1. Spatial distribution of available phosphorus in surface road and trackway surface materials on a sheep farm in Ireland

Abstract: Farm roadway runoff is a high-risk source of pollution when connectivity with waters occurs. Nutrients in this runoff are dominated by fresh animal deposits, but recent dairy and beef farm studies showed that available phosphorus (P) accumulates in roadway surface material and can be lost in runoff. A current knowledge gap is to examine available P concentrations in unsealed roadway and trackway (non-maintained) network of a lowland sheep farm. The present study focused on a 45 ha farm stocked with 544 sheep in south-east Ireland. Ten locations were sampled along with the adjacent fields for available P (i.e., Morgan’s P) and ancillary parameters (e.g., pH, total P and heavy metals) in December 2022. The first sampling location was on an aggregate roadway and the other nine were on trackways representing an older aggregate roadway network used by the flock but now covered with soil and grass. Results showed a distinct difference in surface material pH between roadway and trackway locations. Trackways had a pH that mimicked adjacent fields around the agronomic optimum for grassland of ∼6.2. All sampling locations had elevated available P concentrations, ranging from 26.3 to 111.0 mg L−1 (mean 62.8 mg L−1), similar to the spatial distribution for dairy farms but above those found at beef farms previously studied. The highest available P concentrations were found in roadway and trackway sections adjacent to the farmyard. Other elevated sampling areas were on trackways (87.3 or 97.7 mg P L−1) away from the farmyard where sheep are funnelled to pasture, stop to seek shade, urinate and defecate but do not graze. By contrast the average available P concentration for the surrounding fields was 8.4 mg L−1 with a range of 2.7–15.9 mg L−1. Two sampling areas combine to create a critical source area where a high available P source becomes visibly mobilised as runoff during rainfall, discharges into an open drainage ditch, which is then connected to a local stream. Breaking the pathway before runoff enters the open ditch could be a cheap and effective way of mitigating nutrient losses at these two locations.

2. Phosphorus in farm roadway substrates: contrasting spatial and temporal patterns in dairy and beef farms

Abstract: The mobilisation and impact of roadway runoff on dairy farms has been established as a sub-component of the nutrient transfer continuum. It is acknowledged that fresh nutrient sources deposited on roadways dominate runoff and recent work has shown that available phosphorus (P) in roadway substrates is also an important source component. The objectives of this study were to understand spatial and temporal available P concentrations in roadway substrate (aggregate and soil mix) across dairy and beef farms during the open (February–October, when cows use the roadway network and fields) and closed (November–January, when cows were housed) periods and identify locations which could be considered critical source areas (CSA). For the study, roadway substrate samples were taken at 18 locations on each farm, across eight sampling periods, and were analysed for available P. Results showed that mean available P concentrations (Morgan’s P) in the sampling locations ranged from 15.9 to 101.4 mg L-1 for the Dairy Farm, from 4.1 to 59.4 mg L-1 for Beef Farm 1 and from 6.3 to 23.2 mg L-1 for Beef Farm 2. In open period, the results showed that mean available P concentrations were 75.4 ± 30.9, 14.9 ± 3.2 and 13.4 ± 2.5 mg L-1 for the Dairy Farm, Beef Farm 1 and Beef Farm 2, respectively. In closed period, the mean available P were 40.3 ± 17.2, 10.4 ± 1.0 and 9.8 ± 0.2 mg L-1 for Dairy Farm, Beef Farm 1 and Beef Farm 2, respectively. Overall, P concentrations on the Dairy Farm roadways was up to 4 times greater than that in the Beef Farms’ roadways. Compared to soils in adjacent fields, P concentrations in the Dairy and Beef Farms roadways was up-to 7 and 2 times higher, respectively. Critical roadway sections that required mitigation were two in the Dairy Farm, one in Beef Farm 1 and two in Beef Farm 2. In addition to fresh faeces and urine (i.e., dominant source of nutrients in farm roadways), this study showed that nutrient enriched roadway materials are a labile P source. Thus, future mitigation of roadway runoff must consider all roadway nutrient sources, including livestock fresh excreta as well as used roadway surface materials.

Abstract: Objective We report on: (1) the proportion of patients with known atrial fibrillation (AF); and (2) demographic, clinical or radiological differences between patients with known AF (and not treated) and patients with newly diagnosed AF, in a cohort of patients who presented with ischaemic stroke or transient ischaemic attack (TIA) not previously treated with anticoagulation.Design We reviewed cross-sectional baseline demographic and clinical data from a prospective observational cohort study, (CROMIS-2).Setting Patients were recruited from 79 hospital stroke centres throughout the UK and one centre in the Netherlands.Participants Patients were eligible if they were adults who presented with ischaemic stroke or TIA and AF and had not been previously treated with oral anticoagulation.Main outcome measures Proportion of patients with known AF before index ischaemic stroke or TIA from a cohort of patients who have not been previously treated with oral anticoagulation. Secondary analysis includes the comparison of CHA2DS2-VASc and HAS-BLED scores and other demographics and risk factors between those with newly diagnosed AF and those with previously known AF.Results Of 1470 patients included in the analysis (mean age 76 years (SD 10)), 622 (42%) were female; 999 (68%) patients had newly diagnosed AF and 471 (32%) patients had known AF. Of the 471 patients with known AF, 68% had a strong indication for anticoagulation and 89% should have been considered for anticoagulation based upon CHA2DS2-VASc score. Patients with known AF were more likely to have a prior history of dementia (4% vs 2%, p=0.02) and had higher HAS-BLED scores (median 3 vs 2). CHA2DS2-VASc, other risk factors and demographics were similar.Conclusions About 1/3 of patients who present with stroke and have AF who have not been treated with oral anticoagulation have previously known AF. Of these patients, at least 68% were not adequately treated with oral anticoagulation.Trial registration number NCT02513316.

4. Rehabilitation aimed at improving outdoor mobility for people after stroke: a multicentre randomised controlled study (the Getting out of the House Study)

Abstract: Background: One-third of stroke patients are dependent on others to get outside their homes. This can cause people to become housebound, leading to increased immobility, poor health, isolation and misery. There is some evidence that outdoor mobility rehabilitation can reduce these limitations. Objective: To test the clinical effectiveness and cost-effectiveness of an outdoor mobility rehabilitation intervention for stroke patients. Design: Multicentre, parallel-group randomised controlled trial, with two groups allocated at a 1 : 1 ratio plus qualitative participant interviews. Setting: Fifteen UK NHS stroke services throughout England, Scotland and Wales. Participants: A total of 568 stroke patients who wished to get out of the house more often, mean age of 71 years: 508 reached the 6-month follow-up and 10 were interviewed. Intervention: Control was delivered prior to randomisation to all participants, and consisted of verbal advice and transport and outdoor mobility leaflets. Intervention was a targeted outdoor mobility rehabilitation programme delivered by 29 NHS therapists to 287 randomly chosen participants for up to 12 sessions over 4 months. Main outcome measures: Primary outcome was participant health-related quality of life, measured by the Short Form questionnaire-36 items, version 2 (Social Function domain), 6 months after baseline. Secondary outcomes were functional ability, mobility, number of journeys (from monthly travel diaries), satisfaction with outdoor mobility (SWOM), psychological well-being and resource use [health care and Personal Social Services (PSS)] 6 months after baseline. Carer well-being was recorded. All outcome measures were collected by post and repeated 12 months after baseline. Outcomes for the groups were compared using statistical significance testing and adjusted for multiple membership to account for the effect of multiple therapists at different sites. Interviews were analysed using interpretive phenomenology to explore confidence. Results: A median of seven intervention sessions [interquartile range (IQR) 3–7 sessions], median duration of 369 minutes (IQR 170–691.5 minutes) per participant was delivered. There was no significant difference between the groups on health-related quality of life (social function). There were no significant differences between groups in functional ability, psychological well-being or SWOM at 6- or 12-month follow-ups. There was a significant difference observed for travel journeys with the intervention group being 42% more likely to make a journey compared with the control group [rate ratio 1.42, 95% confidence interval (95% CI) 1.14 to 1.67] at 6 months and 76% more likely (rate ratio 1.76, 95% CI 1.36 to 1.95) at 12 months. The number of journeys was affected by the therapist effect. The mean incremental cost (total NHS and PSS cost) of the intervention was £3413.75 (95% CI –£448.43 to £7121.00), with an incremental quality-adjusted life-year gain of –0.027 (95% CI –0.060 to 0.007) according to the European Quality of Life-5 Dimensions and –0.003 (95% CI –0.016 to 0.006) according to the Short Form questionnaire-6 Dimensions. At baseline, 259 out of 281 (92.2%) participants in the control group were dissatisfied with outdoor mobility but at the 6-month assessment this had reduced to 77.7% (181/233), a 15% reduction. The corresponding reduction in the intervention group was slightly greater (21%) than 268 out of 287 (93.4%) participants dissatisfied with outdoor mobility at baseline to 189 out of 261 (72.4%) at 6 months. Participants described losing confidence after stroke as being detrimental to outdoor mobility. Recruitment and retention rates were high. The intervention was deliverable by the NHS but had a neutral effect in all areas apart from potentially increasing the number of journeys. This was dependent on the therapist effect, meaning that some therapists were more successful than others. The control appeared to affect change. Conclusions: The outdoor mobility intervention provided in this study to these stroke patients was not clinically effective or cost-effective. However, the provision of personalised information and monthly diaries should be considered for all people who wish to get out more. Trial registration: Current Controlled Trials ISRCTN58683841. 5. Impact of agricultural land use and management on available soil phosphorus content in agricultural catchments of Ireland

Abstract: The build-up of soil phosphorus (P) in agricultural soils exceeding crop requirements can lead to diffuse P losses that could impair surface water quality. Therefore, adequate spatial information is required to develop viable tools and recommendations for sustainable P management at the local scale. Here, we present a database of nearly 8.000 samples, collected over a 12-year period in four meso-scale (~10 km2) agricultural catchments in Ireland. The agricultural area of each catchment is divided into sampling units (up to 2 ha) and soil samples are repeatedly taken from each sampling unit every 4 years. Four soil sampling campaigns were carried out to date. The results were analysed in the context of soil test P values (Morgan’s P) and classified according to the P index system as defined in the Ireland’s Nitrates Action Programme.Overall, levels of soil test P did not show substantial changes, with the exception of the most recent sampling campaign. However, when the collected data are considered in a spatial context and accompanied with soil data and land use information, they reveal a more complex story. Notable differences in soil P trends are observed at the individual catchments scale and impacted by land use, agricultural management intensity and some soil properties across and within the catchments. Similarly to the overall soil test P trends, the total area under P index 4 soils (above optimal) decreased in the period preceding the most recent sampling campaign. The most notable decreases in P index 4 soils are found in tillage and drystock fields, but also in the catchment dominated by highly stocked dairy farms availing of a nitrate derogation.Recent increases in soil test P and consequently areas under P index 4 may not be linked to increased organic or mineral P inputs, but rather come as a result of an overall increase in soil pH from increased lime application observed over the most recent period, which had an impact on the extractable Morgan’s P content. On-farm redistribution of fertilizer P inputs to soils with lower P index status has the potential to increase P use efficiency and decrease P loss risk to surface water.

Abstract: Background: The “1-3-6-12-day rule” for starting direct oral anticoagulants (DOACs) in patients with nonvalvular atrial fibrillation after acute ischemic stroke or transient ischemic attack recommends timings that may be later than used in clinical practice. We investigated more practical optimal timing of DOAC initiation according to stroke severity. Methods: The combined data of prospective registries in Japan, Stroke Acute Management with Urgent Risk-factor Assessment and Improvement-nonvalvular atrial fibrillation (September 2011 to March 2014) and RELAXED (February 2014 to April 2016) were used. Patients were divided into transient ischemic attack and 3 stroke subgroups by the National Institutes of Health Stroke Scale score: mild (0–7), moderate (8–15), and severe (≥16). The early treatment group was defined as patients starting DOACs earlier than the median initiation day in each subgroup. Outcomes included a composite of recurrent stroke or systemic embolism, ischemic stroke, and severe bleeding within 90 days. Six European prospective registries were used for validation. Results: In the 1797 derivation cohort patients, DOACs were started at median 2 days after transient ischemic attack and 3, 4, and 5 days after mild, moderate, and severe strokes, respectively. Stroke or systemic embolism was less common in Early Group (n=785)—initiating DOACS within 1, 2, 3, and 4 days, respectively—than Late Group (n=1012) (1.9% versus 3.9%; adjusted hazard ratio, 0.50 [95% CI, 0.27–0.89]), as was ischemic stroke (1.7% versus 3.2%, 0.54 [0.27–0.999]). Major bleeding was similarly common in the 2 groups (0.8% versus 1.0%). On validation, both ischemic stroke (2.4% versus 2.2%) and intracranial hemorrhage (0.2% versus 0.6%) were similarly common in Early (n=547) and Late (n=1483) Groups defined using derivation data. Conclusions: In Japanese and European populations, early DOAC initiation within 1, 2, 3, or 4 days according to stroke severity seemed to be feasible to decrease the risk of recurrent stroke or systemic embolism and no increase in major bleeding. These findings support ongoing randomized trials to better establish the optimal timing of DOAC initiation.

Abstract: Contains fulltext : 235277.pdf (Publisher’s version ) (Closed access) BACKGROUND: Balancing the risks of recurrent ischaemic stroke and intracranial haemorrhage is important for patients treated with antithrombotic therapy after ischaemic stroke or transient ischaemic attack. However, existing predictive models offer insufficient performance, particularly for assessing the risk of intracranial haemorrhage. We aimed to develop new risk scores incorporating clinical variables and cerebral microbleeds, an MRI biomarker of intracranial haemorrhage and ischaemic stroke risk. METHODS: We did a pooled analysis of individual-patient data from the Microbleeds International Collaborative Network (MICON), which includes 38 hospital-based prospective cohort studies from 18 countries. All studies recruited participants with previous ischaemic stroke or transient ischaemic attack, acquired baseline MRI allowing quantification of cerebral microbleeds, and followed-up participants for ischaemic stroke and intracranial haemorrhage. Participants not taking antithrombotic drugs were excluded. We developed Cox regression models to predict the 5-year risks of intracranial haemorrhage and ischaemic stroke, selecting candidate predictors on biological relevance and simplifying models using backward elimination. We derived integer risk scores for clinical use. We assessed model performance in internal validation, adjusted for optimism using bootstrapping. The study is registered on PROSPERO, CRD42016036602. FINDINGS: The included studies recruited participants between Aug 28, 2001, and Feb 4, 2018. 15 766 participants had follow-up for intracranial haemorrhage, and 15 784 for ischaemic stroke. Over a median follow-up of 2 years, 184 intracranial haemorrhages and 1048 ischaemic strokes were reported. The risk models we developed included cerebral microbleed burden and simple clinical variables. Optimism-adjusted c indices were 0·73 (95% CI 0·69-0·77) with a calibration slope of 0·94 (0·81-1·06) for the intracranial haemorrhage model and 0·63 (0·62-0·65) with a calibration slope of 0·97 (0·87-1·07) for the ischaemic stroke model. There was good agreement between predicted and observed risk for both models. INTERPRETATION: The MICON risk scores, incorporating clinical variables and cerebral microbleeds, offer predictive value for the long-term risks of intracranial haemorrhage and ischaemic stroke in patients prescribed antithrombotic therapy for secondary stroke prevention; external validation is warranted. Abstract: Background and Purpose: The causes of recurrent ischemic stroke despite anticoagulation for atrial fibrillation are uncertain but might include small vessel occlusion. We investigated whether magnetic resonance imaging markers of cerebral small vessel disease (SVD) are associated with ischemic stroke risk during follow-up in patients anticoagulated for atrial fibrillation after recent ischemic stroke or transient ischemic attack. Methods: We analyzed data from a prospective multicenter inception cohort study of ischemic stroke or transient ischemic attack anticoagulated for atrial fibrillation (CROMIS-2 [Clinical Relevance of Microbleeds in Stroke Study]). We rated markers of SVD on baseline brain magnetic resonance imaging: basal ganglia perivascular spaces (number ≥11); cerebral microbleeds (number ≥1); lacunes (number ≥1); and white matter hyperintensities (periventricular Fazekas grade 3 or deep white matter Fazekas grade ≥2). We investigated the associations of SVD presence (defined as presence of ≥1 SVD marker) and severity (composite SVD score) with the risk of ischemic stroke during follow-up using a Cox proportional hazards model adjusted for congestive heart failure, hypertension, age >75, diabetes, stroke, vascular disease, age 65–74, female score. Results: We included 1419 patients (mean age: 75.8 years [SD, 10.4]; 42.1% female). The ischemic stroke rate during follow-up in patients with any SVD was 2.20 per 100-patient years (95% CI, 1.60–3.02), compared with 0.98 per 100 patient-years (95% CI, 0.59–1.62) in those without SVD ( P =0.008). After adjusting for congestive heart failure, hypertension, age >75, diabetes, stroke, vascular disease, age 65–74, female score, SVD presence remained significantly associated with ischemic stroke during follow-up (hazard ratio, 1.89 [95% CI, 1.01–3.53]; P =0.046); the risk of recurrent ischemic stroke increased with SVD score (hazard ratio per point increase, 1.33 [95% CI, 1.04–1.70]; P =0.023). Conclusions: In patients anticoagulated for atrial fibrillation after ischemic stroke or transient ischemic attack, magnetic resonance imaging markers of SVD are associated with an increased risk of ischemic stroke during follow-up; improved stroke prevention treatments are required in this population. Abstract: Reducing legacy soil phosphorus (P) is recognised as an effective measure to mitigate diffuse P losses from agricultural landscapes and alleviate trophic pressure to freshwaters systems. Accounting for the distribution of P within farms is critical in identifying fields of agronomic underperformance and/or environmental risk to water as a consequence of inadequately managed re-cycling of P. There is also a need to understand how P use and legacy soil P evolves under the Nitrates Action Programme (NAP) regulations from the European Union (EU) Nitrates Directive. In an Irish case study the aim was to provide a systematic and detailed audit of P balance and soil P responses and trends in two mixed land use agricultural catchments (Arable A and B) across a four year study period. Driven by increased mineral P inputs the field balances in the Arable A catchment had an average surplus P, ranging from 1.9 to 7.5 kg ha−1 yr−1. However, between the study period 2010 to 2013, the average soil test P (STP) levels declined, with the area of excessive soil P concentrations decreasing by 8%. Similarly, in the Arable B catchment the average annual P inputs increased the surplus field P from -0.42 to 25.5 kg ha−1 yr−1, but the area of excessive soil P concentrations increased by 4%. In part, this increase is attributed to some fields receiving excess applications of organic nutrient forms above crop requirements. Whilst, the legacy soil P declined in the Arable A catchment indicating a response to NAP, for both catchments it is evident that the distribution of P sources within farms was poor and P inputs often did not match crop and soil P requirements at the field scale. This study highlights the need for improved support to knowledge transfer mechanisms that can deliver better farm and soil specific nutrient management planning strategies. Without this consideration, achieving the dual benefits of improvement to water quality and increased crop output from agricultural landscapes will be restricted.

Abstract: Background It is likely that a proportion of poststroke cognitive impairment is sometimes attributable to unidentified prestroke decline; prestroke cognitive function is also clinically relevant because it is associated with poor functional outcomes, including death. We investigated the radiological and prognostic associations of preexisting cognitive impairment in patients with ischemic stroke or transient ischemic attack associated with atrial fibrillation. Methods and Results We included 1102 patients from the prospective multicenter observational CROMIS ‐2 (Clinical Relevance of Microbleeds in Stroke 2) atrial fibrillation study. Preexisting cognitive impairment was identified using the 16‐item Informant Questionnaire for Cognitive Decline in the Elderly. Functional outcome was measured using the modified Rankin scale. Preexisting cognitive impairment was common (n=271; 24.6%). The presence of lacunes (odds ratio [OR], 1.50; 95% CI , 1.03–1.05; P =0.034), increasing periventricular white matter hyperintensity grade (per grade increase, OR , 1.38; 95% CI , 1.17–1.63; P OR , 1.26; 95% CI , 1.05–1.51; P =0.011), and medial temporal atrophy grade (per grade increase, OR , 1.61; 95% CI , 1.34–1.95; P mRS >2; adjusted OR , 2.43; 95% CI , 1.42–4.20; P =0.001). Conclusions Preexisting cognitive impairment in patients with atrial fibrillation–associated ischemic stroke or transient ischemic attack is common, and associated with imaging markers of cerebral small vessel disease and neurodegeneration, as well as with longer‐term functional outcome. Abstract: Information on the origin (status) and regeneration of plant species improves our understanding of native distributions and the establishment of non-native species. However, current categories used to record status in Britain and Ireland, whilst conceptually informative, rely on a knowledge of persistence that is impossible to assess objectively during a single (one-off) recording visit. We propose five alternative categories that focus on origin (how a species arrived at a site) rather than persistence. The first two categories apply to nationally native taxa: (1) populations that are unequivocally native and (2) those that are likely to have been introduced and/or are spreading for reasons that are obscure. The other three categories cover the occurrences of any taxon, native or non-native, that is known or suspected to have been introduced to a site: (3) introductions with unknown/obscure origins; (4) deliberate introductions; and (5) accidental introductions. For the introduced categories 3-5 we recommend that botanists also record signs of regeneration, i.e. seedlings or widely scattered patches, as a more objective measure whether a species is likely to be self-sustaining in a given locality.

Abstract: Contains fulltext : 208975.pdf (Publisher’s version ) (Open Access) BACKGROUND: Cerebral microbleeds are a neuroimaging biomarker of stroke risk. A crucial clinical question is whether cerebral microbleeds indicate patients with recent ischaemic stroke or transient ischaemic attack in whom the rate of future intracranial haemorrhage is likely to exceed that of recurrent ischaemic stroke when treated with antithrombotic drugs. We therefore aimed to establish whether a large burden of cerebral microbleeds or particular anatomical patterns of cerebral microbleeds can identify ischaemic stroke or transient ischaemic attack patients at higher absolute risk of intracranial haemorrhage than ischaemic stroke. METHODS: We did a pooled analysis of individual patient data from cohort studies in adults with recent ischaemic stroke or transient ischaemic attack. Cohorts were eligible for inclusion if they prospectively recruited adult participants with ischaemic stroke or transient ischaemic attack; included at least 50 participants; collected data on stroke events over at least 3 months follow-up; used an appropriate MRI sequence that is sensitive to magnetic susceptibility; and documented the number and anatomical distribution of cerebral microbleeds reliably using consensus criteria and validated scales. Our prespecified primary outcomes were a composite of any symptomatic intracranial haemorrhage or ischaemic stroke, symptomatic intracranial haemorrhage, and symptomatic ischaemic stroke. We registered this study with the PROSPERO international prospective register of systematic reviews, number CRD42016036602. FINDINGS: Between Jan 1, 1996, and Dec 1, 2018, we identified 344 studies. After exclusions for ineligibility or declined requests for inclusion, 20 322 patients from 38 cohorts (over 35 225 patient-years of follow-up; median 1.34 years [IQR 0.19-2.44]) were included in our analyses. The adjusted hazard ratio [aHR] comparing patients with cerebral microbleeds to those without was 1.35 (95% CI 1.20-1.50) for the composite outcome of intracranial haemorrhage and ischaemic stroke; 2.45 (1.82-3.29) for intracranial haemorrhage and 1.23 (1.08-1.40) for ischaemic stroke. The aHR increased with increasing cerebral microbleed burden for intracranial haemorrhage but this effect was less marked for ischaemic stroke (for five or more cerebral microbleeds, aHR 4.55 [95% CI 3.08-6.72] for intracranial haemorrhage vs 1.47 [1.19-1.80] for ischaemic stroke; for ten or more cerebral microbleeds, aHR 5.52 [3.36-9.05] vs 1.43 [1.07-1.91]; and for >/=20 cerebral microbleeds, aHR 8.61 [4.69-15.81] vs 1.86 [1.23-1.82]). However, irrespective of cerebral microbleed anatomical distribution or burden, the rate of ischaemic stroke exceeded that of intracranial haemorrhage (for ten or more cerebral microbleeds, 64 ischaemic strokes [95% CI 48-84] per 1000 patient-years vs 27 intracranial haemorrhages [17-41] per 1000 patient-years; and for >/=20 cerebral microbleeds, 73 ischaemic strokes [46-108] per 1000 patient-years vs 39 intracranial haemorrhages [21-67] per 1000 patient-years). INTERPRETATION: In patients with recent ischaemic stroke or transient ischaemic attack, cerebral microbleeds are associated with a greater relative hazard (aHR) for subsequent intracranial haemorrhage than for ischaemic stroke, but the absolute risk of ischaemic stroke is higher than that of intracranial haemorrhage, regardless of cerebral microbleed presence, antomical distribution, or burden. Abstract: Background: Results of small trials indicate that fluoxetine might improve functional outcomes after stroke. The FOCUS trial aimed to provide a precise estimate of these effects.Methods: FOCUS was a pragmatic, multicentre, parallel group, double-blind, randomised, placebo-controlled trial done at 103 hospitals in the UK. Patients were eligible if they were aged 18 years or older, had a clinical stroke diagnosis, were enrolled and randomly assigned between 2 days and 15 days after onset, and had focal neurological deficits. Patients were randomly allocated fluoxetine 20 mg or matching placebo orally once daily for 6 months via a web-based system by use of a minimisation algorithm. The primary outcome was functional status, measured with the modified Rankin Scale (mRS), at 6 months. Patients, carers, health-care staff, and the trial team were masked to treatment allocation. Functional status was assessed at 6 months and 12 months after randomisation. Patients were analysed according to their treatment allocation. This trial is registered with the ISRCTN registry, number ISRCTN83290762.Findings: Between Sept 10, 2012, and March 31, 2017, 3127 patients were recruited. 1564 patients were allocated fluoxetine and 1563 allocated placebo. mRS data at 6 months were available for 1553 (99·3%) patients in each treatment group. The distribution across mRS categories at 6 months was similar in the fluoxetine and placebo groups (common odds ratio adjusted for minimisation variables 0·951 [95% CI 0·839–1·079]; p=0·439). Patients allocated fluoxetine were less likely than those allocated placebo to develop new depression by 6 months (210 [13·43%] patients vs 269 [17·21%]; difference 3·78% [95% CI 1·26–6·30]; p=0·0033), but they had more bone fractures (45 [2·88%] vs 23 [1·47%]; difference 1·41% [95% CI 0·38–2·43]; p=0·0070). There were no significant differences in any other event at 6 or 12 months.Interpretation: Fluoxetine 20 mg given daily for 6 months after acute stroke does not seem to improve functional outcomes. Although the treatment reduced the occurrence of depression, it increased the frequency of bone fractures. These results do not support the routine use of fluoxetine either for the prevention of post-stroke depression or to promote recovery of function.Funding: UK Stroke Association and NIHR Health Technology Assessment Programme.

Abstract: High-resolution water quality monitoring indicates recurring elevation of stream phosphorus concentrations during low-flow periods. These increased concentrations may exceed Water Framework Directive (WFD) environmental quality standards during ecologically sensitive periods. The objective of this research was to identify source, mobilization, and pathway factors controlling in-stream total reactive phosphorus (TRP) concentrations during low-flow periods. Synoptic surveys were conducted in three agricultural catchments during spring, summer, and autumn. Up to 50 water samples were obtained across each watercourse per sampling round. Samples were analysed for TRP and total phosphorus (TP), along with supplementary parameters (temperature, conductivity, dissolved oxygen, and oxidation reduction potential). Bed sediment was analysed at a subset of locations for Mehlich P, Al, Ca, and Fe. The greatest percentages of water sampling points exceeding WFD threshold of 0.035 mg L&minus, 1 TRP occurred during summer (57%, 11%, and 71% for well-drained, well-drained arable, and poorly drained grassland catchments, respectively). These percentages declined during autumn but did not return to spring concentrations, as winter flushing had not yet occurred. Different controls were elucidated for each catchment: diffuse transport through groundwater and lack of dilution in the well-drained grassland, in-stream mobilization in the well-drained arable, and a combination of point sources and cumulative loading in the poorly drained grassland. Diversity in controlling factors necessitates investigative protocols beyond low-spatial and temporal resolution water sampling and must incorporate both repeated survey and complementary understanding of sediment chemistry and anthropogenic phosphorus sources. Despite similarities in elevation of P at low-flow, catchments will require custom solutions depending on their typology, and both legislative deadlines and target baselines standards must acknowledge these inherent differences.

There is some evidence that outdoor mobility rehabilitation can reduce these limitations.To test the clinical effectiveness and cost-effectiveness of an outdoor mobility rehabilitation intervention for stroke patients.Multicentre, parallel-group randomised controlled trial, with two groups allocated at a 1 : 1 ratio plus qualitative participant interviews.Fifteen UK NHS stroke services throughout England, Scotland and Wales.A total of 568 stroke patients who wished to get out of the house more often, mean age of 71 years: 508 reached the 6-month follow-up and 10 were interviewed.Control was delivered prior to randomisation to all participants, and consisted of verbal advice and transport and outdoor mobility leaflets. Intervention was a targeted outdoor mobility rehabilitation programme delivered by 29 NHS therapists to 287 randomly chosen participants for up to 12 sessions over 4 months.Primary outcome was participant health-related quality of life, measured by the Short Form questionnaire-36 items, version 2 (Social Function domain), 6 months after baseline. Secondary outcomes were functional ability, mobility, number of journeys (from monthly travel diaries), satisfaction with outdoor mobility (SWOM), psychological well-being and resource use [health care and Personal Social Services (PSS)] 6 months after baseline. Carer well-being was recorded. All outcome measures were collected by post and repeated 12 months after baseline. Outcomes for the groups were compared using statistical significance testing and adjusted for multiple membership to account for the effect of multiple therapists at different sites. Interviews were analysed using interpretive phenomenology to explore confidence.A median of seven intervention sessions [interquartile range (IQR) 3-7 sessions], median duration of 369 minutes (IQR 170-691.5 minutes) per participant was delivered. There was no significant difference between the groups on health-related quality of life (social function). There were no significant differences between groups in functional ability, psychological well-being or SWOM at 6- or 12-month follow-ups. There was a significant difference observed for travel journeys with the intervention group being 42% more likely to make a journey compared with the control group [rate ratio 1.42, 95% confidence interval (95% CI) 1.14 to 1.67] at 6 months and 76% more likely (rate ratio 1.76, 95% CI 1.36 to 1.95) at 12 months. The number of journeys was affected by the therapist effect. The mean incremental cost (total NHS and PSS cost) of the intervention was £3413.75 (95% CI -£448.43 to £7121.00), with an incremental quality-adjusted life-year gain of -0.027 (95% CI -0.060 to 0.007) according to the European Quality of Life-5 Dimensions and -0.003 (95% CI -0.016 to 0.006) according to the Short Form questionnaire-6 Dimensions. At baseline, 259 out of 281 (92.2%) participants in the control group were dissatisfied with outdoor mobility but at the 6-month assessment this had reduced to 77.7% (181/233), a 15% reduction. The corresponding reduction in the intervention group was slightly greater (21%) than 268 out of 287 (93.4%) participants dissatisfied with outdoor mobility at baseline to 189 out of 261 (72.4%) at 6 months. Participants described losing confidence after stroke as being detrimental to outdoor mobility. Recruitment and retention rates were high. The intervention was deliverable by the NHS but had a neutral effect in all areas apart from potentially increasing the number of journeys. This was dependent on the therapist effect, meaning that some therapists were more successful than others. The control appeared to affect change.The outdoor mobility intervention provided in this study to these stroke patients was not clinically effective or cost-effective. Abstract: Background Up to 42% of all stroke patients do not get out of the house as much as they would like. This can impede a person’s quality of life. This study is testing the clinical effectiveness and cost effectiveness of a new outdoor mobility rehabilitation intervention by comparing it to usual care. Methods/design This is a multi-centre parallel group individually randomised, controlled trial. At least 506 participants will be recruited through 15 primary and secondary care settings and will be eligible if they are over 18 years of age, have had a stroke and wish to get out of the house more often. Participants are being randomly allocated to either the intervention group or the control group. Intervention group participants receive up to 12 rehabilitation outdoor mobility sessions over up to four months. The main component of the intervention is repeated practice of outdoor mobility with a therapist. Control group participants are receiving the usual intervention for outdoor mobility limitations: verbal advice and provision of leaflets provided over one session. Outcome measures are being collected using postal questionnaires, travel calendars and by independent assessors. The primary outcome measure is the Social Function domain of the SF36v2 quality of life assessment six months after recruitment. The secondary outcome measures include: functional ability, mobility, the number of journeys (monthly travel diaries), satisfaction with outdoor mobility, mood, health-related quality of life, resource use of health and social care. Carer mood information is also being collected. The mean Social Function score of the SF-36v2 will be compared between treatment arms using a multiple membership form of mixed effects multiple regression analysis adjusting for centre (as a fixed effect), age and baseline Social Function score as covariates and therapist as a multiple membership random effect. Regression coefficients and 95% confidence intervals will be presented. Discussion This study protocol describes a pragmatic randomised controlled trial that will hopefully provide robust evidence of the benefit of outdoor mobility interventions after stroke for clinicians working in the community. Abstract: Introduction Myasthenia gravis is a commonly undiagnosed condition in the elderly. Statin medications can cause weakness and are linked to the development and deterioration of several autoimmune conditions, including myasthenia gravis. Case presentation We report the case of a 60-year-old Caucasian man who presented with acute onset of dysarthria and dysphagia initially attributed to a brain stem stroke. Oculobulbar and limb weakness progressed until myasthenia gravis was diagnosed and treated, and until statin therapy was finally withdrawn. Conclusion Myasthenia gravis may be underappreciated as a cause of acute bulbar weakness among the elderly. Statin therapy appeared to have contributed to the weakness in our patient who was diagnosed with myasthenia gravis.
