Aim: Flexible bronchoscope is widely used by pediatric pulmonologists as a diagnostic and therapeutic tool. The objective of this study was to present our anesthesia experience in pediatric flexible bronchoscopy in which airway management was provided with laryngeal mask airway (LMA) and the complications developed.Material and Methods: This study was conducted in children aged between 2-15 years who underwent bronchoscopy for diagnosis and/or treatment between January 2017 and November 2018. Patients’ demographic data, diagnosis, anesthesia and airway management were recorded from the patient files. Times of anesthesia, operation and recovery were recorded. Complications during the procedure, awakening and recovery were recorded. Patients’ sore throat and hoarseness during resting and swallowing were recorded. Results: This study included 31 patients whose airway management was provided with LMA. The mean age was 8.58±4.14 years. Persistent cough was the most common indication for bronchoscopy (35.5%). Anesthesia time was 15.46±10.99 minutes, bronchoscopy time 12.87±10.57, awakening time 16.38±4.53 minutes, and recovery time 23.32±10.24 minutes. The most common complication was cough (45.2%). Sore throats of the patients were observed as mild and moderate at the 0th and 2th hours. Both resting and swallowing sore throats were observed as mild at the 4th hour, while no sore throat was seen in any patient at the 12th hour. Hoarseness was observed at mild level in 4 patients (12.9%) at the 0th hour.Conclusion: Providing airway with LMA in pediatric flexible bronchoscopy applications offers a safe anesthetic management, and it has a low rate of complications.Keywords: Flexible bronchoscopy; pediatric bronchoscopy; laryngeal mask airway.

Amaç: Çocuklarda, oksidan ve antioksidan dengenin kistik fibrosis dışı bronşiekatazi (non-CF BE) patogenezi ve prognozundaki etkisinin değerlendirilmesi.Gereç ve Yöntem: Haziran 2009 ve Ekim 2010 tarihleri arasında nonCF BE tanılı 29 çocuk çalışmaya alındı. Otuz sağlıklı çocuk kontrol grubunu oluşturdu. Sağlıklı ve hasta çocuklarda paraoksonaz1 (PON1), total oksidan durum (TOS) ve total antioksidan durum (TAS) serum düzeyleri stabil ve alevlenme dönemlerinde ölçüldü.Bulgular: Hasta grubunda PON1 ve TAS düzeyleri alevlenme döneminde kontrol gurbuna göre daha düşüktü (p 0.05 ve p 0.01, sırasıyla). İmmün yetmezlikli hastalarda oksidatif stresi gösteren TOS düzeyleri ve TAS/TOS oranı kontrol grubuna göre daha düşüktü (p 0.008 ve p 0.01, sırasıyla). TAS düzeyleri ve PON1/TOS oranı orta-şiddetli bronşiektazili hastalarda hafif bronşektazili hastalardan daha düşüktü (p 0.04 ve p 0.03, sırasıyla).Sonuç: Alevlenme döneminde non-CF BE çocuklarda oksidatif stres artmış ve anti-oksidan kapasite azalmıştır. Antioksidan tedavi özellikle immün yetmezliği ve/veya orta-şiddetli bronşiaktazisi olan non-CF BE hastalarda oksidatif stresin neden olduğu hasarın şiddetini azaltmaya yardımcı olacak, özellikle daha iyi prognoza katkıda bulunacaktır., Objective: To evaluate the role of the oxidant and antioxidant balance in the pathogenesis and prognosis of non-cystic fibrosis bronchiectasis (non-CF BE) in children.Materials and Methods: Twenty-nine children with non-CF BE were enrolled between June 2009 and October 2010. Thirty healthy children were enrolled as controls. Paraoxonase 1 (PON1), total oxidant status (TOS), and total antioxidant status (TAS) serum levels were measured in controls and in patients when stable and at acute exacerbation.Results: PON1 and TAS levels were lower in patients at acute exacerbation than in controls (P 0.05 and P 0.01, respectively). TOS levels indicative of oxidative stress were higher, and TAS/TOS levels were lower, in immune-deficient patients than control group (P 0.008 and P 0.01, respectively). TAS levels and PON1/TOS ratio were significantly lower in patients with moderate-severe bronchiectasis than in patients with mild bronchiectasis (P 0.043 and P 0.03, respectively).Conclusion: Oxidative stress was increased and antioxidant capacity decreased in patients with non-CF BE during the exacerbation period. Antioxidant treatment in patients with non-CF BE, especially in patients with immunodeficiency and/or with moderate-severe bronchiectasis, could be helpful to reduce the frequency and severity of the attacks by reducing oxidative stress-induced damage, ultimately contributing to a better prognosis.

Spontan pnömomediastinum (Hamman’s sendromu)nadir görülen, kendini sınırlayan genellikle gençerişkin erkeklerde görülen bir hastalıktır. Pulmoner interstisyumdaalveolar rüptüre sekonder intratorasik basınçartışı sonucu, havanın hilum ve mediastene doğruilerlemesi sonucu ortaya çıkar. Olgular çoğunlukla göğüsağrısı, boyun ağrısı, sırta yayılan ağrı, ses kısıklığı,yutma güçlüğü ve öksürük şikayetleri ile başvururlar. Buyazıda çocuk acil kliniğine göğüs ağrısı ile başvurup,değerlendirme sonrası spontan pnömomediastinum(SPM) saptanan olgumuz, göğüs ağrısının ayırıcıtanısında SPM’ye dikkat çekmek amacıyla sunulmuştur., Spontaneous pneumomediastinum (Hamman’s syndrome)is a rare, self-limiting disorder commonly seenin young adult males. It occurs by the flow of air towardsto the hilum and mediastinum due to increaced intrathoracicpressure secondary to alveolar rupture. Thecases usually present with the complaints of chest pain,neck pain, back pain, dysphonia, dysphagia and coughing.In this article, we present a case of spontaneouspneumomediastinum (SPM), who admitted to the pediatricemergency clinic with the complaint of chest pain.SPM should be kept in mind in the differential diagnosisof chest pain.

Giriş: Bu çalışmanın amacı; Konya ili atmosferindeki Alternaria ve Cladosporium sporlarını etkileyen meteorolojik faktörleri (sıcaklık, nispi nem, rüzgar hızı, rüzgar yönü ve yağış miktarı) saptamaktır. Gereç ve Yöntem: Alternaria ve Cladosporium sporları 1 Ocak 2008-31 Aralık 2009 tarihleri arasında Burkard cihazıyla toplandı. Mikroskobik sayımlar atmos- ferik konsantrasyonlara çevrildi ve spor/m3 şeklinde ifade edildi. Bulgular: 2008-2009 yıllarında sırasıyla, 424 (%19.2) ve 3977 (%8.6) spor/m3 Alternaria sporu, 1784 (%80.8) ve 42.158 (%91.4) spor/m3 Cladosporium sporu tespit edildi. 2008 yılında Cladosporium konsantrasyonuyla günlük ortalama sıcaklık (r 0.181, p 0.045), maksimum sıcaklık (r 0.193, p 0.033) ve rüzgar hızı (r 0.242, p 0.007) arasında pozitif korelasyon; nispi nem (r -0.215, p 0.017) ile negatif korelasyon saptandı. 2009 yılında ise Alternaria ve Cladosporium spor konsantrasyonlarının, günlük ortalama sıcaklık (r 0.44, p 0.001; r 0.44, p 0.001), minimum sıcaklık (r 0.46, p 0.001; r 0.44, p 0.001) ve maksimum sıcaklık (r 0.40, p 0.001; r 0.43, p 0.001) ile pozitif ilişkili olduğu; nispi nem ile (r -0.29, p 0.001; r -0.37, p 0.001) negatif korelasyonu olduğu bulundu. Sonuç: Konya ili atmosferindeki Alternaria ve Cladosporium miktarı meteorolojik faktörlere göre değişiklik göstermektedir. Ayrıca, Cladosporiuma ait sporlar Alternaria sporlarından daha yoğun olarak tespit edilmiştir., Objective: The aim of this study was to determine the meteorological factors (temperature, relative humidity, wind speed, wind direction, rainfall) affecting Alternaria and Cladosporium spores in the atmosphere of Konya, Turkey. Materials and Methods: Measurement of Alternaria and Cladosporium spores were carried out between January 1st 2008 and December 31st 2009, with a Burkard Volumetric 7-Days Spore Trap. Microscope counts were converted into atmospheric concentrations and expressed as spore/m3. Results: While Alternaria spores were identified as 424 (19.2%) and 3977 (8.6%) spores/m3; Cladosporium spores were detected as 1784 (80.8%) and 42158 (91.4%) spores/m3 in 2008 and 2009 years, respectively. The daily concentrations of Cladosporium spore in 2008 were positively correlated with daily mean temperature (r 0.181, p 0.045), maximum temperature (r 0.193, p 0.033) and wind speed (r 0.242, p 0.007) and whereas they were negatively correlated with relative humidity (r -0.215, p 0.017). The daily concentra- tions of Alternaria and Cladosporium spore in 2009 were found to be positively correlated with the daily mean temperature (r 0.44, p< 0.001; r 0.44, p< 0.001), the minimum temperature (r 0.46, p< 0.001; r 0.44, p< 0.001) and the maximum temperature (r 0.40, p< 0.001; r 0.43, p< 0.001) whereas they were negatively correlated with the relative humidity (r -0.29, p< 0.001; r -0.37, p< 0.001), respectively. Conclusion: These findings showed that the concentrations of Alternaria and Cladosporium spores in Konya were affected from meteorological factors. Cladosporium spores were detected higher than Alternaria spores.

Giriş: Etkili tedavi rejimleriyle mortalitesi tüm dünyada azalmış olsa da tüberküloz halen global bir sağlık problemidir. Tedaviye uyumsuzluk, ilaç direnci, tedavi başarısızlığı ve relaps tüberküloz tedavisi sırasında sık karşılaşılan problemlerdir. İntermittan tedavi rejimlerinin, özellikle kaynakları sınırlı bölgelerde, tedavi maliyetini ve tedavinin yan etkilerini azalttığı, hastanın tedaviye uyumunu artırdığı bilinmektedir; ayrıca çocuk hastalarda da her gün verilen tedavi kadar etkili olduğu gösterilmiştir. Bu çalışmada, hastanemizde pulmoner ve ekstrapulmoner tüberküloz tanısı almış çocuk hastalara iki ilaçla verilen 6 ve 9 aylık intermittan tüberküloz tedavi rejimlerinin sonuçları karşılaştırılmıştır. Materyal ve Metod: Hastanemizde 1986 ile 2001 yılları arasında akciğer, lenf nodu, iskelet sistemi, plevral, abdominal, miliyer, perikardiyal, deri ve böbrek tüberkülozu nedeniyle intermittan antitüberküloz tedavi alan 115 hasta retrospektif olarak değerlendirilmiştir. Elli bir hastaya 15 gün süreyle izoniazid ve rifampisin tedavisi sonrası 9 ay süreyle aynı ilaçlarla ve aynı dozlarda haftada iki gün intermittan tedavi verilmiştir. Altmış dört hastaya ise aynı tedavi rejimi 6 ay süreyle uygulanmıştır. Bulgular: Tedavinin ilk ayında 1. gruptaki (9 aylık intermittan tedavi) pulmoner tüberkülozlu hastaların %75'inde, 2. gruptaki (6 aylık intermittan tedavi) pulmoner tüberkülozlu hastaların %79'unda klinik düzelme gözlendi. Pulmoner tüberkülozlu hastalarda ilk 6 ayda, grup 1'de %81'inde, grup 2'de %86'sında radyolojik düzelme görüldü. Klinik ve radyolojik düzelme zamanları karşılaştırıldığında iki grup arasında anlamlı bir farklılık saptanmadı (p 0.05). Benzer sonuçlar ekstrapulmoner tüberkülozlu hastalarda da gözlendi (p 0.05). Hastalar 7 ay ile 15 yıl arasında takip edildi. Hiçbir olguda erken relaps gözlenmedi. Dokuz aylık intermittan tedavi alan (grup 1) 4 hastada, tedavi kesildikten iki yıl sonra geç relaps görüldü. Sonuç:Çocukluk çağı pulmoner ve menenjit dışı ekstrapulmoner tüberkülozda, iki ilaçla 6 aylık intermittan tedavi, 9 aylık intermittan tedavi kadar etkilidir., Introduction: Tuberculosis is still a global health problem all over the world despite its mortality has been decreased with effective treatment regimens. Poor treatment adherence, acquired drug resistance, treatment failure and relapse are the major problems during the course of the tuberculosis treatment. Intermittent regimens have the advantages of reducing the side effects and the cost of the therapy and increasing the adherence, especially in resource-limited areas; and have been documented to be as effective as daily regimen in the paediatric population. In this study, we compared the results of 6-month and 9-month intermittent-therapy regimens with two drugs, given to the children with pulmonary and extrapulmonary tuberculosis at our hospital. Materials and Methods: One hundred and fifteen patients with pulmonary and extrapulmonary tuberculosis other than meningitis, who had been given intermittent anti-tuberculosis therapy between 1986 and 2001, were evaluated retrospectively. Fifty one patients were given isoniazid and rifampin daily for 15 days, followed by the same drugs and doses twice weekly for a total of 9-months. Also, 64 patients were treated with the same regimen for a total of 6-months. Results: Clinical recovery was observed in 75% and 79% of pulmonary tuberculosis patients at the first month of therapy in group 1 (9-month group) and group 2 (6-month group), respectively. Radiological recovery was noted between 0-6 months in 81% of the patients in group 1 and 86% of the patients in group 2. According to the clinical and radiological recovery times, no significant difference was detected between the two groups (p< 0.05). Similar results had been observed in extrapulmonary tuberculosis (p< 0.05). Follow-up periods ranged from 7 months to 15 years. There was no case of early relapse. Late relapse was noted in 4 patients, who had been received 9-month therapy (group 1). Conclusion: Six-month intermittent therapy with two drugs is as efficacious as 9-month intermittent-therapy in childhood pulmonary and extrapulmonary tuberculosis, other than meningitis.

Kistik fibrozis (KF), transmembran ileti regulator genindeki mutasyon sonucu oluşan ve otozomal resesif kalıtım gösteren bir hastalıktır. Kistik fibrozis transmembran regulator protein (KFTR) ter, sindirim sıvıları ve mukus bileşenlerinin düzenlenmesinde gereklidir. Hastalığın sıklığı beyaz ırkta 1/2500-1/3500 civarındadır. Kistik fibrozisin ülkemizdeki sıklığı ise bilinmemektedir. Kistik fibrozis çocukluk yaşlarında ortaya çıkan ve tüm ekzokrin bezlerin fonksiyon bozukluğu ile seyreden kalıtsal bir hastalıktır. Temel bozukluk ter, tükrük, solunum sistemi, kalın barsak, ürogenital sistem ve pankreas ekzokrin bezlerinden anormal sekresyonların oluşumudur. Önde gelen klinik belirtiler, kronik obstrüktif akciğer hastalığı ve pankreas yetersizliğine ait bulgulardır. Pseudomonas aeruginosa KF hastalarında kolonizasyona, kronik ve tekrarlayan enfeksiyonlara neden olan bir bakteridir. Bu yazıda kalıcı öksürüğünün sebebi olarak, Pseudomonas aeruginosa kolonizasyonuna bağlı kronik akciğer enfeksiyonunun akut alevlenmesi tespit edilen kistik fibrozis tanısı almış 10 yaşında bir kız çocuğu sunulmuştur., Cystic fibrosis (CF) is a disease which is caused by a mutation in the transmembrane conductance regulator gene, and displays autosomal recessive heredity. Cystic fibrosis transmembrane regulator protein (CFTR) is required to regulate the components of sweat, digestive fluids, and mucus. The prevalence of the disease is estimated as 1/2500-1/3500 in the white race. But the frequency of CF is not known for our country. Cystic fibrosis occurring in childhood, is a hereditary disease that proceeds with the dysfunction of all exocrine glands. The characteristic feature of the disease is the production of abnormal secretions in sweat, salivary, tracheobronchial, colon, urogenital system and pancreatic exocrine glands. The significant clinical signs of cystic fibrosis are chronic obstructive pulmonary disease and pancreatic insufficiency symptoms. Pseudomonas aeruginosa may lead to colonization, chronic and recurrent infections in patients with CF. In this article, a 10-year-old girl with CF having persistent cough related to acute exacerbation of chronic lung infection due to Pseudomonas aeruginosa colonization, has been presented.

Introduction: The present paper was aimed at indicating and discussing the possible problems related to inhaler devices by considering the knowledge and practices of the physicians regarding the inhalation therapies. Materials and Methods: The present study is a prospective, cross-sectional survey carried out by Turkish Respiratory Society Inhalation Therapy Group between February 2010 and February 2011 with a participation of ten individual centres. Seven inhaler devices that were available on the market in the country were assessed. The data on the problems that 684 clinicians actively attending patients with respiratory disorders experienced in daily clinical practice or their evaluations of their patients were obtained through the questionnaire. Results: The respondents, most of whom were pulmonologist (37.5%), and pediatrist (38.1%), had been, on average, 11.6 years in profession. The source of information on inhalers and administration techniques were reported to be mainly the internet and patient leaflets. Of the participants only 18.5% reported to have had adequate knowledge of inhaler devices and proper administration techniques. Most of the participants stated that they themselves provided the instructions of administration and that the method was often verbal explanation. The physicians believed that although approximately 60% of the patients used the drug correctly, 40.7% made critical mistakes to have adverse effects on the therapeutic outcome. The most important criteria on which the physians lay greater emphasis in choosing the inhaler devices were the physical capability, skills and age of the patients. Conclusion: The awareness of proper use of inhaler devices is a fundamental prerequisite for effective inhalation therapy has been improved in physicians. The results of the present study have shown that more effort is required for professional training. Assisting the physicans with medical personnel for training of the patients and educational motivation are required., Giriş: Bu yazıda, hekimlerin inhaler tedavi konusundaki bilgi düzeyleri ve günlük pratiklerini göz önünde bulundurarak bu tedavilerle ilgili olası problemlerin değerlendirilmesi ve tartışılması amaçlandı. Materyal ve Metod: Bu çalışma, Türkiye Solunum Araştırmaları Derneği (TUSAD) İnhalasyon Tedavileri Çalışma Grubu (INTEDA) tarafından Şubat 2010-Şubat 2011 tarihleri arasında 10 ayrı merkezin katılımıyla yürütülen prospektif, kesitsel bir anket çalışmasıdır. Bu çalışmada, ülkemizde çalışmanın yapıldığı dönemde piyasada bulunan yedi ayrı cihaz değerlendirildi. Aktif olarak solunum yolu hastalarıyla ilgilenen 684 hekimin günlük pratikte karşılaştıkları sorunlar ya da hastaları ile ilgili görüşleri anket yoluyla elde edildi. Bulgular: Çoğunluğunu göğüs hastalıkları uzmanları (%37.5) ve pediatristlerin (%38.1) oluşturduğu katılımcılar ortalama 11.6 7.9 yıldır hekimlik yapıyordu. Hekimlerin inhalasyon cihazları ve uygulama teknikleri konusunda bilgi kaynağı başlıca internet ve ilaç broşürleri olarak bildirilmişti. Katılımcıların yalnızca %18.5'i inhaler cihazlarla ilgili bilgilerini yeterli buluyordu ve uygulama tekniklerini yeterince biliyordu. Katılımcıların çoğunluğu ilaç uygulaması konusunda hastalarına kendilerinin bilgi verdiğini ve bu eğitimin sıklıkla sözlü talimatlar şeklinde olduğunu bildirdiler. Hekimler, hastaların yaklaşık %60'ının ilacını doğru kullandığını düşünüyorken, %40.7'sinin tedavi sonuçlarını olumsuz olarak etkileyebilecek kritik hatalar yaptığına inanıyordu. Hekimlerin inhaler cihaz seçiminde göz önünde bulundurduğu en önemli kriterler, fiziksel yeterliliği, cihaz kullanma becerisi ve hastanın yaşı olarak bildirildi. Sonuç: Etkin inhaler için temel ön koşul olan inhaler cihazların uygun kullanımına ilişkin hekimlerin farkındalığı artmıştır. Bu çalışmanın sonuçları profesyonel bir hasta eğitimi için daha fazla girişimin gerekli olduğunu gösterdi. Hasta eğitiminde hekimlere yardımcı sağlık çalışanlarının destek olması ve hasta eğitimi için hekimleri motive edecek girişimlere ihtiyaç vardır.

ÖZET Giriş: Konjenital lober amfizem (KLA), süt çocuğunda solunum yetmezliğine yol açan nadir akciğer gelişim malformasyonlarındandır. Hastalar, sıklıkla yaşamın ilk 6 ayında tekrarlayan solunum sıkıntısıyla başvururlar. Yabancı cisim aspirasyonu (YCA), çocukluk çağında morbidite ve mortalitenin önemli bir sebebidir. Tekrarlayan alt solunum yolu infeksiyonu hikayesi olan ve tedaviye cevap vermeyen hastalarda YCA akla gelmelidir. Olgu Sunumu: KLAi taklit eden uzun süreli solunum şikayeti olan ve radyolojik bulguları 11 aydır havalanma farkı şeklinde sebat eden 16 aylık YCA olgusu sunuldu. Tartışma: Yabancı cismin akut ve kronik dönemdeki radyolojik bulgusu her iki akciğer arasında havalanma farkı olabilir ve bu bulgu küçük çocuklarda bazen KLAi düşündürebilir. (Gün cel Pediatri 2012; 10: 70-3), SUM MARY Introduction: Congenital lobar emphysema (CLE) is a rare malformation of lung development which may be the cause of respiratory insufficiency of the suckling child. Patients often present within the first 6 months of life with recurrent respiratory distress. Foreign body aspiration (FBA) is an important cause of morbidity and mortality in childhood. Foreign body aspiration should be thought in patients who have a history of recurrent lower respiratory infections and resistance to treatment. Case Report: We report a case of 16 month old girl with FBA mimicking CLE. She was suffering for persistan respiratory symptoms and radiological finding was maintaned as the difference between two sides of the lungs aeration for 11 months. Conclusion: In the acute and chronic period of FBA, the radiological finding can be different between two sides of the lungs aeration and this finding sometimes might suggest CLE in infancy. Loeys-Dietz syndrome is a rare autosomal dominant disorder characterized by the involvement of cardiovascular, craniofacial and skeletal systems. The main etiology of the disease is the mutation in the transforming growth factor betareceptor type 1 and 2 genes. Today, Loeys-Dietz syndrome has been classified into two subtypes due to the presence of craniofacial involvement. These patients have progressive aortic enlargement which increases the risk of dissection and rupture. So, delay in the diagnosis may be associated with poor prognosis. We present a new case with thymoma diagnosed as Loeys-Dietz syndrome type 1. Also, this is the first report of a tumor in Loeys-Dietz syndrome according to the current literature review., Loeys-Dietz sendromu kardiovasküler, kraniofasiyal ve iskelet sisteminin tutulduğu nadir bir otozomal dominant hastalıktır. Esas etyoloji, transforming growth faktör beta-reseptör tip 1 ve 2 genlerindeki mutasyonlardır. Günümüzde, Loeys-Dietz sendromu kraniofasiyal bulguların varlığına göre iki alt gruba ayrılmıştır. Aort genişlemesi olan hastalar diseksiyon ve rüptür açısından risk taşırlar. Sonuçta, tanıdaki gecikme kötü prognoz ile sonuçlanabilir. Timomanın eşlik ettiği ve Loeys- Dietz tip 1 sendromu tanısı konulan yeni bir vakayı sunduk. Ayrıca, vaka sunumu literatürde bir tümörün eşlik ettiği ilk Loeys-Dietz sendromudur.
