Abstract

A 25-year-old woman presented with 1 year of progressive orthopnoea, initially explained as bilateral diaphragmatic paresis caused by seronegative myasthenia gravis. She required assisted ventilation and received pyridostigmine and corticosteroids. She had minimal (particularly proximal) symmetrical tetraparesis with apparent bilateral diaphragmatic weakness, but had normal sensation. Further investigation suggested an overlap myositis with shrinking lung syndrome from systemic lupus erythematosus. She improved following immunosuppression with pulse corticosteroids and rituximab, and at 3 months no longer needed bilevel positive airway pressure support. [ABSTRACT FROM AUTHOR]

Abstract

Background: Primary non-Hodgkin's lymphoma with multiple extra- and intra-calvarial extensions without systemic spread in an immunocompetent patient is extremely rare. They masquerade commonly as meningioma and can present as mass lesions with raised intracranial pressure. Case Description: We report one such case of primary diffuse large B-cell lymphoma (DLBCL) in a young female involving the scalp, dural involvement in the right frontal region, left parietal, and posterior fossa and mimicking both clinically and radiologically as meningioma. She was managed surgically. Histological examination showed features suggestive of DLBCL (germinal center type). She was planned for adjuvant therapy. However, at 2 months following surgery, she succumbed due to systemic involvement of the disease. Conclusion: DLBCL is seen rarely in neurosurgical practice. They can present as tumors with adjacent extra- and intra-cranial masses. They pose a diagnostic challenge as it can be easily confused with meningioma. Tumor resection is performed to confirm diagnosis and in patients who present with raised intracranial pressure. Chemotherapy is the preferred treatment, and adjuvant therapy should be started early. Abstract

Leigh's syndrome is a rare cause of young-onset parkinsonism with dystonia in adults. The case presented a 23-year-old male who developed symptoms such as dragging of the right foot, slowness in daily activities, abnormal hand posturing, and falling backwards. The patient was diagnosed with adult-onset Leigh's syndrome based on clinical symptoms, MRI brain findings, MR spectroscopy, muscle biopsy, and genetic testing. Treatment with various medications did not result in significant improvement after one year of follow-up. Clinicians should be aware of Leigh's syndrome as a potential cause of parkinsonism and dystonia in young adults. [Extracted from the article]

Abstract

Randia genus (Indigo berry) belongs to family Rubiaceae, native to America, allocated in tropical regions. Most of the Species associated with this Genus used as ornamental, folk medicine to treat the disease of renal, malaria, cancer, dysentery, snake-bite etc. All plant parts are used by tribes for various ailment treatments. The phytochemicals generally associated with this genus are terpenoids, saponins, lignans, coumarin, iso-coumarin, flavonoids, tannins, essential oil and acid resin. This review highlights the phytochemicals and pharmacological activity reports. Phytochemical investigation of Randia dumetorum fruit extract using petroleum ether solvent, reveals the presence of 2,4-di-tert-butylphenol, octadecadienoic acid, 23(Phenylsulfanyl) lanosta-8,24-dien-3-ol, hexadecenoic acid, gamma-sitosterol, 4-tert-butylcalix[4]arene, tetracontane, tetratetracontane and octacosanol etc. Compound 1-Octacosanol (A), 9-Octadecenoic acid (B) and gamma-Sitosterol (C) were isolated with the help of column chromatographic techniques and characterized by spectral studies i.e. ¹H NMR, 13C NMR, IR and mass spectroscopy. Abstract

Most of the world’s mountain glaciers have been retreating for more than a century in response to climate change. Glacier retreat is evident on all continents, and the rate of retreat has accelerated during recent decades. Accurate, spatially explicit information on the position of glacier margins over time is useful for analyzing patterns of glacier retreat and measuring reductions in glacier surface area. This information is also essential for evaluating how mountain ecosystems are evolving due to climate warming and the attendant glacier retreat. Here, we present a non-comprehensive spatially explicit dataset showing multiple positions of glacier fronts since the Little Ice Age (LIA) maxima, including many data from the pre-satellite era. The dataset is based on multiple historical archival records including topographical maps; repeated photographs, paintings, and aerial or satellite images with a supplement of geochronology; and own field data. We provide ESRI shapefiles showing 728 past positions of 94 glacier fronts from all continents, except Antarctica, covering the period between the Little Ice Age maxima and the present. On average, the time series span the past 190 years. From 2 to 46 past positions per glacier are depicted (on average: 7.8).
