Pre-operatively, all patients received one drop of tropicamide 0.8% and phenylephrine 5% (combination), 4 times, at an interval of 15 minutes on the day of surgery. Thereafter, Nepafenac drop in arm A/Flurbiprofen drop in arm B was administered 4 times, at an interval of 15 minutes keeping a gap of 10 minutes between tropicamide-phenylephrine and any of the experimental drugs. Phacoemulsification was performed one hour after the administration of last drop. Both vertical and horizontal pupillary diameter were measured at three steps; immediately before the surgical incision (baseline), at the end of emulsification of nucleus (before irrigation and aspiration) and at the end of surgery (after stromal hydration).Results: The difference in pupillary diameter between two groups, was statistically insignificant for vertical diameter (P = 0.08) and horizontal diameter (P = 0.28) at the start of surgery. On the other hand, pupillary diameter difference was statistically significant after emulsification of nucleus and at the end of surgery as well when both vertical (P < 0.05) and horizontal diameter (P < 0.05) were considered. The total reduction in pupillary diameter (both vertically and horizontally) was significantly less in the Nepafenac as compared to Flurbiprofen group (P < 0.05). Analysis of mean cumulative dissipated energy did not document any appreciable difference between the two groups. Phacoemulsification time analysis yielded statistically significant results (P = 0.004) between the Nepafenac and Flurbiprofen group.Conclusion: In the present study, topical Nepafenac (0.1%) proved to be more efficacious in maintaining intra-operative mydriasis during phacoemulsification surgery as compared to topical Flurbiprofen (0.03%).Keywords: topical nepafenac, topical flurbiprofen, intra-operative mydriasis, phacoemulsification

Optical biometry with partial coherence interferometry was performed in all cases. The median absolute error (MedAE) was compared by Friedman’s test, using the optimized lens constant, among seven formulas (Barrett Universal II, Haigis, Hill-RBF, Hoffer Q, Holladay 1, Holladay 2, and SRK/T) in each group.Results: Friedman’s test showed no significant difference in MedAE among all formulas in Groups 1 and 3. However, as the Haigis formula had the highest MedAE and lowest percentage of eyes within ±0.25 Diopter, it is least preferred in Group 1. On the contrary, in Group 3 it fared the best, having the least MedAE and highest percentage of eyes within ±0.25 Diopter. In Group 2, Friedman’s test gave significant difference, and post-hoc analysis showed statistical superiority of Haigis over the Holladay 1 (p=0.02), Holladay 2 (p=0.01), Hill-RBF (p=0.04), and SRK/T (p=0.02) formulas. However, there was no statistical difference between the Barrett Universal II, Haigis, and Hoffer Q formulas.Conclusion: Considering the ACD in eyes with AL less than 22 mm, Haigis is the preferred formula while SRK/T proved to be the worst formula in Groups 2 and 3.Keywords: short axial length, optical biometry, anterior chamber depth, intraocular lens formulas, optimized lens constant

The Government of India initiated a cash incentive scheme--Janani Suraksha Yojana (JSY)--to promote institutional deliveries with an aim to reduce maternal mortality ratio (MMR). An observational study was conducted in a tertiary-care hospital of Madhya Pradesh, India, before and after implementation of JSY, with a sample of women presenting for institutional delivery. The objectives of this study were to: (i) determine the total number of institutional deliveries before and after implementation of JSY, (ii) determine the MMR, and (iii) compare factors associated with maternal mortality and morbidity. The data were analyzed for two years before implementation of JSY (2003-2005) and compared with two years following implementation of JSY (2005-2007). Overall, institutional deliveries increased by 42.6% after implementation, including those among rural, illiterate and primary-literate persons of lower socioeconomic strata. The main causes of maternal mortality were eclampsia, pre-eclampsia and severe anaemia both before and after implementation of JSY. Anaemia was the most common morbidity factor observed in this study. Among those who had institutional deliveries, there were significant increases in cases of eclampsia, pre-eclampsia, polyhydramnios, oligohydramnios, antepartum haemorrhage (APH), postpartum haemorrhage (PPH), and malaria after implementation of JSY. The scheme appeared to increase institutional delivery by at-risk mothers, which has the potential to reduce maternal morbidity and mortality, improve child survival, and ensure equity in maternal healthcare in India. Cauliflower is an important winter crop grown in India, its curds are rich in nutritional profile, containing valuable minerals and vitamins. However, cauliflower harvesting is mainly accomplished by hands, which is time-consuming and requires a high labour force. On the other hand, most developed cauliflower harvesters are once over or single pass type, which harvests all plants irrespective of their maturity. So, the selective harvester could improve the cauliflower curds yield, and then decrease the labour requirement. To improve the cutting performance of the selective cauliflower harvester, the working parameters of the chainsaw cutting mechanism need to be considered and optimized. This research investigates the impact of cutting height, feed (push) force, and cutting speed on the efficiency of the cutter during harvest. The Taguchi approach, together with grey relational analysis (GRA), was employed to identify the most favorable combination of operational parameters. In addition, the variance analysis was conducted to statistically examine the impact of multiple parameters. The findings indicated that the feed force was the major parameter that influenced the cutting force, splitting failure levels, and cutting time. The most effective parameter combination consisted of a cutting height of 15 mm, a feed force of 10 N, and a cutting speed of 5 m/s. The grey relational grade of the ideal parameter combination has shown a 0.322 increase in comparison to the grade achieved with the initially selected parameter combination. Residual blood specimens provide a sample repository that could be analyzed to estimate and track changes in seroprevalence with fewer resources than household-based surveys. We conducted parallel facility and community-based cross-sectional serological surveys in two districts in India, Kanpur Nagar District, Uttar Pradesh, and Palghar District, Maharashtra, before and after a measles-rubella supplemental immunization activity (MR-SIA) from 2018 to 2019. Anonymized residual specimens from children 9 months to younger than 15 years of age were collected from public and private diagnostic laboratories and public hospitals and tested for IgG antibodies to measles and rubella viruses. Significant increases in seroprevalence were observed following the MR SIA using the facility-based specimens. Younger children whose specimens were tested at a public facility in Kanpur Nagar District had significantly lower rubella seroprevalence prior to the SIA compared to those attending a private hospital, but this difference was not observed following the SIA. Similar increases in rubella seroprevalence were observed in facility-based and community-based serosurveys following the MR SIA, but trends in measles seroprevalence were inconsistent between the two specimen sources. Despite challenges with representativeness and limited metadata, residual specimens can be useful in estimating seroprevalence and assessing trends through facility-based sentinel surveillance.

Ableism, akin to other forms of discrimination such as sexism and racism, is increasingly recognized as a pervasive societal concern directed towards individuals with disabilities. This scholarly inquiry delineates the operational definition, manifestations, and ramifications of ableism, with a specific focus on its manifestations within healthcare settings. It underscores the critical need for targeted interventions aimed at mitigating biases among healthcare professionals and promoting inclusivity within healthcare delivery systems. Utilizing insights from cognitive psychology, the study conceptualizes ableism as a cognitive distortion, explicating the mechanisms through which biases shape perceptions and recommending Cognitive Behavioral Therapy (CBT) as a promising intervention modality. The paper advocates for exploration of intersectional analyses, utilization of qualitative research methodologies, and cultivation of collaborative partnerships with disability advocacy organizations to develop more comprehensive intervention approaches. Background: Prior vaccination is often studied for its impact on individuals' post-infection prognosis. Ayurveda, Yoga, Unani, Siddha and Homeopathy (AYUSH) medicines, advised by the Government of India as prophylaxis during the first wave of the coronavirus disease 2019 (COVID-19) pandemic, were consumed by the masses in 2020. A study was therefore undertaken to observe any association between the prior usage of AYUSH prophylactic medicines and post-infection severity as reported by recovered COVID-19 individuals., Methods: This was a retrospective, multi-centre, cohort study conducted in 21 cities of India from 5th August to 30th November 2020. Data from recovered COVID-19 patients, of either sex or any age, captured information about AYUSH prophylactic medicines intake prior to infection, disease severity, symptomatology, duration of complaints, etc. The study participants were grouped into AYUSH intake and non-intake. Primary composite outcome was the disease clinical course. Secondary clinical outcomes were the rate of and time to clinical recovery., Results: Data of 5,023 persons were analysed. Ayurveda or homeopathic prophylactic medicines were consumed by more than half of the study participants: that is, 56.85% ( n  = 1,556) and 56.81% ( n  = 1,555) respectively. The overall adjusted protective effect (PE) of AYUSH prophylactic intake against moderate/severe forms of COVID-19 disease was 56.7% (95% confidence interval [CI], 48.7 to 63.50; p  < 0.001). Adjusted PE for homeopathy and Siddha was 52.9% (95% CI, 42.30 to 61.50; p  < 0.001) and 59.8% (95% CI, 37.80 to 74.10; p  < 0.001), respectively. A statistically significant association was found between AYUSH prophylactic medicine intake and clinical recovery more frequently by the 3rd day of illness (χ 2  = 9.01; p  = 0.002). Time to resolution of symptoms in the AYUSH intake group was on average 0.3 days earlier than in the non-intake group ( p  = 0.002)., Conclusion: AYUSH prophylactics were associated with statistically significant levels of protection against COVID-19 disease severity. Amongst these, previous intake of homeopathy or Siddha medicines was associated with some protection against moderate/severe illness and with a somewhat quicker clinical recovery. Background and aim In India, socioeconomic factors, healthcare access, and cultural views influence HIV epidemiology. With over two million cases, high-risk groups include drivers, sex workers, and intravenous drug users. Early symptoms often go unnoticed, and advanced stages lead to opportunistic infections. Despite antiretroviral therapy (ART) advancements, access remains a challenge. Effective management per National AIDS Control Organization (NACO) guidelines is essential. This study aimed to understand the clinical profile of people living with HIV (PLWH) with sociodemographic variation at tertiary care centers in the western part of Maharashtra. Methods and methodology A retrospective descriptive cross-sectional study was conducted on PLWH from November 2022 to May 2024. The PLWH patients of all age groups and all categories were included in the study. The sociodemographic data were collected, and clinical staging was done according to the World Health Organization (WHO) classification of HIV/AIDS. Data were expressed as mean±standard deviation and percentage proportion. Results Out of 150 PLWH, 77 (51.33%) were male and 73 (48.67%) were female patients. The majority of patients, 102 out of 150, were in the age group of 30-50 years. The most common mode of HIV transmission was heterosexual (143 patients, 95.33%). At the time of presentation, 102 patients (68%) were symptomatic, while 48 patients (32%) were asymptomatic. A total of 77 patients (51.33%) were included in WHO clinical stage 2 of HIV disease. In terms of cluster of differentiation 4 (CD 4) count, 62 patients (41.33%) had counts between 201 and 500, 54 patients (36%) had counts greater than 500, and 34 patients (22.67%) had counts below 200. Most patients, 94 (62.67%), had a BMI of less than 18.5. Pulmonary tuberculosis was found to be the most commonly associated opportunistic infection (OI), constituting 57 (38%) cases, followed by acute gastroenteritis 35 (23.33%). Globoid cell leukodystrophy is a severe rare disorder characterized by white matter degradation, resulting in a progressive loss of physical and mental abilities and has extremely limited therapeutic interventions. Therefore, this study aimed to delve into the Globoid cell leukodystrophy associated intricate network of differentially expressed genes (p < 0.05, |Fc|> 1) to identify potential druggable targets and possible therapeutic interventions using small molecules. The disease-associated neuronal protein circuit was constructed and analyzed, identifying 53 nodes (minimum edge cutoff 1), among which five (FOS, FOSB, GDNF, GFRA1, and JUN) were discerned as potential core protein nodes. Although our research enumerates the potential small molecules to target various protein nodes in the proposed disease network, we particularly underscore T-5224 to inhibit c-Jun activity as JUN was identified as one of the pivotal elements within the disease-associated neuronal protein circuit. The evaluation of T-5224 binding energy (- 11.0 kcal/mol) from docking study revealed that the compound to exhibit a notable affinity towards Jun/CRE complex. Moreover, the structural integrity of complex was affirmed through comprehensive molecular dynamics simulations, indicating a stable hydrophilic interaction between T-5224 and the Jun/CRE complex, thereby enhancing protein compactness and reducing solvent accessibility. This binding energy was further substantiated by free binding analysis, revealing a substantial thermodynamics complex state (- 448.00 ± 41.73 kJ/mol). Given that this investigation is confined to a computational framework, we additionally propose a hypothetical framework to ascertain the feasibility of inhibiting the Jun/CRE complex with T-5224 against Globoid cell leukodystrophy, employing a combination of in vitro and in vivo methodologies as a prospective avenue of this study., (© 2024. Background: Diabetes Mellitus (DM) is an alarming health concern, affecting approximately 537 million people worldwide. As a leading cause of morbidity and mortality, DM demands a comprehensive understanding of its diverse pathophysiological mechanisms and disease progression., Methods: This traditional review has consolidated literature on the pathogenesis of hyperglycemia, its progression into complications, and advances in optimal treatment strategies. The literature in the last two decades has been reviewed using several keywords, including "diabetes," "diabetes-associated complications", "novel therapeutic interventions for diabetes-associated diseases", "phyto-extracts as antidiabetic drugs", etc. in prominent databases, such as PubMed, Scopus, Google Scholar, Web of Science, and ClinicalTrials.gov., Results: We have discussed macrovascular and microvascular complications, such as atherosclerosis, cardiovascular disease, Peripheral Arterial Disease (PAD), stroke, diabetic nephropathy, retinopathy, and neuropathy, as well as various pharmacological and non-pharmacological interventions that are currently available for the management of DM. We have also focused on the potential of natural products in targeting molecular mechanisms involved in carbohydrate metabolism, insulin production, repair of pancreatic cells, and reduction of oxidative stress, thereby contributing to their antidiabetic activity. Additionally, novel therapeutic approaches, like genetic, stem cell, and immunomodulatory therapies, have been explored. Persistent fetal vasculature (PFV) is a rare ocular developmental disorder resulting from incomplete apoptosis of the embryonic hyaloid vasculature during the in-utero period. Variability in the development and regression of hyaloid vasculature is responsible for the wide range of clinical presentation of the disorder. PFV may manifest as anterior segment abnormalities (cataract, glaucoma, microphthalmia, elongated ciliary process with central traction, retrolental membrane, and shallow anterior chamber), posterior segment abnormalities (vitreous stalk, preretinal membranes, optic hypoplasia, and retinal folds), or with a combined anteroposterior disease. The most common associated clinical feature is leukocoria with microphthalmia and usually unilateral presentation. Most of the cases have poor visual prognosis and present early in childhood. Association with myopia is a very rare and atypical presentation, especially unilateral cases which may present later in life and have a good visual prognosis. Chemoresistance, a significant challenge in cancer treatment, is often associated with the cellular glutathione-related detoxification system. The GSTP1 isoenzyme (glutathione S-transferases) plays a critical role in the cytoplasmic inactivation of anticancer drugs. This suggests the identification of GSTP1 inhibitors to combat chemoresistance. We screened Sophoretin (also called quercetin) derivatives for molecular properties, pharmacokinetics, and toxicity profiles. Following that, we conducted molecular docking and simulations between selected derivatives and GSTP1. The best-docked complex, GSTP1-quercetin 7-O-β-D-glucoside, exhibited a binding affinity of -8.1 kcal/mol, with no predicted toxicity and good pharmacokinetic properties. Molecular dynamics simulations confirmed the stability of this complex. Introduction : Cervical cancer is prevalent among women worldwide. It is a type of cancer that occurs in the cells of the cervix, the lower part of the uterus. Mostly, it is observed in developing nations due to limited access to screening tools. Natural products with anticancer properties and fewer side effects have gained attention. Therefore, this study evaluates the potential of Drymaria cordata as a natural source for treating cervical cancer. Methodology : Phytocompounds present in Drymaria cordata were screened for their molecular properties and drug-likeness. The selected compounds were studied using systems biology tools such as network pharmacology, molecular docking, and molecular dynamics simulations, including MMGBSA studies. Results : Through network pharmacology, molecular docking, and molecular dynamics simulations, quercetin 3-O-β-d-glucopyranosyl-(1→2)-rhamnopyranoside was identified as a hit compound targeting HRAS and VEGFA proteins. These proteins were found to be responsible for the maximum number of pathway modulations in cervical cancer. Conclusion : Drymaria cordata exhibits potential for treating cervical cancer due to the presence of quercetin 3-O-β-d-glucopyranosyl-(1→2)-rhamnopyranoside. Delta-9-tetrahydrocannabinol, a component of marijuana, interacts with cannabinoid receptors in brain involved in memory, cognition, and emotional control. However, marijuana use and schizophrenia development is a complicated and contentious topic. As a result, more investigation is needed to understand this relationship. Through the functional enrichment analysis, we report the delta-9-tetrahydrocannabinol to manipulate the homeostatic biological process and molecular function of different macromolecules. Additionally, using molecular docking and subsequent processing for molecular simulations, we assessed the binding ability of delta-9-tetrahydrocannabinol with the estrogen-related protein, dopamine receptor 5, and hyaluronidase. It was found that delta-9-tetrahydrocannabinol may have an impact on the brain's endocannabinoid system and may trigger the schizophrenia progression in vulnerable people. Delta-9-tetrahydrocannabinol may interfere with the biological function of 18 proteins linked to schizophrenia and disrupt the synaptic transmission (dopamine, glutamine, and gamma-aminobutyric acid). It was discovered that it may affect lipid homeostasis, which is closely related to membrane integrity and synaptic plasticity. The negative control of cellular and metabolic processes, fatty acids binding /activity, and the manipulated endocannabinoid system (targeting cannabinoid receptors) were also concerned with delta-9-tetrahydrocannabinol. Hence, this may alter neurotransmitter signaling involved in memory, cognition, and emotional control, showing its direct impact on brain physiological processes. A 55-year-old female was referred to the Department of Ophthalmology with complaints of bilateral loss of vision. She had undergone subtotal gastrectomy with gastrojejunostomy and lymphadenectomy for poorly differentiated gastric adenocarcinoma in the antropyloric region the day before. On the first postoperative day, she complained of generalised weakness, drowsiness, altered sensorium, and acute, painless, bilateral loss of vision. Ocular examination revealed visual acuity as no perception of light, bilaterally, and normal pupillary light reflexes. Anterior and posterior segment examination was within normal limits. This clinical presentation of altered sensorium and cortical blindness along with characteristic radiological findings (hyperintensity on T2/fluid-attenuated inversion recovery sequence involving the bilateral parieto-occipital lobe extending in asymmetric fashion to the bilateral cerebellum, brainstem, and thalami predominantly involving the white matter with few areas of diffusion restriction on diffusion-weighted imaging sequence predominantly on the left side with gyriform pattern) confirmed the diagnosis of posterior reversible encephalopathy syndrome (PRES). In cancer patients, PRES has been reported in patients on chemotherapy regimen or two weeks after surgery for gastric cancer. This study aimed to evaluate the antioxidant, antiarthritic, and anti-inflammatory properties of extracts from the leaves of twelve different medicinal plants in Nepal. We then evaluated the total phenolic, flavonoid, and tannin contents of the extract using in-vitro assays and characterized it using GC-MS analysis. Results revealed that most of the leaf extracts contained phenolic compounds, flavonoids, tannins, alkaloids, and saponins. Few plants also showed the presence of glycosides, phytate, and vitamin C. Among the studied plants, Neolamarckia cadamba exhibited the highest total phenolic and tannin contents, as 241.53 ± 0.20  µ g of gallic acid equivalent/mg and 74.48 ± 1.081  µ g of tannic acid equivalent/mg, respectively. Ipomoea batatas exhibited the highest total flavonoid content, as 53.051 ± 1.11  µ g of quercetin equivalent/mg. Moreover, Raphanus sativus demonstrated significant ferrous ion chelating, 2,2-diphenyl-1-picrylhydrazyl, hydrogen peroxide scavenging, and total antioxidant activities with IC 50 value of 4.76 ± 0.68  µ g/mL, 5.84 ± 0.14  µ g/mL, 6.89 ± 0.16  µ g/mL, and 8.99 ± 0.20  µ g/mL, respectively. Similarly, Colocasia esculenta and Cicer arietinum exhibited the highest hydroxyl radical and nitric oxide scavenging activities, measuring IC 50 value of 7.22 ± 0.56  µ g/mL and 9.06 ± 0.10  µ g/mL, respectively. Among all the extracts, Amorphophallus paeoniifolius displayed significant human red blood cell (HRBC) membrane stabilization activity (IC 50  = 6.22 ± 0.78  µ g/mL). Furthermore, Raphanus sativus , Chenopodium album, Cicer arietinum , and Murraya koenigii exhibited the highest inhibitory activities against protein denaturation with bovine serum albumin, antiarthritic, lipoxygenase inhibitory, and proteinase inhibitory, measuring IC 50 of 7.48 ± 0.48  µ g/mL, 9.44 ± 1.62  µ g/mL, 14.67 ± 1.94  µ g/mL, and 28.57 ± 2.39  µ g/mL, respectively. Background and Objective: According to World Health Organization, melanoma claims the lives of about 48000 people worldwide each year. The purpose of this study was to identify potential phytochemical pool from Diplazium esculentum against proteins that contribute to melanoma development., Methods: The research was carried to locate potentially bioactive molecules and conduct a theoretical analysis of active ingredients from DE to impact melanoma. Network pharmacology, pharmacokinetics, protein network interaction, gene enrichment, survival, and infiltration analysis were conducted. Furthermore, molecular docking and molecular dynamics simulation was carried out for makisterone C-MAPK1, MAPK3, and AKT1 complexes., Results: The potential phytochemical pool were identified (stigmast-5-en-3-ol, esculentic acid, rutin, and makisterone C) and based on network pharmacology and molecular docking studies, makisterone-C was proposed to be the most promising ingredient. Objective: The past few years have seen a rapid advancement in the management of keratoconus (KC). However, there is no prescribed standard of care for the management of KC. This study evaluated the prevailing practice patterns among Indian ophthalmologists in the diagnosis and treatment of KC via an online survey., Methods: This was a survey-based cross-sectional study in which a questionnaire (Supplement 1) was created. Questions pertaining to the practicing experience, setting of practice, and training background were asked in addition to the investigations done and decision making in KC management. Responses were collected via Survey Monkey (Survey Monkey, Palo Alto, California, USA) and statistical analysis performed using R software (4.1.3)., Results: The survey was answered by 273 ophthalmologists. Pentacam was the most used topographer (195 users), followed by Orbscan (41 users), Sirius (34 users), and Galilei (3 users). The lowest limit of pachymetry for performing collagen crosslinking (CXL) was 400μ for most practitioners. More than half the respondents (50.55%) did not perform photorefractive keratectomy (PRK) or intracorneal ring segment (ICRS) implantation in a suitable patient. Accelerated 10-minute protocol (9 mW/cm 2 for 10 minutes) was the most commonly (54.21%) used for CXL, followed by Dresden protocol (3 mW/cm 2 for 30 minutes) (36.63%). When a patient was unsuitable for CXL, 55.31% surgeons advise contact lens (CL) trial, 35.16% surgeons advise keratoplasty, 26.74% surgeons perform stromal augmentation, and 7.69% surgeons advise spectacle correction. Corneal scar was the most common indication (49.45%) for performing keratoplasty., Conclusion: Topography remains the most used diagnostic modality for initial diagnosis. Optical coherence tomography and epithelial mapping are increasingly being used for early diagnosis of KC. Not all ophthalmologists were comfortable performing ICRS or PRK. The evaluation of changes in shear resistance on soft (or weathered) rock joints under cyclic shear loads with constant normal load (CNL) and constant normal stiffness (CNS) significantly contributes to increasing the safety and stability of rock slopes and underground structures. In this study, a series of cyclic shear tests were conducted on simulated soft rock joints with regular (15°-15°, 30°-30°) and irregular (15°-30°) asperities under different normal stiffnesses ( k n ). The results indicated that the first peak shear stress increases with the increase in k n up to the normal stiffness of the joints ( k nj ). Beyond k nj , no significant change was observed in the peak shear stress. The difference in peak shear stress between regular (30°-30°) and irregular joints (15°-30°) increases as k n increases. The minimum difference of peak shear stress between regular and irregular joints was observed (8.2%) under CNL and the maximum difference was found (64.3%) on k nj under CNS. The difference in peak shear stress between the first and subsequent cycles significantly increases as both the joint roughness and k n increases. A new shear strength model is developed to predict peak shear stress of the joints for different k n and asperity angles under cyclic shear loads.

The purpose of the paper is to identify factors associated with operational factor (OF), geo-environmental factor (GEF), and managerial factor (MF) from the literature. After identification, the study intended to assess the impact of geo-environmental factors and managerial factors on operational factors of the mining industry. The study also tests the indirect effect of managerial factors between GEF and OF in the Indian environment. The geographical boundary of the study was 06 large mines of Odisha, Jharkhand, and Chhattisgarh of India. Three hundred and twenty nine number of purposive samples were collected via email and filtered and processed through the SPSS package. To find out the complex role and inter-relationship of GEF and MF on OF, the study adopted the structural equation modelling technique. The finding reflects that MF plays a partial mediation among GEF and OF. This phenomenon is completely novel in its field when it comes to the geo-environmental and management difficulties confronting mining operations. This research can aid managers in identifying key geological and environmental concerns in mining operations, as well as providing data for regulatory compliance. Overall, this study's findings can help management create policies and manage the environmental concerns of the mining sector. Purpose: To analyze the demographics and clinical outcomes of posterior chamber phakic intraocular (IOL) implantation for refractive amblyopia in children and adolescents., Methods: A prospective interventional study was performed on children and adolescents with amblyopia at a tertiary eye care center from January 2021 to August 2022. Twenty-three eyes of 21 anisomyopic and isomyopic amblyopia patients operated for posterior chamber phakic IOL (Eyecryl phakic IOL) as a treatment for amblyopia were included in the study. Patient demographics, pre- and postoperative visual acuity, cycloplegic refraction, anterior and posterior segment examination, intraocular pressure, pachymetry, contrast sensitivity, endothelial count, and patient satisfaction scores were evaluated. Patients were followed up at day 1, 6 weeks, 3 months, and 1 year after surgery, and visual outcomes and complications were documented., Results: The mean age of patients was 14.16 ± 3.49 years (range: 10-19 years). The mean intraocular lens power was - 12.20 diopter spherical (DS) in 23 eyes and - 2.25 diopter cylindrical (DC) in four patients. The mean unaided distant visual acuity (UDVA) and best-corrected visual acuity (BCVA) were 1.39 ± 0.25 and 0.40 ± 0.21 preoperatively on the log of minimum angle of resolution (logMAR) chart. Postoperatively, the visual acuity improved by 2.6 lines in 3 months period and maintained till 1 year. Postsurgery, contrast sensitivity in the amblyopic eyes significantly improved, and the average endothelial loss recorded was 5.78% at 1 year, which was statistically insignificant. Patient satisfaction score was statistically significant, with 4.736/5 recorded on the Likert scale., Conclusion: Posterior chamber phakic IOL is a safe, effective, and alternative method for treating amblyopia patients who are noncompliant with glasses, contact lenses, and keratorefractive procedures., Competing Interests: None

In the recent era, soil nailing is the developing method for slope stability, road widening projects, and constructing deep foundations. This paper suggests a new and improved helical soil nail (HN) that offers an alternative to conventional soil nail (CN). Till now pullout studies on different model nails in c-Φ soil are still rare. In addition, the stability analysis for conventional soil nails was only studied using FEM analysis, whereas for helical soil nails the estimation of stability of slope is still mysterious. So, the present study includes the behavior of conventional and helical soil nails based on both laboratory and analytical methods in c-Φ soil. The present study also incorporated the use of theoretical equations and finite element analysis (using PLAXIS 2D) for comparing the stability of different profiles of soil nails (e.g., CN and HN) in c-Φ soil, which was unique to this study. By using the finite element method (FEM) and the limit equilibrium method (LEM), a factor of safety was also estimated for both types of nails (CN and HN). Background: India did phased measles-rubella supplementary immunisation activities (MR-SIAs; ie, mass-immunisation campaigns) targeting children aged 9 months to less than 15 years. We estimated measles-rubella seroprevalence before and after the MR-SIAs to quantify the effect on population immunity and identify remaining immunity gaps., Methods: Between March 9, 2018 and March 19, 2020 we did community-based, cross-sectional serosurveys in four districts in India before and after MR-SIAs. 30 villages or wards were selected within each district, and one census enumeration block from each was selected as the survey cluster. Households were enumerated and 13 children in the younger age group (9 months to <5 years) and 13 children in the older ager group (5 to <15 years) were randomly selected by use of computer-generated random numbers. Serum samples were tested for IgG antibodies to measles and rubella viruses by enzyme immunoassay., Findings: Specimens were collected from 2570 children before the MR-SIA and from 2619 children afterwards. The weighted MR-SIA coverage ranged from 73·7% to 90·5% in younger children and from 73·6% to 93·6% in older children. Before the MR-SIA, district-level measles seroprevalence was between 80·7% and 88·5% among younger children in all districts, and between 63·4% and 84·5% among older children. After the MR-SIA, measles seroprevalence among younger children increased to more than 90% (range 91·5 to 96·0) in all districts except Kanpur Nagar, in which it remained unchanged 80·4%. Among older children, measles seroprevalence increased to more than 90·0% (range 93·7% to 96·5%) in all districts except Hoshiarpur (88·7%). Background: A diphtheria outbreak occurred in Banaskantha district in the northern part of Gujarat in 2019-2020. This study was undertaken to study and generate information on the resurgence of the disease in this region and to generate information regarding the status of vaccination in this region and what strategies can be adopted to avoid re-emergence of the disease in the future., Materials and Methods: This is a hospital-based, retrospective, descriptive study done on patients of diphtheria who were admitted in Banas Medical College and Research Institute, Palanpur from September 2019 to January 2020. Throat swab was taken from all the patients and information regarding symptoms, vaccination, and demographic characteristic was collected. Treatment in the form of ADS, injection crystalline penicillin/erythromycin and other supportive treatment was given., Result: Out of 188 patients, 27 patients (14.36%) were less than 5 years of age, 118 patients (62.76%) and 38 patients (20.21%) were of age group 5-10 and 11-18 years, respectively. Total of five patients (2.66%) were of age more than 18 years. Out of 188 patients, 102 (54.25%) were males and 86 patients (45.74%) were females. All the patients (188) were found to be unvaccinated. Among 188 throat swabs that were tested, 21 samples (11.17%) were found to be culture positive for Corynebacterium diphtheriae . Antidiphtheric serum was given in 181 patients (96.27%) according to the requirement. Out of 188 patients, 155 patients (82.44%) improved with treatment and were discharged. Total of 23 patients (12.23%) were referred to higher center for tracheostomy and for management of other complications. Six patients (3.19%) left against medical advice and four patients (2.12%) expired despite all measures taken., Conclusion: Diphtheria is a disease that can easily be prevented by vaccination. A case of 50-year-old male who presented with orbital compartment syndrome as a sequela of mid facial trauma, and literature review. Orbital compartment syndrome (OCS) is one of the potentially sight threatening emergencies encountered in clinical practice. Acute rise in pressure within the confined orbital cavity compromises the blood flow to retina and optic nerve leading to irreparable vision loss. Air entrapped in the orbital cavity leading to orbital compartment syndrome has been rarely reported. A brief literature search for the term orbital emphysema in PubMed yielded 352 articles out of which 280 articles were identified after screening for appropriate titles and case reports. A total of 138 patients were reported in the literature with severe orbital emphysema. Acute orbital compartment syndrome needs to be recognized and addressed on an emergency basis to achieve decompression so as to prevent an irreversible vison loss. Microbial interactions with plant roots play an imperial role in tomato plant growth and defense against the Rhizoctonia solani . This study performed a field experiment with two antagonistic bacteria ( Pseudomonas and Bacillus ) inoculated in healthy and Rhizoctonia solani treated soil in tomato rhizosphere to understand the metabolic pattern and microbial function during plant disease suppression. In the present study, we assessed soil and microbial enzymes, bacterial and fungal cell forming unit (CFU), and carbon utilization profiling through Bio-Eco plates of rhizoplane samples. Antagonist bacteria and pathogen interaction significantly ( p < 0.05) influenced the bacterial count, soil enzymes (chitinase and glucanase), and bacterial function (siderophore and chitinase production). These results indicated that these variables had an imperial role in disease suppression during plant development. Furthermore, the metabolic profiling showed that carbon source utilization enhanced under fruit development and ripening stages. These results suggested that carbon sources were essential in plant/pathogen/antagonist interaction. Substrates like β-methyl-D-glucoside, D-mannitol, D-galacturonic acid, N-acetyl-D-glucosamine, and phenylethylamine strongly connect with the suppuration of root rot disease. Diabetes mellitus is the main cause of diabetic retinopathy, the most common cause of blindness worldwide. In order to slow down or prevent vision loss and degeneration, early detection and treatment are essential. For the purpose of detecting and classifying diabetic retinopathy on fundus retina images, numerous artificial intelligence-based algorithms have been put forth by the scientific community. Due to its real-time relevance to everyone's lives, smart healthcare is attracting a lot of interest. With the convergence of IoT, this attention has increased. The leading cause of blindness among persons in their working years is diabetic eye disease. Millions of people live in the most populous Asian nations, including China and India, and the number of diabetics among them is on the rise. To provide medical screening and diagnosis for this rising population of diabetes patients, skilled clinicians faced significant challenges. Our objective is to use deep learning techniques to automatically detect blind spots in eyes and determine how serious they may be. We suggest an enhanced convolutional neural network (ECNN) utilizing a genetic algorithm in this paper. Study Design: Retrospective observational., Objectives: This study aimed to document the safety and efficacy of lumbar corpectomy with reconstruction of anterior column through posterior-only approach in complete burst fractures., Methods: In this retrospective study, we analyzed complete lumbar burst fractures treated with corpectomy through posterior only approach between 2014 and 2018. Clinical and intraoperative data including pre and post-operative neurology as per the ISNCSCI grade, VAS score, operative time, blood loss and radiological parameters, including pre and post-surgery kyphosis, height loss and canal compromise was assessed., Results: A total of 45 patients, with a mean age of 38.89 and a TLICS score 5 or more were analyzed. Preoperative VAS was 7-10. Mean operating time was 219.56 ± 30.15 minutes. Mean blood loss was 1280 ± 224.21 ml. 23 patients underwent short segment fixation and 22 underwent long segment fixation. There was no deterioration in post-operative neurological status in any patient. At follow-up, the VAS score was in the range of 1-3. The difference in preoperative kyphosis and immediate post-operative deformity correction, preoperative loss of height in vertebra and immediate post-operative correction in height were significant (p < 0.05)., Conclusion: The posterior-only approach is safe, efficient, and provides rigid posterior stabilization, 360° neural decompression, and anterior reconstruction without the need for the anterior approach and its possible approach-related morbidity. We achieved good results with an all posterior approach in 45 patients of lumbar burst fracture (LBF) which is the largest series of this nature.

A sudden outbreak of a novel coronavirus SARS-CoV-2 in 2019 has now emerged as a pandemic threatening to efface the existence of mankind. In absence of any valid and appropriate vaccines to combat this newly evolved agent, there is need of novel resource molecules for treatment and prophylaxis. To this effect, flavonol morin which is found in fruits, vegetables and various medicinal herbs has been evaluated for its antiviral potential in the present study. PASS analysis of morin versus reference antiviral drugs baricitinib, remdesivir and hydroxychloroquine revealed that morin displayed no violations of Lipinski's rule of five and other druglikeness filters. Morin also displayed no tumorigenic, reproductive or irritant effects and exhibited good absorption and permeation through GI (clogP <5). In principal component analysis, morin appeared closest to baricitinib in 3D space. Morin displayed potent binding to spike glycoprotein, main protease 3CLPro and papain-like protease PLPro of SARS-CoV-2, SARS-CoV and MERS-CoV using molecular docking and significant binding to three viral-specific host proteins viz. human ACE2, importin-α and poly (ADP-ribose) polymerase (PARP)-1, further lending support to its antiviral efficacy. Additionally, morin displayed potent binding to pro-inflammatory cytokines IL-6, 8 and 10 also supporting its anti-inflammatory activity. MD simulation of morin with SARS-CoV-2 3CLPro and PLPro displayed strong stability at 300 K. Both complexes exhibited constant RMSDs of protein side chains and Cα atoms throughout the simulation run time. In conclusion, morin might hold considerable therapeutic potential for the treatment and management of not only COVID-19, but also SARS and MERS if studied further. Communicated by Ramaswamy H. Background: The consumption of green juice of Barley and wheat grass is widely increases because of its therapeutic benefits., Objective: The study aimed to investigate phytochemicals and evaluate the antioxidant and antidepressant activity of aqueous extract of barley and wheat grass., Material and Methods: This study included phytochemical screening, evaluation of antioxidant and antidepressant activities. Four groups consisting of six mice in each group. Negative control with stressed induced mice; imipramine group (100 mg/kg), barley and wheat extract group (400 mg/kg) respectively. Forced swim, tail suspension and elevated plus maize test were carried out to evaluate the antianxiety and antidepressant activity., Results: Phytochemical screening of barley and wheat grass extract showed secondary metabolites such as alkaloids, Saponin, Tannins, Phenolic, Carbohydrate, Glycosides, Flavonoids, and Proteins. The mean total phenolic content of aqueous extract of barley and wheat grass was 160.996 ± 0.656, 135.63 ± 1.184 mg equivalent of GAE/g respectively. The total flavonoid content of aqueous extract of barley and wheat grass was 153.42 ± 0.40, 133.14 ± 0.43 mg equivalent of quercetin/g respectively. The extracts were proved to be an effective radical scavenger in all antioxidant assays. Forced swim and tail suspension test showed a significant (∗p < 0.05 and ∗∗p < 0.01) decrease in an immobility time. This review article attempts to evaluate the accuracy of intraocular lens power calculation formulae in short eyes. A thorough literature search of PubMed, Embase, Cochrane Library, Science Direct, Scopus, and Web of Science databases was conducted for articles published over the past 21 years, up to July 2021. The mean absolute error was compared by using weighted mean difference, whereas odds ratio was used for comparing the percentage of eyes with prediction error within ±0.50 diopter (D) and ±1.0 D of target refraction. Statistical heterogeneity among studies was analyzed by using Chi-square test and I 2 test. Fifteen studies including 2,395 eyes and 11 formulae (Barrett Universal II, Full Monte method, Haigis, Hill-RBF, Hoffer Q, Holladay 1, Holladay 2, Olsen, Super formula, SRK/T, and T2) were included. Although the mean absolute error (MAE) of Barrett Universal II was found to be the lowest, there was no statistically significant difference in any of the comparisons. The median absolute error (MedAE) of Barrett Universal II was the lowest (0.260). Holladay 1 and Hill-RBF had the highest percentage of eyes within ±0.50 D and ±1.0 D of target refraction, respectively. Yet their comparison with the rest of the formulae did not yield statistically significant results. Thus, to conclude, in the present meta-analysis, although lowest MAE and MedAE were found for Barrett Universal II and the highest percentage of eyes within ±0.50 D and ±1.0 D of target refraction was found for Holladay 1 and Hill-RBF, respectively, none of the formulae was found to be statistically superior over the other in eyes with short axial length., Competing Interests: None

LncRNAs (long noncoding RNAs) are 200 bp length crucial RNA molecules, lacking coding potential and having important roles in regulating gene expression, particularly in response to abiotic stresses. In this study, we identified salt stress-induced lncRNAs in chickpea roots and predicted their intricate regulatory roles. A total of 3452 novel lncRNAs were identified to be distributed across all 08 chickpea chromosomes. On comparing salt-tolerant (ICCV 10, JG 11) and salt-sensitive cultivars (DCP 92-3, Pusa 256), 4446 differentially expressed lncRNAs were detected under various salt  treatments. We predicted 3373 lncRNAs to be regulating their target genes in cis regulating manner and 80 unique lncRNAs were observed as interacting with 136 different miRNAs, as eTMs (endogenous target mimic) targets of miRNAs and implicated them in the regulatory network of salt stress response. Functional analysis of these lncRNA revealed their association in targeting salt stress response-related genes like potassium transporter, transporter family genes, serine/threonine-protein kinase, aquaporins like TIP1-2, PIP2-5 and transcription factors like, AP2, NAC, bZIP, ERF, MYB and WRKY. Furthermore, about 614 lncRNA-SSRs (simple sequence repeats) were identified as a new generation of molecular markers with higher efficiency and specificity in chickpea. Background: We performed a retrospective study of 67 patients and their data for radiological investigations by serial Xrays, computed tomography, magnetic resonance imaging, uniform surgical procedure of craniotomy. The results were analyzed to determine the natural course of the disease, anatomical changes at various intervals following trauma, and outcome of surgical procedure in terms of cranial reconstruction, seizures, and progress in neurological deficit., Results: Among 67 patients, 34 (50.74%) were male and 33 (49.26%) were female patients. About 86.67% of patients sustained the injury before the age of 3 years. Development of seizures in 28 patients (41.80%) is the most common symptom. In our study, 43.28% of patients (29 cases) had a combination of Type I and II of growing skull fracture. The dural defects confirmed in all cases were nearly twice (average 1.42) as large as the bone defects. All patients under the age of 3 years with diastatic skull fracture should be closely followed up and should be examined 2-3 months later to look for evidence of a growing skull fracture. Linear fractures and burst fractures in an infant with a scalp swelling must be corrected early to prevent a growing skull fracture., Conclusion: Early management can avoid difficult surgical dissection and progressive neurological sequelae seen with delayed intervention. Surgical correction results in the prevention of brain shift and increase in meningocerebral cicatrices. Meticulous surgery and vigilant postoperative care reduce the morbidity and mortality. Purpose: Optimization of lens constants is a critically important step that improves refractive outcomes significantly. Whether lens constants optimized for the entire range of axial length would perform equally well in short eyes is still a matter of debate. The aim of this study was to analyze whether lens constants need to be optimized specifically for short eyes., Methods: : This retrospective observational study was conducted at a tertiary care hospital in Central India. Eighty-six eyes of eighty-six patients were included. Optical biometry with IOLMaster 500 was done in all cases and lens constants were optimized using built-in software. Barrett Universal II, Haigis, Hill-RBF, Hoffer Q, Holladay 1, and SRK/T formulae were compared using optimized constants. Mean absolute error, median absolute error (MedAE), and percentage of eyes within ±0.25, ±0.50, ±1.00, and ±2.00 diopter of the predicted refraction, of each formula were analyzed using manufacturer's, ULIB, and optimized lens constants. MedAE was compared across various constants used by Wilcoxon signed-rank test and among optimized constants by Friedman's test. Cochran's Q test compared the percentage of eyes within ± 0.25, ±0.50, ±1.00, and ± 2.00 diopter of the predicted refraction. A value of P < 0.05 was considered statistically significant., Results: : Optimized constant of Haigis had significantly lower MedAE (P < 0.00001) as compared to manufacturers. However, there was no statistically significant difference between ULIB and optimized constants. Purpose: COVID-19-associated rhino-orbital-cerebral mucormycosis (ROCM) has reached epidemic proportion during India's second wave of COVID-19 pandemic, with several risk factors being implicated in its pathogenesis. This study aimed to determine the patient demographics, risk factors including comorbidities, and medications used to treat COVID-19, presenting symptoms and signs, and the outcome of management., Methods: This was a retrospective, observational study of patients with COVID-19-associated ROCM managed or co-managed by ophthalmologists in India from January 1, 2020 to May 26, 2021., Results: Of the 2826 patients, the states of Gujarat (22%) and Maharashtra (21%) reported the highest number of ROCM. The mean age of patients was 51.9 years with a male preponderance (71%). While 57% of the patients needed oxygen support for COVID-19 infection, 87% of the patients were treated with corticosteroids, (21% for > 10 days). Diabetes mellitus (DM) was present in 78% of all patients. Most of the cases showed onset of symptoms of ROCM between day 10 and day 15 from the diagnosis of COVID-19, 56% developed within 14 days after COVID-19 diagnosis, while 44% had delayed onset beyond 14 days. Orbit was involved in 72% of patients, with stage 3c forming the bulk (27%). Overall treatment included intravenous amphotericin B in 73%, functional endoscopic sinus surgery (FESS)/paranasal sinus (PNS) debridement in 56%, orbital exenteration in 15%, and both FESS/PNS debridement and orbital exenteration in 17%. Intraorbital injection of amphotericin B was administered in 22%. At final follow-up, mortality was 14%. Disease stage >3b had poorer prognosis. Paranasal sinus debridement and orbital exenteration reduced the mortality rate from 52% to 39% in patients with stage 4 disease with intracranial extension (p < 0.05)., Conclusion: : Corticosteroids and DM are the most important predisposing factors in the development of COVID-19-associated ROCM. COVID-19 patients must be followed up beyond recovery. Awareness of red flag symptoms and signs, high index of clinical suspicion, prompt diagnosis, and early initiation of treatment with amphotericin B, aggressive surgical debridement of the PNS, and orbital exenteration, where indicated, are essential for successful outcome.

This paper proposes two differential detection techniques for signal detection in mobile molecular communication (MMC) for targeted drug delivery (TDD) application. In MMC, a nano-transmitter and a nano-receiver are considered to be in Brownian motion in an extracellular fluid medium. Transmitter uses calcium molecules to communicate with the receiver. Detection is performed using concentration difference based detector (CDD) at the receiver which calculates the maximum absolute concentration difference of the received signal within the same bit interval to detect the bit. This improves the bit error rate (BER) performance in MMC. The performance is further enhanced using manchester coded transmission with differential detection (MCD). In MCD, Bit-1 is coded by the symbol [1 0] and Bit-0 is coded by the symbol [0 1] and the difference between peaks of signals received in consecutive bit duration is taken to detect the bit. Simulation results prove that the MCD technique is 3 dB less sensitive to inter symbol interference (ISI) than the CDD technique. The detection threshold is selected using maximum a posteriori probability (MAP) rule. The performance of these detectors is compared with other existing detection techniques. Results reveal that BER performance of the CDD and MCD better by at least 3 dB and 6 dB, respectively. The proposed CDD and MCD techniques perform better in different bit-sequence length, various initial distance and different bit duration than other existing techniques.
