Background: Low back pain is highly prevalent and the main cause of years lived with disability (YLDs). We present the most up-to-date global, regional, and national data on prevalence and YLDs for low back pain from the Global Burden of Diseases, Injuries, and Risk Factors Study 2021. Methods: Population-based studies from 1980 to 2019 identified in a systematic review, international surveys, US medical claims data, and dataset contributions by collaborators were used to estimate the prevalence and YLDs for low back pain from 1990 to 2020, for 204 countries and territories. Low back pain was defined as pain between the 12th ribs and the gluteal folds that lasted a day or more; input data using alternative definitions were adjusted in a network meta-regression analysis. Nested Bayesian meta-regression models were used to estimate prevalence and YLDs by age, sex, year, and location. Prevalence was projected to 2050 by running a regression on prevalence rates using Socio-demographic Index as a predictor, then multiplying them by projected population estimates. Findings: In 2020, low back pain affected 619 million (95% uncertainty interval 554–694) people globally, with a projection of 843 million (759–933) prevalent cases by 2050. In 2020, the global age-standardised rate of YLDs was 832 per 100 000 (578–1070). Between 1990 and 2020, age-standardised rates of prevalence and YLDs decreased by 10·4% (10·9–10·0) and 10·5% (11·1–10·0), respectively. A total of 38·8% (28·7–47·0) of YLDs were attributed to occupational factors, smoking, and high BMI. Interpretation: Low back pain remains the leading cause of YLDs globally, and in 2020, there were more than half a billion prevalent cases of low back pain worldwide. While age-standardised rates have decreased modestly over the past three decades, it is projected that globally in 2050, more than 800 million people will have low back pain. Challenges persist in obtaining primary country-level data on low back pain, and there is an urgent need for more high-quality, primary, country-level data on both prevalence and severity distributions to improve accuracy and monitor change. In this work, we applied and validated an artificial intelligence technique known as generative adversarial networks (GANs) to create large volumes of high-fidelity synthetic anteroposterior (AP) pelvis radiographs that can enable deep learning (DL)-based image analyses, while ensuring patient privacy.AP pelvis radiographs with native hips were gathered from an institutional registry between 1998 and 2018. The data was used to train a model to create 512 × 512 pixel synthetic AP pelvis images. The network was trained on 25 million images produced through augmentation. A set of 100 random images (50/50 real/synthetic) was evaluated by 3 orthopaedic surgeons and 2 radiologists to discern real versus synthetic images. Two models (joint localization and segmentation) were trained using synthetic images and tested on real images.The final model was trained on 37,640 real radiographs (16,782 patients). In a computer assessment of image fidelity, the final model achieved an "excellent" rating. In a blinded review of paired images (1 real, 1 synthetic), orthopaedic surgeon reviewers were unable to correctly identify which image was synthetic (accuracy = 55%, Kappa = 0.11), highlighting synthetic image fidelity. The synthetic and real images showed equivalent performance when they were assessed by established DL models.This work shows the ability to use a DL technique to generate a large volume of high-fidelity synthetic pelvis images not discernible from real imaging by computers or experts. These images can be used for cross-institutional sharing and model pretraining, further advancing the performance of DL models without risk to patient data safety.Level III.

PURPOSE: To develop a multimodal machine learning–based pipeline to predict patient-specific risk of dislocation following primary total hip arthroplasty (THA). MATERIALS AND METHODS: This study retrospectively evaluated 17 073 patients who underwent primary THA between 1998 and 2018. A test set of 1718 patients was held out. A hybrid network of EfficientNet-B4 and Swin-B transformer was developed to classify patients according to 5-year dislocation outcomes from preoperative anteroposterior pelvic radiographs and clinical characteristics (demographics, comorbidities, and surgical characteristics). The most informative imaging features, extracted by the mentioned model, were selected and concatenated with clinical features. A collection of these features was then used to train a multimodal survival XGBoost model to predict the individualized hazard of dislocation within 5 years. C index was used to evaluate the multimodal survival model on the test set and compare it with another clinical-only model trained only on clinical data. Shapley additive explanation values were used for model explanation. RESULTS: The study sample had a median age of 65 years (IQR: 18 years; 52.1% [8889] women) with a 5-year dislocation incidence of 2%. On the holdout test set, the clinical-only model achieved a C index of 0.64 (95% CI: 0.60, 0.68). The addition of imaging features boosted multimodal model performance to a C index of 0.74 (95% CI: 0.69, 0.78; P = .02). CONCLUSION: Due to its discrimination ability and explainability, this risk calculator can be a potential powerful dislocation risk stratification and THA planning tool. Minimizing bias is critical to adoption and implementation of machine learning (ML) in clinical practice. Systematic mathematical biases produce consistent and reproducible differences between the observed and expected performance of ML systems, resulting in suboptimal performance. Such biases can be traced back to various phases of ML development: data handling, model development, and performance evaluation. This report presents 12 suboptimal practices during data handling of an ML study, explains how those practices can lead to biases, and describes what may be done to mitigate them. Authors employ an arbitrary and simplified framework that splits ML data handling into four steps: data collection, data investigation, data splitting, and feature engineering. Examples from the available research literature are provided. A Google Colaboratory Jupyter notebook includes code examples to demonstrate the suboptimal practices and steps to prevent them. The increasing use of machine learning (ML) algorithms in clinical settings raises concerns about bias in ML models. Bias can arise at any step of ML creation, including data handling, model development, and performance evaluation. Potential biases in the ML model can be minimized by implementing these steps correctly. This report focuses on performance evaluation and discusses model fitness, as well as a set of performance evaluation toolboxes: namely, performance metrics, performance interpretation maps, and uncertainty quantification. By discussing the strengths and limitations of each toolbox, our report highlights strategies and considerations to mitigate and detect biases during performance evaluations of radiology artificial intelligence models. Whole-body low-dose CT is the recommended initial imaging modality to evaluate bone destruction as a result of multiple myeloma. Accurate interpretation of these scans to detect small lytic bone lesions is time intensive. A functional deep learning) algorithm to detect lytic lesions on CTs could improve the value of these CTs for myeloma imaging. Our objectives were to develop a DL algorithm and determine its performance at detecting lytic lesions of multiple myeloma.Axial slices (2-mm section thickness) from whole-body low-dose CT scans of subjects with biochemically confirmed plasma cell dyscrasias were included in the study. Data were split into train and test sets at the patient level targeting a 90%/10% split. Two musculoskeletal radiologists annotated lytic lesions on the images with bounding boxes. Subsequently, we developed a two-step deep learning model comprising bone segmentation followed by lesion detection. Unet and "You Look Only Once" (YOLO) models were used as bone segmentation and lesion detection algorithms, respectively. Diagnostic performance was determined using the area under the receiver operating characteristic curve (AUROC).Forty whole-body low-dose CTs from 40 subjects yielded 2193 image slices. A total of 5640 lytic lesions were annotated. The two-step model achieved a sensitivity of 91.6% and a specificity of 84.6%. Lesion detection AUROC was 90.4%.We developed a deep learning model that detects lytic bone lesions of multiple myeloma on whole-body low-dose CTs with high performance. External validation is required prior to widespread adoption in clinical practice.

Considering the immense pace of developments in deep learning (DL), its applications in medicine are relatively limited. One main issue that hinders the utilization of DL in the medical practice workflow is its reliability. A radiologist interpreting an image can easily say “I don’t know”, while a DL model is forced to output a result. Evidential deep learning (EDL) is one of the methods for uncertainty quantification (UQ). In this work, we aimed to use EDL to express model uncertainty in detecting COVID-19. We used SIIM-FISABIO-RSNA COVID-19 chest x-ray dataset and trained a model to diagnose typical COVID-19 pneumonia. When applied to a separate test set, it yielded an accuracy of 88% with median uncertainty scores of 0.25 and 0.07 for normal and typical COVID-19 images, respectively. Moreover, the model labeled unseen indeterminate and atypical COVID-19 x-rays with median uncertainties of 0.32 and 0.35, respectively. Our model’s performance was superior to the exact model trained with conventional approach of DL (i.e., using the cross-entropy loss), which is not able to express the uncertainty level. Overall, this study demonstrates applicability of UQ in disease detection that could facilitate the use of DL in practice by increasing its reliability.

Background: The 8-item myasthenia gravis activity of daily living (MG-ADL) questionnaire is a valid and reliable instrument for evaluating myasthenia gravis (MG)-associated disability. This study aims to assess its validity and reliability in the Iranian population. Methods: A total number of 58 patients with MG were qualified for the examination. All 58 patients completed the Persian translation of 15-item Myasthenia Gravis Quality of Life (MG-QOL15) and MG-ADL questionnaires initially, and 30 filled out the MG-ADL questionnaire 2 to 4 weeks after the initial visit. Pearson correlation coefficient of questionnaires, internal consistency using Cronbach's alpha (α), and test-retest repeatability of the questionnaire were evaluated. Results: The Pearson correlation coefficient of Persian versions of MG-QOL and MG-ADL was 0.93 (P < 0.01). The Persian MG-ADL showed satisfactory internal consistency (Cronbach’s α = 0.89) and test-retest reliability (r = 0.99, P < 0.01). Conclusion: The Persian MG-ADL is a valid and reliable questionnaire for the determination of activities of daily life in Iranian patients with MG.

Glioblastoma (GBM) is the most common primary malignant brain tumor in adults. The standard treatment for GBM consists of surgical resection followed by concurrent chemoradiotherapy and adjuvant temozolomide. O-6-methylguanine-DNA methyltransferase (MGMT) promoter methylation status is an important prognostic biomarker that predicts the response to temozolomide and guides treatment decisions. At present, the only reliable way to determine MGMT promoter methylation status is through the analysis of tumor tissues. Considering the complications of the tissue-based methods, an imaging-based approach is preferred. This study aimed to compare three different deep learning-based approaches for predicting MGMT promoter methylation status. We obtained 576 T2WI with their corresponding tumor masks, and MGMT promoter methylation status from, The Brain Tumor Segmentation (BraTS) 2021 datasets. We developed three different models: voxel-wise, slice-wise, and whole-brain. For voxel-wise classification, methylated and unmethylated MGMT tumor masks were made into 1 and 2 with 0 background, respectively. We converted each T2WI into 32 × 32 × 32 patches. We trained a 3D-Vnet model for tumor segmentation. After inference, we constructed the whole brain volume based on the patch's coordinates. The final prediction of MGMT methylation status was made by majority voting between the predicted voxel values of the biggest connected component. For slice-wise classification, we trained an object detection model for tumor detection and MGMT methylation status prediction, then for final prediction, we used majority voting. For the whole-brain approach, we trained a 3D Densenet121 for prediction. Whole-brain, slice-wise, and voxel-wise, accuracy was 65.42% (SD 3.97%), 61.37% (SD 1.48%), and 56.84% (SD 4.38%), respectively.

There are increasing concerns about the bias and fairness of artificial intelligence (AI) models as they are put into clinical practice. Among the steps for implementing machine learning tools into clinical workflow, model development is an important stage where different types of biases can occur. This report focuses on four aspects of model development where such bias may arise: data augmentation, model and loss function, optimizers, and transfer learning. The risk of progression in Barrett's esophagus (BE) increases with development of dysplasia. There is a critical need to improve the diagnosis of BE dysplasia, given substantial interobserver disagreement among expert pathologists and overdiagnosis of dysplasia by community pathologists. We developed a deep learning model to predict dysplasia grade on whole-slide imaging.We digitized nondysplastic BE (NDBE), low-grade dysplasia (LGD), and high-grade dysplasia (HGD) histology slides. Two expert pathologists confirmed all histology and digitally annotated areas of dysplasia. Training, validation, and test sets were created (by a random 70/20/10 split). We used an ensemble approach combining a "you only look once" model to identify regions of interest and histology class (NDBE, LGD, or HGD) followed by a ResNet101 model pretrained on ImageNet applied to the regions of interest. Diagnostic performance was determined for the whole slide.We included slides from 542 patients (164 NDBE, 226 LGD, and 152 HGD) yielding 8596 bounding boxes in the training set, 1946 bounding boxes in the validation set, and 840 boxes in the test set. When the ensemble model was used, sensitivity and specificity for LGD was 81.3% and 100%, respectively, and90% for NDBE and HGD. The overall positive predictive value and sensitivity metric (calculated as F1 score) was .91 for NDBE, .90 for LGD, and 1.0 for HGD.We successfully trained and validated a deep learning model to accurately identify dysplasia on whole-slide images. This model can potentially help improve the histologic diagnosis of BE dysplasia and the appropriate application of endoscopic therapy.

INTRODUCTION Glioblastomas (GBMs) are highly aggressive tumors. Despite multimodal treatment, its median overall survival ranges between 16 and 20 months. The standard treatment regimen consists of surgical resection followed by concurrent chemoradiotherapy and adjuvant temozolomide. Despite temozolomide’s effectiveness, it may cause the clinical challenge of treatment-related progression also known as pseudoprogression(PsP). Usually, PSP resolves or stabilizes without further treatment, whereas a true progression (TP) requires more aggressive management. Identifying PSP from TP will affect the patient’s treatment plan. Conventional magnetic resonance imaging (MRI) reading techniques cannot distinguish these entities. This study investigated the feasibility of using deep learning to distinguish PsP from TP. METHOD We included GBM patients who met our inclusion criteria. We evaluated all cases to see if they had a new enhancing lesion within the original radiation field or an increase in the size of an existing lesion. The challenging MRIs were collected. Clinical notes regarding tumor and recurrence location, clinical history, and medication were collected. We labeled the ones who stayed stable or improved in the imaging and clinical situation as PSP and those with further imaging and clinical deterioration as TP. We coregistered Contrast-enhanced-T1 MRIs with T2-weighted images for each patient. We performed five-fold cross-validation to generalize the performance. We trained A 3-D Densenet121 model to establish the prediction. We selected the best models with the highest accuracy. RESULT After reviewing 1000 patients, we included 124 patients whose imaging showed suspicious progression and their medicational histories were completely retrievable; 63 PsP, and 61 TP. We developed a deep learning model based on the whole dataset. The 5-fold cross-validation revealed that the mean area under the curve (AUC) was 0.81. CONCLUSION We report the development of a deep learning model that diagnoses PsP from TP in patients who received temozolomide. Further refinement and external validation are required prior to widespread adoption in clinical practice.

GBMs are highly aggressive tumors, evidenced by their rapid growth. Despite maximal treatment, median overall survival ranges between 16 and 20 months. Several studies use patients' diagnosis time(pre-surgical) imaging to predict their survival, but having a model that can predict the days till death for the patients who received the first standard step treatments (Stupp protocol) and now showing secondary progression signs is highly needed. In this study, we aimed to predict the overall survival of Glioblastoma(GBM) patients by developing a multimodal deep learning model using post-operative magnetic resonance imaging(MRI) and patients' age. We included GBM patients who met the following inclusion criteria: having maximum surgical resection followed by concurrent chemoradiation and showing secondary progression signs in follow-up imaging while being on adjuvant temozolomide treatment. We obtained contrast-enhanced T1(CET1) and T2-weighted(T2W) MRIs at progression time and patients' age and death dates. We calculated the days of survival from the day of the MRI. Dataset was split into train, validation, and test set at the patient level. CET1 and T2W MRIs were skull-stripped and coregistered for each patient. To mitigate data imbalance, we used the deep imbalance regression method for training a 3-D Densenet121 model with the concatenated CET1 and T2W as inputs. One hundred features were extracted from the optimized model and concatenated with the normalized age of each patient; the resulting output vectors were passed through the second dense neural network for final prediction. We included 110 patients who met the inclusion criteria. With the overall survival ranging from 16 to 4500 days, a mean of 582.4 and a standard deviation of 694.3, we achieved the test set's root mean square error of 261 days. Further refinement with more data, including clinical data such as comorbidities and external validation, is required prior to adoption in clinical practice.

INTRODUCTION Glioblastomas (GBMs) are highly aggressive tumors. A common clinical challenge after standard of care treatment is differentiating tumor progression from treatment-related changes, also known as pseudoprogression (PsP). Usually, PsP resolves or stabilizes without further treatment or a course of steroids, whereas true progression (TP) requires more aggressive management. Differentiating PsP from TP will affect the patient's outcome. This study investigated using deep learning to distinguish PsP MRI features from progressive disease. METHOD We included newly diagnosed GBM patients who met the inclusion criteria, including a new or increasing enhancing lesion size within the original radiation field and who had clinical, medication, and any histopathology available. The interpretation of the MR images at the image inclusion time point had to be indeterminate. We labeled those who subsequently were stable or improved on imaging and clinically as PSP and those with clinical and imaging deterioration as TP. A subset of subjects underwent a second resection. We labeled these subjects as PSP or TP based on the histological diagnosis. We performed skull stripping, coregistered contrast-enhanced T1 MRIs with T2-weighted images for each patient and used them as input to a 3-D Densenet121 model. We used several augmentation techniques and five-fold cross-validation to achieve more robust predictions. RESULT We included 124 patients who met the criteria, and of those, 63 were PsP and 61 were TP. We trained a deep learning model that achieved 76.4% (ranged 70%-84%, SD 5.122) mean accuracy over the 5 folds, 0.7560(ranged 0.6553-0.8535, SD 0.069) mean AUROCC, 88.72% (SD 6.86) mean sensitivity, and 62.05%(SD 9.11) mean specificity. CONCLUSION We report the development of a deep learning model that distinguishes PsP from TP in GBM patients treated per the Stupp protocol. Further refinement and external validation are required prior to widespread adoption in clinical practice.

PURPOSE Though patients with glioblastoma generally have poor overall survival (OS), OS estimates range significantly. We sought to develop a radiomics-based model predictive of OS in glioblastoma using preoperative MRI scans to identify these differences and guide clinical care. METHODS The primary endpoint was OS, determined from the date of surgery to death or last follow-up. T1, FLAIR, T2, and post-contrast T1 (T1c) MRI sequences were obtained for all patients. Using HD-GLIO-AUTO, lesion segmentation yielded three masks corresponding to the regions of contrast enhancement, non-enhancement, and total volume. Radiomics features were extracted from each region from the T2 and T1c sequences independently (i.e. either from T2 or T1c) and in combination. Age at surgery and gender were also considered. The data was split into training (75%) and test (25%) sets, stratifying for age at surgery. Kaplan-Meier analysis and the Coxnet model were used. For Coxnet, Elastic Net was used as the penalty, with optimization of the L1 coefficient using 5-fold cross-validation and evaluation via the concordance index (C-index). RESULTS From 3 institutions, 383 patients were included. Median OS was 395 days: 346 patients died, and 37 were right-censored. Age > 65 years was predictive of decreased OS (p < 0.005), but gender was not (p=0.16). For the Coxnet model, the C-index was 0.67 (standard deviation: 0.01) considering T2 and T1c sequences for the total lesion volume, with a C-index of 0.65 for the test set. For the enhancing volume, the model had a C-index of 0.71 for the test set using T1c sequences. Overall, C-indices ranged from 0.62-0.71. CONCLUSIONS We present a novel radiomics-based model predictive of OS in glioblastoma across a multi-institutional dataset. Ongoing efforts will incorporate immediate postoperative imaging and IDH and MGMT status into the proposed model to further improve prognostic ability and help guide clinical decision-making.

Background: Mild cognitive impairment (MCI) is a state between normal cognition and dementia. However, MCI diagnosis does not necessarily guarantee the progression to dementia. Since no previous study investigated brain positron emission tomography (PET) imaging of MCI-- to-normal reversion, we provided PET imaging of MCI- to-normal reversion using the Alzheimer's Disease Neuroimaging Initiative (ADNI) database. Methods: We applied comprehensive neuropsychological criteria (NP criteria), consisting of memory, language, and attention/executive function domains, to include patients with a baseline diagnosis of MCI (n=613). According to the criteria, the year 1 status of the patients was categorized into three groups (reversion: n=105, stable MCI: n=422, conversion: n=86). Demographic, neuropsychological, genetic, CSF, and cognition biomarker variables were compared between the groups. Additionally, after adjustment for confounding variables, the deposition pattern of amyloid-β and cerebral glucose metabolism were compared between three groups via AV45- and FDG-PET modalities, respectively. Results: MCI reversion rate was 17.1% during one year of follow-up. The reversion group had the lowest frequency of APOE ε4+ subjects, the highest CSF level of amyloid-β, and the lowest CSF levels of t-tau and p-tau. Neuropsychological assessments were also suggestive of better cognitive performance in the reversion group. Patients with reversion to normal state had higher glucose metabolism in bilateral angular and left middle/inferior temporal gyri, when compared to those with stable MCI state. Meanwhile, lower amyloid-β deposition at baseline was observed in the frontal and parietal regions of the reverted subjects. On the other hand, the conversion group showed lower cerebral glucose metabolism in bilateral angular and bilateral middle/inferior temporal gyri compared to the stable MCI group, whereas the amyloid-β accumulation was similar between the groups. Conclusions: This longitudinal study provides novel insight regarding the application of PET imaging in predicting MCI transition over time.

Chronic social isolation stress (SIS) could impair learning and memory-related behaviors. Herein, we investigated the efficacy of Melatonin in treatment of memory despair and also its possible underlying mechanism of action in an animal model of SIS. For this purpose, mice were allocated to two opposing conditions, including social condition (SC) and isolated condition (IC), for five weeks. The study consisted of three groups, including saline-treated SC, saline-treated IC, Melatonin-treated IC (10 mg/kg/day for five successive days). At the end of the isolation period, mice underwent three neurobehavioral tests: passive avoidance (PA), Morris water maze (MWM), and Y maze (YM) tests. Hippocampus samples were obtained and the expressions of BDNF, TrkB, phosphorylated TrkB (pTrkB), CREB, phosphorylated CREB (pCREB), as well as M1 and M2 microglia were assessed. Interpreting the behavioral tests, we found that isolated mice showed lower freezing response in the PA test, lower number of novel arm visits in the YM, and higher escape latency and less time spent in the target quadrant in the MWM, when compared to SC rodents (P values 0.001). The isolated group had higher M1/M2 relative ratio (P 0.001), as well as lower concentrations of BDNF mRNA (p 0.001) and protein (P 0.001), TrkB protein (P = 0.035), CREB mRNA (P 0.001) and protein (P = 0.012), pTrkB (P 0.001), and pCREB (P = 0.035). However, Melatonin relatively reversed the behavioral, cellular, and molecular effects of SIS. Taken together, melatonin therapy could alleviate memory impairment through switching microglial polarization from M1 to M2 phenotype along with altered expression and function in the BDNF/TrkB/CREB signaling pathway.

Despite the great search for an effective approach to minimize secondary injury in spinal cord injury (SCI) setting, there have been limited advances. Roflumilast is a selective inhibitor of phosphodiesterase 4 with potent anti-inflammatory properties. Here, we sought to explore Roflumilast efficacy in the improvement of locomotor and sensory deficits of SCI. In an animal setting, 50 male rats were randomly assigned to five groups: an SCI group receiving Placebo, three SCI groups receiving Roflumilast at the doses of 0.25, 0.5, and 1 mg/kg prior to T9 vertebra laminectomy, and a sham-operated group. Locomotor, mechanical, and thermal activities were evaluated for 28 days. At the end of the study, spinal cord samples were taken to assess the relative ratio of microglial subtypes, including M1 and M2, histopathological changes, levels of pro-inflammatory (TNF-α and IL-1β) and anti-inflammatory (IL-10) biomarkers, and cAMP level. Repeated measure analysis revealed significant effect for time-treatment interaction on locomotion [F (24, 270) = 280.7, p 0.001], thermal sensitivity [F (16, 180) = 4.35, p 0.001], and mechanical sensitivity [F (16, 180) = 7.96, p 0.001]. As expected, Roflumilast significantly increased the expression of spinal cAMP. HE staining exhibited lesser histopathological disruptions in Roflumilast-treated rodents. We also observed a significant reduction in the M1/M2 ratio (p values 0.001) as well as in pro-inflammatory biomarkers following the administration of Roflumilast to the injured rats. Furthermore, IL-10 level was increased in rodents receiving 1 mg/kg of the reagent. In conclusion, the increased spinal cAMP following Roflumilast therapy might attenuate neuroinflammation via altering microglial activity; therefore, it could be considered as an alternative therapeutic agent for SCI complications.
