Background: Pneumonia has a high incidence in traumatic brain injury (TBI) patients and lacks effective treatments. Early mobilization (EM) may be a potentially effective treatment. Aim: To explore the impact of EM on TBI‐related pneumonia in the neurosurgical intensive care unit (NICU). Study Design: This study was a historical control study. 100 TBI patients who received EM intervention were prospectively included as the experimental group (EM cohort), and 250 TBI patients were retrospectively included as the control group. The propensity score matching (PSM) method was employed to balance baseline and minimize potential bias. The relationship between EM and TBI‐related pneumonia was investigated by univariate and multivariate logistic regression, then further determined by subgroup analysis. The influence of other variables was excluded by interaction analyses. Finally, the effect of EM on the prognosis of TBI patients was analysed by comparing the Glasgow Coma Scale (GCS) and the hospital stay. Results: After screening, 86 patients were included in the EM cohort and 199 patients were included in the control cohort. There were obvious differences between the two cohorts at baseline, and these differences were eliminated after PSM, when the incidence of pneumonia was significantly lower in the EM cohort than in the control cohort (35.0% vs. 61.9%, p <.001). Multivariate logistic regression showed that EM was an independent risk factor for TBI‐related pneumonia and was significantly associated with a decreased incidence of pneumonia. This correlation was present in most subgroups and was not affected by other variables (p for interaction >.05). Patients in the EM cohort had shorter length of ICU stay (6 vs. 7 days, p =.017) and higher GCS at discharge (12 vs. 11, p =.010). Conclusion: EM is a safe and effective treatment for TBI patients in NICU, which can reduce the incidence of pneumonia, help to improve prognosis and shorten the length of ICU stay. Relevance to Clinical Practice: Although the utilization rate of EM is low in TBI patients for various reasons, EM is still an effective method to prevent complications. Our study confirms that a scientific and detailed EM strategy can effectively reduce the incidence of pneumonia while ensuring the safety of TBI patients, which is worthy of further research and clinical application. Increasingly, the traditional professional development (PD) model at 4-year institutions fails to meet the evolving needs of faculty. The traditional model of PD faces unique implementation challenges, including developing offerings that appeal to a wide range of disciplines and topics, dealing with limited faculty availability, physical/campus location barriers, and limited training staff. This article discusses a collaborative initiative across five State University of New York (SUNY) colleges and universities to develop a web-based, on-demand, self-directed PD opportunity that can easily be shared and replicated throughout SUNY and the world under a Creative Commons BY-NC-SA 2.5 license. The "Tools of Engagement Project: On-demand Discovery Learning Professional Development" (hereafter referred to as "TOEP") allows faculty to explore emerging technologies and Web 2.0 tools at their own pace through a variety of rich media and textual tutorials. As a part of the grant research, the interactive community of "TOEP" learners also reflect on how they could incorporate emerging technologies and Web 2.0 tools into their pedagogy, thereby creating a knowledge base of technology implementation.

Instructors have many tools to consider when offering online courses: chat rooms, discussion boards, and interactive Web environments, to name a few. The selection of applications for instructional purposes can be a complicated task as the number available grows. Not only must the application work, but it also must have a pedagogical purpose or serve an instructional requirement. The investigation reported in this article looked at synchronous online application selection in the e-conferencing environment. The authors studied 23 instructors who used e-conferencing in their courses for two semesters. The instructors were surveyed to determine which tools proved effective in accomplishing specific instructional tasks. Results from this study indicate that using e-conferencing tools for synchronous online instruction is effective. Results also point to the development of common techniques for using specific practices and e-conferencing tools to reinforce good instructional principles. Further exploration of this powerful tool set is necessary, and targeted research into the effective use of specific e-conferencing tools may lead to major improvements in the delivery of synchronous online education. By studying the combination of online tools provided by this software, specifically as it relates to specific learning/teaching objectives, researchers can improve the instructional value of its implementation. Background: After craniotomy, patients require rehabilitation efforts for the recovery process, and neurologists are mostly engaged for that (in the management of post-craniotomy complications). However, neurologists are not always available for care after neurosurgery during follow-up (situation of our institute). The objectives of the study were to compare the effects of two different types of care (nurse-led and neurologist-led) on various long-term outcomes in patients who have undergone craniotomy due to traumatic brain injuries., Methods: Electronic medical records of patients (aged ≥18 years) who underwent craniotomy for traumatic brain injuries and their caregivers were extracted and retrospectively reviewed. Patients received nurse-led care (NL cohort, n  = 109) or neurologist-led care (GL cohort, n  = 121) for 6 months after craniotomy., Results: Before the nurse-or neurologist-led care (BC), all patients had activities of daily living (ADL) ≤ 11, ≤ 50 quality of life (QoL), and 69% of patients had definitive anxiety, 87% of patients had definitive depression, and all caregivers had Zarit Burden interview scores ≥50. Nurse-led post-surgical care was associated with improved ADL and QoL, relieved anxiety and depression of patients, relieved the burden on caregivers, and the higher overall satisfaction of patients and their caregivers after 6-months of care (AC) as compared to their BC condition ( p  < 0.05) and also compared to those of patients in the GL cohort under AC condition ( p  < 0.01). Patients in the GL cohort reported pressure sores ( p  = 0.0211) and dizziness [15 (12%) vs. 5 (5%)] after craniotomy during follow-up than those in the NL cohort., Conclusion: ADL, QoL, and psychological conditions of patients who undergo craniotomy for traumatic brain injuries must be improved and the burdens of their caregivers must be relived. HER2-positive breast cancer is a highly heterogeneous tumor, and about 30% of patients still suffer from recurrence and metastasis after trastuzumab targeted therapy. Predicting individual prognosis is of great significance for the further development of precise therapy. With the continuous development of computer technology, more and more attention has been paid to computer-aided diagnosis and prognosis prediction based on Hematoxylin and Eosin (H&E) pathological images, which are available for all breast cancer patients undergone surgical treatment. In this study, we first enrolled 127 HER2-positive breast cancer patients with known recurrence and metastasis status from Cancer Hospital of the Chinese Academy of Medical Sciences. We then proposed a novel multimodal deep learning method integrating whole slide H&E images (WSIs) and clinical information to accurately assess the risk of relapse and metastasis in patients with HER2-positive breast cancer. Specifically, we obtained the whole H&E staining images from the surgical specimens of breast cancer patients, and these images were adjusted to size 512 × 512 pixels. The deep convolutional neural network (CNN) was applied to these images to retrieve image features, which were combined with the clinical data. Based on the combined features. After that, a novel multimodal model was constructed for predicting the prognosis of each patient. The model achieved an area under curve (AUC) of 0.76 in the two-fold cross-validation (CV). To further evaluate the performance of our model, we downloaded the data of all 123 HER2-positive breast cancer patients with available H&E image and known recurrence and metastasis status in The Cancer Genome Atlas (TCGA), which was severed as an independent testing data. Despite the huge differences in race and experimental strategies, our model achieved an AUC of 0.72 in the TCGA samples.
