Rhinoscleroma is a chronic indolent granulomatous infection of the nose and the upper respiratory tract. The average time of presentation between the onset and diagnosis is about a decade. An unusual case of Rhinoscleroma in a young immune-competent male with a two month history mimicking as invasive fungal sinusitis is being presented. While Radiological appearance is not pathognomonic of rhinoscleroma, nasal endoscopy is both diagnostic and therapeutic. Histopathology report is confirmatory for rhinoscleroma. Subperiosteal hematoma of the orbit is a rare complication of ethmoidal sinusitis presenting most commonly in adults. The authors report a unique case who had subperiosteal hematoma of the orbit secondary to ethmoidal sinusitis. She was referred to our hospital with the diagnosis of orbital abscess and magnetic resonance imaging disclosed an extraconal mass in the upper part of the left orbit. A left frontal craniotomy was performed after aspiration of purulent material from ethmoid sinus using endoscopic approach, and a brownish black hematoma material was removed. This patient is the second reported case of subperiosteal hematoma of the orbit misdiagnosed as orbital abscess in the current literature. The presented case exemplifies the fact that early diagnosis and prompt intervention is very important to avoid possible complications often associated with this disease. Background and Objective: Intracranial abscesses complicating rhinosinusitis are now uncommon. Attitude of patients to the so called common cold, delay in diagnosis and relative inaccessibility to computed tomography scan contribute to persistence of the entity and its sequelae in our environment. Case Description: Four patients presented to our centre with this condition over an eighteen month period. All were male teenagers. Late presentation was common to all, and rhinological symptoms were less emphasized. Radical treatment of the intracranial abscesses and three of the primary foci was employed. Three of the patients were left with morbidities while one died. Conclusion: Claims of its rarity may not continue to be true in the developing countries where the factors associated with poverty and low socioeconomic standard of living are prevalent. Therefore clinicians need to be more alert and work as a team for early diagnosis and reduce its morbidity and mortality. Parents need to be educated. Nasotracheal intubation, a procedure in which a tube is inserted through the nose into the trachea (windpipe), is used to give oxygen and other respiratory therapy. Sinusitis (inflammation of the sinuses) is a well documented complication of this procedure. As a result of sinus obstruction, caused either directly by the tube or indirectly by tissue swelling, the normal bacterial flora of the sinuses become pathogenic, causing sinusitis. Because of this complication it is recommended that the airway be accessed through the mouth rather than the nose. However, for burn patients this conversion to an oral access can cause complications and even death. Infection in the paranasal sinuses may be difficult to diagnose in the critically ill patient. A study was undertaken of 22 nasally intubated burn patients to determine the incidence, complications and treatment of sinusitis. There were five patients with inhalation burn injury, while 17 had burns of the face and neck. Patients had been intubated for more than seven days when a computed tomographic (CT) scan was performed; average time from injury to CT scan was 13 days. There was CT evidence of sinusitis for eight patients (36 percent); half of these patients had a pre-existing sinus condition. All eight patients with sinusitis required tracheostomy (creation of an airway through a surgical incision in the trachea) for prolonged mechanical ventilation. During the same time period there were 67 burn patients intubated for fewer than seven days with no sign of infection. It is suggested that patients with pre-existing sinus disease be converted to an oral airway early in treatment; otherwise, the frequency and complications of sinusitis in nasotracheally intubated burn patients do not warrant the risk of such conversion. Dental infection or sepsis, either known or hidden, is frequently the cause of maxillary sinusitis (inflammation of the maxillary sinus, one of a pair of air pockets in the upper jaw bone). Incomplete extraction of the roots of the upper teeth, as well as severe dental caries (cavities) in the upper jaw, may cause maxillary sinusitis. A case is presented in which the use of osseointegrated titanium implants for cosmetic dental reconstruction caused unilateral (on one side) maxillary sinusitis. Osseointegrated implants require that the jaw bone be healthy and have a good blood supply. The screw of the implant is imbedded in the jaw bone and buried under the gum. This implant remains in place for several months; following this the permanent plate or individual teeth are attached to the implant. In this case a 44-year-old woman developed one-sided facial pain and puffiness in the cheek region. Three weeks earlier three osseointegrated implants had been placed in her upper jaw bone. Sinus X-rays showed an opaque maxillary sinus with two of the dental implants dislodged. The implants were removed and the infected bone was scraped away. She was treated with antibiotics and recovered. These implants are often inserted very close to the mucosa of the maxillary sinus, and when there is insufficient bone depth the implant may enter the sinus cavity. When foreign body implants are inserted in the upper jaw, great care must be taken, especially if the bone has been chronically infected from infected teeth.
