Zusammenfassung Hintergrund Die chronische Nierenkrankheit (CKD) ist eine häufige Erkrankung, insbesondere im höheren Alter. Um der Progression der Erkrankung und deren Komplikationen vorzubeugen, ist eine leitliniengerechte ambulante Versorgung von Patient:innen mit CKD anzustreben. Zur Messung und Bewertung der Versorgungsqualität können Qualitätsindikatoren (QI) genutzt werden. In Deutschland existieren bisher keine QI für CKD. Ziel der Arbeit war die Entwicklung von QI für die Qualitätsüberprüfung der ambulanten Versorgung von Patient:innen über 70 Jahren mit nichtdialysepflichtiger CKD. Material und Methoden Auf Grundlage der nationalen S3-Leitlinie CKD und eines Reviews internationaler QI wurde eine Liste von QI erstellt. Die ausgewählten QI wurden in 2 Sets eingeteilt: basierend auf Routinedaten (z. B. Abrechnungsdaten der Krankenkassen) und auf Datenerhebung in der Praxis (Chart-Review). Expert:innen verschiedener Fachrichtungen sowie ein Patient:innenvertreter bewerteten diese in einem Delphi-Verfahren mit 2‑stufiger Onlinebefragung im Oktober 2021 und Januar 2022 und abschließender Konsensuskonferenz im März 2022. Zusätzlich wurden Ranglisten der wichtigsten QI von jedem Set erstellt. Ergebnisse Ein Inzidenz- und ein Prävalenzindikator wurden a priori festgelegt und standen nicht zur Abstimmung. Weitere 21 QI standen zur Abstimmung durch die Expert:innen. Für jedes QI-Set wurden die 7 wichtigsten Indikatoren ausgewählt. Nur 1 QI wurde von dem Expert:innenpanel für den zusätzlichen Einsatz bei Erwachsenen unter 70 Jahren als nicht geeignet eingestuft. Diskussion Die QI sollen es ermöglichen, die Qualität der ambulanten Versorgung von Patient:innen mit CKD zu untersuchen, mit dem Ziel, die leitlinienkonforme ambulante Versorgung zu optimieren.

Guidelines for low back pain (LBP) management recommend addressing psychosocial risk factors such as stress and depression, which have been shown to play a prognostic role in nonspecific LBP. LBP management has been found to diverge from published recommendations. The reasons why remain unclear and may be related to patient views and expectations regarding the causes and treatment of LBP.We examined the degree to which patient views regarding psychosocial factors coincided with core recommendations and statements of the German national guideline for nonspecific LBP, as well as factors affecting those views.Cross-sectional study.Data were gathered from June 2018 through September 2018 in 13 general practices in Mecklenburg-Wets Pomerania, Germany.Practice staff approached all patients entering the practice, regardless of the reason for consultation, during 3 consecutive days and offered study participation. After providing informed consent, patients received a questionnaire to complete prior to consultation. Nonresponse bias was addressed by using inverse probability weights. Descriptive analysis and multivariate logistic regression models were performed.A total of 977 patient questionnaires were included in the analysis. One-third to one-half of the patients disagreed and one-third agreed that psychological problems and their treatment play a role in LBP management. A significant proportion (13-25%) was undecided. However, relaxation techniques were well accepted. Patients with higher education levels, poorer health status, and more severe LBP but no pain medication in the last 12 months were more likely to expect psychosocial diagnostics and treatment and regarded relaxation techniques as potentially helpful. More severe pain and lower levels of education were associated with disagreement with guideline recommendations and statements regarding management of psychosocial factors.Recall bias is possible, as patients were asked to recall their LBP history. However, we limited the recall time to the last 12 months. Data on income, employment status and co-morbidities were not collected and may have affected the responses. However, educational status, health status, and age were collected.A significant portion of patients did not agree that psychosocial aspects should be addressed in LBP. Pain severity, health status, level of education, and previous treatment experience appear to affect patient views. These results highlight the importance of careful patient counseling regarding psychosocial factors and screening for psychosocial problems in LBP, when indicated. Additionally, educational initiatives may help bring patient expectations into agreement with recommendations.

Background: Fatigue, dyspnea, and lack of energy and concentration are commonly interpreted as indicative of symptomatic anemia and may thus play a role in diagnostic and therapeutic decisions. Objective: To investigate the association between symptoms commonly attributed to anemia and the actual presence of anemia. Methods: Data from two independent cohorts of the Study of Health in Pomerania (SHIP) were analyzed. Interview data, laboratory data, and physical examination were individually linked with claims data from the Association of Statutory Health Insurance Physicians. A complete case analysis using logistic regression models was performed to evaluate the association of anemia with symptoms commonly attributed to anemia. The models were adjusted for confounders such as depression, medication, insomnia, and other medical conditions. Results: A total of 5979 participants (53% female, median age 55) were included in the analysis. Of those, 30% reported fatigue, 16% reported lack of energy, 16% reported lack of concentration, and 29% reported dyspnea and/or weakness. Anemia was prevalent in about 6% (379). The symptoms were more prevalent in participants with anemia. However, participants with anemia were older and had a poorer health status. There was no association in multivariate logistic regression models between the symptoms fatigue, lack of concentration, dyspnea, and/or weakness and anemia. Anemia was associated (OR: 1.45; 95% CI: 1.13–1.86) with lack of energy in the multivariate analysis. Other factors such as depression, insomnia, and medication were more strongly associated with the symptoms. Conclusion: The clinical symptoms commonly attributed to anemia are unspecific and highly prevalent both in non-anemic and anemic persons. Even in the presence of anemia, other diagnoses should be considered as causes such as depression, heart failure, asthma, and COPD, which are more closely associated with the symptoms. Further diagnostic research is warranted to explore the association of symptoms in different subgroups and settings in order to help clinical decision making.

BackgroundThyroid disorders are common in the adult German population. Little is known about guideline implementation in clinical practice and the prevalence of diagnostic procedures in ambulatory care. The study aims to investigate the use of thyroid hormone measurements, thyroid ultrasound, thyroid scintiscan and associated costs in ambulatory care at population level.MethodsData were derived from two independent population-based cohorts of the Study of Health In Pomerania (SHIP). Ambulatory billing data from the Association of Statutory Health Insurance Physicians Mecklenburg-Vorpommern were individually linked for the period 2002–2016 with SHIP data. The main outcomes were the frequency of outpatient ultrasound, scintiscan, serum TSH level measurement, free triiodothyronine (fT3) and free thyroxine (fT4) measurement, TSH-receptor-antibodies and microsomal antibodies measurement within 1 year and 3 years prior to the study entrance of the participants. Multinomial logistic regression models were used to assess the association of age, sex, thyroid medication intake and Charlson-Comorbidity-Index with frequency of TSH measurements and ultrasound examinations.ResultsA total of 5552 participants (47% male, median age 55) were included in the analysis. 25% (1409/5552) had a diagnosed thyroid disorder or treatment, 40% (2191/5552) had clinical findings based on ultrasound or laboratory testing in SHIP only and 35% (1952/5552) neither a coded thyroid disorder or clinical finding nor thyroid medication. In the total study population 30% (1626/5552) received at least one TSH measurement, 6.8% (378/5552) at least one thyroid ultrasound and 2.6% (146/5552) at least one scintiscan within the past year before the study examination. Tests were performed more frequently in patients with thyroid medication and coded thyroid disorders. Hence, this group caused the highest expenditures.ConclusionsGiven the high prevalence of thyroid disorders, diagnostic and monitoring tests should be used rationally with regard to costs. TSH levels should be monitored regularly in patients on thyroid medication. A consensus on monitoring frequency and iteration of monitoring of morphological thyroid disorders with TSH and ultrasound and specific guideline recommendations are needed.Trial registrationVersorgungsforschung Deutschland (VfD_17_003880).

Die Ambulante Geriatrische Komplexbehandlung (AGKB) ist ein Modellprojekt fur multimorbide Patienten, mit der vertraglichen Vereinbarung, Hospitalisierungen und Pflegebedurftigkeit abzuwenden sowie Selbststandigkeit zu fordern. Sie bietet Gelegenheit, Polypharmazie zu reduzieren und potenziell inadaquate Arzneimittel (PIM) abzusetzen. Den Effekt der AGKB auf die Anzahl der Arzneimittel sowie der PIM zu erheben. Die Beobachtungsstudie basiert auf Abrechnungsdaten der AOK Nordost. Es erfolgte ein Abgleich der verordneten Arzneimittel mit der PRISCUS-Liste. Die Anzahl der Arzneimittel und der PIM im Zeitraum von 2 Quartalen vor und 2 Quartalen nach der AGKB wurden untersucht. Die Anzahl der verordneten Statine, Opioide, Antidementiva, Protonenpumpenhemmer und Antidepressiva wurde ermittelt und in Weiterverordnungen, Neuverordnungen und abgesetzte Medikamente eingeteilt. Von 699 Teilnehmern konnten 682 (73 % Frauen, Durchschnittsalter 79 Jahre, SD ± 5) analysiert werden. Eine Veranderung in der Anzahl der Wirkstoffe und der PIM wurde nicht beobachtet. Psycholeptika, Antiphlogistika und Psychoanaleptika waren die am haufigsten verordneten PIM. Die Mehrheit der Statine, Opioide, Protonenpumpenhemmer, Antidepressiva und Antidementiva wurde weiterverordnet. Antidementiva waren die Medikamente mit den meisten Neuverordnungen. Durch die AGKB erfolgte keine Anderung der Anzahl der verordneten Arzneimittel und PIM. Ein strukturierter Medikamentenreview, einschlieslich eines Gesprachs mit dem weiterbetreuenden Hausarzt, sollte zur Verbesserung der Arzneimittelsicherheit und Reduktion von Polypharmazie und Unterversorgung in der AGKB gestarkt werden.

Kassenarztliche Abrechnungsdiagnosen werden u. a. zur Qualitatssicherung und fur Schatzungen von Pravalenz und Ressourcenverbrauch genutzt. Untersuchungen zur Validitat zeigten erhebliche Limitationen. Bisher gibt es in Deutschland keine populationsbasierten Daten zur Ubereinstimmung von ambulanten Abrechnungsdiagnosen mit klinischen Primardaten von Schilddrusenveranderungen. Untersucht wurde die Ubereinstimmung kassenarztlicher Abrechnungsdaten fur die ICD-Diagnosen Schilddrusenknoten, Struma, Hyperthyreose, Hypothyreose und Thyreoiditis mit Untersuchungsdaten und Angaben aus dem Interview der populationsbasierten Kohortenstudie der Study of Health in Pomerania (SHIP). Abrechnungsdaten der Kassenarztlichen Vereinigung Mecklenburg-Vorpommerns wurden auf individueller Ebene fur den Zeitraum 2002–2016 mit Daten der SHIP gelinkt. Die Ubereinstimmung wurde mittels Sensitivitat, Spezifitat sowie positiv und negativ pradiktiven Wertes (PPW, NPW) untersucht. Alle Angaben wurden gewichtet, um die Bevolkerungsreprasentativitat zu gewahrleisten. In die Analyse wurden Daten von 5746 Probanden (46 % Manner, durchschnittliches Alter 55 Jahre, SD [Standardabweichung] ± 15, Min.: 20 Jahre, Max.: 93 Jahre) eingeschlossen. Auf Basis der Untersuchungsdaten hatten 63 % der Probanden eine Schilddrusenveranderung (3451/5511, fehlende Werte n = 235), auf Basis der Abrechnungsdaten waren es 25 % (1421/5746). Die Sensitivitat betrug abhangig von der untersuchten Schilddrusenveranderung 12–36 %, die Spezifitat 84–98 %, der PPW lag am hochsten bei Schilddrusenknoten (75 %) und Hypothyreose (70 %); der NPW lag zwischen 63 % und 94 %. Schilddrusenveranderungen sind in der Bevolkerung sehr haufig und oft nicht diagnostiziert. Kassenarztliche Abrechnungsdaten haben eine geringe Sensitivitat, um klinisch relevante Schilddrusenveranderungen abzubilden.

Background Ear irrigation is a commonly used method for removing earwax in general practice. There is no firm evidence if no pre-treatment is as good as pre-treatment with various standard preparations. Aim To assess the effectiveness of no pre-treatment compared to pre-treatment with commercially available cerumenolytics and to assess which preparation is best suited for pre-treatment. Methods This is a pragmatic observational study of patients with cerumen treated from a single GP with 3 different preparations or no preparation prior to standardized ear irrigation. Generalized linear mixed models with logit link function were performed to assess the effectiveness of pre-treatment with different preparations and no pre-treatment. The models were adjusted for age group (Results A total of 168 patients (298 ears, 58 % female, median age 65 years) consulted for obstructive cerumen, some of them several times. The cerumen was successfully removed in 70% (208/298). Comparing any preparation to no preparation (aggregated comparison), the odds ratio for complete clearance was 1.35 (95%confidence interval: 0.69-2.65). Comparing the preparations individually, the odds ratio of the docusate-sodium-based preparation was 1.87 (95% CI: 0.79-4.42) indicating a higher effectiveness. Although, not statistically significant. Ear irrigation was less successful for patients aged ≥ 70 years (OR = 0.48, 95% CI: 0.23-0.98). Conclusions The aggregated comparison indicates a slight trend toward a higher effectiveness of any pre-treatment compared to no pre-treatment. The effect-size of docusate-sodium-based pre-treatment indicates a higher effectiveness of cerumen impaction removal. Nevertheless, superiority could not be shown conclusively according to the statistical significance given the restricted sample size.

BackgroundLow back pain is a common reason for patients to seek medical care. Physician non-adherence to clinical guidelines has been observed. We investigated the extent to which patient expectations correspond to recommendations of the German national guideline for management of low back pain (G-LBP) and whether patient characteristics, history of LBP and previous treatment experience are associated with expectations.MethodsA cross-sectional study including patients from 13 general practices was conducted. Data were collected using a questionnaire. Inverse probability weights were used to address non-response bias. Descriptive analysis and multivariate logistic regression models were performed.ResultsA total of 977 patients were included in analyses (median age 57 years, 39% male). 75% of patients reported experiencing LBP currently or within the last year. More than 65% indicated they would agree to forgo further examinations if their LBP was judged by their physician to be of no serious concern. This was associated with the highest level of education and no prior imaging, and negatively associated with good-to-poor health status and moderate-to-severe pain intensity. 40% of participants expected imaging. The highest educational level, female gender and no prior imaging were associated with a decreased expectation of imaging. 70% expected prescriptions for massages. Females, participants with good-to-poor health status, current LBP or LBP in the last 12 months had an increased expectation for massages. Expectations for injection therapy (45%) were mainly associated with previous injections. Expectations for physiotherapy (64%) were associated with female gender, lower educational level, good-to-poor health status, current LBP or in the last 12 months. The perspective that daily activities should be continued (66%) was associated with female gender and higher educational level. Participants who agreed to the statement ‘There is no effective treatment for LBP’ (11%) had a poor health status, current LBP and a severe pain intensity.ConclusionPatient views regarding LBP management are partially concordant with guideline recommendations and are strongly influenced by previous treatment experiences and education level. Exploration of patient expectations and experiences in LBP treatment may help minimize dissatisfaction of patients expecting treatments not endorsed by guidelines and simultaneously increase physician guideline adherence.

Zusammenfassung Einleitung Für eine korrekte Einnahme und einen sicheren Umgang mit der eigenen Medikation ist es wichtig, dass Patienten bestimmte Informationen über ihre einzunehmenden Medikamente kennen und Auskunft geben können. Die Studie untersucht, welche Auskunft Patienten über ihre verordneten Medikamente geben können und welche Faktoren die Auskunftsfähigkeit beeinflussen. Methoden In dieser Querschnittstudie wurden 637 Patienten in 10 Allgemeinarztpraxen (Teilnahmerate 57 %) über Name, Dosis, Einnahmefrequenz und Indikation ihrer verordneten Medikamente befragt. Die Patientenangaben wurden mit der Praxisdokumentation verglichen. Ergebnisse Die Patientenangaben zur Anzahl der Medikamente stimmten zu 54 % vollständig mit der Praxisdokumentation überein. Es besteht eine relevante Diskrepanz zwischen den dokumentierten Medikamentenverordnungen und den Angaben von Patienten. Die Patienten konnten am besten über den Medikamentennamen (75 %) Auskunft geben und am schlechtesten über die Indikationen (47 %). Die Auskunftsfähigkeit ließ bei Patienten mit ≥ 5 Medikamenten nach. Eine höhere Schulbildung war mit einem höheren Anteil korrekter Angaben assoziiert. 65 % der Patienten besaßen einen Medikationsplan und 19 % nutzen diesen zur Beantwortung der Fragen. Der Besitz eines Medikationsplans alleine zeigte keinen positiven Effekt auf die korrekte Auskunftsfähigkeit, jedoch seine Anwendung. Dieser Effekt war bei Patienten mit ≥ 5 Medikamenten stärker ausgeprägt. Diskussion Die meisten Patienten nutzten den Medikationsplan nicht, weswegen die Ausstellung eines Medikationsplans per se nicht zu einer besseren Auskunftsfähigkeit führt. Technische Lösungen, wie zentrale Datenspeicherung, sollten entwickelt werden, die unabhängig von einem unvollständigen und im Bedarf oft nicht vorhandenen Medikationsplan aus Papier oder elektronischer Gesundheitskarte die Versorgungs- und Arzneimitteltherapiesicherheit und den Austausch zwischen verschiedenen Ärzten und Institutionen gewährleisten können.

Zusammenfassung Ziel der Studie Die Beobachtungsstudie untersucht den Verlauf klinischer Endpunkte von multimorbiden geriatrischen Patienten, die an einer 4-wöchigen Ambulanten Geriatrischen Komplexbehandlung (AGKB) teilgenommen haben. Methodik Die Analysen basierten auf Daten von 128 Patienten. Tests des geriatrischen Basisassessments und die EQ5D-Skala wurden vor, unmittelbar nach sowie 6 und 12 Monate nach der AGKB eingesetzt. Analysen erfolgten mittels Multilevel Mixed-Effekt-Regressions Modellen. Ergebnisse Die AGKB verbesserte die Selbstständigkeit, die Mobilität, das Gleichgewicht, das Sturzrisiko und den allgemeinen Gesundheitszustand. Die kognitive Leistung nahm geringfügig ab. Schlussfolgerung Die patientenbezogenen klinischen Endpunkte geben Hinweise auf einen positiven Einfluss funktioneller Defizite.

Hintergrund Akupunktur wurde 2007 fur die Indikationen chronische Knie- und Ruckenschmerzen in den Leistungskatalog der gesetzlichen Krankenversicherung aufgenommen. Die Studie untersucht, wer Akupunktur erhalt, wer Akupunktur anbietet und ob es zeitliche und regionale Unterschiede gibt.

Billing diagnoses are used for quality assurance, estimates of prevalence and resource allocation. Validity studies showed relevant limitations. In Germany, there are no population-based data on the agreement of outpatient billing diagnoses with clinical data of thyroid disorders.The study investigated the agreement of ICD-diagnosed thyroid nodules, goitre, hyperthyroidism, hypothyroidism and thyroiditis with clinical and self-reported data from the population-based cohort study called the Study of Health in Pomerania (SHIP).Billing data from the Association of Statutory Health Insurance Physicians Mecklenburg-Vorpommern were linked on an individual level for the period from 2002-2016 with data from SHIP. The agreement was evaluated using sensitivity, specificity and positive and negative predictive value (PPW, NPW). Data were weighted to ensure population representativeness.The data of 5746 participants were analysed (46% male, average age 55 years, SD [standard deviation] ± 15, min: 20 years, max: 93 years). Based on clinical data, 63% (3451/5511, missing values n = 235) and based on billing data 25% (1421/5746) of the participants had thyroid disorders. The sensitivity was 12-36%, the specificity was 84-98%, the PPW was highest for thyroid nodules (75%) and hypothyroidism (70%) and the NPW was between 63 and 94%, depending on the investigated thyroid disorder.Thyroid disorders are common and often undiagnosed. Billing data have a low sensitivity to identify clinically relevant thyroid disorders.

Ambulatory geriatric rehabilitation (AGR) is a prevention program for multimorbid patients with the contractual agreement to prevent the need for care and hospital admissions and to promote self-sufficiency. It provides an opportunity to reduce the prevalence of polypharmacy and to discontinue potentially inadequate medication (PIM).The study investigated the effect of AGR on the number of drugs and PIM.The observational longitudinal study was based on claims data from the statutory health insurance AOK Nordost. A comparison of the prescribed drugs with the PRISCUS list was performed. The study investigated the effect of AGR on the number of drugs prescribed and PIM in the period of two quarterly periods before and two quarterly periods after AGR. The numbers of prescribed statins, opioids, antidementia drugs, proton pump inhibitors and antidepressants were assessed and grouped into further prescriptions, new prescriptions and discontinued drug prescriptions.Out of 699 participants 682 (73% women, mean age 79 years, SD ± 5 years) were analyzed. The number of substances and PIM remained at the same levels after AGR. Psycholeptic, antiphlogistic and psychoanaleptic drugs were the most frequently prescribed PIM. The majority of statins, opioids, antidementia drugs, proton pump inhibitors and antidepressants were prescribed further. Antidementia drugs were the medication with the most newly started prescriptions.The AGR has no influence on the number of prescribed drugs and PIM. A structured drug review and a conversation with the general practitioner should be strengthened to improve drug safety and reduce polypharmacy as well as undertreatment.

Objective To evaluate the effectiveness of an intervention including psychoeducation and yoga for depression management at the primary healthcare level in one district in the Ha Nam province, Vietnam. Method The Patient Health Questionnaire-9 (PHQ-9) was used for depression screening and follow-up. Screened patients were further diagnosed with the Mini-International Neuropsychiatric Diagnostic Interview, by a trained general doctor. A linear regression model, adjusted for age, gender and baseline PHQ-9 score was used to assess whether the intervention leads to decreased depression severity compared to standard care in the control communes. Results Both groups had similar PHQ-9 scores at baseline. The intervention group had on average significantly lower PHQ-9 scores after the intervention than the control group. Almost half of the patients in the intervention group recovered from depression, whereas nobody did in the control group. Conclusion The results indicate that the intervention can be more effective than standard care in treating depression. The mean change of the PHQ-9 score after the intervention is deemed to be of clinical relevance. Objectif Evaluer l'efficacite d'une intervention comprenant la psychoeducation et le yoga dans la prise en charge de la depression au niveau des soins de sante primaires dans un district de la province de Ha Nam, au Vietnam. Methode Le Patient Health Questionnaire-9 (PHQ-9) a ete utilise pour le depistage de la depression et pour le suivi. Les patients depistes ont encore ete diagnostiques avec la Mini-International Neuropsychiatric Diagnostic Interview, par un medecin generaliste forme. Un modele de regression lineaire, ajuste pour l’âge, le sexe et le score PHQ-9 de la ligne de base a ete utilise pour determiner si l'intervention entrainait une diminution de la severite de la depression par rapport au traitement standard dans les communes temoins. Resultats Les deux groupes avaient des scores PHQ-9 similaires au depart. Le groupe d'intervention a eu en moyenne des scores PHQ-9 significativement plus faibles apres l'intervention que le groupe temoin. Pres de la moitie des patients dans le groupe d'intervention a recupere de la depression, alors qu'aucun ne l'a fait dans le groupe temoin. Conclusion Les resultats indiquent que l'intervention peut etre plus efficace que le traitement standard dans le traitement de la depression. La variation moyenne du score PHQ-9 apres l'intervention est consideree comme cliniquement pertinente. Objetivo Evaluar la efectividad de una intervencion que incluia psicoeducacion y yoga para el manejo de la depresion a nivel de atencion primaria en un distrito de provincia de Ha Nam, Vietnam. Metodo Se utilizo el Cuestionario de Salud del Paciente - 9 (PHQ-9) para realizar un cribado de depresion y su seguimiento. Los pacientes evaluados fueron diagnosticados por un medico general entrenado para ello, utilizando la MINI Entrevista Psiquiatrica Internacional. Se utilizo un modelo de regresion lineal, ajustado por edad, genero y puntaje basal del PHQ-9 para evaluar si la intervencion conllevaba a una disminucion en la severidad de la depresion, comparada con los cuidados estandares en los controles. Resultados Ambos grupos tenian puntuaciones basales similares en el PHQ-9. El grupo que recibio la intervencion tenia en promedio una puntuacion promedio significativamente menor en el PHQ-9 despues de la intervencion que el grupo control. Casi la mitad de los pacientes en el grupo de intervencion se recuperaron de la depresion, mientras que nadie lo hizo en el grupo control. Conclusion Los resultados indican que la intervencion puede ser mas efectiva que los cuidados estandares para el tratamiento de la depresion. El cambio promedio en la puntuacion del PHQ-9 despues de la intervencion podria ser considerado de relevancia clinica.

For the correct intake and safe handling of their own medication it is important that patients know about their own medication and can give certain information. This study examines what information patients can provide about their prescribed medication and which factors influence the ability to provide information.A cross-sectional study was conducted. In 10 General Practice, 637 patients (participation rate 57 %) were asked about the name, dosage, frequency of intake and indication of their prescribed medication. The patient information were compared with the practice documentation.Patient data on the number of medications were 54 % consistent with the practice documentation. There is a relevant discrepancy between the documented drug prescription and the information provided by patients. The patients were best able to provide information about the names (75 %) of their medications and worst about the indication (47 %). The ability to provide information decreased in patients with ≥ 5 medications. An association between higher education and correct information was found. 65 % of the participants had a medication plan and 19 % used the plan to answer the questions. The possession of a medication plan showed no effect. However the use of a medication plan had an effect, which was even stronger in patients with ≥ 5 medications.Most patients did not use the medication plan, which is why the issue of a medication plan does not lead to better information. Technical solutions, such as centralized data storage, should be developed, which can guarantee the security of supply and drug therapy and the exchange between different physicians and institutions, regardless of an incomplete and often non-existent paper or electronic health card medication plan.Für eine korrekte Einnahme und einen sicheren Umgang mit der eigenen Medikation ist es wichtig, dass Patienten bestimmte Informationen über ihre einzunehmenden Medikamente kennen und Auskunft geben können. Die Studie untersucht, welche Auskunft Patienten über ihre verordneten Medikamente geben können und welche Faktoren die Auskunftsfähigkeit beeinflussen.In dieser Querschnittstudie wurden 637 Patienten in 10 Allgemeinarztpraxen (Teilnahmerate 57 %) über Name, Dosis, Einnahmefrequenz und Indikation ihrer verordneten Medikamente befragt. Die Patientenangaben wurden mit der Praxisdokumentation verglichen.Die Patientenangaben zur Anzahl der Medikamente stimmten zu 54 % vollständig mit der Praxisdokumentation überein. Es besteht eine relevante Diskrepanz zwischen den dokumentierten Medikamentenverordnungen und den Angaben von Patienten. Die Patienten konnten am besten über den Medikamentennamen (75 %) Auskunft geben und am schlechtesten über die Indikationen (47 %). Die Auskunftsfähigkeit ließ bei Patienten mit ≥ 5 Medikamenten nach. Eine höhere Schulbildung war mit einem höheren Anteil korrekter Angaben assoziiert. 65 % der Patienten besaßen einen Medikationsplan und 19 % nutzen diesen zur Beantwortung der Fragen. Der Besitz eines Medikationsplans alleine zeigte keinen positiven Effekt auf die korrekte Auskunftsfähigkeit, jedoch seine Anwendung. Dieser Effekt war bei Patienten mit ≥ 5 Medikamenten stärker ausgeprägt.Die meisten Patienten nutzten den Medikationsplan nicht, weswegen die Ausstellung eines Medikationsplans per se nicht zu einer besseren Auskunftsfähigkeit führt. Technische Lösungen, wie zentrale Datenspeicherung, sollten entwickelt werden, die unabhängig von einem unvollständigen und im Bedarf oft nicht vorhandenen Medikationsplan aus Papier oder elektronischer Gesundheitskarte die Versorgungs- und Arzneimitteltherapiesicherheit und den Austausch zwischen verschiedenen Ärzten und Institutionen gewährleisten können.

The observational study aims to investigate the course of clinical outcomes of geriatric, multimorbid patients who received ambulatory geriatric rehabilitation (AGR) for four weeks.Analyses were based on data from 128 patients. Basic geriatric assessment tests and the EQ5D-Scale were used before, directly after the intervention, as well as after 6 and 12 months. Data were analyzed using Mixed-Effects Regression-Models.AGR improved self-dependence, mobility, balance, risk of falls and general state of health. The cognitive conditions reduced slightly.The patient-related clinical endpoints indicate a positive influence of functional deficits.Die Beobachtungsstudie untersucht den Verlauf klinischer Endpunkte von multimorbiden geriatrischen Patienten, die an einer 4-wöchigen Ambulanten Geriatrischen Komplexbehandlung (AGKB) teilgenommen haben.Die Analysen basierten auf Daten von 128 Patienten. Tests des geriatrischen Basisassessments und die EQ5D-Skala wurden vor, unmittelbar nach sowie 6 und 12 Monate nach der AGKB eingesetzt. Analysen erfolgten mittels Multilevel Mixed-Effekt-Regressions Modellen.Die AGKB verbesserte die Selbstständigkeit, die Mobilität, das Gleichgewicht, das Sturzrisiko und den allgemeinen Gesundheitszustand. Die kognitive Leistung nahm geringfügig ab.Die patientenbezogenen klinischen Endpunkte geben Hinweise auf einen positiven Einfluss funktioneller Defizite.

Ambulatory geriatric rehabilitation (AGR) is a community based outpatient intervention which aims to improve physical function, maintain independent living of geriatric patients, avoiding hospitalisation and institutionalisation. It should therefore reduce health care costs. The objective of our study is to evaluate the effectiveness of AGR for frail elderly patients insured by the statutory health insurance AOK Nordost compared to care as usual. Outcome variables are progression to higher nursing care levels, admission to nursing home, incident fractures, hospital admission, ambulatory care sensitive hospital admissions, days spent in hospital, and health care costs. This matched cohort study aims to estimate average treatment effects of AGR. For this purpose we will compare patients receiving AGR with matched patients receiving care as usual. Patients in the intervention group were treated between the years 2009 and 2013 from three centres in Mecklenburg-Vorpommern, Germany. Matching will be conducted using propensity score techniques. Claims data will be provided by the statutory health insurance AOK Nordost. The primary outcomes are the progression of nursing care levels, incident fractures, and admission to nursing home. Secondary outcomes are total and ambulatory care sensitive hospital admissions, and health care costs from the statutory health insurance perspective. Data will be analysed using appropriate regression models. This study aims to quantify the effectiveness of AGR. Results will be important for providers of AGR, policy makers and stakeholders to make informed decisions on whether to continue, modify or extend AGR.
