- Abstract
Background and Design.- In chronic renal insufficiency, many toxic metabolites which are normally nontoxic but become toxic when combined improving oxidative metabolism, accumulates in the body fluids. We studied the effect of the most frequently used cuprophan and polysulfone dialysers in hemodialysis treatment on oxidant/antioxidant status. For this purpose, 26 cases (9 females, 17 males, mean age of 36) that are used cuprophan dialysers (n=15) and polysulfone dialysers (n=ll) were examined. The cases with COPD, DM and the ones using vitamin E and C, were excluded. First the plasma and red blood cells of the venous blood taken before and after hemodialyses were separated and than plasma MDA and intraerythrocyter GSH levels, activities of GSH-Px and SOD were measured.Results.- When the groups that are used cuprophan and polisulfone dialysers are compared; It is observed that the cuprophan dialysers increased plasma MDA levels (p0.05).Conclusion.- Hemodialysis treatment is an oxidant stress for the organism and some of used dialysers is effective on this oxidant stress. - Abstract
Background and Design.- The harmful effects of acute and chronic iron loading are well documented. Increased lipid peroxidation is an important expression of both chronic and acute iron toxicity. Our aim was to study the effect of iron owr1oading on the different tissue lewls of iron.Results.- 14 adult male Wistar albino rats weighing 200-250g were used. The animals were dhlided into 2 weight-matched groups. Group 1: Control group (n=7) and Group 2: Iron owr1oading group (n=7). The rats in group 2 receiwd intraperitoneal iron (Ferro Ill hydroxide polymaltose) at a dose of 250 mg/kg/day for 10 days. At the end of theadministration period, the rats were anesthetized by ketamine (50 mg/kg). Heparinized blood samples were obtained \Jia aorta abdominalis. The samples of the tissue from liwr, heart, spleen, brain and skeletal muscle were obtained. The plasmaand the tissue lewls of iron were measured by atomic absorbtion spectrophotometer. 5. VİTAMİN E UYGULANAN HİPERTİROİDİLİ SIÇANLARDA PANKREAS DOKUSUNUN İNCELENMESİ

- Abstract
Background.- In our former experiments we observed that oxidant stress seen in hyperthyroidic conditions was reduced by vitamin E administration. In this study, examination of the pancreatic tissue was planed.Design.- Female, Wistar albino rats weighting 180-200 g. were used in groups; 1st group: Control (n=8), 2nd group: Vitamin E administered (n=9), 3rd group: Hyperthyroidic group (n=10) and 4th group: Hyperthyroidic and Vitamin E administered group (n=6). Hyperthyroidism was formed by giving L-tiroxin (0.4 mg/ 100 g food) to rats during 24 days. Vitamin E (500 mg/ kg-i.p.) is administrated in 1, 4, 7, 11, 14, 18, 21, 24 days from the beginning of the experiment. At the end of this period blood samples taken from the heart and the whole pancreatic tissue of the rats which are sacrificed under ether anesthesia. T3, T4, TSH levels were measured in the blood samples by RIA method. Pancreatic tissue was prepared for routine histologic examination.Results and Conclusion.- Insulin levels (μIU/ ml) are 8.45±5.18 in 1st group, 4.78±3.46 in 2nd group, 12.87±5.81 in 3 rd group, 3.63±2.42 in 4th group. In histologic examination of pancreatic tissues, exocrin pancreatic tissue and Langerhans islets of 1st and 3rd groups showed the same morphology. In the 4th group exocrin paranchyma destruction (acinar athrophy), lymphocyte infiltrasyon, fibrosis and fat accumulation were observed. Langerhans islets were lost or became smaller in the areas which the exocrine paranchyma was destructed. In the islets aldehide fuchcine (+) stained beta cells were seen. 2nd and 4th groups showed the same morphologic changes. Nabız dalga hızı (NDH), periferik arter dalga formlarından yararlanılarak geniş arterlerin elastisitesini değerlendirmede kullanılan bir tekniktir. Bu çalışmada perindoprilin primer hipertansif olgularda arteriyel duvar sertliğinin bir indeksi olan NDH ile nabız dalga hızı ilerleme zamanına (NDHİZ) etkisi incelenmiştir. Çalışmaya primer hipertansiyon tanısı almış 24 (17 kadın, 7 erkek) hasta dahil edildi. Hastalara perindopril 4 - 8 mg tedavisi başlandı; tedavi 10 haftaya tamamlandı. NDH, karotis ve femoral arterler kullanılarak, online nabız dalga kaydına ve NDH'nın otomatik hesaplanmasına imkan veren Complior Colson cihazı (Createch Industrie, Fransa) ile hesaplanmıştır. On haftalık perindopril tedavisini takiben sistolik kan basıncı, diyastolik kan basıncı ve NDH değerlerinde istatistiksel olarak anlamlı azalma olurken; NDHİZ'nda anlamlı uzama saptanmıştır (sırası ile p ≤ 0.001, < 0.001, 0.003, 0.01). Tedavi öncesi ve sonrasında kalp hızında anlamlı değişiklik olmamıştır (p = 0.50). Sonuç olarak; esansiyel hipertansiyonlu hastalarda perindopril tedavisi ile kan basıncı ve karotis-femoral NDH değerlerinde azalma, NDHİZ'da uzama tesbit edilmiştir.

Chronic renal failure in a 47-year-old woman led to intense visceral calcification, mainly in the lungs, heart, and stomach. Changes secondary to multiple organ calcification were seen on whole-body bone scintigraphy. This is a rare finding in patients with end-stage renal failure.

Amaç: Hepatit C enfeksiyonu olan hastalarda, kronik enflamasyona bağlı olarak hepatosellüler depolardan ferritin salınmakta ve bu hepatosellüler hasarın bağımsız bir göstergesi olarak kabul edilmektedir. Serum eritrosit ferritin seviyelerinin, serum ferritin düzeyleri gibi, karaciğer hasarını göstermede aminotransferazlar ile birlikte kullanılıp kullanılamayacağı bilinmemektedir. Çalışmamızda amacımız, anti-HCV pozitif hemodiyaliz hastalarında, karaciğer hasarının göstergesi olan aminotransferaz enzim düzeyleri ile eritrosit ve serum ferritin düzeyleri arasındaki ilişkiyi ortaya koymaktır. Gereç ve yöntem: Beş farklı diyaliz merkezinde diyalize giren, aktif enfeksiyonu, malignitesi ve polisitemisi bulunmayan, kan transfüzyonu yapılmamış, anti-HCV’si pozitif 40 hastada eritrosit ferritin, serum ferritin, CRP, HBsAg, anti-HİV, anti-HCV, ALT (alaninaminotransferaz ) ve AST (aspartat aminotransferaz) düzeylerine bakıldı. SPSS (ver. 12) istatistik programı kullanılarak sonuçlar değerlendirildi. Bulgular: Hastaların 17’ si erkek 23’ü kadın olup yaş ortalamaları 54±16 yıl, ALT 25 ±19 IU/L, 20±12 IU/L, eritrosit ferritin değerleri 1,8 ± 1,72 ng/gHb, serum ferritin değerleri 992±424 ng/ml olarak bulundu. Yapılan değerlendirmede, serum aminotransferaz değerleri ile serum ferritin değerleri arasında istatistiksel olarak anlamlı pozitif korelasyon saptandı (p < 0,05), ancak eritrosit ferritin ile aminotransferazlar düzeyleri arasında istatistiksel olarak anlamlı pozitif korelasyon saptanmadı (p > 0,05). Sonuç:Anti-HCV pozitif hastalarda hepatosellüler depolardan salınan serum ferritin seviyeleri aminotransferaz enzimleri ile birlikte karaciğer hasarının göstergelerinden biridir. Yaptığımız çalışmada, aminotransferaz seviyeleri ile serum ferritin düzeyleri arasında anlamlı korelasyon saptandı, ancak eritrosit ferritin düzeyleri ile aminotransferaz seviyeleri arasında anlamlı bir korelasyon saptanamamıştır. Bulgular HCV pozitif hemodiyaliz hastalarında, eritrosit ferritin seviyelerinin karaciğer hasarını göstermede serum ferritin gibi iyi bir belirteç olarak kullanılamayacağını düşündürmektedir.

In a 40-year-old man, three-phase bone scintigraphy (TPBS) revealed a periaortic hypervascular mass, delayed abdominal aorta transit time and bilateral hydronephrosis in angiogram and blood pool phases of TPBS. Abdominal ultrasonography and computed tomography showed retroperitoneal mass; right ureter was not visualized. A biopsy of this mass showed histopathologic findings of retroperitoneal fibrosis. A percutaneous right nephrostomy was performed and steroid therapy was initiated. In this case, early incidental diagnosis with TPBS provided excellent renal and patient outcome. © 2003 Elsevier Ireland Ltd. All rights reserved.

Background: We investigated the effects of Botox-A on weight loss and gastric emptying in an experimental obese rat model. Although there is evidence of weight loss in normal-weight rats after Botox-A injection, there are no studies indicating the effect of Botox-A injection on weight loss and gastric emptying time in obese rats. Methods: 37 female Wistar Albino rats were given high calorie diet for 90 days. They were separated into 3 groups. The first group (Botox group) consisted of 15 obese rats whose gastric antrum was injected with 20 U of Botulinum Toxin Type A. The second group (Saline group) consisted of 15 obese rats whose gastric antrum was injected with 20 U of saline. The third group (Control group) had no surgical intervention. Gastric scintigraphy was performed in the 3 groups pre- and postoperatively. Results: The saline group had a weight reduction in the early postoperative days but began to gain weight thereafter. ÖZET GÖRSEL UYARILMIŞ YANITLARDA LABORATUAR NORMLARININ OLUŞUMU Beynin, çevresinde olup bitenlerin farkında olması yönünde en değerli ve en vazgeçilmez karmaşık duyu şüphesiz görme duyuşudur. Görme oluşumundaki bu karmaşık sistemle orantılı olarak, çok çeşitli hastalık gurupları ve çözülmesi gereken görme ile ilişkin problemler de tanısal alanda, bilimsel ilerlemeye eşit hızda gelişen yeni metotların klinik kullanıma sunulması ihtiyacını doğurmuştur. Teknolojideki baş döndürücü ilerlemeler her gün bu amaca hizmet edecek yeni bir cihazı araştırmacıların kullanımına sunmaktadır. Bu gelişmelere paralel olarak, yeni geliştirilen ve bilgisayarlı uyarılmış potansiyel kayıt ve analiz sistemleri bu amaçla denenmektedir. Biz bu çalışmada, görme yollarını ilgilendiren patolojilerde görsel uyarılmış potansiyel çalışmalarının rolünü ve bu amaçla kullanılan bilgisayarlı sistemlerin laboratuar ortamında kullanım şekillerini araştırmaya ve İ.Ü. Cerrahpaşa Tıp Fakültesi Fizyoloji Anabilim dalında bu tanı metodunu kullanılabilir hale getirmeye çalıştık Elektroretinogram denen ve başlıca retinada bulunan fotoreseptör hücrelerinin fonksiyonlarını yansıtan yöntemle birlikte, görsel uyarılmış potansiyeller, klinik olarak bulgu vermemiş bile olsa birçok hastalık formunda tanı koydurmaktadır. Retrobulber nörit, papillit, iskemik optik nöropati, toksik ve metabolik optik nöropatiler, optik sinir basısı ve optik atrofı gibi klinik durumlarla Parkinson, Alzheimer ve kortikal körlük gibi hastalıklarda VEP ölçümleri tanıya yardımcıdır. Hasta yönünden hiçbir girişimsel ve rahatsızlık verici yönü olmamasına rağmen diğer son derece girişimsel ve uygulaması zor tetkiklerin çok daha geç dönemde ipucu verdiği birçok klinik durumu çok daha erken dönemde yakalamamıza imkan veren bu yöntemin yaygınlaşması ile kısa zaman içerisinde faydalı olduğu tüm hastalıklarda ilk tercih tanı metodu olarak kullanılmaya başlanması çok uygun olacaktır. 29 SUMMARY SETTLEMENT OF LABORATORY STAND ARTS FOR VISUAL EVOKED POTENTIALS The most significant and precious complex sense for the brain to be aware of enviroment is certainly vision. Proportionally to this complexity, there are many groups of diseases and problems to be solved which require new methods in field of diagnosis developing with the same rate of improvements of science. Dazzling advances in technology offers everyday a new method for researchers. Parallel to all those improvements, recently developed computerised visual evoked potentials recording and analysing systems are being used for this purpose. In this study, we tried to explore the role of visual evoked potentials in pathologies regarding to visual pathways and usage of computerised systems controlling this system in laboratory enviroment for using of this method in Istanbul University Cerrahpaşa medical Fakulty department of Physiology. With a method called electro retinograms, which is basicly projecting the functions of photoreceptor cells in retina, visual evoked potentials can help to diagnose many subclinical disease forms. VEP recordings are helpful in diagnosis of retrobulber neuritis, papillitis, ischemic optic neuropathies, toxic and metabolical optic neuropathies, optic nerve compression, optical atrophy and diseases of parkinsonism, Alzheimer and Cortical blindness. Even it is not an invasive and disturbing method for the patient, this method can help us to seize many clinical conditions earlier which are normally diagnosed much more lately with other diffucult and invasive methods. Engineering - Abstract
The chemical composition of the fatty acids methyl esters (FAMEs) and other lipids in leaves and roots of Crocus vallicola were analyzed by gas chromatography-mass spectrometry (GC-MS). In this work, twenty-eight compounds, including 22 FAMEs, 1 aldehyde, 3 hydrocarbons (substitute alkane and alkene), 2 alcohols in the leaves and twenty-one compounds (17 FAMEs, 1 anhydride, I substitute alcohol, 1 ketone, 1 substitute amide) in the roots were identified by GC-TVS from C. vallicola. The FAMEs in leaves and roots were highly similar; only the amounts of the esters were different.

- Abstract
Background: To investigate the predictable parameters associated with downgrading in patients with a Gleason score (GS) 8 (4+4) in prostate biopsy after radical prostatectomy., Methods: We retrospectively analyzed 62 patients with a GS of 4+4 on prostate biopsy who underwent robotic radical prostatectomy between 2017 and 2022., Results: 38 of 62 (61.2%) were downgraded. In multivariable logistic regression model, Ga-68 prostate-specific membrane antigen (PSMA) positron-emission tomography (PET)/computed tomography (CT) SUV max was independent predictor of downgrading (OR 0.904; p = 0.011) and a Logistic Regression model was constructed using the following formula: Y = 1.465-0.95 (PSMA PET/CT SUV max). The model using this variable correctly predicted the downgrading in 72.6% of patients. The AUC for PSMA PET/CT SUV max was 0.709 the cut off being 8.8. A subgroup analysis was performed in 37 patients who had no other European Association of Urology (EAU) high risk features. 25 out of 37 (67.5%) were downgraded, and 21 of these 25 had organ confined disease. Low PSMA SUV max (<8.1) and percentage of GS 4+4 biopsy cores to cancer bearing cores (45.0%) were independently associated with downgrading to GS 7., Conclusion: PSMA PET/CT can be used to predict downgrading in patients with GS 4+4 PCa. Patients with GS 4+4 disease, but no other EAU high risk features, low percentage of GS 4+4 biopsy cores to cancer bearing cores, and a low PSMA PET/CT SUV max are associated with a high likelihood of the cancer reclassification to intermediate risk group., (© 2024. Introduction and Objectives: To investigate the additive role of prostate-specific membrane antigen (PSMA) positron emission tomography/computed tomography (PET/CT) independent from multiparametric magnetic resonance imaging (mpMRI) and clinical-pathological parameters to predict pathological upgrading in patients with ISUP grade group (GG) 1 prostate cancer (PCa) at prostate biopsy., Materials and Methods: A total of 41 patients who underwent robotic radical prostatectomy (RP) for GG1 disease at prostate biopsy with preoperative PSMA PET/CT and mpMRI images available for central review were included in the study. Univariate and multivariate logistic regression analyses were performed to determine the independent predictors of pathological upgrading (GG ≥ 2)., Results: Final RP pathology revealed upgrading in 26 patients (65.9%); to GG 2 disease in 25 cases and GG 4 disease in one case. International Society of Urological Pathology (ISUP) upgrading rates for prostate imaging-reporting and data system (PIRADS)-5, PIRADS-4, and PIRADS ≤ 3 lesions were 78%, 74%, and 38%, respectively. Fourteen out of 15 (93.3%) patients with an SUVmax ≥ 5.6 and all patients with an SUVmax ≥ 6.5 (n = 10) had pathological upgrading. The upgrading rate in patients with SUV < 5.6 was 46.2% (12/26). Intraprostatic SUVmax ≥ 5.6 was found as the only independent predictor of pathological upgrading in multivariate analysis., Conclusion: High prostatic PSMA uptake was found to be a very reliable predictor of pathological upgrading, but low PSMA uptake cannot exclude pathological upgrading. Intraprostatic PSMA uptake along with previously known mpMRI and biopsy-related parameters should be considered when making a treatment decision in patients with GG1 PCa at prostate biopsy., (© 2023 Wiley Periodicals LLC.)

19. Radioligand Therapy With 177 Lu-PSMA-I&T in Patients With Metastatic Prostate Cancer : Oncological Outcomes and Toxicity Profile.

Introduction: This study aimed to investigate the oncological outcomes and toxicity profile of 177 Lu-PSMA-I&T radioligand therapy (RLT) in patients with metastatic castration-resistant prostate cancer (mCRPC), as well as our initial experience in metastatic hormone-sensitive prostate cancer (mHSPC)., Patients and Methods: A total of 38 consecutive patients with metastatic prostate cancer (33 mCRPC and 5 mHSPC) received 177 Lu-PSMA-I&T RLT, with a median of 2 cycles per patient (range, 1-7). Response to RLT was evaluated based on prostate-specific antigen (PSA) changes and imaging response. Clinical progression-free survival and overall survival were used to report oncological outcomes. Toxicity was assessed using the Common Toxicity Criteria for Adverse Events criteria., Results: In mCRPC, 22 (69%), 18 (56%), and 11 (34%) patients achieved any PSA decline, PSA response of ≥30%, and PSA response of ≥50%, respectively. The clinical progression-free survival and overall survival after the first cycle of RLT were 6.3 and 21.4 months, respectively. In mHSPC, 177 Lu-PSMA-I&T RLT resulted in excellent PSA response (93.0%-99.9%) in all cases. Clinical progression and cancer-related mortality occurred in only 1 case. Toxicity profile was favorable in both mHSPC and mCRPC., Conclusions: 177 Lu-PSMA-I&T RLT demonstrated favorable PSA response (≥30%) in over half of the patients with mCRPC and excellent PSA response in all patients with mHSPC. Toxicity profile was favorable in both mHSPC and mCRPC settings. 20. Intraoperative Frozen Section via Neurosafe During Robotic Radical Prostatectomy in the Era of Preoperative Risk Stratifications and Primary Staging With mpMRI and PSMA-PET CT: Is There a Perfect Candidate?

Background: We aimed to analyze the effect of preoperative risk assessment including Ga-68 PSMA PET and multiparametric magnetic resonance imaging (mpMRI) on nerve sparing practices, positive surgical margin (PSM) rates and oncological outcomes based on a comparison between patients underwent RARP with and without Neurosafe (NS)., Methods: Patients underwent RARP with NS (RARP-NS) or without (RARP-only) NS retrospectively evaluated. Suspicion for extracapsular extension on mpMRI and/or Ga-68 PSMA PET was recorded as i(imaging)T3. NS was performed according to the Martini-Klinik technique. PSM at preserved bundle side were called PSM at region of interest (ROI) while the others were elsewhere., Results: A total of 208 patients (90 in RARP-NS, 118 in RARP-only groups) were included. Preoperatively the RARP-only group showed significantly higher mean PSA (p = .01) and PIRADS 5 (p = .002) findings and had more D'Amico high risk (DAHR) patients (p = .08). The overall PSM rates for pT2 versus pT3 disease were 7.5% versus 21.6 and 15.6% versus 55% in RARP-NS and RARP-only groups, respectively. NS resulted in more bilaterally preserved bundles (81.1% vs. 66.3%) and less PSM at the ROI (3.3% vs. 23.4%) than RARP-only group. NS outperformed RARP-only in all clinical settings had its highest differential benefit in more bilateral nerve sparing and less PSM at ROI in patients with both DAHR and iT3 disease. BCR rates were 2.2% and 2.5% for RARP-NS and RARP only groups, respectively (p = .4). One patient in RARP-NS and 9 in RARP-only groups had PSA persistence (p = .02)., Conclusion: RARP-NS led to more preserved bundles with less PSM. Background: Treatment response is traditionally monitored using prostate-specific antigen (PSA) and conventional imaging in patients with metastatic prostate cancer (mPCa)., Objective: To assess the diagnostic performance of prostate-specific membrane antigen (PSMA) positron emission tomography (PET)/computed tomography (CT) when monitoring mPCa patients receiving systemic treatment and also to investigate the concordance between PSMA PET response according to the PSMA PET progression (PPP) criteria and biochemical response., Design, Setting, and Participants: A total of 96 patients with
68 Ga-PSMA-11 PET/CT-detected mPCa at baseline PSMA PET/CT (bPSMA) who underwent at least one follow-up scan after receiving systemic treatment were included in the study. PSA levels at bPSMA and follow-up PSMA PET (fPSMA) scans were recorded. The PPP criteria were used to define PSMA progression. Biochemical progression was defined as ≥25% increase in PSA. PSMA PET and PSA responses were dichotomized into progressive disease (PD) versus non-PD, and the concordance between PSA and PSMA responses was evaluated., Outcome Measurements and Statistical Analysis: The concordance between PSA and PSMA PET responses was presented using frequencies, percentages, and Cohen's kappa test., Results and Limitations: A total of 345 serial PSMA PET/CT (96 bPSMA and 249 fPSMA) scans were evaluated. The positivity rates of PSMA PET scans for PSA levels of <0.01, 0.01-0.2, 0.2-4, and >4 ng/ml were 55.6%, 75.0%, 100%, and 98.8%, respectively. PSA and PSMA responses showed moderate-to-high concordance (Cohen's κ = 0.623, p < 0.001). PSA-PSMA discordance was detected in 39 scans (17%). The most common cause of discordance was the discordant results between different metastatic lesions (16/28, 57.1%) in patients with PPP without PSA progression and local progression in prostate (n = 7/11, 63.6%) in patients with PSA progression without PPP., Conclusions: PSMA PET/CT showed very high detection rates of malignant lesions even at very low PSA values and showed significant concordance with PSA response when monitoring treatment response in patients receiving systemic treatment for mPCa., Patient Summary: This study describes that prostate-specific membrane antigen positron emission tomography (PSMA PET), a new sensitive imaging tool, can detect malignant lesions even at very low prostate-specific antigen values when monitoring metastatic prostate cancer. - Abstract
Background: Inflammation is known to play a crucial role in many diseases, including COVID-19., Objective: Using flow-mediated dilatation (FMD), we aimed to assess the effects of inflammation on endothelial function in COVID-19 patients., Methods: This study was conducted with a total of 161 subjects, of whom 80 were diagnosed with COVID-19 within the last six months (comprising 48 women and 32 men with a mean age of 32.10 ± 5.87 years) and 81 were healthy controls (comprising 45 women and 36 men with a mean age of 30.51 ± 7.33 years). We analyzed the findings of transthoracic echocardiography and FMD in all subjects. All results were considered statistically significant at the level of p < 0.05., Results: The echocardiography and FMD of the COVID-19 group were performed 35 days (range: 25-178) after diagnosis. There was no statistically significant difference in echocardiographic parameters. Differently, FMD (%) was significantly higher in the control group (9.52 ± 5.98 vs. 12.01 ± 6.18, p=0.01). In multivariate analysis with the forward stepwise model, FMD was significantly different in the control group compared to the COVID-19 group (1.086 (1.026 - 1.149), p=0.04). A Spearman's correlation test indicated that FMD (r=0.27, p=0.006) had a weak positive correlation with the presence of COVID-19., Conclusion: Our findings point to COVID-19-induced endothelial dysfunction, as assessed by FMD, in the early recovery phase.

Objective: Lymph node metastasis is the most important factor both in the selection of treatment since many alternatives have been created in recent years, and in the evaluation of prognosis in lung cancer. The most unpredictable cause of lymph node false positivity in fluorine-18-fluorodeoxyglucose (
18 F-FDG) positron emission tomography/computed tomography (PET/CT) is anthracosis. The aim of this study is to compare18 F-FDG PET/CT texture information of anthracotic (ALN) and metastatic (MLN) lymph nodes, after re-evaluation of the cytological samples obtained from anthracotic lymph nodes by EBUS-TBNA., Subjects and Methods: Ninety nine patients, 78 of whom had primary lung cancer were included in the study. Two hundred and three lymph nodes from 99 patients sampled by EBUS-TBNA and diagnosed cytologically as ALN or MLN were evaluated retrospectively. All ALN were classified as grades 1, 2 and 3 cytologically. Volume of interest (VOI) of 203 lymph nodes was re-drawn and maximum standardized uptake value (SUVmax), metabolic tumor volume (MTV) and total lesion glycolysis (TLG) values were recorded., Results: There was a statistically significant difference in MTV and TLG values in MLN and all ALN grades. However, only grade 1-2 ALNs could be differentiated from MLNs with SUVmax, and no statistically significant difference was found in grade 3 ALN and MLN. Metabolic tumor volume and TLG values over 4.10cm3 and 26.57 showed 60% and 59% sensitivity and 83% and 94 specificity respectively for the identification of MLN., Conclusion: The contribution of MTV and TLG values of18 F-FDG PET/CT to the differential diagnosis of ALN is much more valuable than SUVmax values, especially for grade 3 anthracosis. It was thought that cytological reporting of only grade 3 ALN could make a better contribution to the18 F-FDG PET/CT evaluation analysis.- Published

25. Assessment of Aortic Stiffness by Transthoracic Echocardiographic in Young COVID-19 Patients.

Background: Deteriorated aortic elasticity is part of the atherosclerotic process. Inflammation is an underlying factor in both COVID-19 and atherosclerosis., Aims and Objectives: Using aortic elastic properties, we aimed to assess the subclinical indicators of susceptibility to inflammatory atherosclerosis in patients with COVID-19., Materials and Methods: Out of 194 participants included in this study, 100 were diagnosed with COVID-19 in the last 6 months (60 women and 40 men with a mean age of 34.13 ± 6.45 years) and 94 were healthy controls (55 women and 39 men with a mean age of 30.39 ± 7.21 years). We analyzed transthoracic echocardiographic and aortic stiffness parameters in all participants., Results: Values of systolic blood pressure (110 [85-140] vs. 110 [80-140], P = 0.037) and pulse pressure (PP) (37 [25-55] vs. 40 [25-55], P < 0.01) were significantly different between the groups. As for laboratory parameters, levels of glucose (97.89 ± 20.23 vs. 92.00 ± 9.95, P = 0.003) and creatinine (0.80 ± 0.13 vs. 0.75 ± 0.09, P = 0.003) were significantly higher in the COVID-19 group. Echocardiographic parameters showed that both groups differed significantly in diastolic aortic diameter (2.42 ± 0.28 vs. 2.31 ± 0.35, P = 0.017), aortic strain (9.66 [1.20-31.82] vs. 12.82 [2.41-40.11], P = 0.025), aortic distensibility (0.502 [0.049-2.545] vs. 0.780 [0.120-2.674], P < 0.01), and aortic stiffness (16.67 [4.19-139.43] vs. 11.71 [3.43-65.21], P = 0.006)., Conclusion: Measurement of aortic stiffness is a simple, practical yet inexpensive method in COVID-19 patients, and therefore, may be used as an early marker for COVID-19-induced subclinical atherosclerosis., Competing Interests: The authors report no conflict of interest., (Copyright: © 2022 Journal of Cardiovascular Echography.)

Background: To evaluate the additive role of Ga-68 PSMA PET as a primary staging tool in patients bearing prostate cancer in single PIRADS 4 or 5 index lesions., Methods: Eighty-one biopsy-naive patients with preoperative mpMRI and Ga-68 PSMA PET who underwent radical prostatectomy (RP) were evaluated retrospectively. Forty-nine patients had PIRADS 4 and 32 had PIRADS 5 index lesions. The localization, grade, and volumetric properties of dominant (DT) and non-dominant tumors (NDT) in RP were compared to the index lesions of mpMRI and Ga-68 PSMA PET., Results: The median age and PSA level were 62 (IQR; 59-69) years and 7 (IQR; 2-8) ng/ml, respectively. Ga-68 PSMA PET detected DTs in 100% of the patients including 13 patients in whom mpMR failed. In 45 patients an NDT was reported in RP. Ga-68 PSMA PET accurately detected NDT in 24 of 45 (53.3%) patients. Six patients (12.2%) in PIRADS 4 and 8 (25%) in PIRADS 5 group showed upgrading. In PIRADS 4, Ga-68 PSMA PET localized DT in all patients with upgraded tumors whereas mpMRI missed exact location in 2 of 6 (33.3%). In PIRADS 5 both mpMRI and Ga-68 PSMA PET accurately located all DTs. Overall detection rates of extracapsular extension (ECE) and seminal vesicle invasion (SVI) by mpMRI were 51.1% and 53.8%, respectively. Ga-68 PSMA PET detected ECE and SVI in 27.9% and 30.7%, respectively. When mpMRI and Ga-68 PSMA PET were used in combination detection rates of ECE and SVI increased to 65.1 and 61.5%. Ga-68 PSMA PET-detected six of ten patients with positive lymph nodes whereas mpMRI could not identify any., Conclusions: Ga-68 PSMA PET has a better diagnostic accuracy in detecting DT, NDT, upgrading, adverse pathology in patients with PIRADS 4 index lesions. However, mpMRI better predicted ECE and SVI than Ga-68 PSMA PET.

Background: Results from prospective trials have shown higher accuracy of prostate-specific membrane antigen (PSMA)-based positron emission tomography (PET)/computed tomography (CT) in detection of lymph node metastasis (LNM) compared to conventional imaging., Objective: To evaluate the accuracy of
68 Ga-PSMA-11 PET/CT for LNM detection in patients undergoing radical prostatectomy (RP) and extended pelvic lymph node dissection (PLND)., Design, Setting, and Participants: Between June 2014 and November 2020, 96 patients with68 Ga-PSMA PET/CT for primary staging underwent RP and extended PLND., Outcome Measurements and Statistical Analysis: The results from68 Ga-PSMA PET/CT were compared with histologic data from primary PLND in 96 patients. All68 Ga-PSMA PET/CT scans were centrally reviewed., Results and Limitations: Of 96 patients, 15.6% (n = 15) harbored LNMs. The median prostate-specific antigen at68 Ga-PSMA PET/CT was 8.0 ng/ml (interquartile range 5.5-11.7). The majority of patients had intermediate- (52.1%) or high-risk disease (41.7%). Biopsy grade group 4 and 5 was present in 22.9% and 15.6%, respectively. The68 Ga-PSMA PET/CT scans identified eight of 15 patients (53.3%) as LN-positive (true positive). The calculated per-patient sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of68 Ga-PSMA PET/CT in the detection of LNM were 53.3%, 98.8%, 88.9%, 92.0%, and 91.7%, respectively. The per-patient sensitivity and specificity in the detection of LNMs larger than 2 mm were 61.5% and 98.8%, respectively. The main limitation is the retrospective design of the study., Conclusions:68 Ga-PSMA PET/CT is accurate in lymph node staging and the results support its use for primary staging of prostate cancer., Patient Summary: We compared prostate-specific membrane antigen (PSMA)-based positron emission tomography (PET)/computed tomography (CT) findings with histopathology results after extended lymph node dissection and showed that it is accurate in detecting lymph node metastases. We aimed to estimate the absorbed radiation doses for critical organs (eg, kidneys, parotid glands, submandibular glands, and lacrimal glands) of patients treated with 4 to 6 cycles by Lu-PSMA inhibitor RLT, retrospectively, and to evaluate the findings extensively in order to determine the critical organ radiation-absorbed limitations and the number of prospective RLT., Materials and Methods: A total of 51 cycles Lu-PSMA inhibitor RLT in 10 patients was analyzed. Therapies have been applied in 4 to 6 cycles with 8 to 10 weeks' intervals. Dosimetric estimates of kidneys, parotid glands, submandibular glands, and lacrimal glands have been calculated based on MIRD scheme pamphlet no. 16. Regions of interest were drawn with GE Xeleris Functional Imaging Workstation. OLINDA/EXM 1.1 simulation software was used to calculate radiation-absorbed doses., Results: Mean radiation-absorbed doses were 0.70 ± 0.24 Gy/GBq for kidneys, 1.34 ± 0.78 Gy/GBq for parotid glands, 0.94 ± 0.45 Gy/GBq for submandibular glands, and 2.28 ± 1.29 Gy/GBq for lacrimal glands., Conclusions: Due to the critical target organ risks and the optimal therapy doses, patient-specific dosimetry is a deterministic factor in radionuclide therapy. Even when the absorbed kidney doses were above the ICRP critical dose limits in patients who had 4 to 6 cycles of therapy, mortality due to nephrotoxicity has not been observed. Mild increased tolerated radiation dose is acceptable for the patient groups with very low survival rate.

Imaging is critical for primary staging of prostate cancer. Traditional imaging modalities (computerized tomography scan and nuclear medicine bone scans) are limited by their suboptimal diagnostic performance. Recent meta-analyses have demonstrated that nuclear imaging with
68 Ga-labeled prostate-specific membrane antigen ligand (68 Ga-PSMA) using positron emission tomography/computed tomography (PET/CT) has higher sensitivity and specificity in this setting compared to conventional imaging techniques.68 Ga-PSMA PET/CT for whole-body assessment can be used as the sole imaging modality for primary lymph node and bone staging of prostate cancer. PATIENT SUMMARY: There is a rapidly growing body of evidence that nuclear imaging with68 Ga-labeled prostate-specific membrane antigen ligand using positron emission tomography/computed tomography has a higher detection rate for lymph node and bone metastases in prostate cancer patients. - Abstract
Aims: To evaluate the diagnostic accuracy of ultrasound (US) assessing the lateral compartment lymph node metastasis in patients with primary papillary thyroid carcinoma (PTC), and to demonstrate the incidence and patterns of the lateral lymph node metastasis., Methods: We retrospectively reviewed 198 patients with primary PTC who underwent thyroidectomy in addition to modified lateral neck dissections (MLND) involving level II to level V due to clinically positive lateral neck disease. A skilled and experienced single operator performed all US examinations. Surgical pathology results were accepted as the reference method and sensitivity, specificity, and diagnostic accuracy of US in detecting metastatic lymph nodes established using level-by-level analysis., Results: In the study cohort, 10.1% of the patients had lateral compartment lymph node metastases without any central compartment involvement. For the lateral compartment, 48.5% had level II, 74.7% had level III, 64.6% had level IV, and 29.3% of the patients had level V metastasis. None of the patients had isolated level V metastasis. The sensitivity, specificity, and diagnostic accuracy of US in identifying lateral lymph compartment metastasis ranged from 87% to 91.4%, 92% to 98.6% 92.4% to 96%, respectively. However, the sensitivity (74.7%) and diagnostic accuracy (76.2%) of US significantly decreased for the central compartment while specificity (90%) remained similar., Conclusion: US performed by a skilled operator has an excellent diagnostic accuracy for the evaluation of lateral cervical lymph nodes in primary PTC; thus, might enable precise tailoring of the management strategies. - Abstract
Objective: In renal transplant recipients, cyclosporine treatment appears to cause more frequent hyperlipidemia than tacrolimus usage. In this study, hyperlipidemic renal transplant recipients who use cyclosporine were investigated for changes in high-density lipoprotein (HDL)-2/3, apolipoprotein (Apo) A1/B, other lipid and biochemical parameters, and body mass index after prospective cyclosporine to tacrolimus switching., Materials and Methods: Fifteen patients, including 9 females of overall mean age of 33.2 +/- 10.7 years and posttransplantation time of 78.06 +/- 42.93 months with a mean body mass index of 23.77 +/- 3.34 kg/m(2), were included if they were nondiabetic, hyperlipidemic, and had undergone renal transplantation between 1992 and 2000, using cyclosporine and candidates for a switch to tacrolimus due to hyperlipidemia. Before switching to tacrolimus and at 12 months of tacrolimus use we studied fasting blood samples for creatinine, uric acid, glucose, triglyceride, Apo A1, Apo B, low-density lipoprotein (LDL), HDL2, HDL3, and total cholesterol., Results: There were no significant differences in creatinine, uric acid, glucose levels, or body mass index before tacrolimus versus 12 months thereafter. It was observed that tacrolimus significantly decreased triglyceride, Apo A1, Apo B, LDL, HDL, and total cholesterol levels (P < .001; P = .006; P = .01; P < .001; P = .03; P = .001, respectively), but had no effect on homocysteine, Apo A1/B, HDL 2, HDL 3, or HDL 2/3 levels (P > .05)., Conclusion: Switching from cyclosporine to tacrolimus was associated with a more favorable cardiovascular risk profile by improving hyperlipidemia.

- Abstract
Background: We investigated the effects of Botox-A on weight loss and gastric emptying in an experimental obese rat model. Although there is evidence of weight loss in normal-weight rats after Botox-A injection, there are no studies indicating the effect of Botox-A injection on weight loss and gastric emptying time in obese rats., Methods: 37 female Wistar Albino rats were given high calorie diet for 90 days. They were separated into 3 groups. The first group (Botox group) consisted of 15 obese rats whose gastric antrum was injected with 20 U of Botulinum Toxin Type A. The second group (Saline group) consisted of 15 obese rats whose gastric antrum was injected with 20 U of saline. The third group (Control group) had no surgical intervention. Gastric scintigraphy was performed in the 3 groups pre- and postoperatively., Results: The saline group had a weight reduction in the early postoperative days but began to gain weight thereafter. The mean weight of the Botox group between the 16th and 28th days postoperatively was significantly lower than the mean weights of the control and the saline groups (P<0.05, P<0.001). The results of gastric emptying scintigraphy in all 3 groups at day 20 revealed significantly higher T1/2 values in the Botox-A group when compared to the results of the control and saline groups (P<0.001)., Conclusion: Botox-A application to the gastric antrum in obese rats leads to weight loss by increasing the gastric emptying time.
