A cirrose hepática (CH) é um dos principais fatores de risco para CHC, por isso, é de grande importância que todo paciente cirrótico, independente da etiologia, seja submetido ao rastreamento do CHC a cada 6 meses. Objetivo: Realizar uma análise crítica do rastreamento ultrassonográfico do CHC nos pacientes cirróticos acompanhados em serviços de referência em hepatologia, público e privados, no Espírito Santo. Pacientes e métodos: Estudo descritivo, transversal, através da análise retrospectiva de dados obtidos em prontuários de pacientes cirróticos de quaisquer etiologias atendidos nos serviços de referência em hepatologia do Espírito Santo. Com esses dados foi construída uma planilha no programa SPSS Statistics 17.0, para análise estatística. Resultados: Dos 255 pacientes analisados, 72,5% eram do sexo masculino e a principal etiologia da CH foi o uso abusivo do álcool (51,4%). O rastreamento ultrassonográfico foi realizado conforme preconizado, a cada 6 meses, em apenas 16/255 (6,3%) dos pacientes. O maior percentual dos pacientes, 43,5%, realizou Ultrassonografia de abdome (US), com intervalo de tempo maior 1 ano. O CHC foi diagnosticado em 32 pacientes (12,5%) e o vírus hepatite C (VHC), foi o fator etiológico da CH que se apresentou com significância estatística (p=0,021) no surgimento do CHC. Pacientes que realizaram US idealmente a cada 6 meses, ou próximo ao ideal, quando diagnosticados com CHC, a maioria (58,3%) foram identificados com nódulo menor 3 cm. Ao contrário, pacientes que realizaram seguimento ultrassonográfico de forma irregular, quando diagnosticados com CHC, a maioria (85,7%), já se apresentava com nódulos maiores que 5 cm. Conclusão: O rastreamento ultrassonográfico realizado de maneira ideal, ou próximo ao ideal, permitiu diagnóstico precoce do CHC. Pacientes em que o rastreamento foi realizado de forma irregular, em especial, com intervalo de tempo entre os US > 1ano, quando diagnosticados com o CHC, o mesmo se fez quando o nódulo já se apresentava com tamanho acima do preconizado para a realização de medidas terapêuticas que possibilitassem a cura e com isso redução de mortalidade. Abstract

A multicentre prospective survey on the use of artificial nutrition (AN) and its complications has been carried out on 1657 hospitalised patients including 7.8% in surgical departments, 7.1% in medical departments and 59.9% in intensive care units. Gastrointestinal diseases and cancer were the most frequent pathologies among patients requiring AN. Parenteral nutrition (PN) alone was employed in 1103 (66.5%) patients. In 267 (16.1%) PN was associated with enteral nutrition (EN). 287 (17.3%) patients were fed by the enteral route alone. The average daily energy intake was 35 kcal/kg for central PN, 26 kcal/kg for peripheral PN and 33 kcal/kg for EN. The mean daily nitrogen intake was 0.19 g/kg for central PN, 0.17 g/kg for peripheral PN and 0.20 g/kg for EN. In 10% of patients PN was electrolyte free and in only 50% all the main electrolytes were added. Trace elements were added in 48% of formulations, mainly as multiple combination. Insulin was added in 61% of PN patients, while albumin was given with PN in 20% of patients. Adverse reactions were observed in 10.9% of PN patients and in 11.5% of EN patients. The most frequent complications occurring in parenterally fed patients were infections of the central venous catheter and metabolic alterations (severe electrolyte abnormalities, liver or renal dysfunction, hypo or persistent hyperglycemia). In enterally fed patients the frequent complications were abdominal distension, cramps and diarrhoea. Adverse reactions were responsible for discontinuation of AN in 2.2% of parenterally supported patients and in 5.7% of enterally fed patients.

Abstract

A multicentre prospective survey on the use of artificial nutrition (AN) and its complications has been carried out on 1657 hospitalised patients including 7.8% in surgical departments, 7.1% in medical departments and 59.9% in intensive care units. Gastrointestinal diseases and cancer were the most frequent pathologies among patients requiring AN. Parenteral nutrition (PN) alone was employed in 1103 (66.5%) patients. In 267 (16.1%) PN was associated with enteral nutrition (EN). 287 (17.3%) patients were fed by the enteral route alone. The average daily energy intake was 35 kcal/kg for central PN, 26 kcal/kg for peripheral PN and 33 kcal/kg for EN. The mean daily nitrogen intake was 0.19 g/kg for central PN, 0.17 g/kg for peripheral PN and 0.20 g/kg for EN. In 10% of patients PN was electrolyte free and in only 50% all the main electrolytes were added. Trace elements were added in 48% of formulations, mainly as multiple combination. Insulin was added in 61% of PN patients, while albumin was given with PN in 20% of patients. Adverse reactions were observed in 10.9% of PN patients and in 11.5% of EN patients. The most frequent complications occurring in parenterally fed patients were infections of the central venous catheter and metabolic alterations (severe electrolyte abnormalities, liver or renal dysfunction, hypo or persistent hyperglycemia). In enterally fed patients the frequent complications were abdominal distension, cramps and diarrhoea. Adverse reactions were responsible for discontinuation of AN in 2.2% of parenterally supported patients and in 5.7% of enterally fed patients.

Abstract

The Authors report on tpo cases of strangulated obturator hernia observed in their department during the period 1977-1987 and successfully operated on by mid-line laparotomy, both pith a diagnosis of occlusion of the small bowel. The Authors review the clinical signs and instrumental investigations which may be of use in reaching an early pre-operative diagnosis, which constitutes the only possibility of reducing the mortality associated with this condition. Particular attention is devoted to the somatic characteristics of the patients: elderly, thin, chronic bronchitic, hypertensive cardiopathic. In one of the two cases, there was a concomitant crural hernia. In both cases, which presented a picture of acute intestinal occlusion, the type of surgery opted for was a mid-line laparotomy. In one case, an ileal resection was performed. To close the orifice of the obturator canal, plain suture of the peritoneum was used in one case and a mersilene patch in the other.
