Abstract

An 8-cavity, 1.3 GHz, LCLS-II-HE cryomodule was assembled and tested at Fermilab to verify performance before the start of production. Its cavities were processed with a novel nitrogen doping treatment to improve gradient performance. The cryomodule was tested with a modified protocol to process sporadic quenches, which were observed in LCLS-II production cryomodules and are attributed to multipacting. Dedicated vertical test experiments support the attribution to multipacting. The verification cryomodule achieved an acceleration voltage of 200 MV in continuous wave mode, corresponding to an average accelerating gradient of 24.1 MV/m, significantly exceeding the specification of 173 MV. The average Q0 (3.0x10^10) also exceeded its specification (2.7x10^10). After processing, no field emission was observed up to the maximum gradient of each cavity. Abstract

Mapping the behaviour of dark energy is a pressing task for observational cosmology. Phenomenological classification divides dynamical dark energy models into freezing and thawing, depending on whether the dark energy equation of state is approaching or moving away from $w=p/\rho=-1$. Moreover, in realistic dynamical dark energy models the dynamical degree of freedom is expected to couple to the electromagnetic sector, leading to variations of the fine-structure constant $\alpha$. We discuss the feasibility of distinguishing between the freezing and thawing classes of models with current and forthcoming observational facilities and using a parametrisation of the dark energy equation of state, which can have either behaviour, introduced by Mukhanov as fiducial paradigm. We illustrate how freezing and thawing models lead to different redshift dependencies of $\alpha$, and use a combination of current astrophysical observations and local experiments to constrain this class of models, improving the constraints on the key coupling parameter by more than a factor of two, despite considering a more extended parameter space than the one used in previous studies. We also briefly discuss the improvements expected from future facilities and comment on the practical limitations of this class of parametrisations. The aim of this study was to compare the effectiveness of biologics in these two conditions. We included 318 patients (24 ACO and 297 asthma) treated with monoclonal antibodies and followed for at least 12 months. Omalizumab was the most frequently employed biologic agent both in patients with ACO and asthma. Asthma control test (ACT) scores after at least 12 months of biologic therapy were not significantly different between groups. The percentage of patients with ≥ 1 exacerbation and ≥ 1 corticosteroid burst was significantly higher in ACO patients (70.8 vs 27.3 and 83.3% vs 37.5%, respectively), whereas the percentage of “controlled” patients (with no exacerbations, no need for corticosteroids and ACT ≥ 20) was significantly lower (16.7% vs 39.7%). In conclusion, this report suggests that patients with ACO treated with biologics reach worse outcomes than asthma patients.Keywords: asthma, asthma–COPD overlap, COPD

Abstract

Background & aims: Severe COVID-19 infection is characterized by an inflammatory response and lung injury that can evolve into an acute respiratory distress syndrome that needs support treatment in intensive care unit. Nutritional treatment is an important component of the management of critically ill patients and should be started in the first 48 h of ICU admission to avoid malnutrition. This study describes the characteristics of the patients treated in a tertiary hospital in Madrid during the months of March-May 2020 (first wave), the medical nutrition treatment employed and its influence in the clinical outcome of these patients. Methods: This is a retrospective study including COVID-19 patients admitted in ICU that needed medical nutrition treatment (MNT). Collected variables included sex, age, BMI, underlying diseases, time from hospitalisation to ICU admission, type of respiratory support (invasive mechanical ventilation (IMV) or high flow nasal cannula (HFNC) or non-invasive ventilation (non-IMV)), caloric and protein requirements (25 kcal/kg adjusted body weight (ABW), 1.3 g/kg ABW/day), MNT type (enteral nutrition (EN), parenteral nutrition (PN), mixed EN + PN), total calories (including propofol) and proteins administered, percentage of caloric and protein goal in ICU day 4th and 7th, metabolic complications, acute kidney failure (AKF), length of stay (LOS) and mortality. Data are expressed as mean ± SD, median (IQR) or frequencies. Statistical analysis was performed with the IBM SPSS Statistics for Windows, Version 25.0. p < 0.05 were considered statistically significant. Results: A total of 176 patients were included (72.7% male), 60.1 ± 13.5 years, BMI 29.9 ± 5.4 kg/m2. Underlying diseases included 47.4% overweight, 39.8% obesity, 49.1% hypertension, 41.4% dyslipidaemia. 88.6% of patients needed IMV, 89.1% prone position, 2.9% ECMO. Time to ICU admission: 2 (4.75) days. Abstract

This study aims to explore the relationship between relational reflexivity and the well-being of young couples with children. The research involved 154 individuals belonging to 77 young couples with children, and we used a dyadic approach (Actor-Partner interdependence Model) to explore the association between the two dimensions of relational reflexivity (self-detachment and relational agency) and the well-being of parents. To study relational reflexivity, we collected data on self-detachment and we validated a measure to approach relational commitment and functioning within the couple (family relational agency). Then, we explored differences in couple’s well-being (relationship satisfaction and generativity). Results showed that the two dimensions of reflexivity are differently associated with the well-being of the couple. Important differences emerged between actor and partner effects, suggesting that couple generativity and quality of marriage outcomes are differently related to relational reflexivity, wich are linked to gender differences.

Abstract

A total of 32% of children whose parents have some mental health problems are estimated to be diagnosed with some mental disorder later in life. As a consequence, a need arises to offer preventive psychological interventions aimed at these children. The aims were to investigate whether there are significant changes before and after the KidsTime program. In total, 101 people participated in the program, and pre‐intervention and post‐intervention data on self‐stigma, self‐esteem, resilience, parenting practices and strength and difficulties of thirty‐three parents with mental illness were obtained. Significant pre‐post differences were found in the 'expression of affection' subscale of the parenting practices and in self‐stigma. In the group of parents with a mental illness, the KidsTime program showed improvement of parents' emotional support for their children and a reduction in their self‐stigma as well. Multi‐family interventions are key to improving self‐stigma and parenting skills, and this can lead to prevention of future mental health problems in their children. [ABSTRACT FROM AUTHOR]

In particular, cardiac toxicity is a potentially fatal AE, and introduces new challenges regarding its underlying molecular mechanisms of occurrence, optimal treatment and follow up, and prevention. We present a clinical case of a patient with advanced kidney cancer treated with nivolumab as a third line treatment. After four cycles, the patient developed nonspecific symptoms and was hospitalized, identifying a set of clinical, analytical and electrocardiographic alterations compatible with myocarditis. Despite the intensive support, the patient died and a necropsy study was performed. We present a detailed description of the clinical case including the pathological and molecular findings, and we conduct a review of the available evidence related to immune-mediated cardiac toxicity to offer some new highlights in the management of this AE.Keywords: myocarditis, nivolumab, kidney, immune-related adverse events

Abstract

The NGLS collaboration is developing design concepts for a multi-beamline soft x-ray FEL array powered by a superconducting linear accelerator, operating with a high bunch repetition rate of approximately 1 MHz. The CW superconducting linear accelerator design is based on developments of TESLA and ILC technology, and is supplied by an injector based on a high-brightness, highrepetition- rate photocathode electron gun. Electron bunches from the linac are distributed by RF deflecting cavities to the array of independently configurable FEL beamlines with nominal bunch rates of ∼100 kHz in each FEL, with uniform pulse spacing, and some FELs capable of operating at the full linac bunch rate. Individual FELs may be configured for different modes of operation, including self-seeded and external-laser-seeded, and each may produce high peak and average brightness x-rays with a flexible pulse format, and with pulse durations ranging from femtoseconds and shorter, to hundreds of femtoseconds. 46. Psychosocial risk in healthcare workers after one year of COVID-19.

Abstract

Background The COVID-19 pandemic overwhelmed the capacity of the healthcare system, affecting the volume of demands and the care tasks of healthcare workers. Aims To examine the health indicators and exposure to psychosocial risks of Spanish healthcare workers 1 year into the COVID-19 pandemic and compare them with the results of the first wave. Methods We conducted a cross-sectional study using an online questionnaire (April–May 2020 and 2021). The data stem from the COTS 1 project database, corresponding to the first wave of COVID-19 (n = 1989) and COTS 2 (n = 1716) corresponding to 1 year later. The samples were independent. The prevalence of exposure to psychosocial risks and adverse health indicators was estimated for every occupational group, segregating the data by sex. Results Professionals of all types presented worse perception of health. In general, the results were worse for women, while geriatric assistants presented the greatest exposure to psychosocial risk in COTS 2 compared to COTS 1. Sleep problems, high quantitative demands and high concern about becoming infected and spreading COVID-19 were cross-disciplinary in COTS 1, while worse perception of health, high pace of work, high work–life conflict and low development opportunities stood out in COTS 2. Conclusions Exposure to psychosocial risks was already high during the first wave and a significant decline in working conditions was observed. The prolongation of the pandemic exacerbated these results and seems to have multiplied the pre-existing inequalities between the axes of segregation in the labour market. [ABSTRACT FROM AUTHOR]
