Dyadic conditions of patients with heart failure and their caregivers may affect both patient self-care and caregiver contribution to patient self-care (CCPS). The purpose of this study was to examine the relationships of patient-caregiver physical function and depressive symptoms to the patient self-care (maintenance and management) and CCPS. Data from 55 were analyzed using an Actor-Partner Interdependence Model to address the aim through AMOS. Patient self-care was very poor. Better patient physical function was related to better patient self-care management (actor effect) and poorer CCPS maintenance (partner effect). Better caregiver physical function was related to CCPS management (actor effect). Severer patient depressive symptoms were related to poorer patient self-care maintenance (actor effect) and poorer CCPS management (partner effect). Physical function and depressive symptoms in patient-caregiver dyads were related to patient self-care and CCPS. To improve patient self-care and CCPS, dyadic support for physical function and depressive symptoms is needed.

Introduction: This study assessed the therapeutic effectiveness of a single-pill combination (SPC) of olmesartan/amlodipine plus rosuvastatin for blood pressure (BP) and low-density lipoprotein cholesterol (LDL-C) in patients with hypertension and dyslipidemia. Methods: Adult patients with hypertension and dyslipidemia who were decided to be treated with the study drug were eligible. The primary endpoint was the proportion of patients who achieved BP, LDL-C and both BP and LDL-C treatment goals at weeks 24–48. Secondary endpoints were assessed at weeks 24–48 and included changes in BP and LDL-C levels from baseline; the proportion of patients who achieved treatment goals who were initially classified as uncontrolled at baseline; changes and percent changes in lipid parameters; changes in both BP and LDL-C levels among patients who reached treatment goals who were followed for more than 24 weeks; and the overall safety profile. Results: A total of 5476 patients were enrolled, and 4411 patients comprised the effectiveness evaluation set. The proportions of patients who reached the treatment goals for BP, LDL-C levels, and both BP and LDL-C levels were 67.93% [95% confidence interval (CI) 66.52–69.32], 80.19% [95% CI 78.85–81.49], and 58.07% [95% CI 56.43–59.7], respectively. Secondary endpoints showed statistically significant changes. Overall, the treatment was well tolerated. Conclusions: The treatment of patients with hypertension and dyslipidemia with the olmesartan/amlodipine plus rosuvastatin SPC was associated with significant decreases in SBP/DBP and LDL-C levels, and a high proportion of patients achieved the BP and LDL-C treatment goals. The finding of this study is worthwhile in that this study evaluated the effectiveness and safety in a broad patient population representative of those seen in everyday clinical practice. Background: Hepatocellular carcinoma (HCC) is one of the most common malignancies and a leading cause of cancer-related mortality and morbidity worldwide. Objectives: This study evaluated the enhanced efficacy of a combination of herbal medicine with sorafenib (MIX) in a transplantation tumor model induced by HepG2 cells in HCC. Materials and Methods: The HepG2 cell transplantation tumor model was established by injecting HepG2 cells subcutaneously into BALB/c nude mice. The impact of MIX on tumor growth was observed. Granzyme B was used to measure natural killer (NK) cell activity. Prognostic markers of HCC, telomerase reverse transcriptase (TERT), and alpha-fetoprotein (AFP) were analyzed via real-time polymerase chain reaction. Additionally, inflammation and apoptosis-related markers were assessed through serum analysis and Western blotting. Results: Among the MIX groups, MIX25 (sorafenib 10 mg/kg + Dioscoreae rhizoma and Polygalae radix mixture 25 mg/kg) demonstrated the most significant effect. MIX25 effectively suppressed tumor volume and weight, enhanced apoptosis-promoting proteins (Bax, Cytochrome c, and Caspase-3), and reduced apoptosis-inhibiting proteins like survivin. Furthermore, MIX25 inhibited the mitogen-activated protein kinase inflammatory response and inflammatory markers, including tumor necrosis factor-alpha and cyclooxygenase-2. The MIX25 group exhibited superior effects on NK cell activity and cancer marker genes (AFP and TERT) compared to other drug-treated groups. Conclusion: MIX25 effectively suppressed the proliferation of HepG2 cell transplantation tumors in BALB/c nude mice. It not only inhibited inflammatory markers but also enhanced cell apoptosis. Additionally, MIX25 significantly increased NK cell activity and reduced mRNA expressions of AFP and TERT. Overall, the combination of MIX25 with sorafenib shows promise as a potential treatment for patients with HCC. [ABSTRACT FROM AUTHOR]
