stable. Conclusion The method of minimally invasive fixation of the pubic symphysis with a system of transpedicular screws meets current requirements in the treatment of patients with polytrauma and unstable pelvic injury, and it may be successfully used in the acute period of trauma.

The transition to a digital model of providing the public service of conferring academic titles implies fundamental changes in this public institution, which are associated with both the need to make a technological map for digitizing the service and optimization of the very procedure for awarding academic titles. The paper discusses technological and institutional aspects of providing the public service of conferring academic titles in a digital form. The theories of public administration and public services, digitalization, jurisprudence and project management constitute the methodological framework of the article. The classical research methods, such as analysis and synthesis, are used. The empirical data are obtained through integration of the legal acts regulating the principles and mechanisms for rendering public services and conferring academic titles with the project documents of the Department of Scientific and Scientific-Pedagogical Workers Certification of the Ministry of Science and Higher Education of the Russian Federation. We indicate the need to legalize the procedures for initiating the nomination of an applicant for academic titles and justify the attribution of academic titles to the academic rights of scientific and scientific-pedagogical workers. Special focus of the study is on the reorganization of the academic titles institution and its role in assessing scientific qualifications and developing academic career. We address the controversial issue of granting the leading universities the right to independently confer academic titles (transition to a hybrid format) and emphasize the importance of balance between local and centralized regulation. In prospect the initiative to update the quantitative requirements and criteria for applicants for academic titles should be a key tool in revising the content of academic titles.

Aim. To evaluate the effect of uterine artery embolization (UAE) on reproductive health in women 40 years and older. Materials and methods. A retrospective study was conducted at the perinatal center of the District Clinical Hospital of Khanty-Mansiysk. Patients were divided into two groups: the main group (group 1; n=57) included women with uterine fibroids after UAE, and the control group (group 2; n=42) included women without a history of uterine fibroids. Results. Women with uterine fibroids were statistically more likely to have grade 1 obesity (χ2=4.39; p=0.04) and less likely to have normal body weight (χ2=8.73; p=0.00) compared to women without fibroids. Smoking increased the risk of uterine fibroids twofold (χ2=4.49; p=0.03; odds ratio 1.97; confidence interval 0.64-6.11). More women with uterine fibroids had a history of abortions (n=42; 73.7%) than those without uterine fibroids (n=19; 45.2%; χ2=8.27; p=0.01). The most common gynecological comorbidities in women with uterine fibroids included inflammatory disease of the uterus (N71), endometrial polyp (N84), and endometriosis (N80); p0.05. The most common extragenital diseases in patients with uterine fibroids included thyroid disease, hypertension, and iron deficiency anemia (p0.05). Contour SE embolization agent with spherical particles was more often associated with pain (χ2=0.18; p=0.67) and less often with fever (χ2=4.47; p=0.03). There was a decrease in the number of short menstrual cycles (less than 24 days) from the moment before surgical treatment and after 24 months (χ2=34.580; p0.001) and an increase in the number of cycles of 24–38 days (χ2=14.887; p=0.005). At 24 months after UAE, there were more women with a menstrual period of less than 3 days (χ2=10.385; p=0.035) and fewer with a menstrual period of more than 8 days (χ2=34.573; p0.001). Ultrasound showed a decreased ovarian reserve at 6, 12 and 24 months after UAE (p0.05). Conclusion. When assessing the reproductive health of women 40 years and older after embolization of the uterine arteries, it was found that this method of treatment of symptomatic uterine fibroids leads to a decrease in the duration and heaviness of menstruation but increases the risk of an ovarian reserve decrease.

While sperm-borne RNAs have demonstrated potential involvement in embryo development, the underlying mechanisms remain unclear. Aim. To determine the possibility of identifying patients with significant coronary artery disease (CAD) by evaluation of the left ventricular (LV) myocardial work indicators by constructing pressure-strain loops despite a negative test result during exercise stress echocardiography. Materials and methods. The study included 79 patients with suspected or previously confirmed CAD, of which 47 (59%) men, who had a negative test result during exercise stress echocardiography on the treadmill. Global work index (GWI), global constructive work (GCW), global wasted work (GWW) and global work efficiency (GWE) were evaluated using the technique of constructing pressure-strain loops at rest and at peak exercise. Results. With similar systolic blood pressure and LV ejection fraction at peak exercise between the control group and the group of patients with single-vessel CAD, there was revealed significant difference in GWI, GCW and GWE at peak exercise. In patients with multi-vessel CAD, GWI and GCW also significantly differed from the control group at peak exercise. Conclusion. The evaluation of LV myocardial work indicators may be recommended for a more accurate diagnosis of CAD in negative or unreliable test results during exercise stress echocardiography.

Introduction: Whole-genome DNA methylation identification is crucial for profiling physiologically and clinically relevant epigenetic changes. Although there are multiple experimental methods, their accuracy, advantages, and disadvantages need to be investigated in their application to complex tissue objects. In this study, we performed a benchmark of 5mC detection with Oxford Nanopore and enzymatic methyl-sequencing (EM-seq) methods.Material and Methods: To this end, we profiled in a genome-wide manner 5mC sites in colorectal tumors and normal tissues for three patients and applied the HumanMethylationEPIC BeadChip as an additional control approach. We estimated the whole-genome scale of the methylation detection that each method yields.Results: Our investigation describes the sensitivity and specificity of each platform and the impact that sequencing coverage brings. Our analysis revealed the higher sensitivity and specificity of Nanopore sequencing over the EM-seq method. Moreover, Oxford Nanopore Technology (ONT) sequencing, followed by Megalodon methylation detection, demonstrates better quantitative agreement of the epigenetic signals between biological replicates. Furthermore, our analysis highlights that with 40× and above coverage, EM-seq slightly outperforms ONT and yields highly accurate detection of the 5mC signals (AuPR = 0.99178 and AuROC = 0.98161).Conclusion: The study was performed on colon cancer and adjacent normal tissue samples, placing our investigation close to the real application of methylation studies in oncology.

Relevance. The necessity to know the conditions and forms of W concentration in coals for solving a number of scientific and engineering problems at complex development of coal deposits. Aim. Complex estimation of W modes of occurrence in coal for development of measures for rational ecologically safe use of coal. Methods. Correlation analysis, scanning electron microscopy, infrared spectroscopy, coal group analysis, inductively coupled plasma mass spectrometry, instrumental neutron activation analysis. Results and conclusions. The modes of occurrence of W in coal were studied by a complex of methods. In the majority of W-enriched coal deposits, a negative significant correlation of its content in coal and coal ash with ash yield was found, indicating its association with organic matter. Using the method of coal group composition analysis, it was found that the main carrier and concentrator of W in the studied lignite deposits is organic matter. The contribution of the mineral phase in general in W-rich coals and in coals with its normal content does not exceed 20%, usually less than 5%. These conclusions are also confirmed by infrared spectroscopy data, according to which no more than 15% of the metal in the samples studied is associated with mineral phases. The association of W with high molecular humic acids predominates. In anomalously W-enriched lignites, the humic acid phase represents 76 to 88% of the gross metal content. The role of bitumen and low-molecular-weight humic acids in the balance of W is marginal at their different levels in the coals. Mineral phases of W are not characteristic of coal. Nano-microinclusions of scheelite, wolframite, hubnerite and ferberite were recorded as isolated occurrences. The main mineral phases are associated with Fe and Mn hydroxides, in which W is presented as a trace element in the amount of 1–5% together with other elements (Ge, As, etc.). In more metamorphosed bituminous and anthracite coals, authigenic mineral formations were found, represented by tungstite, phyllotungstite, native W and complex Fe-Mn-Ca-W-O mineral phases.

The unique ability of ferroelectrics to generate high voltage under shock loading is limited by electrical breakdown within the shock-compressed ferroelectric material. Breakdown is a hybrid process of initiation and growth. The possible mechanisms of electrical breakdown in ferroelectric films and bulk ceramics subjected to high-pressure shock loading are discussed and experiments designed to elucidate which mechanisms govern breakdown. Gigapascal shock loading experiments were performed on poled Pb0.99(Zr0.95Ti0.05)0.98Nb0.02O3 ferroelectric film specimens in the range of 32–156 μm thickness to determine the dependence of the breakdown field on thickness and on film specimens in the range of 4–16 mm length to determine the dependence of the breakdown field on the duration of shock compression. The resulting breakdown-field vs thickness and breakdown-field vs shock transit time dependencies are consistent with a hybrid electron emission initiation and Joule heating microchannel growth mechanism. Further analysis of data previously obtained on shock-compressed 0.27Pb(In1/2Nb1/2)O3–0.47Pb(Mg1/3Nb2/3)O3–0.26PbTiO3 ferrvoelectric single crystals and Pb(Zr0.65Ti0.35)O3, Pb0.99(Zr0.52Ti0.48)0.99Nb0.01O3, Pb0.99(Zr0.95Ti0.05)0.98Nb0.02O3 bulk ceramics is consistent with this dual mechanism. It appears that neither chemical composition nor microstructure (single crystal vs polycrystalline) of the ferroelectric material has a significant effect on the breakdown mechanism in shocked ferroelectrics. The advancement in material platforms exhibiting strong and robust electro-optic effects is crucial for further progress in developing highly efficient and miniaturized optoelectronic components with low power consumption for modern optical communication systems. In this work, we investigate thin-film lead zirconate titanate (PZT) substrates grown by a chemical solution deposition technique as a potential platform for on-chip plasmonic electro-optic modulators. A high modulation depth (>40 %) is achieved with 15 μm-long electro-optic directional coupler modulators. An unusual cutoff in the modulation frequency response at ∼200 kHz is observed and further studied with respect to possible reorientation effects. Second-harmonic generation signals are found influenced by the externally applied electric field, indicating that the domain reorientation effect can be responsible for the unusual frequency response observed.
