The role of subclinical atrial fibrillation as a cause of cryptogenic stroke is unambiguously established. Long-term electrocardiogram (ECG) monitoring remains the sole method for determining its presence following a negative initial workup. This position paper of the European Society of Cardiology Working Group on e-Cardiology first presents the definition, epidemiology, and clinical impact of cryptogenic ischaemic stroke, as well as its aetiopathogenic association with occult atrial fibrillation. Then, classification methods for ischaemic stroke will be discussed, along with their value in providing meaningful guidance for further diagnostic efforts, given disappointing findings of studies based on the embolic stroke of unknown significance construct. Patient selection criteria for long-term ECG monitoring, crucial for determining pre-test probability of subclinical atrial fibrillation, will also be discussed. Subsequently, the two major classes of long-term ECG monitoring tools (non-invasive and invasive) will be presented, with a discussion of each method’s pitfalls and related algorithms to improve diagnostic yield and accuracy. Although novel mobile health (mHealth) devices, including smartphones and smartwatches, have dramatically increased atrial fibrillation detection post ischaemic stroke, the latest evidence appears to favour implantable cardiac monitors as the modality of choice; however, the answer to whether they should constitute the initial diagnostic choice for all cryptogenic stroke patients remains elusive. Finally, institutional and organizational issues, such as reimbursement, responsibility for patient management, data ownership, and handling will be briefly touched upon, despite the fact that guidance remains scarce and widespread clinical application and experience are the most likely sources for definite answers.

Left ventricular diastolic dysfunction is the underlying pathophysiological disorder in patients with heart failure and preserved left ventricular ejection fraction. According to the latest guidelines of the European Society of Cardiology, the diagnosis requires the presence of three factors: I. Symptoms and signs of heart failure II. Preserved or slightly abnormal systolic function - ejection fraction over 50% III. Evidence of diastolic dysfunction - measured via invasive or non-invasive methods Arterial hypertension is a socially significant disease afflicting an increasing part of the population. It is defined as office values of systolic blood pressure above 140 mmHg and/or values of diastolic pressure above 90 mmHg. It is an independent nosological unit, but also a major risk factor for the development of cardiovascular, cerebrovascular and kidney diseases. This article presents the main parameters for determining diastolic function, the characteristics of the most commonly used medications for the treatment of arterial hypertension and a design of a clinical study based on the effects of combination antihypertensive therapy candesartan and amlodipine on diastolic function parameters and on the values of systolic and diastolic blood pressure.

Представяме клиничен случай на 78-годишна жена с анамнеза за артериална хипертония, пароксизмално предсърдно мъждене (ППМ) и исхемичен мозъчен инсулт, която е хоспитализирана по спешност с гръдна болка с давност от 1 час и ЕКГ данни за новопоявил се ляв бедрен блок. Ехокардиографията (ЕхоКГ) при постъпването показа септо-апикална хипокинезия, редуцирана фракция на изтласкване на лявата камера (ФИЛК) от 38%, умерена митрална регургитация. На контролната ЕхоКГ на следващия ден е установен дефект на междукамерната преграда (с размер 7/14 mm), причиняващ значителен ляво-десен шънт (Qp/Qs = 1.8/1.0). Установеният междукамерен дефект (МКД) при пациентката беше лекуван успешно перкутанно. Селективната коронарна ангиография (СКАГ) демонстрира липса на обструкция на коронарните артерии, поради което се постави работна диагноза – миокарден инфаркт с необструктивни коронарни артерии – MINOCA. Въведе се интрааортна балонна помпа (ИАБП), вследсвие на което се постигна подобряване в помпената функция на лявата камера и хемодинамично стабилизиране на пациентката. При интер-венцията бе използван двоен достъп през дясната югуларна вена и дясната радиална артерия. Затварянето на МКД беше извършено с имплантиране на устройство за затваряне на междупредсърдни дефекти (с размери 20.5/16.5 mm), въведен през венозния интродюсер под постоянен ангиографски и трансезофагеален ехографски (ТЕЕ) контрол. Незабавното елиминиране на шънта бе потвърдено ехографски и ангиографски. Постпроцедурно пациентката бе хемодинамично стабилна, без оплаквания. В хода на 6-месечно проследяване се обективизираха липса на сърдечно-съдови събития и стабилна позиция на имплантирания оклудер без данни за шънт. We present a case of a 78-year-old female with a history of hypertension, paroxysmal atrial fi brillation and ischemic stroke which was admitted due to an hour-long chest pain with ECG changes indicating sinus rhythm and a newly formed LBBB. Subsequently a ventricular septal defect (VSD) was discovered which we successfully treated percutaneously. At admission, echocardiography revealed septo-apical hypokinesis with an accompanying reduced left ventricular ejection fraction of 38%, in addition to the mid-ventricular septal defect (7/14 mm in size) causing a signifi cant left to right shunt (QP/QS = 1.8/1.0). Coronary angiography demonstrated lack of obstruction of the coronary vessels – MINOCA. Rapid response and Intensive care treatment, including Intra-Aortic Balloon Catheter insertion, lead to LV-functional improvement and hemodynamic stabilization. A dual access approach through the right jugular vein and right radial artery was used and eventual closure of the VSD was achieved with an Occlutech ASD occluder (20.5/16.5 mm), inserted via the venous introducer, while under constant angiographic and transesophageal echo guidance. Immediate VSD shunt elimination was disclosed by EchoCG and angiography. Overall, an uneventful in-hospital stay and six months event-free follow-up period were registered including further EchoCG which confi rmed lack of major cardio-vascular events and the stable position of the occluder, without a visible shunt.

Транстиретиновата сърдечна амилоидоза е рестриктивна кардиомиопатия (ATTR-КМП), резултат от извънклетъчно натрупване на неразтворими транстиретинови амилоидни фибрили в миокарда, има прогресиращ ход и е възможен летален изход в рамките на 2-6 години от поставяне на диагнозата. Бива два вида – наследствена и див тип. Данни от последните години показват, че дивият тип АТТР-КМП е относително честа причина за сърдечна недостатъчност със запазена фракция на изтласкване, особено при по-възрастни мъже. В същото време в България, наследствената транстиретинова амилоидоза не е толкова рядка. Диагностицирни са пет различни патологични мутации. Най-разпространена е р.Glu89Gln, като по данни от юни 2019 г има установени 62 несвързани семейства с 117 пациенти и 72 носители. Диагнозата АТТР-КМП често се поставя със закъснение или се пропуска, а нейното ранно диагностициране е изключително важно, тъй като има одобрен медикамент (стабилизатор на транстиретина), който е по-ефикасен приложен в начален стадий на заболяването. Други медикаменти са в клинични изпитания. Диагностицирането на АТТР-КМП е процес, който изисква мултидисциплинарен подход с участието на подготвени специалисти, мултимодална образна диагностика, добре оборудвани хистопатологична и генетична лаборатории. Изграждането на експертни центрове на функционален принцип би могло да допринесе за по-ранното откриване, своевременното лечение и проследяването на пациентите с АТТР-КМП, което съответно да подобри тяхната прогноза. Transthyretin cardiac amyloidosis is a restrictive cardiomyopathy ((ATTR-CM), caused by an extracellular deposition of insoluble amyloid fibrils in the myocardium. It is a life threatening disease with life expectancy of 2 to 6 years after diagnosis. There are two types – hereditary and wild type. Recent data reveal that the wild type ATTR-CM is a common cause of heart failure with preserved ejection fraction, especially in elderly men. Hereditary ATTR amyloidosis is not so rare in Bulgaria. Five different mutations have been diagnosed, the most common being p.Glu89Gln, identified in 62 unrelated families with 117 patients and 72 mutation carriers. ATTR-CM diagnosis is often delayed or even missed, however its early recognition has become very important as a new drug, which is a transthyretin stabilizer is now available and other drugs are under development. Updated knowledge about the clinical presentation, diagnostic algorithm, available and future therapeutic options for ATTR-CM are a prerequisite for an early identification, timely treatment and better prognosis of the affected patients. The diagnosis requires a multidisciplinary approach with the participation of experienced specialists, multimodality imaging, well equipped histopathological and genetic laboratories. Establishing centres of expertise could improve the management of the patients with ATTR-CM.

С нарастване на заболяемостта и обхвата на пандемията от COVID-19 по света е от голямо значение безопасното използване на потенциално ефективни медикаменти за лечение на инфекцията. В последните седмици се появиха редица публикации, описващи вероятен ефект от употребата на познати медикаменти, сред които хлорохин и хидроксихлорохин, както и комбинацията им с азитромицин. Един от най-сериозните рискове, свързани с употребата на тези медикаменти, e медикаментозно-обусловеното удължаване на QT-интервала, което от своя страна повишава риска от развитие на torsades de pointes (TdP) и внезапна сърдечна смърт (ВСС). В настоящия документ описваме подход за скриниране на пациентите и тяхното проследяване с оглед редукция на риска от нежелани лекарствени реакции. With the rising incidence and wider spread of the COVID-19 pandemic worldwide, it is of great importance to safely use potentially effective medicines to treat the infection. In recent weeks, a number of publications have emerged describing the likely effect of the use of known drugs, including chloroquine and hydroxychloroquine, and their combination with azithromycin. One of the most serious risks associated with the use of these medications is the drug-induced QT prolongation, which in turn increases the risk of developing torsades de pointes (TdP) and sudden cardiac death (SCD). In this document, we describe an approach to screen patients and monitor them with the aim to reduce the risk of drug-induced side effects.

Миокардитът представлява възпалително заболяване на миокарда, в основата на което стоят множество и различни причинители, затрудняващи поставянето на етиологична диагноза. Макар и рядко миокардитът може да е усложнение на грипна инфекция. В болшинството от случаите вирусният миокардит е самоограничаваща се инфекция, преминаваща без да оставя трайни изменения в миокарда. Представяме клиничен случай на 30 годишна жена, развиваща остра сърдечна недостатъчност в хода на респираторна инфекция съпроводена от тежка систолна дисфункция (левокамерна (ЛК) фракция на изтласкване (ФИ) 16%). При пациентката се проведе лечение съобразено с настоящите препоръки за лечение на остра и хронична сърдечна недостатъчност в комбинация с имуномодулираща и метаболитна терапия. В хода на хоспитализацията от микробиологичните и вирусологични изследвания се установиха острофазови анти-тела към парагрипни вируси. В резултат на проведеното лечението се отчете пълно обратно развитие на симптомите на сърдечна недостатъчност и възстановяване на изходната ФИ (40%). Навременното започване на лечение при миокардит, съпроводено с добър клиничен ход и обратно развитие на ЛК систолната дисфункция, в някой случай може да отмени провеждането на инвазивни процедури като ендомиокардна биопсия. Myocarditis is an inflammatory disease of the myocardium, with heterogeneous etiology. Although rare, myocarditis can be a complication of the influenza infection. In the majority of cases, viral myocarditis is a self-limiting infection passing without permanent changes in the myocardium. We present a clinical case of a 30-year-old woman with acute heart failure in the course of respiratory infection accompanied by severe systolic dysfunction (left ventricular (LV) ejection fraction (EF) 16%). The patient was treated according to the current guideline recommendations for the treatment of acute and chronic heart failure with the conjunction of immunomodulating and metabolic therapy. During hospitalization from microbial and viral testing, acute-phase antibodies to parainfluenza virus were identified. Following the treatment, a complete resolution of heart failure symptoms and restoration of LVEF to baseline (40%) was observed. Timely initiation of treatment in myocarditis, followed by good clinical course and regression of LV systolic dysfunction, in some cases may cancel invasive procedures such as endomyocardial biopsy.
