Patients and Methods: In this single-center randomized trial, patients with resectable or borderline resectable pancreatic cancer were randomly assigned to receive preoperative chemoradiotherapy or upfront surgery. Neo-adjuvant therapy consisted of two cycles of chemotherapy of gemcitabine and Nab-paclitaxel, followed by concurrent chemoradiotherapy with oral capecitabine combined with radiation therapy to a dose of 25 fractions x 1.8 Gray, followed by surgery and four cycles of adjuvant therapy or upfront surgery and six cycles of adjuvant chemotherapy. The primary end point was overall survival. Results: From March 2019 to March 2021, 80 patients were randomly assigned into two groups: 41 in neo-adjuvant therapy and 39 in upfront surgery. Per protocol analysis, 25/31 (80.65) patients completed the intended neo-adjuvant treatment, with a dropout rate of 19.35. Recurrent cholangitis due to stent block was the foremost reason for dropout. The median overall survival was 18.90 months with neo-adjuvant and 14.7 months in the upfront surgery group (hazard ratio, 0.78; 95 confidence interval, 0.58 to 1.05; P = .096). The resection rate was 36/41 (87.80) in the upfront surgery group versus 14/31 (45.61) in the NACRT group (P = Conclusion: The median overall survival and disease-free survival showed an advantage with neo-adjuvant therapy but did not reach statistical significance. The secondary end points favoured neo-adjuvant treatment, but logistics, multiple visits, prolonged treatment, and financial constraints are some of the main hurdles for developing countries to deliver multimodality treatment. Abstract

Esophageal cancer is one of the common cancers. Risk factors are well recognized and lead most commonly to two distinct histological subtypes (squamous cell carcinoma and adenocarcinoma). The diagnosis is based on endoscopic evaluation. The most challenging aspect of management is accurate staging as it guides appropriate management. Endoscopic ultrasound, computed tomography (CT), positron emission tomography-CT, and magnetic resonance imaging are the imaging tests employed for the staging. Each imaging test has its own merits and demerits. Imaging is also critical to evaluate posttreatment complication and for response assessment. In this review article, we discuss in detail the risk factors, anatomical aspects, and role of imaging test in staging and evaluation of complications and response after treatment. Abstract

In recent years, rapid solidification techniques have garnered considerable attention due to their capacity to yield metals with unique microstructures and properties unattainable through conventional processing methods. These techniques involve swiftly cooling the metal, resulting in a fine-grained microstructure, unlike the coarse-grained microstructures typically obtained through traditional methods. This paper offers an overview of several rapid solidification techniques, such as melt spinning, spray forming, and gas atomization. These methods entail rapidly cooling molten metal by either quenching it onto a swiftly rotating wheel or spraying it onto a substrate. The cooling rates achieved, typically ranging from 104 to 107 K/s, far exceed those of conventional casting methods. The distinctive microstructure resulting from rapid solidification techniques presents numerous advantages over conventional methods. These include enhanced homogeneity, increased strength, improved ductility, heightened thermal stability, and augmented corrosion resistance. Consequently, rapid solidification techniques hold significant promise for applications across aerospace, automotive, and medical industries. This review also underscores recent advancements in rapid solidification techniques and their potential applications. These encompass the production of high-strength, wear-resistant materials for cutting tools, the development of lightweight alloys for aerospace applications, and the manufacture of biocompatible alloys for medical implants. In summation, this review offers a comprehensive examination of the latest progress in rapid solidification techniques for metal processing and their diverse applications across various industries.

Abstract

This study investigates the enhancement of aluminum-based composite manufacturing through the utilization of ZrB2 reinforcement via Friction Stir Process (FSP). Achieving a uniform distribution of ZrB2 particles is crucial for optimizing material properties. The addition of ZrB2 nanoparticles using FSP has led to notable improvements in various mechanical properties of aluminum. Tensile strength increased by 20.25%, hardness by 35.67%, fatigue strength by 23.67%, and wear resistance by 29.45%. These enhancements underscore the effectiveness of nanoparticle reinforcement in fortifying aluminum matrices against mechanical stresses and wear mechanisms. The results demonstrate the potential of FSP-based techniques in tailoring the mechanical properties of aluminum-based composites for diverse applications. This research contributes valuable insights into advanced manufacturing methods for developing high-performance materials with enhanced mechanical characteristics, facilitating the advancement of aluminum composite technologies for industries requiring superior strength, durability, and wear resistance.
