Abstract

Mechanisms of resilience against tau pathology in individuals across the Alzheimer's disease spectrum are insufficiently understood. Longitudinal data are necessary to reveal which factors relate to preserved cognition (i.e. cognitive resilience) and brain structure (i.e. brain resilience) despite abundant tau pathology, and to clarify whether these associations are cross-sectional or longitudinal. We used a longitudinal study design to investigate the role of several demographic, biological and brain structural factors in yielding cognitive and brain resilience to tau pathology as measured with PET. In this multicentre study, we included 366 amyloid-β-positive individuals with mild cognitive impairment or Alzheimer's disease dementia with baseline 18F-flortaucipir-PET and longitudinal cognitive assessments. A subset (n = 200) additionally underwent longitudinal structural MRI. We used linear mixed-effects models with global cognition and cortical thickness as dependent variables to investigate determinants of cognitive resilience and brain resilience, respectively. Models assessed whether age, sex, years of education, APOE-ε4 status, intracranial volume (and cortical thickness for cognitive resilience models) modified the association of tau pathology with cognitive decline or cortical thinning. We found that the association between higher baseline tau-PET levels (quantified in a temporal meta-region of interest) and rate of cognitive decline (measured with repeated Mini-Mental State Examination) was adversely modified by older age (Stβinteraction = -0.062, P = 0.032), higher education level (Stβinteraction = -0.072, P = 0.011) and higher intracranial volume (Stβinteraction = -0.07, P = 0.016). Younger age, higher education and greater cortical thickness were associated with better cognitive performance at baseline. Greater cortical thickness was furthermore associated with slower cognitive decline independent of tau burden. Higher education also modified the negative impact of tau-PET on cortical thinning, while older age was associated with higher baseline cortical thickness and slower rate of cortical thinning independent of tau. Our analyses revealed no (cross-sectional or longitudinal) associations for sex and APOE-ε4 status on cognition and cortical thickness. In this longitudinal study of clinically impaired individuals with underlying Alzheimer's disease neuropathological changes, we identified education as the most robust determinant of both cognitive and brain resilience against tau pathology. The observed interaction with tau burden on cognitive decline suggests that education may be protective against cognitive decline and brain atrophy at lower levels of tau pathology, with a potential depletion of resilience resources with advancing pathology. Finally, we did not find major contributions of sex to brain nor cognitive resilience, suggesting that previous links between sex and resilience might be mainly driven by cross-sectional differences.

Abstract

BackgroundRecruiting to multi-site trials is challenging, particularly when striving to ensure the randomized sample is demographically representative of the larger disease-suffering population. While previous studies have reported disparities by race and ethnicity in enrollment and randomization, they have not typically investigated whether disparities exist in the recruitment process prior to consent. To identify participants most likely to be eligible for a trial, study sites frequently include a prescreening process, generally conducted by telephone, to conserve resources. Collection and analysis of such prescreening data across sites could provide valuable information to improve understanding of recruitment intervention effectiveness, including whether traditionally underrepresented participants are lost prior to screening.MethodsWe developed an infrastructure within the National Institute on Aging (NIA) Alzheimer's Clinical Trials Consortium (ACTC) to centrally collect a subset of prescreening variables. Prior to study-wide implementation in the AHEAD 3-45 study (NCT NCT04468659), an ongoing ACTC trial recruiting older cognitively unimpaired participants, we completed a vanguard phase with seven study sites. Variables collected included age, self-reported sex, self-reported race, self-reported ethnicity, self-reported education, self-reported occupation, zip code, recruitment source, prescreening eligibility status, reason for prescreen ineligibility, and the AHEAD 3-45 participant ID for those who continued to an in-person screening visit after study enrollment.ResultsEach of the sites was able to submit prescreening data. Vanguard sites provided prescreening data on a total of 1029 participants. The total number of prescreened participants varied widely among sites (range 3-611), with the differences driven mainly by the time to receive site approval for the main study. Key learnings instructed design/informatic/procedural changes prior to study-wide launch.ConclusionCentralized capture of prescreening data in multi-site clinical trials is feasible. Identifying and quantifying the impact of central and site recruitment activities, prior to participants signing consent, has the potential to identify and address selection bias, instruct resource use, contribute to effective trial design, and accelerate trial enrollment timelines.

Abstract

Conflict in its various manifestations continues to be a defining feature in many places throughout the world. In an attempt to address such conflict, various forms of a Truth and Reconciliation Commission (TRC) have been introduced to facilitate the transition from social conflict to a new dispensation. The introduction and subsequent proceedings of TRCs in South Africa, Canada and Norway are widely regarded as good examples of this approach. Against this background, a number of researchers from VID Specialized University and the University of the Western Cape had an exploratory meeting in Oslo in 2018 where the possibility for a joint research project under the broad theme of ‘discourses on reconciliation’ was first discussed. This led to two further research symposia in Cape Town and Tromsø in 2019. With the inclusion of specialists working on the Canadian Truth and Reconciliation process, these meetings demonstrated common ground and a shared understanding of the issues at stake. Moreover, it pointed to the differences between the South African, Canadian and Norwegian Commissions. In comparing the South African, Canadian and Norwegian experiences, researchers identified that these countries were, in fact, at different stages of their respective truth and reconciliation processes. This has prompted scholars to revisit and problematise these processes in relation to ongoing societal challenges. In all cases, it is quite apparent that reconciliation between individuals and groups remains a significant challenge.

7. If Not Us, Who? Social Media Policy and the Ischool Classroom

Abstract

Social networking tools offer opportunities for innovative, participative pedagogical practice within traditional institutional frameworks. However, tensions continue to develop within this space: between creativity and security, personal and professional identity, privacy and openness. We argue that iSchools are uniquely positioned to create proactive, adaptive policies guiding the pedagogical use of social media and offer initial recommendations toward the crafting of such policies. If we expect information school graduates to be proficient and critical users of perpetually evolving social media technologies, we need to create learning environments that support the ethical, reflective and effective use of these tools. As an initial step in that direction, we report on three explorations we conducted to identify the challenges and opportunities that are part of today's educational social media landscape. Informed by these investigations we put forward initial guidelines for developing a "Social Media & the iSchool Classroom" policy for other schools to appropriate, modify and enhance.

25. Social media in the Canadian government : an exploratory study of emerging practice

Abstract

Records held in national and institutional archives can serve as instruments of accountability and transparency for government actions (or inaction) and aid in constructing social memory. As digital technologies advance, records that were traditionally analogue are increasingly generated within networked digital platforms. In efforts contribute to archival and records theory on social media and accountability, this dissertation investigates emergent practices in the Government of Canada’s (GC) early use of social media, (2013–2014), when agencies and public servants were in the nascent stages of adoption. This study undertakes a qualitative examination of two main areas of the GC’s social media use: social media and recordkeeping practices and their implications for records generation and retention, and policy instruments and frameworks regarding the role of information management and recordkeeping in its social media use and outputs. Empirical data gathered included 28 interview participants, 34 legislative and policy instruments, online and offline observations, and 35 documentary sources, which were analyzed using a practice lens—introducing the utility of a practice lens for archival research. The main objectives of the study were to gain an understanding of the relationships between social media practices, policy, and information management and recordkeeping practices in the GC during an early phase of social media adoption and to contribute to the archival and records theory discourse surrounding shifting social media and recordkeeping practices and the implications for records as instruments of accountability. Findings suggest that emerging social media practices at that time put a strain on existing government frameworks, particularly with regard to retaining, collecting, and preserving its own records and records under its collections mandate. Despite implementing social media, findings also indicate that GC social media adoption and use operated in a bureaucratic environment that struggled to effectively adopt the ethos of these platforms (e.g. horizontal collaborations, ease of information access, etc.). The research surfaced constraints in policy development: policies intended to support increased collaboration were challenged by a hierarchical decision-making model. Moving forward, this research suggests an agile approach to policy development and an exploration of global treaty approaches in exploration of social media platform governance models.

One purpose of this article is to chronicle the progression of affirmative action. A second purpose is to show how affirmative action has benefited all of society and not just a select few. The authors of this article move from a critique of the history of affirmative action to an assessment of the current political climate. Abstract

The anthracycline antibiotics, daunorubicin, doxorubicin, and the newer derivatives, are important components of many antineoplastic chemotherapeutic regimens. Their usefulness is limited by their cardiotoxicity. Sequential monitoring of cardiac functions of patients undergoing chemotherapy allows identification of subclinical cardiotoxicity. In many patients monitoring can thus guide the modification of the chemotherapy to minimize cumulative cardiotoxicity, reducing acute and long-term clinical and subclinical sequelae. Such monitoring also aids in the comparison of cardiotoxicity produced by different drugs and different methods and schedules of drug administration. The considerable variability of monitoring regimens between institutions and in the literature has detracted from its usefulness. The Cardiology Committee of the Childrens Cancer Study Group has, therefore, reviewed the field and has formulated recommendation for standardized noninvasive monitoring of children during and immediately after chemotherapy and for the modification of the chemotherapy where indicated. Abstract

This session was about libraries and archives embracing open source software for digital preservation as demonstrated by large institutions such as MIT, Harvard, and The National Archives of the UK, to name a few. These organizations have developed and distributed open source tools for digital preservation ranging from format identification tools to repository software. The development of open source records management software by contrast has been slow to develop. This presentation discussed the reasons for this dichotomy through a discussion of the ongoing work of the InterPARES 3 general study – Open Source Records Management Software and reports on an early evaluation of the first open source records management software. This session took place on October 19, 2010 in the Lillooet Room of the Irving K. Barber Learning Centre at the University of British Columbia.

Abstract

There are dedicated efforts around the world to steward material related to human atrocities, with aspirations to acknowledge, learn from, and perhaps lessen the probability of further harm. Yet for the archivists charged with stewarding these collections, there remain ethically fraught questions of how to do this work. Through this article, we identify systemic, structural challenges that confront efforts to ethically steward collections of trauma. Our scholarly reflections are grounded in the context of an institution being created to hold materials related to Canada's Truth and Reconciliation Commission (TRC). We reflect on ongoing discussions with those involved in the inception and development of the National Centre for Truth and Reconciliation (NCTR) and our review of court documents and media accounts related to the TRC and development of the NCTR. We call attention to the assemblage of socio-technical, political, and legal forces that influence the actions of those managing collections of trauma. We highlight how those involved in the NCTR's development are participants in and descendants of Canada's legacy of colonizing initiatives (as are we, researchers studying the NCTR's development). To support those facing questions of action in this complicated space, we draw on the work of Iris Marion Young to inform archivists' practice related to the issues of conflict, plurality, agency, and distrust that underlie the design and management of collections of trauma.
