7. Burden of Care, Quality of Life, and Coping Strategies among Spouses of Alcohol-Dependence Patients in Tertiary Health Care Centre.

Abstract

Background: Caregiving to a patient with substance-abuse is quite stressful and places an additional burden to the caregiver which in turn have a huge impact on their quality of life. To overcome this burden and improve their quality of life, caregivers adopt some coping strategies. Aim: To study the burden of care, coping strategies and quality of life among spouses of alcohol-dependence patients. Methodology: In this case control study, 100 spouses of diagnosed alcohol-dependence patients were taken as cases and the same number of age and sex-matched spouses of normal healthy individuals were taken as controls. AUDIT - C scale was applied to diagnose alcohol dependence in patients. M.I.N.I scale used on spouses, to rule out any chronic psychiatric illness. Burden Interview Schedule (BIS), EUROHIS-QOL scale, and Brief COPE Scale were used to find the burden on spouses, their quality of life, and coping strategies used by them. Results: In this study, the mean burden of care score (BIS) was significantly higher in cases (35.11 ± 4.97) as compared to that in controls (16.56 ± 9.98). Mean scores for BIS subdomains were also significantly higher in cases as compared to that in controls. There was no significant difference in the EUROHIS-QOL ratings between the cases and controls. Mean BRIEF COPE scale scores were significantly lower in cases (77.39 ± 18.10) as compared to that in controls (82.89 ± 16.43). The only difference between the two groups that was statistically significant was the avoidant coping score. Conclusion: The findings of this study showed that spouses of alcohol-dependent patients experience a high burden of care in all subdomains. Adaptive coping strategies were mainly taken by spouses of alcohol dependence patients, to cope with the situation. The quality of life of spouses of alcohol-dependent patients was not affected. 19. Histone lactylation drives CD8+T cell metabolism and function

Abstract

The activation and functional differentiation of CD8+T cells are linked to metabolic pathways that result in the production of lactate. Lactylation is a lactate-derived histone post-translational modification; however, the relevance of histone lactylation in the context of CD8+T cell activation and function is not known. Here, we show the enrichment of H3K18 lactylation (H3K18la) and H3K9 lactylation (H3K9la) in human and mouse CD8+T cells, which act as transcription initiators of key genes regulating CD8+T cell function. Further, we note distinct patterns of H3K18la and H3K9la in CD8+T cell subsets linked to their specific metabolic profiles. Additionally, we find that modulation of H3K18la and H3K9la by targeting metabolic and epigenetic pathways influence CD8+T cell effector function, including antitumor immunity, in preclinical models. Overall, our study uncovers the potential roles of H3K18la and H3K9la in CD8+T cells.

22. In vivo anthelmintic therapeutic efficacy of aqueous and alcoholic extracts of moringa oleifera leaves in naturally infected goats

Abstract

Gastrointestinal helminths are one of the major constraints affecting the production performance of small ruminants throughout the world including India. A number of medicinal plants have been used to treat intestinal parasites in man and animals. The present study aimed to evaluate the in vivo anthelmintic therapeutic efficacy of aqueous and alcoholic extracts of Moringa oleifera leaves in goats naturally infected with gastrointestinal parasites. Twenty-four goats (either sex) naturally infected with gastrointestinal parasites were randomly divided into four groups each having six goats. Group I was untreated control; Group II received a single dose of albendazole @ 7.5 mg/kg body weight. Group III and IV were respectively administered aqueous and alcoholic extracts of Moringa oleifera leaves @ 300 mg/kg body weight, orally daily for 30 days. Faecal egg count, haematological estimations, and biochemical estimations were conducted on the zero, 10th, 20th, and 30th days of the experiment. Groups II, III, and IV showed a significant reduction in EPG count following treatment. All treated animals showed a significant increase in Hb, TEC, PCV, and lymphocytes and a significant decrease in TLC, neutrophil, and eosinophil levels The results indicated possession of good anthelmintic activity by both extracts. These herbal extracts can be a safe and therapeutically effective substitute for standard chemical drugs. KEYWORDS: Goat, Moringa oleifera, Albendazole, Anthelmintic, Gastrointestinal parasites.

23. Why do people speed? A Qualitative Synthesis on Facilitators and Barriers to Speeding

Abstract

Death, disability, and economic burden due to road traffic crashes (RTCs) are a major public health problem globally and are expected to increase for the next two decades. Every year approximately 1.3 million people die as a result of a RTC, and 20- 50 million more suffer non-fatal injuries, with many incurring permanent disabilities. People aged 5-29 years are at highest risk of road crash fatalities. A major risk factor of RTCs is speeding- contributing to 15-35% of all reported road crash deaths. Every 1% increase in mean speed by driver is directly related to a 4% increase in a fatal crash risk and a 3% increase in a non-fatal crash. The Stockholm Declaration signed at the 2020 Global Ministerial Conference on Road Safety states that global leaders have a shared responsibility to protect road users from crash forces beyond the limits of human injury tolerance. To fulfill this, a focus on safe speeds is required which can be achieved through (i) Zero Speeding- use of effective speed management approaches, and (ii) 30 KM/H: mandating a 30 km/h speed limit in urban areas to prevent serious injuries and deaths to vulnerable road users when human errors occur. These interventions are similar to the World Health Organization (WHO) identified best practices for reducing speeding. As of 2018, only 46 countries have anti-speeding legislation that aligns with these best practices. Despite such existing evidence and legal stipulations, ensuring appropriate road user behavior around speeding continues to remain a challenge. While some studies have assessed factors associated with speeding, there has been no qualitative synthesis of evidence to understand enabling and obstructive factors to behavior change. Understanding the factors influencing speeding behavior is a formative step towards the development of innovative and empirical intervention strategies. Therefore, this study will aim to identify and explore the facilitators and barriers towards speeding through an evidence synthesis.

Abstract

Stimulant laxatives are a common class of laxatives that is abused by patients with eating disorders. We present a case of a 30-year-old female who presented with dyspnea, peripheral edema and weight gain who had been chronically using laxatives. Her symptoms were consistent with rebound edema caused by sodium and free water shifts with abrupt cessation of excessive stimulant laxative use. This case highlights the use of furosemide as the mainstay treatment for rebound edema and weight gain.

Abstract

Gastric diverticula are the least common gastrointestinal diverticula. Patients can be diagnosed incidentally on EGD or present with variable symptoms such as abdominal fullness, anorexia, and perforation. Gastric diverticula can be acquired from malignancy, peptic ulcer disease, or prior surgery or be congenital. Treatment varies based on symptomatology ranging from conservative medical management with proton pump inhibitors to surgical treatment with open or laparoscopic resection. We present a case of a 73-year-old female with acquired gastric diverticulum presenting as a gastric outlet obstruction who was successfully treated with conservative medical therapy.

39. Ruptured Subcapsular Liver Hematoma: A Rare Complication of HELLP Syndrome

Abstract

Subcapsular liver hematoma (SLH) is a rare complication of HELLP (hemolysis, elevated liver enzymes, low platelets) syndrome. We report a previously healthy 16-year-old female presenting with pre-eclampsia requiring emergent C-section, who developed immediate postoperative bleeding and abdominal distention. Abdominal computed tomography angiography (CTA) revealed a large encapsulated liver hematoma with active extravasation. The patient was successfully treated with a multidisciplinary approach with medical and surgical management.
