Abstract

Vocal cord palsy can have myriad causes. Unilateral vocal cord palsy is common and frequently asymptomatic. Trauma, head, neck and mediastinal tumors as well as cerebrovascular accidents have been implicated in causing unilateral vocal cord palsy. Viral neuronitis accounts for most idiopathic cases. Bilateral vocal cord palsy, on the other hand, is much less common and is a potentially life-threatening condition. Myasthenia gravis, an autoimmune disorder caused by antibodies targeting the post-synaptic acetylcholine receptor, has been infrequently implicated in its causation. We report here a case of bilateral vocal cord palsy developing in a 68-year-old man with no prior history of myasthenia gravis 2 months after he was operated on for diverticulitis of the large intestine. Delay in considering the diagnosis led to endotracheal intubation and prolonged mechanical ventilation with attendant complications. Our case adds to the existing literature implicating myasthenia gravis as an infrequent cause of bilateral vocal cord palsy. Our case is unusual as, in our patient, acute-onset respiratory distress and stridor due to bilateral vocal cord palsy was the first manifestation of a myasthenic syndrome.

Abstract

Bupropion is a well known atypical antidepressant marketed in the United States under the trade name Wellbutrin. In its sustained release form, it is also used for smoking cessation. It lowers seizure threshold and its potential to cause seizures has been widely publicized. Lamotrigine is a broad spectrum anticonvulsant with an extremely favorable side-effect profile and has thus rapidly become the preferred drug for a broad range of seizure types such as absence seizures, primary generalized epilepsy syndromes such as idiopathic generalized tonic-clonic seizures, juvenile myoclonic epilepsy as well as partial (focal) epilepsy. It is also used by psychiatrists as a mood stabilizer. We report a patient who presented with delayed status epilepticus following a failed suicidal attempt after overdosing on bupropion XL and lamotrigine.

Abstract

Idiopathic primary generalized epilepsy (IPGE) is characterized by generalized convulsions with loss of consciousness (LOC) at times associated with lateral tongue bite and loss of bladder control. Interictal EEG record shows an organized background with a 9-12 Hz posterior dominant rhythm (PDR) and generalized spike wave and polyspike wave discharges. We present here a patient whose typical spells were characterized by abrupt LOC accompanied with tonic posturing and convulsive movements and occasional bladder incontinence. Interictal EEG was reported as intermittent generalized spike wave discharges leading to initiation of levetiracetam therapy. Marked sinus bradycardia was noted during examination during inpatient video-EEG admission. A typical event captured on video-EEG with simultaneous telemetry revealed sinus arrest leading to cerebral hypoperfusion and seizure as the underlying cause of his “epilepsy”.

Abstract

Dyke-Davidoff-Masson syndrome (DDMS) is characterized by unilateral cerebral atrophy or hypoplasia (hemiatrophy), ipsilateral osseous hypertrophy with hyperpneumatization of paranasal sinuses accompanied with varying degrees of contralateral hemiparesis, hemifacial atrophy and mental retardation. DDMS may be infantile or acquired later in life as a result of trauma, infection, ischemic or hemorrhagic insult to the central nervous system (CNS). Abstract

Lyme disease is a tick borne spirochete infection caused by Borrelia burgdorferi that may present with multisystem involvement. It can affect both the central and peripheral nervous systems. The classical triad of Lyme neuroborreliosis called Bannwarth’s syndrome includes lymphocytic meningitis, cranial neuropathy and radiculoneuritis. An ever increasing number of peripheral neuropathies have been associated with Lyme borreliosis. We present here an asymptomatic 57-year-old man with right sided diaphragmatic paralysis noted on a chest radiograph performed pre-operatively prior to a total knee replacement surgery. Lyme serology came back positive. Our case is unique as phrenic nerve palsy was the sole clinical manifestation of Lyme disease and it emphasis that Lyme disease be considered in the differential diagnosis of otherwise unexplained phrenic nerve palsy specially in patients residing in endemic areas.Key words: Lyme disease; phrenic nerve palsy

45. CYP2C9, VKORC1, CYP4F2, ABCB1and F5variants: Influence on quality of long-term anticoagulation

Abstract

Aims: The study aims to evaluate the impact of genetic, demographic and clinical data on various measures of outcome of anticoagulation quality in patients. Patients and methods: The study consisted of 310 patients receiving long-term oral anticoagulation therapy in our hospital. Apart from demographic and clinical variables, 21 SNPs (in 7 genes) were analyzed and compared with the outcomes of anticoagulation therapy. Various outcomes that were measured are; supra therapeutic INRs (INR >3, >6), anticoagulation stabilization, time taken to stabilize and proportion of INRs within (2–3), above (>3) and below (<2) therapeutic range. Results: Supra therapeutic INRs were influenced by CYP2C9*2, *3, CYP4F2rs2108622, VKORC1-1639G>A, 1173C>T, rs55894764 along with concomitant drugs, smoking, body weight and height. Persistently fluctuating INRs/absolute instability correlated with VKORC1-1639G>A, gender, height and body mass index. The time taken to stabilize was associated with CYP4F2rs2108622, CYP2C9*14, smoking, clinical indication and concomitant drugs. The overall distribution of INR was influenced by variants in CYP4F2rs2108622, CYP2C9*3, rs9332230, VKORC11173C>T, -1639G>A, rs55894764, ABCB1rs2032582, rs1128503, rs1045642 and F5rs6025, age, smoking and concomitant drugs. Conclusions: Knowledge of factors influencing the quality of long term anticoagulation can help clinicians to customize therapy either by dose variation, therapy with alternate choice of drug, concurrent heparin therapy and/or frequent INR monitoring.
