Abstract

Objectives. We examined the major causes of and risk factors for death among migrants who died while making unauthorized border crossings into the United States from Mexico. Methods. Decedents were included in the study if (1) their remains were found between January 1, 2002, and December 31, 2003, in any US county along the 650-mi (1040-km) section of the US-Mexican border from Yuma, Ariz, to El Paso, Tex; (2) their immigration status was unauthorized; and (3) they were believed to have died during transit from Mexico to the United States. Characteristics of the decedents and causes of and risk factors for their deaths were examined. Results. Among the 409 decedents meeting our inclusion criteria, environmental heat exposure (n = 250; 61.1%) was the leading cause of death, followed by vehicle crashes (n = 33; 8.1%) and drownings (n = 24; 5.9%). Male decedents (n = 298; 72.8%) outnumbered female decedents (n = 105; 25.6%) nearly 3 to 1. More than half of the decedents were known to be Mexican nationals (n = 235; 57.5%) and were aged 20 to 39 years (n = 213; 52.0%); the nationality of 148 (36.2%) decedents was undetermined. Conclusions. Deaths among migrants making unauthorized crossings of the US-Mexican border are due to causes that are largely preventable. Prevention strategies should target young Mexican men, and focus on preventing them from conceiving plans to cross the border, discouraging them from using dangerous routes as crossing points, and providing search-and-rescue teams to locate lost or injured migrant crossers.

25. Pedestrian and hypothermia deaths among Native Americans in New Mexico: between bar and home

Abstract

Native Americans who travel long distances on foot to obtain alcohol may be at risk for accidental death from in pedestrian-motor vehicle crashes or hypothermia. A study of New Mexico residents who died from accidental injuries over a nine-year period found that Native Americans were eight times more likely to die in pedestrian-motor vehicle crashes, and 30 times more likely to die from hypothermia than other residents. Of 1,585 Native Americans who died from accidental injuries, 347 died from pedestrian-motor vehicle crashes, and 166 died from hypothermia. Ninety percent of Native Americans who died in pedestrian-motor vehicle crashes or from hypothermia and were tested for blood alcohol levels were highly intoxicated. Sixty-seven percent of pedestrian or hypothermic deaths occurred outside the reservation. The sale of alcohol is illegal on many reservations so Native Americans must travel long distances to buy alcohol.

Abstract

Multiple primary tumors (MPTs) affect a substantial proportion of cancer survivors and can result from various causes, including inherited predisposition. Currently, germline genetic testing of MPT-affected individuals for variants in cancer-predisposition genes (CPGs) is mostly targeted by tumor type. We ascertained pre-assessed MPT individuals (with at least two primary tumors by age 60 years or at least three by 70 years) from genetics centers and performed whole-genome sequencing (WGS) on 460 individuals from 440 families. Despite previous negative genetic assessment and molecular investigations, pathogenic variants in moderate- and high-risk CPGs were detected in 67/440 (15.2%) probands. WGS detected variants that would not be (or were not) detected by targeted resequencing strategies, including low-frequency structural variants (6/440 [1.4%] probands). In most individuals with a germline variant assessed as pathogenic or likely pathogenic (P/LP), at least one of their tumor types was characteristic of variants in the relevant CPG. However, in 29 probands (42.2% of those with a P/LP variant), the tumor phenotype appeared discordant. The frequency of individuals with truncating or splice-site CPG variants and at least one discordant tumor type was significantly higher than in a control population (χ2 = 43.642; p ≤ 0.0001). 2/67 (3%) probands with P/LP variants had evidence of multiple inherited neoplasia allele syndrome (MINAS) with deleterious variants in two CPGs. Together with variant detection rates from a previous series of similarly ascertained MPT-affected individuals, the present results suggest that first-line comprehensive CPG analysis in an MPT cohort referred to clinical genetics services would detect a deleterious variant in about a third of individuals.

35. Eosinophilia-myalgia syndrome: a clinical case series of 21 patients

Abstract

The first case of an illness that would later be called eosinophilia-myalgia syndrome (EMS) was reported in 1988. Many more cases were reported during 1989, and the disorder was linked to the consumption of the supplement L-tryptophan. Twenty-one cases of EMS, which occurred in New Mexico between May 1 and November 17, 1989, were recently reviewed. Of these, 20 cases involved women and 20 involved the use of products containing L-tryptophan, which were usually taken for insomnia. All the patients with EMS developed intense myalgia (muscle pain), and over 80 percent developed fever, weakness, and headache. Three-quarters of the patients also developed some type of respiratory problem, including shortness of breath; one patient had to be put on a ventilator. All had eosinophilia, or increased blood levels of eosinophils, a type of white blood cell. Muscle biopsies from four patients revealed perimyositis, or inflammation of the connective tissue around the muscle. Most of the EMS patients who had blood tests for liver function had abnormal results. Physicians advised most of the patients to discontinue L-tryptophan. Half the patients had been given prednisone, a corticosteroid; of six patients so treated, five reported decreased muscle pain, and eosinophil counts returned to normal in four. The remaining patients received a variety of treatments, including analgesics (both narcotic and nonnarcotic pain killers), nonsteroidal anti-inflammatory drugs (NSAIDS), calcium channel blockers, diuretics, and antibiotics. However, the effectiveness of these treatments could not be determined. It is likely that patients with milder forms of EMS were excluded from the study because of the strict criteria used to select patients. EMS resembles toxic oil syndrome (TOS), an illness reported in Spain in 1981, which was linked to the ingestion of contaminated cooking oil. Many TOS patients went on to develop chronic illness; it is possible that this could occur with EMS. More experience with EMS will be needed to determine the appropriate treatment. Abstract

To determine the prevalence of firearm ownership and storage practices in New Mexico, we did a random-digit-dialing survey of New Mexico residents in October 1991. Of 200 households surveyed, 79 (40%) had 1 or more firearms in the home. Rural households were more likely than urban households to have firearms (44% versus 30%), and households with annual incomes of greater than $25,000 were more likely to have a firearm than households with incomes of $25,000 or less (41% versus 33%).
