Background: Prenatal serological screening is commonly used to screen for trisomy 21 syndrome (T21); however, it carries a risk of missed detection. In this study, we explored how to reduce missed diagnosis of T21 in serological screening. Methods: A total of 116 pregnant women with T21 fetuses confirmed by prenatal diagnosis were evaluated. Serological screening and non-invasive prenatal test (NIPT) findings were analyzed. Results: Twenty-nine T21 fetuses were missed in serological screening; the missed diagnosis rate was 25%, 67.65% of the missed cases were of moderate risk, and 79.31% of the missed pregnant women were under 35 years of age. Nuchal translucency (NT) and the free beta subunit of human chorionic gonadotropin (free-HCGβ) were higher in the detected T21 cases than in the missed T21 cases, while alpha-fetoprotein (AFP) levels were decreased. Fortyeight pregnant women who underwent NIPT in the second trimester were at high-risk for T21. Conclusions: Prenatal screening should not be ignored in young pregnant women. INTRODUCTION: Rapid initiation of antiretroviral therapy (ART) in early HIV infection is important to limit seeding of the viral reservoir. A number of studies have shown that if ART is commenced prior to seroconversion, the seroconversion may, or may not, occur. We aimed to assess whether seroreversion or no seroconversion occurs using samples collected during an early treatment study in South Africa. METHODS: We tested 10 longitudinal samples collected over three years from 70 blood donors who initiated ART after detection of acute or early HIV infection during donation screening on fourth- and fifth-generation HIV antibody and RNA assays, and three point of care (POC) rapid tests. Donors were allocated to three treatment groups: (1) very early, (2) early, and (3) later. Longitudinal samples were grouped into time bins post-treatment initiation. RESULTS: On all three high-throughput HIV antibody assays, no clear pattern of declining signal intensity was observed over time after ART initiation in any of the treatment initiation groups and 100% detection was obtained. The Abbott Determine POC assay showed 100% detection at all time points with no seroreversion. However, the Abbott ABON HIV1 and OraSure OraQuick POC assays showed lower proportions of detection in all time bins in the very early treated group, ranging from 50.0% (95% CI: 26.8-73.2%) to 83.1% (95% CI: 64.2-93.0%), and moderate detection rates in the early and later-treated groups. CONCLUSION: While our findings are generally reassuring for HIV detection when high-throughput serological screening assays are used, POC assays may have lower sensitivity for detection of HIV infection after early treatment. Findings are relevant for blood safety and other settings where POC assays are used.

Thirty-five exhibited low avidity, and seven infants developed cCMV. Of 19,435 women screened in 2013–2017, 184 (1.0%) had IgM above the revised cutoff, 67 had low avidity, and 1 had cCMV. The 2017–2019 results were not significantly different from the 2013–2017 results. The diagnosis of celiac disease (CD) at the first diagnostic step requires the detection of specific class A antibodies to tissue transglutaminase type-2 (TG2 IgA) and the measurement of total immunoglobulin A (tIgA) to exclude IgA deficiency. The aim of the study was to evaluate the new quantitative immunoassay panel allowing for the detection of celiac-specific antibodies with the simultaneous determination of tIgA from the same sample of blood at one time. This retrospective study included 104 pediatric patients divided into groups with recognized CD and IgA deficiency (n = 20; 19%), immunocompetent children with CD (n = 28; 27%), children with IgA deficiency and without CD (n = 28; 27%), and the control group of immunocompetent children without CD (n = 28; 27%). Intestinal biopsy with histopathological evaluation (except five patients with CD who were diagnosed without biopsy) and measurement of reference celiac specific antibodies were performed in all children. Multiparametric quantitative immunoassay Polycheck® Celiac IgA plus total IgA test was used to evaluate its usefulness in CD screening and IgA deficiency diagnosis. The statistical analysis showed the high sensitivity and specificity of both TG2 IgA and tIgA on the multiparametric panel (sensitivity 96% and 100%; specificity 100% and 79%, respectively). The accuracy and area under the ROC curve for tIgA were 0.904 and 0.955, while for TG2 IgA they were 0.982 and 1.000, respectively. Although the sensitivity of IgA antibodies against deaminated gliadin peptides was low (20%), the specificity reached 100%. The study showed that Polycheck® Celiac IgA plus total IgA test is a specific and sensitive tool for simultaneous serological CD screening and recognition of IgA deficiency. [ABSTRACT FROM AUTHOR]

We analyzed 221 coronavirus disease 2019 cases identified between June 2020 and January 2021 in 6074 individuals screened for immunoglobulin G antibodies in May 2020, representing 77% of residents of 5 Italian municipalities. Objective: To assess the cost‐effectiveness of prenatal detection of congenital cytomegalovirus (cCMV) following maternal primary infection in the first trimester within standard pregnancy follow‐up or involving population‐based screening (serological testing at 7 and 12 weeks of gestation), with or without secondary prevention (valaciclovir) in maternal CMV primary infection. Design: Cost‐effectiveness study from the perspective of the French national health insurance system. Setting: Cost‐effectiveness based on previously published probability estimates and associated plausible ranges hypothetical population of 1,000,000 pregnant women. Population: Hypothetical population of 1,000,000 pregnant women. Methods: Cost‐effectiveness of detecting fetal cCMV in terms of the total direct medical costs involved and associated expected outcomes. Main outcome measures: Detection rates and clinical outcomes at birth. Results: Moving to a population‐based approach for targeting fetal CMV infections would generate high monetary and organizational costs while increasing detection rates from 15% to 94%. This resource allocation would help implementing horizontal equity according to which individuals with similar medical needs should be treated equally. Secondary prevention with valaciclovir had a significant effect on maternal‐fetal CMV transmission and clinical outcomes in newborns, with a 58% decrease of severely infected newborns for a 3.5% additional total costs. Accounting for women decision‐making (amniocentesis uptake and termination of pregnancy in severe cases) did not impact the cost‐effectiveness results. Conclusions: These findings could fuel thinking on the opportunity of developing clinical guidelines to rule identification of cCMV infection and administration of in‐utero treatment. Infections acquired during pregnancy add up to the overall perinatal rate of mortality and morbidity. Because it has the potential to be screened in all pregnant women, infection with cytomegalovirus continues to raise awareness among obstetricians and healthcare providers, given the perinatal complications involved, but also because not all parturient women are equally informed about the impact of the disease. A significant possibility of vertical transmission is attainable if primary infection is acquired in pregnancy, especially in the first trimester, when the risk of developing a congenital neonatal disease is higher. Even so, there are many more steps to be taken into consideration in order to fully understand the consequences of the infection, given the fact that the leading consequence of the congenital disease is the sensorineural hearing loss in the neonates, thus raising an important question: why a generalized serological screening in early pregnancy for CMV is just not justifiable?

Prenatal infections (PI) remain an actual problem. Taking into account the epidemiological data on the increase in the incidence of syphilis in many countries, a review of modern algorithms for diagnosing this disease in pregnant women and newborns has been carried out. Universal screening for syphilis at the first prenatal examination is recommended by WHO and CDC, and is practiced in most countries. The article also presents the characteristics of modern laboratory methods of serological screening of pregnant women for syphilis and their combinations, one of which is primary screening using the treponemal test “total antibodies to syphilis” with confirmation of a positive result by immunoblotting. Given the controversial nature of approaches to the prevention of other PI and their consequences, this review provides current recommendations for countries that screen and actively manage infections such as cytomegalovirus (CMV) and toxoplasmosis (T). Despite the proven significance of CMV as a factor in PI, universal prenatal screening, similar to screening for syphilis is not practiced in any of the countries due to the absence of prenatal treatment with proven efficacy. According to the national protocol of Germany, serostatus (titers IgG, IgM) to CMV are determined in early pregnancy. Confirmation of congenital CMV infection (HCMVI) is a positive PCR result in the amniotic fluid after 20–21 weeks of gestation. The main method of laboratory diagnostics of HCMVI in a newborn is PCR for CMV in urine. The article also presents the main approaches to the prevention of congenital toxoplasmosis, which have been practiced in France since the 90s and demonstrate effectiveness. Thus, despite the fact that the use of effective prenatal treatment is not possible for all PI, their diagnosis using a complex of modern laboratory and instrumental methods is critical for the child’s health, since it allows the application of an appropriate algorithm for specific treatment and observation from birth

Chagas disease (CD) is among the top 10 causes of inability to blood donation. Blood donation centers screen for anti-Trypanosoma cruzi antibodies using highly sensitive immunoenzymatic (ELISA) or chemiluminescent methods, which can lead to false positive results. Since positive samples cannot be used, to avoid the loss of valuable blood donations, it is necessary to improve specificity without reducing the sensitivity of the tests used for blood screening. For this purpose, our group has developed four chimeric proteins (IBMP-8.1, IBMP-8.2, IBMP-8.3, and IBMP-8.4) that have been evaluated in phase I and II studies with high performance and low cross-reactivity rates. The study included a panel of 5,014 serum samples collected from volunteer blood donors at the Hematology and Hemotherapy Foundation of the State of Bahia (Brazil). They were subjected to the detection of anti-T. cruzi antibodies, using all four IBMP antigens individually and latent class analysis (LCA) as a reference test, since there is no gold standard test for this purpose. Considering the sample size analyzed, LCA classified 4,993 (99.6%) samples as T. cruzi-negative and 21 (0.42%) as T. cruzi-positive. Sensitivity values ranged from 85.71% for IBMP-8.1 and 90.48% for IBMP-8.2–95.24% for IBMP-8.3 and 100% for IBMP-8.4, while specificity ranged from 99.98% for IBMP-8.3 and IBMP-8.4–100% for IBMP-8.1 and IBMP-8.2. Accuracy values ranged from 99.4 to 99.98%. The pretest probability for the molecules was 0.42, whereas the positive posttest probability ranged from 95.24 to 99.95% and the negative posttest probability ranged from 0.00001 to 0.0006% for all antigens. The higher odds ratio diagnosis was found for IBMP-8.4, which has been shown to be a safe single antigen for serological screening of CD in blood samples. The use of chimeric IBMP antigens is an alternative to reduce the number of bags discarded due to false-positive results. These molecules have high diagnostic performance and were shown to be suitable for use in screening CD in blood banks, isolated (IBMP-8.4) or in combination; and their use in blood banks could significantly reduce unnecessary disposal of blood bags or the risk of T. cruzi transmission.

Conclusions: The detection of antibodies against T. cruzi in dried blood samples shows a higher sensitivity when using E-CLIA compared with ELISA. Trypanosoma Detect™ is easier to use but has a lower sensitivity. Hence, we propose a sequential strategy based on performing the rapid test first, and a negative result will be confirmed by DBS-ECLIA for use in community Chagas disease screening programs. Background: Chagas disease (CD) serological screening at blood banks is usually performed by a single highly sensitive serological assay, with chemiluminescent immunoassays (CLIAs) being the method of choice. CLIAs employ recombinant, fusion peptides and/or chimeric antigens that selectively capture anti‐Trypanosoma cruzi antibodies. However, despite high sensitivity, the ability of these tests to identify CD‐positive cases should be evaluated against T. cruzi strains circulating in specific locales. Herein, we used a latent class analysis (LCA) approach employing an array of four chimeric antigens to assess the diagnostic performance of the Liaison XL Murex Chagas CLIA for the detection of anti‐T. cruzi IgG in serum samples. Study Design and Methods: The study included a panel of 5014 serum samples collected from volunteer blood donors at the Hematology and Hemotherapy Foundation of the State of Bahia, submitted to anti‐T. cruzi antibody detection using Liaison Chagas CLIA and LCA as a reference test in the absence of a gold standard. Results: LCA classified 4993 samples as negative, while positivity for T. cruzi antibodies was predicted in 21 samples. Compared with LCA, CLIA demonstrated sensitivity and specificity of 76.2% and 99.5%, respectively, providing an overall accuracy of 99.4%. Discussion: In blood banks lacking a de facto highly sensitive screening immunoassay, the low sensitivity offered by Liaison Chagas CLIA renders it unsuitable for standalone use in serological screening procedures for CD. Prenatal cytomegalovirus (CMV) screening is not routinely conducted in Bulgaria. According to our previous data, CMV infection is widespread in our region with a total seroprevalence of 78.4%. Such high seroprevalence may have profound effect over the incidence of congenital CMV (cCMV) infections. We aimed in determination of the serological CMV status of both women in reproductive age and pregnant women. Materials and methods: Retrospective: 1612 women in childbearing age (15-49 y) Prospective: 121 pregnant women (14-41 y) ELISA tests for detection of anti-CMV IgM/IgG and for avidity determination of anti-CMV IgG, EUROIMMUN, Germany. Results: From a total of 1612 women in childbearing age, 1362 (84.5%, 95% CI: 82.73 - 86.27) were anti-CMV IgG positive. From a total of 121 pregnant women, 105 (86.8%, 95% CI: 80.77 - 92.83) were anti CMV IgG positive and 12 (9.9%, 95% CI: 4.58 - 15.22%) were anti CMV IgM positive. We got a high IgG avidity index in all seropositive pregnant women, including those with IgM positive results. Conclusions: CMV infection is highly prevalent in women in reproductive age in Bulgaria and the estimated hypothetical population transmission risk in our region is a total of 7%. We compared the preliminary retrospective data with the data obtained from a real situation of 121 pregnancies and got very similar results. Since almost 10% of the pregnant women had evidence of acute or recent infection, this required us to perform the anti-CMV IgG avidity test. We got a high IgG avidity index in all pregnant women.

Abstract Background Cytomegalovirus (CMV) is one of the most frequent pathogens for congenital infections. Most cases of congenital CMV infection (cCMV) are asymptomatic at birth, but sensorineural hearing loss (SNHL) or neurodevelopmental delay can appear later in childhood. This prospective study examined the practicability of serological screening for anti-CMV immunoglobulin (Ig) G and anti-CMV IgM in pregnant women. Methods A total of 11,753 pregnant women were examined for CMV IgG and CMV IgM during the first or second trimester. When IgM was positive, IgG was reevaluated more than two weeks later. When IgG was negative, IgG was reevaluated in the second or third trimester. All neonates from mothers with positive/borderline IgM or IgG seroconversion underwent polymerase chain reaction assay for CMV using urine samples to diagnose cCMV. Levels of IgG and IgM were compared between mothers with and without cCMV. Receiver operating characteristic (ROC) curves for IgM titers were analyzed. Results Eight of 500 neonates (1.6%) born from mothers with positive IgG and positive IgM, and 3 of 13 neonates (23.1%) born from mothers with IgG seroconversion were diagnosed with cCMV. Neither IgM titers nor IgG titers differed significantly between cCMV and non-cCMV groups. The area under the ROC curve was 0.716 and the optimal cut-off for IgM was 7.28 index (sensitivity = 0.625, specificity = 0.965, positive predictive value = 0.238, negative predictive value = 0.993). Titers of IgG were not frequently elevated in pregnant women with positive IgM during the observation period, including in those with cCMV. All 11 cCMV cases were asymptomatic at birth and none had shown SNHL or developmental delay as of the last regular visit (mean age, 40 months). Conclusions Seroconversion of CMV IgG and high-titer IgM during early pregnancy are predictors of cCMV. High IgM titer (> 7.28 index) is a predictor despite relatively low sensitivity. Levels of IgG had already plateaued at first evaluation in mothers with cCMV. Maternal screening offered insufficient positive predictive value for diagnosing cCMV, but allowed identifying asymptomatic cCMV cases in an early stage.

Abstract Nasopharyngeal carcinoma (NPC), which is closely associated with Epstein–Barr virus (EBV), is one of the most prevalent cancers in southeast China. Most NPC patients are diagnosed at late stage due to inconspicuous symptoms at the early stage, and the prognosis of these patients is poor. The early diagnosis rate of NPC could be significantly increased by serological screening, but the positive predictive value (PPV) is relatively low. A simple two‐step serological screening scheme was established to improve the PPV of the screening strategy and was validated by a prospective cohort. Serum antibodies specific for EBNA1, Zta, Thymidine Kinase (TK), EAD, EAR, and VCA were detected by enzyme‐linked immunosorbent assay. The combination of EBNA1/IgA and VCA/IgA was used in the first step of screening, and anti‐early antigens (EAs) were used in the second step of screening. EAD/IgA was the most prominent marker in the second step of screening, and other anti‐EAs were complementary to EAD/IgA. As validated by a prospective cohort including 4200 participants, using the combination of EAD/IgA and TK/IgA in the second step decreased the number of high‐risk participants from 128 to 27, and increased the PPV from 4.69% to 18.52%, with only one very early‐stage case missed. The two‐step screening scheme provides a standardized approach for NPC screening with an improved PPV and may be used in future field studies. With this two‐step serological screening method, more people benefit from the screening program without increasing the need for fiberoptic endoscopy.

Objectives: The study evaluates the trend over time and the results of serological screening for hepatitis C infection in pregnancy and obstetric and neonatal outcomes.Study Design: It is a retrospective observational study of all pregnant women delivered in maternity units in Trento province, North East Italy, between 2009-2018. Serological screening data for hepatitis C virus were collected through the birth attendance certificate that is the mandatory tool in Italy for the birth report and the monitoring of the pregnancy, childbirth and health of the newborn. We also used the informations collected in the Hospital information system in order to get, for confirmed positive cases, the data of the entire serological profile. On this basis, we calculated the coverage of serological screening, the seroprevalence of the infection and the obstetric and neonatal outcomes.Results: A total of 45,493 pregnant women were analysed, 75.5% Italians and 24.5% foreigners. The mean coverage for serological HCV screening in pregnancy was 99.4%. 177 women tested positive for HCV antibodies (107 Italian and 70 foreign women). Mean overall seroprevalence in pregnant women was 3.9‰ (95% CI: 3.8-4.0); amongst Italians it was 3.2‰ (95% CI: 3.0-3.4) and amongst foreigners it was 6.1‰ (95% CI: 5.9-6.2). The highest seroprevalence was recorded in mothers coming from Asian countries. HCV positive mothers present an excess risk for intrahepatic cholestasis and gestational diabetes compared to HCV negative mothers. The neonatal outcomes considered bring to light a statistically significant increase in preterm births, low birth weight, elective Caesarean births, hospitalisation at birth and bottle- or mixed-feeding vs breast-only feeding. There are differences in obstetric and neonatal outcomes in relation to the HCV positive subgroup of the mother defined in particular by exposure to smoke and/or opioids.Conclusions: The availability and integration of current information sources allows you to verify compliance with national recommendations on the serological screening of hepatitis C virus infection, to determine infection seroprevalence, the characteristics of the cases and the obstetric and neonatal outcomes. Infections acquired during pregnancy add up to the overall perinatal rate of mortality and morbidity. Because it has the potential to be screened in all pregnant women, infection with cytomegalovirus continues to raise awareness among obstetricians and healthcare providers, given the perinatal complications involved, but also because not all parturient women are equally informed about the impact of the disease. A significant possibility of vertical transmission is attainable if primary infection is acquired in pregnancy, especially in the first trimester, when the risk of developing a congenital neonatal disease is higher. Background: To explore the clinical value of secondary screening using noninvasive prenatal testing (NIPT) for women with critical-risk and low-risk pregnancies who had multiple of the median (MoM) abnormalities in serological screening. Methods: NIPT was used to analyze fetal free DNA in the peripheral blood of 2,325 women with critical-risk pregnancies and 239 women with low-risk pregnancies with MoM abnormalities in serological screening. Based on NIPT results, women with high-risk pregnancies were recommended for amniocentesis for fetal karyotype analysis. Results: Among 2,325 women with critical-risk pregnancies as determined by serological screening, NIPT indicated 15 high-risk pregnancies (11 cases of trisomy 21 and 4 cases of trisomy 18). Of the 15 patients, 1 case refused prenatal diagnosis. The other 14 cases underwent invasive amniocentesis for fetal karyotype analysis, and 13 cases of fetal chromosomal abnormalities were diagnosed, including ten cases of trisomy 21 and three cases of trisomy 18. NIPT of 239 patients with low-risk pregnancies but abnormal MoM showed one case with a high risk of trisomy 21, which was diagnosed as a false positive by amniotic fluid karyotype analysis, and one case with a high risk of trisomy 13 (stillbirth) that was diagnosed by karyotype analysis. A case of gender chromosome abnormality was diagnosed as aneuploidy by karyotype analysis. Conclusions: The application of NIPT as a secondary screening for women with low- and critical-risk pregnancies as determined by serological screening but with MoM abnormalities will greatly reduce the number of invasive prenatal diagnosis procedures, significantly improve the rate and accuracy of fetal chromosomal abnormality detection. Aim. To determine of the prevalence of viral hepatitis A, E, B, C and D markers in migrant workers.Materials and methods. Blood serum samples from 1,333 migrant workers recently arrived in Russia from Uzbekistan (n = 464), Tajikistan (n = 415), Ukraine (n = 308) and Moldova (n = 146) were analyzed. Anti-HAV IgG, anti-HEV IgM and IgG, HBsAg, anti-HBV and anti-HCV were tested using commercial ELISA tests.Results. The frequency of HBsAg detection was significantly higher among migrants from Tajikistan and Uzbekistan (5,3% and 5,2%, respectively) compared to migrants from Ukraine (1,0%) and Moldova (3,4%). No anti-HDV was detected in any positive HBsAg sample. The Austrian province of Tyrol belongs to the areas where the alveolar echinococcosis (AE) caused by the fox tapeworm Echinococcus multilocularis (E. multilocularis) is highly endemic. In Central Europe and since 2011 in Austria, a growing incidence of human cases of AE has been observed, presumably linked with increasing fox populations infected by the fox tapeworm E. multilocularis. Hunting and the related activities put hunters in a high-risk group, and they are considered particularly vulnerable for the contraction of an AE. In light of this risk and the increased number of AE cases made public in Austria, the objective of the study was to investigate the prevalence of AE in hunters and to provide a possible connection to the incidence increase. In 2015 and 2016, we examined 813 serums of active hunters from all nine districts of Tyrol and serologically tested them for E. multilocularis antibodies. Twenty-one (2.58%) positive results in ELISA were detected via Western blot (WB), and only one (0.12%) serum showed a low positive reaction. No lesion in the liver parenchyma could be detected by abdominal ultrasonography in this patient so far, but the risk of developing alveolar echinococcosis remains for this WB-positive hunter. Risk factor analysis of these 813 hunters revealed that 697 (85.7%) hunted red foxes regularly and 332 (40.8%) of those skinned them as well. Three hundred and eighteen (39.1%) out of the 813 hunters were owners of hunting dogs; 89 (10.9%) and 243 (29.9%) were owners of non-hunting dogs and cats, respectively. Our results indicate that hunters do not have a greater risk of infection with E. multilocularis compared to non-hunters in Austria. The cause of the unexpected increase in AE cases in Austria remains unclear.

Résumé La toxoplasmose est habituellement une infection bénigne, sauf chez l'immunodéprimé et chez le fœtus lors d'une contamination in utero. Le risque de transmission materno-fœtale est de l'ordre de 29 % mais s'avère très variable selon le terme de la grossesse (de 6 % au premier trimestre à environ 80 % en fin de grossesse) tandis que la sévérité de la maladie évolue de façon inverse (formes graves en cas de transmission en début de grossesse, formes infra-cliniques en fin de grossesse). Le nombre de toxoplasmoses congénitales est relativement constant depuis plusieurs années autour de 250 cas par an, ce qui représente une incidence de 0,3 pour 1 000 naissances vivantes en France. Depuis 1978, la France a instauré un programme de prévention de la toxoplasmose congénitale chez les femmes enceintes, avec un dépistage sérologique (IgG et IgM) obligatoire avant la fin du premier trimestre et la surveillance jusqu'à la fin de la grossesse des femmes non-immunisées. En cas de contamination durant la grossesse, il est fondamental de dater la contamination afin de proposer un diagnostic prénatal. Amebiasis, which is caused by Entamoeba histolytica ( E. histolytica ), is the second leading cause of parasite-related death worldwide. It manifests from asymptomatic carriers to severe clinical conditions, like colitis and liver abscesses. Amebiasis is commonly seen in developing countries, where water and food are easily contaminated by feces because of the poor sanitation. However, a recently challenge in many developed countries is the increase in domestic cases of invasive amebiasis as a sexually transmitted infection (STI amebiasis). In contrast to food-/ waterborne transmission of E. histolytica in developing countries, transmission of STI amebiasis occurs directly through human-to-human sexual contact ( e.g. , men who have sex with men and people who engage in oral-anal sex); in this setting, asymptomatic infected individuals are the main reservoir of E. histolytica . The Development of screening methods for the early diagnosis of asymptomatic E. histolytica infection is the key to epidemiologic control. Moreover, delay in diagnosis of severe cases ( e.g. , fulminant amebiasis) leads to death even in developed countries. Objective: To explore the distribution and epidemiological characteristics of patients with syphilis in a first-class tertiary hospital and to evaluate the coincidence rate between chemiluminescence immunoassay (CLIA) and Treponema pallidum particle agglutination assay (TPPA)., Methods: The medical records of 247,501 outpatients and inpatients were retrospectively analyzed. TPPA was used to verify positive and suspected cases, and the coincidence rate between CLIA and TPPA was evaluated. Receiver operating characteristic (ROC) curve was used to determine optimal diagnostic thresholds., Results: Of the 247,501 serum samples, 5,173 were detected positive for syphilis using CLIA, with a detection rate of 2.09% and a men-to-women ratio of 1.39. The chi-square test showed that sex and age were both factors that affected the detection rate (χ 2 =229.51, P < 0.0001). In addition, urology, orthopedics, cardiology, general surgery, gastroenterology, and gynecology represented the top six departments with the highest numbers of positive cases. Comparative analysis showed that the overall coincidence rate between CLIA and TPPA was 80.24%. Analysis of the ROC curve showed that the area under the curve (AUC) was 0.936 (95% confidence interval [CI]: 0.929-0.942, P < 0.0001) using sample/cut-off value (S/CO) as a diagnostic indicator. The results showed that an S/CO value of 3.945 was the best diagnostic value for the CLIA method, with a diagnostic specificity of 93.64% and a sensitivity of 81.90%., Conclusions: Syphilis is widely distributed in various hospital departments and primarily affects middle-aged and older individuals. To assess the predictive value of integrating multiple of the median (MOM) with the risk cutoff value for serological screening of Down syndrome. In this retrospective study, women with singleton pregnancies who underwent triple serological screening for Down syndrome were followed, and their screening results and pregnancy outcomes were recorded. The range of MoM value of each indicator was calculated, different protocols integrating various MoM values with the risk cut-off value were compared. A total of 120,269 women with singleton pregnancy were screened and included in the analysis, of those 52 fetuses were confirmed as trisomy-21 by amniocentesis chromosomal karyotyping. Using a risk cut-off value of 1:380, 8,809 samples tested positive and the screen positive rate was 7.32% (8,809/120,269). The normal reference ranges (5-95%) of the MoM value of AFP, β-hCG, and uE3 were 0.60-1.72, 0.43-2.21 and 0.60-1.58, respectively. The detection rate of each screening protocol integrating different MoM percentile values was between 75% and 79%, the positive rate was between 7% and 18%, and the false positive rate was between 7% and 18%. Protocol-6 which combined the screening risk cut-off value and β-hCG MoM ≥ 97.5% percentile is an optimal protocol with a relatively high detection rate (78.8%) and low false positive rate (8.2%). BACKGROUND: Ovarian cancer is frequently diagnosed at an advanced stage and 70% of patients experience recurrence months to years from initial diagnosis. The expression of paraneoplastic antigens can result in the occurrence of onconeural autoantibodies in ovarian cancer that may be associated with neurological disorders that are clinically manifested in patients before diagnosis of ovarian cancer. These paraneoplastic antigens can serve as excellent biomarkers not only for early detection but also for monitoring ovarian cancer recurrence. OBJECTIVE: To assess the immunoreactivity of our previous 3 biomarkers along with 3 paraneoplastic antigens, HARS, Ro52 and CDR2 for the evaluation of their sensitivity in predicting recurrence before the clinical relapse of the ovarian cancer. METHODS:Western blot immunoassays were performed to assess the immunoreactivity of 6 antigens with 21 recurrent ovarian cancer patients. RESULTS: The results indicated that antibodies to HARS, Ro52, CDR2 and 5H6 antigens predicted ovarian cancer recurrence 5.03 months before the clinical or symptomatic relapse in 21 ovarian cancer patients with a sensitivity of 90.5% when CA125 levels were below the standard cutoff (35 U/ml). CONCLUSION: Our study suggests that appearance of onconeural antibodies prior to the rise in CA125 during post treatment surveillance can be a useful diagnostic to predict ovarian cancer recurrence. A doença de Chagas é uma infecção sistêmica de evolução crônica cujo agente etiológico é o parasita Trypanosoma cruzi. O último relato encontrado sobre a soroprevalência da doença em doadores de sangue realizado na capital pernambucana, Recife, data de 1970, onde foi encontrada uma prevalência de 4,4% em doadores de um hospital local. Devido à falta de informações divulgadas sobre a infecção por T. cruzi e sendo Pernambuco uma região endêmica para esta enfermidade, o presente estudo se propôs a analisar o perfil dos doadores de sangue do Hemocentro de Pernambuco (Hemope), que apresentaram reatividade para doença de Chagas, no período de 2002 a 2007. O perfil dos doadores inaptos foi avaliado de acordo com gênero, idade e procedência segundo as mesorregiões de Pernambuco. Foi encontrada uma prevalência de 0,17% para doença de Chagas e 6,89% das bolsas descartadas deveram-se a essa reatividade. Em relação ao gênero dos doadores, foi significativamente maior a contribuição dos homens (pChagas disease is a systemic infection with a chronic onset transmitted by Trypanosoma cruzi. The aim of the present study was to outline the serological profile of hepatitis C among blood donors seen at the Uberaba Regional Blood Center, Hemominas Foundation, over the last 14 years. The frequency of hepatitis C was compared between first-time and repeat donors and the epidemiological characteristics of those with positive and indeterminate ELISA anti-HCV (third and fourth generation) were analyzed based on the donor histories kept in the archives of the Uberaba Regional Blood Center. The serological ineligibility rate was 0.3%, with higher prevalence in the group of first-time donors. We did not find any significant differences regarding age, skin color, marital status or place of residence between eligible and ineligible donors; however, the frequency of positive serology was higher among men. The lower (0.3%) rate of ineligibility due to hepatitis C that was observed at the Uberaba Regional Blood Center, in relation to most Brazilian blood centers, is probably due to the large number of repeat donors (83.3%). Não encontramos diferenças significativas quanto à idade, cor da pele, estado civil e local de residência entre os aptos e os inaptos, porém a ocorrência de sorologia positiva foi maior nos homens. A menor prevalência de inaptidão para hepatite C (0,3%) no Hemocentro Regional de Uberaba, em relação à maioria dos hemocentros do país, é provavelmente devido ao grande (83,3%) número de doadores de repetição, reforçando assim a importância da fidelização do doador para o aumento da segurança transfusional.

As the result of a huge work on the study of the human genome and based on the results obtained, one can conclude that the single nucleotide polymorphisms are the reasons of the vast majority of non-communicable diseases as well as the diseases other than injuries and poisoning, i.e. the diseases where the cause is not obvious. To clarify the role of single nucleotide polymorphism in the occurrence of atrophic gastritis, it is required to perform full-genome sequencing and human genome scanning with a simultaneous mass serological screening of atrophic gastritis. As a result, it will be possible to establish which single nucleotide polymorphism is responsible for mild, moderate, and severe mucosal atrophy in the antrum and the body of the stomach. The recrudescence of syphilis is leading to the development of new serological tests. The goal of this study was to compare the performance of the more recent Elecsys Syphilis assay, the Electro Chemiluminescence Immunoassay (ECLIA), with the former Architect Syphilis TP assay, the Chemiluminescent Microparticle Immunoassay (CMIA), for the detection of antibodies againstTreponema pallidumin blood donors. Serum samples of 5543 voluntary blood donors were screened in parallel with two tests. All repeatedly reactive (RR) samples by one or both assays were further analysed for confirmation by immmunoblot INNO-LIA and TPHA. Of 32 RR samples by CMIA, 21 were confirmed positive; of 21 RR samples by ECLIA, 20 were confirmed positive. The sensitivities of CMIA and ECLIA were 100% and 95.24% (95% CI = 85.71–100), respectively, not significant (p> 0.05). The specificity and predictive positive value (PPV) of CMIA were 99.86% (95% CI = 99.74–99.94) and 72.41%, respectively, while the specificity and PPV of ECLIA were both 100%, being statistically significant (p= 0.01 for both). The Brazilian Ministry of Health has made tests for HIV1 and HIV2, HTLV I and HTLV II, HCV, HBV, T. cruzi, T. pallidum and Plasmodium in endemic areas, mandatory for all blood collection bags used in the country. However, blood-borne infectious diseases are not investigated in blood recipients before transfusion. For this study, a serological evaluation of recipients before transfusion was carried out. Prior to transfusion, serum samples from 159 blood recipients were analyzed using the same tests used in the serological screening of blood donors. The blood recipients were divided into three groups: Group 1 (G1), patients who had never received blood, Group 2 (G2), patients who had received multiple transfusions and Group 3 (G3) one-off recipients. A triagem sorológica em doadores de sangue, não possibilita segurança de 100% quanto à possibilidade de transmissão de agentes infecto-contagiosos. O Ministério da Saúde determina a realização de testes para sífilis, hepatite B e C, HIV, doença de Chagas, HTLV I/II e malária nas áreas endêmicas, em todas as unidades de sangue coletadas no Brasil. A amostra do doador deve ficar armazenada por um período mínimo de seis meses. Com relação aos receptores de sangue, o Ministério determina a realização de testes imuno-hematológicos pré-transfusionais tais como classificação ABO/Rh, pesquisa de anticorpos irregulares e testes de compatibilidade. Nesse caso, a amostra do receptor deve ficar armazenada por um período de dez dias. Considerando que algumas patologias testadas, quando não detectadas no doador, podem ser transmitidas e cursar durante décadas sem apresentar sintomas, um estudo de provas entre receptores e seus respectivos doadores fica comprometido. Um recente estudo no Brasil, envolvendo receptores sem passado transfusional, eventual e politransfundidos mostrou uma importante prevalência de patologias que podem ser transmitidas pelo sangue. O estudo revelou também que uma elevada percentagem dos receptores que apresentaram reatividade não tinha conhecimento prévio à transfusão do seu estado sorológico. A segurança transfusional e a importância da aplicação de testes sorológicos em receptores de sangue antes da transfusão são pontos discutidos na presente revisão.The serological screening of blood donors does not constitute a 100% guarantee of protection against the possible transmission of infectious and contagious agents. The Ministry of Health determines that tests for syphilis, hepatitis B and C, Aids, Chagas' disease and HTLV I/II, as well as malaria in endemic areas, are mandatory in all the blood collection bags used in Brazil. The samples from blood donors must be stored for six months. Concerning the recipients of blood, the Ministry determines that pre-transfusional immunohematological tests such as ABO/Rh classification, irregular antibody titers and compatibility must be preformed. In these cases, the sample from the recipient is stored for ten days. Considering that some pathologies tested, when not detected in donors, can still be transmitted to the recipients and remain asymptomatic for several decades, a study of cause-effect among recipients and their respective donors is impaired. A recent study in Brazil analyzed recipients of blood without any history of transfusion as well as eventual and multitransfused recipients. It showed an important prevalence of the pathologies transmitted by blood transfusion. The study also revealed that a high percentage of recipients that were reactive, were unaware of their serological status before their transfusion. Transfusional safety and the serological pre-transfusional evaluation in recipients of blood are points discussed in this review.

This study evaluated the usefulness of the anti-HBc, hepatitis C virus antibodies (anti-HCV), human T cell lymphotropic virus I and II antibodies (anti-HTLV I/II), serologic tests for syphilis, and surface antigen of hepatitis B virus (HBsAg) as surrogate markers for the risk for HIV infection in 80,284 serum samples from blood donors from the Blood Bank of "Hospital Universitário Regional Norte do Paraná", Londrina, Paraná State, Brazil, analyzed from July 1994 to April 2001. Among 39 blood donors with positive serology for HIV, 12 (30.8%) were anti-HBc positive, 10 (25.6%) for anti-HCV, 1 (2.6%) for anti-HTLV I/I, 1 (2.6%) was positive for syphilis, and 1 (2.6%) for HBsAg. Among the donors with negative serology for HIV, these markers were detected in 8,407 (10.5%), 441 (0.5%), 189 (0.2%), 464 (0.6%), and 473 (0.6%) samples, respectively. The difference was statistically significant (p < 0.001) for anti-HBc and anti-HCV. Although the predictive positive value for these surrogate markers were low for HIV infection, the results confirmed the anti-HBc and anti-HCV as useful surrogate markers for HIV infection thus reinforcing the maintenance of them in the screening for blood donors contributing to the prevention of the small number of cases in which HIV is still transmitted by transfusion.
