Abstract

Background: Posterior fossa tumors represent two thirds of brain tumors in children. Although progress in treatment has improved survival rates over the past few years, long-term memory impairments in survivors are frequent and have an impact on academic achievement. The hippocampi, cerebellum and cerebellar-cortical networks play a role in several memory systems. They are affected not only by the location of the tumor itself and its surgical removal, but also by the supratentorial effects of complementary treatments, particularly radiotherapy. The IMPALA study will investigate the impact of irradiation doses on brain structures involved in memory, especially the hippocampi and cerebellum. Methods/design: In this single-center prospective behavioral and neuro-imaging study, 90 participants will be enrolled in three groups. The first two groups will include patients who underwent surgery for a posterior fossa brain tumor in childhood, who are considered to be cured, and who completed treatment at least 5 years earlier, either with radiotherapy (aggressive brain tumor; Group 1) or without (low-grade brain tumor; Group 2). Group 3 will include control participants matched with Group 1 for age, sex, and handedness. All participants will perform an extensive battery of neuropsychological tests, including an assessment of the main memory systems, and undergo multimodal 3 T MRI. The irradiation dose to the different brain structures involved in memory will be collected from the initial radiotherapy dosimetry. Discussion: This study will provide long-term neuropsychological data about four different memory systems (working memory, episodic memory, semantic memory, and procedural memory) and the cognitive functions (attention, language, executive functions) that can interfere with them, in order to better characterize memory deficits among the survivors of brain tumors. We will investigate the correlations between neuropsychological and neuroimaging data on the structural (3DT1), microstructural (DTI), functional (rs-fMRI), vascular (ASL) and metabolic (spectroscopy) impact of the tumor and irradiation dose. This study will thus inform the setting of dose constraints to spare regions linked to the development of cognitive and memory functions. Abstract

Background:BRCC3/MTCP1 deletions are associated with a rare familial moyamoya angiopathy with extracranial manifestations.Case: We report the case of an adolescent male presenting with progressive and symptomatic moyamoya angiopathy and severe dilated cardiomyopathy caused by a hemizygous deletion of BRCC3/MTCP1. He was treated for renovascular hypertension by left kidney homograft and right nephrectomy in infancy and had other syndromic features, including cryptorchidism, growth hormone deficiency, and facial dysmorphism. Due to worsening of the neurological and cardiac condition, he was treated by a direct superficial temporal artery to middle cerebral artery bypass to enable successful cardiac transplant without cerebral damage.Conclusions:BRCC3-related moyamoya is a devastating disease with severe heart and brain complications. This case shows that aggressive management with cerebral revascularization to allow cardiac transplant is feasible and efficient despite end-stage heart failure.

Abstract

Introduction: Antenatal presentation of hypertrophic cardiomyopathy (HCM) is rare. We describe familial recurrence of antenatal HCM associated with intrauterine growth restriction and the diagnostic process undertaken. Methods: Two pregnancies with antenatal HCM were followed up. Biological assessment including metabolic analyses, genetic analyses, and respiratory chain study was performed. We describe the clinical course of these two pregnancies, antenatal manifestations as well as specific histopathological findings, and review the literature. Results: The assessment revealed a deficiency in complex I of the respiratory chain and two likely pathogenic variations in the ACAD9 gene. Discussion and Conclusion: Antenatal HCM is rare and a diagnosis is not always made. In pregnancies presenting with cardiomyopathy and intrauterine growth restriction, ACAD9 deficiency should be considered as one of the potential underlying diagnoses, and ACAD9 molecular testing should be included among other prenatal investigations.

International audience; La sécurité est devenue un élément essentiel des systèmes électroniques utilisés dans le stockage de données personnelles, bancaires ou dans des équipements critiques tels que les systèmes militaires ou les infrastructures énergétiques. En effet, les systèmes électroniquesmanipulent de plus en plus des données sensibles dans des environnements non contrôlés et peuvent être confrontés à des problèmes de piratage [1]. Plusieurs technologies électroniques anti-effraction et anti-rétro-ingénierie existent. Les plus simples sont basées sur des cléscryptographiques ou logicielles, des fusibles de sécurité pour empêcher les accès non autorisés. Sont explorées aussi des composants à clef d’indentification (ID) ou fonctions non-clonables (PUF) [2-5], le camouflage des topologies [6-7] ou, le renforcement du packaging [8] pouréviter la pénétration physique.La mise en place de ces protections impose des contraintes lors de la conception du système, et est souvent accompagnée d’une augmentation de son coût, de sa taille et parfois affecte ses performances fonctionnelles. De plus, elles ne sont nullement efficaces contre le reverse engineering [8-10] puisque l’attaquant peut identifier la structure du composant par imagerie optique, électronique ou rayon-X.Nous avons développé un nouveau stérilisateur de données miniature, hautement réactif (destruction physique des données en moins de 1 ms), sûr (pas de matériau énergétique sensible tel qu’un explosif), qui agit comme un module complémentaire à placer sur le composantélectronique ou la mémoire à protéger. Ce stérilisateur aussi appelé dispositif de sécurité ultime (USD) est actionné par un pyroMEMS [11-12] pour désintégrer mécaniquement un composant contenant des données sensibles en moins de 100 μs après détection de l’intrusion. Sonactionnement est basé sur l’utilisation d’un matériau à base de nanothermite Al/CuO déposée par 3D printing [13-15]. Abstract

International audience; This work presents a triggerable ultimate security device (USD) capable of physically destroying a memory chip that contains classified data within a few milliseconds upon the detection of intrusion. The USD is designed as an add-on module that can be positioned onto any electronic chip, IC circuit, or memory card without requiring any specialized chip design, substrate type or encapsulation solution. Its operation is simply based on the exothermic reaction of a solid-state printed energetic composite that physically disintegrates the sensible component in less than 1 ms. The solid-state energetic composite consists of a mixture of Al/CuO nanothermite with a copper ammine complex densely printed onto the chip that is meant to be destroyed. This energetic layer can be ignited through resistive heating or capacitance discharge in less than 50 µs. The paper details the fabrication and assembly of a test USD device and demonstrates that 400 mg of energetic composite can be used to irreversibly destroy one silicon chip (~120 mm3) in less than 10 ms. The outcome of this work also shows that the reaction time and efficiency are highly dependent on the reactive material loading, i.e., the mass of nanothermites deposited into the device and memory card.

National audience; Nous avons développé un système de sécurité ultime (USD) capable de répondre en quelques millisecondes à une intrusion sur un système en venant physiquement détruire les données avant toutes altérations, vole ou revers engineering. Pour ce faire nous avons dans un premier temps élaboré notre Matériau Energétique (ME) à base de nanothermite Al/CuO et complexe de cuivre et mélangé à du polymère, PVP, pour le stockage en solution et sa dépose par direct writing. Avec 450 mg de matériau stocké dans un boitier muni d’un initiateur, capable de s’initier en quelques dizaines de μs, nous sommes capables de détruire de manière irréversible un composant sensible.

Abstract

Pediatric spinal low-grade glioma (LGG) and glioneuronal tumours are rare, accounting for less 2.8–5.2% of pediatric LGG. New tumour types frequently found in spinal location such as diffuse leptomeningeal glioneuronal tumours (DLGNT) have been added to the World Health Organization (WHO) classification of tumours of the central nervous system since 2016, but their distinction from others gliomas and particularly from pilocytic astrocytoma (PA) are poorly defined. Most large studies on this subject were published before the era of the molecular diagnosis and did not address the differential diagnosis between PAs and DLGNTs in this peculiar location. Our study retrospectively examined a cohort of 28 children with LGGs and glioneuronal intramedullary tumours using detailed radiological, clinico-pathological and molecular analysis. 25% of spinal PAs were reclassified as DLGNTs. PA and DLGNT are nearly indistinguishable in histopathology or neuroradiology. 83% of spinal DLGNTs presented first without leptomeningeal contrast enhancement. Unsupervised t-distributed stochastic neighbor embedding (t-SNE) analysis of DNA methylation profiles showed that spinal PAs formed a unique methylation cluster distinct from reference midline and posterior fossa PAs, whereas spinal DLGNTs clustered with reference DLGNT cohort. FGFR1 alterations were found in 36% of spinal tumours and were restricted to PAs. Spinal PAs affected significantly younger patients (median age 2 years old) than DLGNTs (median age 8.2 years old). Progression-free survival was similar among the two groups. In this location, histopathology and radiology are of limited interest, but molecular data (methyloma, 1p and FGFR1 status) represent important tools differentiating these two mitogen-activated protein kinase (MAPK) altered tumour types, PA and DLGNT. Thus, these molecular alterations should systematically be explored in this type of tumour in a spinal location.

Abstract

Highlights • Common long-term memory defects after pediatric brain tumor affect school achievement • The hippocampi, the cerebellum and cerebellar-cortical networks play a role in several memory systems. • This study will provide long-term neuropsychological data about four different memory systems. • We will investigate the correlations between neuropsychological, neuroimaging and radiotherapy dose data. • Imaging will be structural (3DT1), microstructural (DTI), functional (rs-fMRI), vascular (ASL) and metabolic (spectroscopy)., Background Posterior fossa tumors represent two thirds of brain tumors in children. Although progress in treatment has improved survival rates over the past few years, long-term memory impairments in survivors are frequent and have an impact on academic achievement. The hippocampi, cerebellum and cerebellar-cortical networks play a role in several memory systems. They are affected not only by the location of the tumor itself and its surgical removal, but also by the supratentorial effects of complementary treatments, particularly radiotherapy. The IMPALA study will investigate the impact of irradiation doses on brain structures involved in memory, especially the hippocampi and cerebellum. Methods/design In this single-center prospective behavioral and neuro-imaging study, 90 participants will be enrolled in three groups. The first two groups will include patients who underwent surgery for a posterior fossa brain tumor in childhood, who are considered to be cured, and who completed treatment at least 5 years earlier, either with radiotherapy (aggressive brain tumor; Group 1) or without (low-grade brain tumor; Group 2). Group 3 will include control participants matched with Group 1 for age, sex, and handedness. All participants will perform an extensive battery of neuropsychological tests, including an assessment of the main memory systems, and undergo multimodal 3 T MRI. The irradiation dose to the different brain structures involved in memory will be collected from the initial radiotherapy dosimetry. Discussion This study will provide long-term neuropsychological data about four different memory systems (working memory, episodic memory, semantic memory, and procedural memory) and the cognitive functions (attention, language, executive functions) that can interfere with them, in order to better characterize memory deficits among the survivors of brain tumors. We will investigate the correlations between neuropsychological and neuroimaging data on the structural (3DT1), microstructural (DTI), functional (rs-fMRI), vascular (ASL) and metabolic (spectroscopy) impact of the tumor and irradiation dose. This study will thus inform the setting of dose constraints to spare regions linked to the development of cognitive and memory functions. Abstract

International audience; We developed an Ultimate Security Device (USD) that can, in case of intrusion or external attack, blow up a safetycritical component such as memory device. The device consists of two active parts (1) a pyroMEMS ignites in a fraction of millisecond (2) a mass of reactive composite, both encapsulated into a printed hermetic cap and placed over the sensitive component to be protected. After the presentation of the design and integration of the USD, we demonstrated that 400 mg of reactive composite permits to irreversibly destroy the silicon chips (~118 mm 3) in less than 10 ms. This ultimate security device provides a speedy and automatic response and can be programmed for tunable actions (generation of pressure burst, heat, chemical species) to implement relevant emergency safety responses.

Abstract

International audience; A prior investigation by the authors demonstrated that incorporating 25% of copper complex (Cu(NH3)4(NO3)2) into Al/CuO nanothermite enables to produce highly-reactive gasgenerating energetic composites for emerging micro-airbag applications. To further improve the decomposition of the copper complex into gaseous species (N2, O2, N2O), during the thermite reaction, we employed ball milling technique to diminish its grain size down to the nanoscale. Results show that premilling the copper complex i.e. refining its grains without much modifying their structures, increases the pressure generation and burn rate by a factor 1.5 and 2, respectively. It also maintains a high degree of performances along a wider range of thermite to copper complex mass ratio.

Abstract

Patients with pituitary stalk interruption syndrome (PSIS) are initially referred for hypoglycemia during the neonatal period or growth retardation during childhood. PSIS is either isolated (nonsyndromic) or associated with extra-pituitary malformations (syndromic).To compare baseline characteristics and long-term evolution in patients with PSIS according to the initial presentation.Sixty-seven patients with PSIS were included. Data from subgroups were compared: neonates (n = 10) versus growth retardation patients (n = 47), and syndromic (n = 32) versus nonsyndromic patients (n = 35).Neonates displayed a more severe hormonal and radiological phenotype than children referred for growth retardation, with a higher incidence of multiple hormonal deficiencies (100% versus 34%; P = 0.0005) and a nonvisible anterior pituitary lobe (33% versus 2%; P = 0.0017). Regular follow-up of growth might have allowed earlier diagnosis in the children with growth retardation, as decreased growth velocity and growth retardation were present respectively 3 and 2 years before referral. We documented a progressive worsening of endocrine impairment throughout childhood in these patients. Presence of extra-pituitary malformations (found in 48%) was not associated with more severe hormonal and radiological characteristics. Growth under GH treatment was similar in the patient groups and did not vary according to the pituitary MRI findings.PSIS diagnosed in the neonatal period has a particularly severe hormonal and radiological phenotype. The progressive worsening of endocrine impairment throughout childhood justifies periodic follow-up to check for additional hormonal deficiencies.
