BACKGROUND: It is well established that mothers' education has positive effects on child nutrition in developing countries. Less explored is the effect exerted by the education of other individuals--mothers' friends, neighbours and family. OBJECTIVES: To examine independent effects of mothers', fathers' and grandmothers' education on child height-for-age and weight-for-age z-score, and the role of community-level maternal literacy over and above parental education and other individual-level factors. METHODS: Cross-sectional data were analysed for 5692 children from Andhra Pradesh State in India and Vietnam sampled within "sites" (20 from each country) and then within "communities" (31 from Vietnam and 102 from India). Multilevel regression analysis was undertaken to account for confounders and geographical clustering of observations. RESULTS: Child nutrition is positively and independently associated with mothers', fathers' and grandmothers' education. The association with grandmothers' education was statistically significant in the India sample only and was stronger for boys: the adjusted mean difference in height-for-age z-scores between boys living with an educated grandmother and those not was 0.64 (95% CI 0.29 to 0.99, p

ObjectiveIt is hypothesised that mothers' social networks can positively affect child nutrition through the sharing of health knowledge and other resources. The present study describes the composition of mothers' networks, examines their association with child nutrition, and assesses whether health knowledge is shared within networks.Design and settingCross-sectional data for mothers of young children from Andhra Pradesh (south India) were combined with existing data from the Young Lives study, in which the mothers were participating (n = 282).ResultsThe composition of social networks varied between urban and rural areas, with urban networks being larger, more female, more literate and with a greater proportion of members living outside the household and being non-family. There was a positive association between child's height-for-age Z-score and mother's network size and network literacy rate. The association with network literacy was stronger among the poorest households. Women commonly reported seeking or receiving health advice from network members.ConclusionBig and literate social networks are associated with better child nutrition, especially among the poor. The dissemination of health knowledge between network members is a plausible way in which social networks benefit child nutrition in India. Further research into the underlying mechanisms is necessary to inform the development of interventions that channel health information through word of mouth to the most excluded and vulnerable families.

The aim of the study was to compare the development of children with and without a programme of psychosocial stimulation in 'control' and 'intervention' sites in a poorly resourced area of northeast Brazil. The sample (n = 156, born 1998) was from a larger cohort. The cohort was tested at 12 months (baseline) with the Bayley Scales of Infant Development. All children in the intervention site with a mental development index (MDI) and/or psychomotor development index (PDI) < or = 100 were enrolled. Each time such a child was enrolled, the next child tested in that site of the same sex and with an index of 101 to 115 was also enrolled, and the next two children matched for sex and scores of < or = 100 and 101 to 115 in the control sites were recruited in parallel. The intervention comprised 14 contacts between 13 and 17 months of age. All children were tested again at age 18 months. The intervention and control groups were similar at baseline for a range of socioeconomic, demographic, environmental, and biological variables, and their MDI and PDI were also similar. At 18 months, the mean differences between the intervention and control groups were + 9.4 points for MDI and + 8.2 points for PDI (p < 0.001 in each case). For children with an initial score of < or = 100, the mean difference between the intervened and control groups was + 11.2 points for MDI (p < 0.001), and + 10.8 points for PDI (p = 0.001). The intervention was thus associated with significant improvements in cognitive and motor development.

Women and the poor are disproportionately affected by common mental disorders (CMD), and women in low income countries are particularly at risk. Social capital may explain some of the geographical variation in CMD, but the association between social capital and CMD in low income countries has rarely been studied. This paper aims to explore the relationship between individual and ecological measures of social capital and maternal CMD in four low income countries. Cross-sectional data from the Young Lives (YL) study with information across 234 communities in Peru, Ethiopia, Vietnam and Andhra Pradesh (India) were used. The mental health of mothers of one-year-old children (n=6909), and the individual cognitive and structural social capital of all respondents was assessed. Ecological social capital was calculated by aggregating individual responses to the community level. Multi-level modelling was used to explore the association between individual and ecological (community level) social capital and maternal CMD in each of the four countries, adjusting for a wide range of individual and community level confounders. The analysis shows that individual cognitive social capital is associated with reduced odds of CMD across all four countries. The results for structural social capital are more mixed and culturally specific, with some aspects associated with increased odds of CMD. This suggests that structural social capital has context-specific effects and cognitive social capital more universal effects on maternal CMD.

This paper presents a case study to illustrate the range of decisions involved in designing a sampling strategy for a complex, longitudinal research study. It is based on experience from the Young Lives project and identifies the approaches used to sample children for longitudinal follow‐up in four less developed countries (LDCs). The rationale for decisions made and the resulting benefits, and limitations, of the approaches adopted are discussed. Of particular importance is the choice of sampling approach to yield useful analysis; specific examples are presented of how this informed the design of the Young Lives sampling strategy.

OBJECTIVE: To assess the occurrence of child injury in four developing country settings and to explore potential risk factors for injury. METHODS: Injury occurrence was studied in cohorts of 2000 children of age 6-17 months at enrolment, in each of Ethiopia, Peru, Vietnam and India (Andhra Pradesh). Generalized estimating equation models were used to explore potential risk factors for child injury. RESULTS: Occurrence of child injury was high in all countries. Caregiver depression emerged as a consistent risk factor for all types of injury measured (burns, serious falls, broken bones and near-fatal injury) across all countries. Other risk factors also showed consistent associations, including long-term child health problems, region of residence and the regular care of the child by a non-household member. CONCLUSIONS: This report provides further evidence of the importance of childhood injury in developing countries and emphasizes the importance of including infants in injury research and prevention strategies. It provides strong evidence of an association between caregiver mental health and child injury risk and contributes to the limited knowledge base on risk factors for child injury in developing countries.

The fetal origins hypothesis states that nutritional deprivation in utero affects fetal development and contributes to the incidence of diseases associated with the metabolic syndrome in later life. This study investigated whether haemoglobin (Hb) A(1c), an indicator of blood glucose, varied among healthy male adolescents according to their fetal growth rate, in a middle-income setting. Participants were men aged 18 years, belonging to the 1982 Pelotas birth cohort. Complete data, including gestational age and Hb A(1c) at age 18 years, were available for 197 individuals. There was an inverse association between mean Hb A(1c) and birthweight for the gestational age, but not birthweight alone. The association remained significant after adjustment for family income and mother's education, as well as for body mass index at 18 years (P for trend=0.01 and 0.03, respectively).

Social capital is a relatively new concept which has attracted significant attention in recent years. No consensus has yet been reached on how to measure social capital, resulting in a large number of different tools available. While psychometric validation methods such as factor analysis have been used by a few studies to assess the internal validity of some tools, these techniques rely on data already collected by the tool and are therefore not capable of eliciting what the questions are actually measuring. The Young Lives (YL) study includes quantitative measures of caregiver's social capital in four countries (Vietnam, Peru, Ethiopia, and India) using a short version of the Adapted Social Capital Assessment Tool (SASCAT). A range of different psychometric methods including factor analysis were used to evaluate the construct validity of SASCAT in Peru and Vietnam. In addition, qualitative cognitive interviews with 20 respondents from Peru and 24 respondents from Vietnam were conducted to explore what each question is actually measuring. We argue that psychometric validation techniques alone are not sufficient to adequately validate multi-faceted social capital tools for use in different cultural settings. Psychometric techniques show SASCAT to be a valid tool reflecting known constructs and displaying postulated links with other variables. However, results from the cognitive interviews present a more mixed picture with some questions being appropriately interpreted by respondents, and others displaying significant differences between what the researchers intended them to measure and what they actually do. Using evidence from a range of methods of assessing validity has enabled the modification of an existing instrument into a valid and low cost tool designed to measure social capital within larger surveys in Peru and Vietnam, with the potential for use in other developing countries following local piloting and cultural adaptation of the tool.

Social capital is broadly defined as the 'value' of social relationships. A substantial amount of work has been conducted into the role of social capital, but little of it has focused on Latin America and Peru in particular. Standard tools are often used to measure social capital in different cultural settings. We argue that while there are common themes, the exact nature of social capital is dependent upon the specific norms of behaviour, networks and organizations that characterize a given setting. Social capital is a multidimensional and culturally specific concept. The contribution that different organizations and networks make to different aspects of social capital varies by setting (rural/urban), person involved (male/female), and over time, highlighting the need for culturally sensitive measures. Two approaches are used to describe the characteristics of social capital in Peru: a literature review of research on social capital in Peru, and qualitative in-depth interviews with twenty respondents from one rural and two urban areas of Peru.

Aim: To identify biological and environmental factors associated with poorer mental and motor development at age 12 mo in urban communities in northeast Brazil. Methods: A cohort of 245 infants born during January-August 1998 in six hospitals in the interior of Pernambuco was followed twice weekly from birth until 12 mo of age. Socio-economic, demographic and environmental data were collected, together with daily information on morbidity and feeding patterns. Gestational age, birth anthropometry and nutritional status at 12 mo were measured. Multiple linear regression analysis was used to identify variables that had independent effects on mental and motor development assessed at 12 mo of age with the Bayley Scales of Infant Development. Results: Environmental factors explained about 21% and 19% of the variance in mental and motor development, respectively. Of these, the most important were poverty-related. Significant biological factors associated with mental development were birthweight and infant sex. For motor development, the biological factors were weight-for-age and haemoglobin concentration. Biological factors explained only 6% and 5% of the variance in mental and motor development, respectively. Conclusion: Of the variables examined, environmental factors had a greater detrimental effect on child development than biological factors in this population. Interventions that enhance social capital and alleviate poverty are advocated.

OBJETIVOS: apresentar as características socioeconômicas e demográficas, o perfil nutricional de crianças ao nascer e aos 12 meses de vida, além dos dados longitudinais sobre aleitamento, diarréia e situação vacinal durante o primeiro ano de vida. MÉTODOS: uma amostra de 652 recém-nascidos foi recrutada de setembro de 1997 a agosto de 1998 e acompanhada durante os primeiros 18 meses de vida. Essas crianças residiam nas áreas urbanas de quatro municípios da zona da mata meridional de Pernambuco. A coleta de dados foi realizada através de visitas domiciliares. RESULTADOS: cerca de 60% das famílias tinham uma renda per capita £ ½ salário mínimo e 41% das mães referiam menos de quatro anos de escolaridade. A mediana do aleitamento materno exclusivo e total foi de 0 dias e 94 dias, respectivamente. Moraes.pdf: 689039 bytes, checksum: 916e0ec7587f749ff5f5172d60d34222 (MD5) Previous issue date: 2003 A longitudinal prospective study of the effect of drainage and sewerage systems on diarrhoea in children aged < 5 years was conducted in 9 poor urban areas of the city of Salvador (population 2.44 million) in north-east Brazil in 1989-90. Due to complex political and administrative reasons, 3 areas had benefited from drainage improvements, 3 from both drainage and sewerage improvements, and 3 from neither. An extensive questionnaire was applied to collect information on each child and on the conditions of the household, and mothers recorded diarrhoea episodes in their children aged < 5 years daily for 1 year, using calendars. Fortnightly home visits were made to collect the data. The incidence of diarrhoea in children in neighbourhoods with drainage was less than two-thirds, and in neighbourhoods with drainage and sewerage less than one-third, of the incidence in neighbourhoods with neither. After controlling for potential confounders, the proportion of children with ‘frequent diarrhoea’ showed the same significant trend across the study groups. Though the groups were not exactly comparable, more than one child was monitored per household, and it was not possible to rotate field-workers between study groups, the study provides evidence that community sanitation can have an impact on diarrhoeal disease, even without measures to promote hygiene behaviour.

In the developed world the importance of evidence-based policy is increasingly recognised. Panel studies – a long-term study design where a cross-sectional sample of units is selected and surveyed at regular intervals – are being used to gather information about the same people or communities over a number of years. Panel studies distinguish between transitory and persistent states and help explain relationships between variables, such as health, age, and education. They are, however, complex and costly to undertake. A growing number of developing countries are now implementing or considering starting panel studies. This paper identifies challenges that arise in panel studies and shows possible ways to address them when resources are scarce. It discusses how to develop a conceptual framework which links macro and micro contexts; cost-effective methods of sampling; how to track individuals and the importance of ethics and data management and analysis. Panel studies require long-term funding and a stable institutional environment. Policy-makers must realise that in its early stages a panel study will not yield results. Insights are derived only after the study has been going for several years.

Exclusive breastfeedingis rare in Bangladesh. About 90% ofwomen have home deliveries, so the Baby-Friendly Hospital Initiative has no mechanism to reach them. Mother support groups do not exist, and community healthworkers do not have time to promote and support exclusive breastfeeding. To provide this kind of support at the community level, an area in Dhakawas selected for a peer-counselinginterv ention program. Usingcertain selection criteria, 1 woman from each community was trained as a peer counselor. The trainingw as based on the World Health Organization/United Nations International Children's Emergency Fund 40-hour breastfeedingcounselingcourse and related books. Counselingskills were taught usingdemonstrations and role play, followed by practical trainingin the project area. The intervention was very successful, as 70% of the mothers in the project area breastfed their infants exclusively for 5 months compared to only 6% in the control area. The authors describe the peer counseling training, strategies used for peer counseling visits, and lessons learned.

Summary Background Most mothers breastfeed in Bangladesh, but they rarely practise exclusive breastfeeding. Hospital-based strategies for breastfeeding promotion cannot reach them because about 95% have home deliveries. We postulated that with the intervention of trained peer counsellors, mothers could be enabled to breastfeed exclusively for the recommended duration of 5 months. Methods 40 adjacent zones in Dhaka were randomised to intervention or control groups. Women were enrolled during the last trimester of pregnancy between February and December, 1996. In the intervention group, 15 home-based counselling visits were scheduled, with two visits in the last trimester, three early postpartum (within 48 h, on day 5, between days 10 and 14), and fortnightly thereafter until the infant was 5 months old. Peer counsellors were local mothers who received 10 days' training. Findings 363 women were enrolled in each group. Peer counselling significantly imporved breastfeeding practices. For the primary outcome, the prevalence of exclusive breastfeeding at 5 months was 202/228 (70%) for the intervention group and 17/285 (6%) for the control group (difference=64%; 95% Cl 57%–71%, p>0·0001). For the secondary outcomes, mothers in the intervention group initiated breastfeeding earlier than control mothers and were less likely to give prelacteal and postlacteal foods. At day 4, significantly more mothers in the intervention group breastfed exclusively than controls. Interpretation Peer counsellors can effectively increase the initiation and duration of exclusive breastfeeding. We recommend incorporation of peer counsellors in mother and child health programmes in developing countries.

Several million children are killed each year by diarrhoeal diseases; preventive strategies appropriate for developing countries are vital. Despite strong circumstantial evidence that flies are vectors of diarrhoeal diseases, no convincing studies of the impact of fly control on diarrhoea incidence in developing countries have been reported. We undertook a randomised study of the effect of insecticide spraying on diarrhoea incidence.Six study villages were randomly assigned to two groups. Flies were controlled through insecticide application in group A in 1995 and in group B in 1996. In 1997 the effectiveness of baited fly traps was tested in group A villages. Diarrhoea episodes were monitored in children under 5 years through mothers' reports during weekly visits by a health visitor. Fly density was monitored by use of sticky fly-papers hung in sentinel compounds.During the fly seasons (March-June) of both 1995 and 1996, insecticide application practically eliminated the fly population in the treated villages. The incidence of diarrhoea was lower in the sprayed villages than in the unsprayed villages in both 1995 (mean episodes per child-year 6.3 vs 7.1) and 1996 (4.4 vs 6.5); the reduction in incidence was 23% (95% CI 11-33, p=0.007). At times other than the fly season there was no evidence of a difference, in diarrhoea morbidity between sprayed and unsprayed villages. Fly density data for 1997 indicate the ineffectiveness of baited traps in this setting.Fly control can have an impact on diarrhoea incidence similar to, or greater than, that of the interventions currently recommended by WHO for inclusion in diarrhoeal disease control programmes in developing countries. This important finding needs confirmation in other settings in developing countries. The incidence of diarrhea was lower in the sprayed villages than in the unsprayed villages in both 1995 and 1996, for an overall 23% reduction in incidence. This paper describes a methodology to design feasible interventions to improve weaning food hygiene practices of families living in extreme poverty. Educational messages to promote specific behavioural changes were defined and tested by utilizing a combination of ethnographic, survey and observational methods, and integrating viewpoints and suggestions of mothers and caretakers into the decision-making process. This new approach culminated in a household trial in which five groups, each of 15 non-practising mothers, were invited to adopt defined behaviours (handwashing before and after defined events, boiling water for reconstituting powdered milk, feeding gruel by spoon rather than bottlefeeding, not storing gruels and milks, and all four together). All initiated the advocated behaviours and most (53-80%) sustained the new behaviours and practised them every time during a one-month period. Of the four advocated behaviours, spoon-feeding was the most difficult to adopt wholly. The methodology was developed in response to the high priority given to reducing weaning food contamination for diarrhoeal disease control, and the lack of any existing methodology for defining appropriate educational interventions in resource-poor regions. This approach, with its combination of qualitative and quantitative methods and community focus, is recommended for future studies to design hygiene and other health education interventions in developing countries.

Objectives: To measure the institutional maternal mortality ratio (MMR) in Mali and suggest ways to reduce it. Methods: Routinely recorded data from 24 health institutions in three regions were reviewed for 1988 to 1992. Results: The overall MMR in the institutions was 201 maternal deaths per 100 000 live births. Hemorrhage, toxemia and infections accounted for 80% of the 360 recorded maternal deaths, almost all of which were preventable. The main reasons why these conditions result in death lie in poor quality and maldistribution of health services, lack of transport and late use of allopathic services. Conclusions: Maternal mortality is still a major public health problem in Mali, even among the small proportion of women who reach health facilities. Substantial new initiatives are urgently needed to reduce this major cause of preventable adult female mortality.

A case-control study of risk factors of clinical marasmus was undertaken to guide intervention efforts in rural Bangladesh. Cases were children whose mid-upper arm circumference measured110 mm and controls were children matched for age and sex with arm circumference120 mm. Between June 1988 and June 1989, 164 such pairs of children aged 1-4 years were studied. The effects of various demographic, socioeconomic, environmental, and health factors, reported by mothers, were investigated in a multivariate analysis using conditional logistic regression. Results showed an increased risk of marasmus among children from families with other children under 5 years of age (odds ratio [OR] = 2.51; 95% confidence interval [CI]: 1.33-4.74), and children who consumed formula foods (OR = 16.41, 95% CI: 3.39-79.36). Higher maternal education was associated with reduced risk of marasmus, compared with no education, the OR for5 years of schooling = 0.57, 95% CI: 0.23-1.41; OR foror = 5 years of schooling = 0.34, 95% CI: 0.15-0.76. The strong association of childhood marasmus with mother's education and child spacing supports the notion that non-nutritional factors should be essential components of efforts to reduce severe malnutrition in Bangladesh.

The wealth index is a commonly-used measure of socio-economic position (SEP) in low- and middle-income settings, but there is concern that it is strongly influenced by community-level as well as household-level factors. Subjective SEP indicators are infrequently used in health research.We use data from 11 280 households included in the Malawi Integrated Household Survey 2004/5. We compare the wealth index with four subjective measures of SEP: perceived food consumption adequacy, perceived overall consumption adequacy, an economic ladder question, and perceived income sufficiency. The wealth index is compared with each subjective SEP measure in terms of: (i) agreement of classification of households, (ii) targeting accuracy with respect to US$1-a-day poverty based on consumption expenditure, and (iii) the socio-economic processes (household- and community-level) giving rise to the SEP scores.Each subjective SEP indicator resulted in considerable differential classification of households compared with the wealth index. Three measures of subjective SEP (perceived food consumption adequacy, economic ladder question, and perceived income sufficiency) identified a higher proportion of dollar-a-day poor households as poor than the wealth index. The wealth index was strongly influenced by community infrastructure, but all subjective SEP indicators were free from strong community-level influence.The strengths and limitations of any measure of SEP depend on the context and purpose for which it is being used. In these data, the wealth index was strongly influenced by community infrastructure, whereas the subjective SEP measures were not, perhaps allowing analyses using them to disentangle household and community influences. Several subjective measures also corresponded to dollar-a-day poverty more strongly than the wealth index. Subjective measures may therefore be preferable to the wealth index in some circumstances, although they have their own set of potential biases.

In a population-based study, all infant deaths occurring in a one-year period in the metropolitan areas of Porto Alegre and Pelotas, in southern Brazil, were studied. There were 227 infants who presented diarrhoea during the fatal illness, and in 75% of these diarrhoea was considered to be the underlying cause of death. Acute diarrhoea (14 days' duration) accounted for 28% of the deaths, persistent diarrhoea for 62% and dysentery for a further 10%. Approximately one-half of the children with persistent diarrhoea were admitted to a hospital in the first two weeks of the episode. Hospital-acquired infections were likely to have contributed to one- to two-thirds of deaths due to dysentery and persistent diarrhoea. A comparison with neighbourhood controls showed that breast milk provided substantial protection against deaths due to either acute or persistent diarrhoea.

To determine whether clinical marasmus occurs in small groups of children from easily recognizable high-risk households, the authors conducted a case-control study to identify risk indicators that may be used in targeted interventions. Cases were children whose mid-upper arm circumference measured less than 110 mm, and controls were children matched for age and sex with arm circumferences greater than 120 mm. Between June 1988 and June 1989, 164 such pairs of children aged 1-4 years were studied in Matlab, Bangladesh. Conditional logistic regression analysis showed an increased risk of marasmus among children from families with other children under 5 years of age (odds ratio = 2.80, 95% confidence interval 1.56-5.02) and children who consumed formula foods (odds ratio = 18.81, 95% confidence interval 4.15-85). Higher maternal education was associated with reduced risk of marasmus. Further examination of these risk indicators suggests that the resources saved through targeting fewer households will be negated by missing many children with marasmus. The authors conclude that the application of targeted interventions against marasmus, using the risk approach, is unlikely to be efficient.

In a population-based cohort of approximately 6000 Brazilian children, the associations between maternal education and a number of child health outcomes were studied while controlling for potentially confounding variables such as family income and education of the husband. In the crude analyses, maternal education was associated with perinatal and infant mortality, hospital admissions in the first 20 months of life and the three nutritional indicators (length-for-age, weight-for-age and weight-for-length) at mean age 20 months. After adjustment for confounding, the apparent associations with outcomes in early infancy--birthweight and perinatal mortality--were no longer present, while that with infant mortality persisted despite being reduced. Strong associations remained with later outcomes including hospital admissions, length-for-age and weight-for-age at mean age 20 months. Among infants born to women with little or no schooling, deaths due to diarrhoea, pneumonia and other infectious diseases were particularly common. These findings support the hypothesis that maternal education has an effect on child health which is partly independent from that of other socioeconomic factors; they also suggest that maternal care is more important than the biological characteristics of the mothers since stronger effects were observed for the late (postneonatal mortality, hospital admissions and nutritional status) than for the early (birthweight, perinatal mortality) outcomes.

Background: Many epidemiological studies require a measure of socioeconomic position. The monetary measure preferred by economists is consumption expenditure; the wealth index has been proposed as a reliable, simple alternative to expenditure and is extensively used. Methods: A systematic review was conducted of the agreement between wealth indices and consumption expenditure, summarising the agreement and exploring factors affecting agreement. Results: Seventeen studies using 36 datasets met the inclusion criteria. Of these, 22 demonstrated weak agreement, 10 moderate agreement, and four strong agreement. There was some evidence that agreement is higher: in middle-income settings; in urban areas; for wealth indices with a greater number of indicators; and for wealth indices including a wider range of indicators. Conclusions: The wealth index is mostly a poor proxy for consumption expenditure.

We examined the association between prolonged breastfeeding and anthropometric status in a population-based cohort study of 5,914 liveborns from the city of Pelotas in Southern Brazil. When children from all socioeconomic groups were studied, there was no important association between current breastfeeding and anthropometric status at age 12 months. Children who were still breastfed at age 20 months--and, to a lesser extent, at 43 months--presented with poorer anthropometric status than their nonbreastfed counterparts. We did not find the same pattern in all socioeconomic groups, however. Children from low-income families who were breastfed tended to present better anthropometric status than those who were not, whereas the reverse was observed for children of middle- and high-income families. After controlling for confounding variables, the nutritional advantage of breastfeeding among low-income families was no longer clear, while the superiority of nonbreastfed infants amongst middle- and high-income children persisted. These findings indicate that some of the controversy regarding the nutritional effects of prolonged breastfeeding may have been caused by confounding and effect modification. Any decisions on whether or not breastfeeding should be encouraged after the first year of life should take into account the characteristics of the population as well as the anti-infective and birth-spacing properties of breastfeeding.

The problem of recall bias when reporting breast feeding duration is investigated. Data are presented from the follow-up of over 1,000 children from a birth cohort (1982) in southern Brazil, when they were on average 11, 23, and 47 months of age. Compared with the response given at 11 months of age, 24% of mothers misclassified the duration (grouped into 3-month categories) at age 23 months and 30% at age 47 months. Women who were richer and/or better educated were significantly more likely to report longer durations, while those poorer and less educated did not tend to misclassify more in one direction than in the other.

The impact of a water, sanitation and hygiene education intervention project on diarrhoeal morbidity in children under 5 years old was evaluated in a rural area of Bangladesh. Data were collected throughout 1984-1987, covering both pre- and post-intervention periods, from an intervention and a control area. The 2 areas were similar with respect to most socio-economic characteristics and baseline levels of diarrhoeal morbidity. The project showed a striking impact on the incidence of all cases of diarrhoea, including dysentery and persistent diarrhoea. By the end of the study period, children in the intervention area were experiencing 25% fewer episodes of diarrhoea than those in the control area. This impact was evident throughout the year, but particularly in the monsoon season, and in all age groups except those less than 6 months old. Within the intervention area, children from households living closer to handpumps or where better sanitation habits were practised experienced lower rates of diarrhoea. These results suggest that an integrated approach to environmental interventions can have a significant impact on diarrhoeal morbidity.

SUMMARYThe aim of this study was to determine what weaning foods and food preparation practices expose children to a high risk of diarrhoeal disease through exposure to a contaminated diet. Bacterial contamination of 897 food and 896 drinking water samples was assessed in a water and santitation intervention project.The geometric mean of faecal coliforms per g or ml was 7·5 x 103 in left-over rice, 1·4 x 102 in other types of boiled rice, 2·5 x 102 in milk, 4·8 in household drinking water, and 3·5 in bread. Multiplication of faecal coliforms occurred when there was a delay of more than 4 h between preparation and consumption of food. All samples were more contaminated in the rainy than in the dry season. Strategies to reduce contamination should therefore focus on ‘wet’ foods, early consumption after preparation, and re–heating of left–over foods. Understanding the reasons for the faulty practices is also essential to the formulation of effective measures.

Petrakova and Sadana make an important distinction between the science and the art of public health, where the art is concerned with application. However, while it is correct to say there is still much to be learned about how to deliver public health interventions, there is a growing body of research on health systems and policies that helps to guide the delivery of preventive and curative services at different levels of socioeconomic development.1 Schools of public health should therefore aim to address health systems and policies through research and teaching as well as through the traditional public health approaches to understanding the causation of disease, the determinants of health and the evaluation of specific interventions.

Objective: To test the hypothesis that maternal common mental disorders (CMD) are associated with poorer child nutritional status in four developing countries (Ethiopia, India, Vietnam, and Peru). Design: Community based cross sectional survey in 20 sites in each of the four countries. Maternal CMD measured by the self reporting questionnaire 20 items (SRQ20). Potential confounding factors include: household poverty, household composition, maternal characteristics such as age and education, child characteristics such as birth weight, age, and sex. Possible mediating factors included the child’s physical health and breast feeding status. Setting: Urban and rural, poor and middle income areas in each country. Participants: 2000 mothers and their children aged 6–18 months in each country. Main outcome measures: Child stunting and underweight measured using standard anthropometric techniques. Results: Levels of maternal CMD and child malnutrition are high in each study setting. After adjusting for confounding factors, the odds ratios (OR) for the association of maternal CMD with child stunting are: India 1.4 (95%CI 1.2 to 1.6), Peru 1.1 (0.9 to 1.4), Vietnam 1.3 (0.9 to 1.7), and Ethiopia 0.9 (0.7 to 1.2). For child underweight, the confounder adjusted ORs are: India 1.1 (0.9 to 1.4), Peru 0.9 (0.6 to 1.2), Vietnam 1.4 (1.1 to 1.8), and Ethiopia 1.1 (0.9 to 1.4). No clear evidence for effect modification by the child’s age or sex was found. Possible mediating factors for the effect of maternal CMD on child malnutrition did not provide strong suggestions for potential mechanisms. Conclusions: There was a relation between high maternal CMD and poor child nutritional status in India and Vietnam. However, the findings from Peru and Ethiopia do not provide clear evidence for a similar association being present in non-Asian countries. Regardless of the direction of the relation, child nutrition programmes in Asia should consider incorporating promotion of maternal mental health.

Study objective: The concept of social capital has influenced mental health policies of nations and international organisations despite its limited evidence base. This papers aims to systematically review quantitative studies examining the association between social capital and mental illness. Design and setting: Twenty electronic databases and the reference sections of papers were searched to identify published studies. Authors of papers were contacted for unpublished work. Anonymised papers were reviewed by the authors of this paper. Papers with a validated mental illness outcome and an exposure variable agreed as measuring social capital were included. No limitations were put on date or language of publication. Main results: Twenty one studies met the inclusion criteria for the review. Fourteen measured social capital at the individual level and seven at an ecological level. The former offered evidence for an inverse relation between cognitive social capital and common mental disorders. There was moderate evidence for an inverse relation between cognitive social capital and child mental illness, and combined measures of social capital and common mental disorders. The seven ecological studies were diverse in methodology, populations investigated, and mental illness outcomes, making them difficult to summarise. Conclusions: Individual and ecological social capital may measure different aspects of the social environment. Current evidence is inadequate to inform the development of specific social capital interventions to combat mental illness.

Si bien en el Perú el índice de matrícula en las escuelas primarias es muy alto, más de la mitad de los niños que asisten a la escuela primaria se encuentra uno o más grados por debajo de la media para su edad. Además, las evaluaciones han demostrado que, si se someten a prueba, los escolares peruanos se ubican bastante por debajo de lo que se espera para su edad. Sus puntajes se encuentran también por debajo de los niveles promedio de países que atraviesan circunstancias socioeconómicas similares. No se ha estudiado el papel del capital social1 en explicar estos hallazgos, aunque la investigación efectuada en EE.UU. ha sugerido asociaciones positivas entre capital social y rendimiento educativo. Las organizaciones sociales que se concentran en el cuidado infantil son un ejemplo de recursos de redes sociales sólidas en el Perú. El estudio de Niños del Milenio ofrece una oportunidad para investigar si el capital social se asocial con el progreso y el rendimiento a nivel educativo.

La salud del niño, en particular su estado nutricional de largo plazo, está estrechamente relacionado con las características de las familias, las comunidades y los niños, incluida la educación de la madre y el acceso a los servicios públicos. Ahora bien, el acceso a los servicios públicos puede tener efectos diferentes en los niños, dependiendo de las características de la comunidad y la familia, específicamente, de la educación de la madre o cuidador. Pese al gran aumento en la inversión en salud en el Perú durante la última década, el acceso a ese servicio todavía es muy desigual. Este trabajo utiliza datos del estudio de largo plazo Niños del Milenio para explorar la interacción de los bienes privados, como la educación, con algunos servicios públicos, a nivel comunitario y familiar. Los autores analizan la relación entre la educación materna y el acceso a agua potable y la disponibilidad de sistema de aguas residuales, así como la calidad de los servicios de salud, la proximidad a las carreteras pavimentadas o de ingeniería, y el acceso a la electricidad. El estudio también observa el impacto directo e indirecto de la educación de la madre en las mediciones de talla por edad de sus hijos. El estudió halló que la educación de la madre es un factor determinante de la condición nutricional de los niños. Sólo en el caso de madres que son más educadas y tienen acceso a los servicios no hay una diferencia significativa con el estado nutricional. Estos resultados comprueban el argumento de que la inversión en servicios públicos es aún más necesaria en las zonas de madres con poca educación.

Los hogares requieren diversificar sus fuentes de ingreso durante crisis económicas, pero la población pobre suele carecer de recursos para afrontar estas situaciones de emergencia. Peor aún es el caso de los hogares con un recién nacido, puesto que tienen dos restricciones más simultáneas: la capacidad de generar ingresos de parte de las madres se reduce y, al mismo tiempo, las familias tienen más gastos por el bebe. Sin embargo, mayor investigación muestra que las madres urbanas que dejan a sus niños en centros de cuidado temporal pueden seguir trabajando y obtener dinero. Por ello, si eventualmente las familias no tuvieran dicha posibilidad, se podrían generar graves consecuencias para el bienestar del infante, justamente en un momento crítico en que necesita desarrollarse. En el caso de las madres rurales, para que ellas generen más ingresos, no basta con dejar a sus niños a cuidado externo: necesitan recibir capacitación. Este hallazgo sustentaría que se promueva una política pública que permita trabajar a las madres de niños pequeños. En este sentido, ameritaría examinar la mejor manera de ampliar la cobertura del programa Wawa Wasi, que tiene impactos positivos, pero es insuficientemente financiado.

This paper describes the design, implementation and evaluation of an intervention to promote improved hygiene practices in a shanty town area of Lima, Peru. Following formative research, the intervention focused on behaviors associated with the hygienic use of potties by children aged 15-47 months and keeping the home environment free from feces. A health communications strategy was developed involving delivery through routine health services, and using video presentations, leaflets and counseling by health staff during consultations. Intervention activities occurred during a 6-month period in health centers and posts of four intervention communities; four other communities acted as a comparison group. Process and impact indicators were measured through questionnaires and 4-h structured observations conducted in over 600 households both pre- and post-intervention implementation, and through intervention monitoring activities. The intervention materials and approach were well received by the study community; however, in the time-frame of the project only limited coverage of the target audience was achieved, which was insufficient to result in an impact on behaviors. Nevertheless sufficient positive features existed to suggest that with higher coverage, an impact on target behaviors might have been achieved. Reasons for the intervention's shortcomings are discussed and suggestions made for more effective implementation.

Objectives. To describe breastfeeding practices from 0 to 12 months of age in 4 small towns that are representative of urban northeast Brazil and to identify factors associated with introduction of other milk in the first month of life. Methods. From January to August 1998, 364 mothers were interviewed at delivery to ascertain antenatal care; delivery room practices; and their intentions regarding breastfeeding, pacifiers, and introduction of water, teas, and other milk. Their perceptions of home support and the advantages of breastfeeding also were assessed. Thereafter, daily information about feeding practices was collected at twice-weekly home visits. When other milk was started, a second interview was conducted to ascertain initial and current breastfeeding problems and use of a pacifier. Reasons for starting other milk were investigated using 5-point Likert scales. Results. Mothers were positive toward breastfeeding, and 99% breastfed their new infant. Few intended to breastfeed exclusively, and in the first week 80% gave water/tea and 56% used a pacifier. The median duration of exclusive breastfeeding was 0 days, and the median age for starting other milk was 24 days. The median duration of breastfeeding was 65 days for mothers who started other milk within 1 month and 165 days for other mothers. After adjustment for confounding variables, the main factors associated with introduction of other milk within 1 month were pacifier use in the first week (odds ratio [OR], 4.01; 95% confidence interval [CI]: 2.07–7.78), intention to start other milk in the first month (OR, 3.79; 95% CI: 1.74–8.24), giving water/tea in the first week (OR, 3.07; 95% CI: 1.56–6.03), and leaving the maternity ward before breastfeeding was started (OR, 2.59; 95% CI: 1.34–5.04). Conclusion. Although breastfeeding is common in this community, it rarely is exclusive and takes place for a relatively short duration. Identification of risk factors for early introduction of other milk offers potential avenues for future intervention, including improvement of breastfeeding support in antenatal and maternity services.

Brazil has one of the highest rates of caesarean section in the world. Patterns of caesarean sections were studied in a cohort of 5960 mothers followed from 1982 to 1986 in southern Brazil. Overall, 27.9% were delivered by caesarean section in 1982, this proportion being 30% for nulliparae, 80% for second deliveries when the first was by caesarean, and over 99% for third births when the first two were by caesarean. Socioeconomic status and requests for sterilisation by tubal ligation were important underlying factors. 9.4% of the women were sterilised during a caesarean section (3.7% in the lowest income group and 20.2% in the highest). 31% of women who had had their first child by a caesarean section and who were having a second operative delivery were sterilised. The high rates of caesarean sections and accompanying sterilisations reflect the lack of appropriate reproductive and contraceptive policies in the country.

Little is known about feces disposal practices, their determinants and feasibility for change, despite their importance in the control of diarrheal diseases. We report here the results of formative research for the development of an intervention to promote sanitary disposal of feces of young children. The study was conducted in a densely populated shanty town area of Lima, where water and sanitation systems are scarce. In-depth interviews were undertaken with mothers, husbands and community leaders. Group discussions were held with mothers in order to validate findings from the interviews, investigate particular topics further and explore reactions to possible intervention strategies. The principal defecation sites for young children were diapers, potties, the ground in or near the home, the hill, latrines and flush toilets. The main determinants found were the age of the child, the effort required by the method, perceptions of dirtiness and the availability of resources. Almost all children under one year of age use diapers but the high resource cost of diaper washing is a strong motivation for mothers to move their children on as early as possible. Potties were considered the most socially acceptable and `hygienic' defecation method for children between one and three years of age. Nevertheless, defecation directly onto the ground is common at this age. Potty training is deemed to be quite difficult and the long term achievements are determined by the initial training success. In most cases, the training process is authoritative and inconsistent. The use of latrines and flush toilets is not considered appropriate for children until they are three to four years old. Based on these initial findings, a micro-trial was conducted to assess the feasibility and acceptability of promoting greater use of potties and associated practices. The results of the trial were very encouraging and provided valuable information for the design of a community-wide intervention. Our findings help explain why the emphasis given in most sanitation projects, where efforts have been concentrated on the promotion of latrines, has failed to induce their utilization by small children. Sanitation projects should incorporate interventions that will promote hygienic defecation and stool clearance practices for infants and small children.

Epidemiological publications on the relationship of caffeine to birth weight and duration of human pregnancy, from 1966 to 1995, were searched through Medline. Each study was treated as the stratification variable, and its weight in the weighted average was proportional to the inverse of its variance. Twenty-six studies were located. Among the twenty-two studies on birth weight, eleven were on mean birth weight, nine on low birth weight (LBW), and four on intrauterine growth retardation (IUGR). Combined analysis of mean birth weigh study results showed a significant decrease in birth weight of nearly 43g among newborns of the heaviest caffeine-consuming mothers. LBW, IUGR, and preterm delivery displayed significant homogeneity in the test results, indicating that a pooled estimate should not be taken as an adequate measure. The high heterogeneity of the available literature on the effects of caffeine on LBW, IUGR, and preterm delivery prevents estimation of reliable pooled estimates through meta-analysis. Further assessment of caffeine intake during pregnancy is needed in future research. Cada estudo foi tratado como uma categoria de uma variável e seu peso foi proporcional ao inverso de sua variância. Foram localizados vinte e seis estudos. Entre os vinte e dois estudos sobre peso ao nascer, onze foram sobre peso médio ao nascer, nove sobre baixo peso ao nascer (BPN) e quatro sobre retardo do crescimento intra-uterino (RCIU). O efeito agregado sobre o peso médio ao nascer mostrou uma redução estatisticamente significativa de 43 gramas entre os recém-nascidos de mães que consumiam maiores quantidades de cafeína. A análise agregada do efeito sobre BPN, RCIU e nascimentos pré-termos apresentou teste de homegeneidade estatisticamente significativo, indicando que uma estimativa combinada não seria confiável. A grande heterogeneidade da literatura disponível quanto ao efeito da cafeína sobre o BPN, RCIU e partos pré-termo não permite o cálculo confiável de estimativas agrupadas através de meta-análise. Torna-se necessária uma avaliação mais cuidadosa do consumo de cafeína durante a gestação em estudos futuros.

Over 1300 severely malnourished children (60% of US National Center for Health Statistics weight-for-height, with edema, or both) are admitted each year to the Children's Nutrition Unit in Dhaka. Fatality during treatment is low and recovery is rapid. Our aim was to determine whether this initial success is sustained when children return home. A previous attempt to address this question was frustrated by the difficulty in tracing children after discharge because most are from slum settlements and families move frequently. This prospective study with fortnightly monitoring was therefore undertaken. The main outcomes of interest were anthropometric status, relapse, morbidity, and mortality. Children (n = 437) who had been treated for severe malnutrition when aged 12-59 mo and had reached the discharge criterion of 80% of weight-for-height, were followed for the next 12 mo. During follow-up, 7.5% were lost without trace, 0.6% relapsed, and 2.3% died. Morbidity was high, with a mean of seven episodes of diarrhea during the year. Outpatient visits for diarrhea occurred for 67% of children, and 58% had pneumonia (10% had pneumonia three times). After 12 mo, mean weight-for-height was 91% (-0.92 z score) but mean height-for-age remained at 84% (-4.14 z score). Weight gain, but not height gain, tended to be lower in children who experienced more diarrhea. Fever and cough were not associated with either weight or height gain. The high prevalence of illness highlights the need for continued accessible health care and for interventions to reduce disease acquisition.

The authors conducted a matched case-control study to investigate the effects of caffeine intake during pregnancy on birth weight. From January to November 1992, in the first 24 hours after delivery, 1,205 mothers (401 cases and 804 controls) were interviewed and their newborns were examined to assess birth weight and gestational age by means of the method of Capurro et al. (J Pediatr 1978;93:120-2). The cases were children with birth weight2,500 g and gestational ageor = 28 weeks. Cases and controls were matched for time of birth and hospital of delivery and were recruited from the four maternity hospitals in Pelotas, southern Brazil. Daily maternal caffeine intake during pregnancy for each trimester was estimated. To assess caffeine intake, 10% of the mothers were reinterviewed at their households and samples of reported information on drip coffee and maté (a caffeine-containing drink widely used in South America) were collected and sent to the laboratory for caffeine determination through liquid chromatography. When instant coffee was reported, the weight of powder was measured using a portable scale, and caffeine intake was estimated from a reference table. Caffeine intake from tea, chocolate, soft drinks, and medicines was estimated from a reference table. Analyses were performed by conditional logistic regression. Crude analyses showed no effect of caffeine on low birth weight, preterm births or intrauterine growth retardation. The results did not change after allowing for confounders.

Methodological issues in the design and interpretation of cross-sectional interview surveys of the prevalence of acute respiratory infections (ARI) were assessed among young children. A cross-sectional survey was conducted in approximately 20000 children in the north of Ghana. Approximately half were administered a questionnaire in which the initial questions about recent illnesses were direct questions about the presence or absence of 3 specific ARI-related symptoms (cough rapid breathing difficulty breathing) while the other half were administered a questionnaire which started with an open-ended questions on whether the child was ill designed to elicit spontaneous responses. A 2-week recall period was used in addition to point prevalence questions for half of the children in each group while 4 weeks was used for the other half. The results were compared with those from a longitudinal morbidity surveillance system in an adjacent population of children. The repeatability of the responses to each of the symptoms/conditions was assessed in a subsample of the children. The point and period prevalence rates of ARI symptoms or conditions based on spontaneously elicited responses were more likely to be valid than those based on prompted responses. Furthermore using a 2-week recall period appeared to give more valid period prevalence rates than a 4-week recall period. The repeatability of the various ARI questions was not high (kappas 0.14 to 0.49) irrespective of the questionnaire design. Whether these findings will also be true in other populations needs to be assessed. (authors)
