International audience; A total diet study (TDS) was conducted in France to assess the health risks related to the chemicals in food of non-breastfed children under three years of age (Infant TDS). For the first time, substances coming from food contact materials, such as bisphenol A (BPA), bisphenol A diglycidyl ether (BADGE) and its derivatives, some phthalates, and some ink photoinitiators, were targeted because of growing interest in these substances. Food samples were collected to be representative of the whole diet of non-breastfed children aged 1–36 months, and prepared as consumed prior to analysis. Dietary exposure was assessed for 705 representative children under three years of age. Generally, the substances from food contact materials were detected in few samples: 38% for BPA, 0% for BADGE and its derivatives, 0–35% for phthalates, 1.9% for benzophenone, and 0% for the other ink photoinitiators. Regarding exposure levels, the situation was deemed tolerable for BADGE and its hydrolysis products, di-isodecyl phthalate, dibutyl phthalate, butyl benzyl phthalate, bis(2-ethylhexyl) phthalate, and di-isononyl phthalate, benzophenone, and 4-methylbenzophenone. Only for BPA, the exposure levels of some children exceeded the lowest toxicological value established by the French Agency for Food, Environmental and Occupational Health & Safety at 0.083 µg.kg bw−1.d−1. The temporary tolerable daily intake of the European Food Safety Authority (EFSA), set at 4 µg.kg bw−1.d−1, was never exceeded. However, actual exposure to BPA was probably overestimated, as well as the associated risk, because the foods were sampled prior to the recent regulations banning BPA in food packaging. This study is the first worldwide to provide an estimate of infant food contamination levels and exposures of children under 3 years of age, based on a TDS approach. It therefore provides key data on the exposure of this particularly sensitive population to substances released from food contact materials, and presents useful data for studies evaluating exposure to mixtures or aggregated exposure.

Online; Background. Our diets are undergoing a transition towards a lower consumption of animal protein, in line with the sustainability of diets. Given the differences in nutritional profile between protein sources, changes in consumption, even marginal, raise the question of the nutritional quality of diets. Objectives. Our objective was to identify the consequences, on sustainability parameters, of rearrangements of protein intake aiming at improving nutritional adequacy, depending on whether or not we constrained an increase of the proportion of vegetable proteins.Methods. Based on the consumption data from the INCA2 study (2006-2007), we simulated, for each individual, all possible substitutions of a portion of a protein food for a portion of another protein food, and selected the substitution that increased nutritional adequacy the most (estimated with probabilistic PANDiet score). This step was iterated 20 times for each individual under two different scenarios: by constraining (P) or not (N) an increase in the percentage of plant protein at each iteration. The sustainability parameters studied were diet costs, greenhouse gas emissions (GHGE), exposure to food contaminants and premature deaths avoided (estimated with the PRIME model).Results. The percentage of plant protein (31.1%) decreased slightly in N (30.0%) and increased in P (37.7%). The food groups whose contribution to protein intake increased the most were legumes (+225%), fatty fish (+151%) and lean chicken (+82%) in N and legumes (+502%), pizzas and quiches (+190%) and fatty fish (+102%) in P. The PANDiet increased slightly more in N (+7.5) than in P (+6.2) due to higher probabilities of adequacy in EPA + DHA, iron, iodine, potassium, zinc, riboflavin, vitamin B-6 and B-12 despite lower probabilities of adequacy in fiber and folates. GHGE increased in N and decreased in P. The diet cost increased more in N than in P. Risks related to exposure to inorganic arsenic, dioxins and furans and nickel increased in N and P. The modifications identified could lead to 1,700 and 2,200 premature deaths avoided / year in N and P, respectively.Conclusion. We identified simple modifications of the protein intake that significantly increased the nutritional adequacy of diets. By forcing these changes to systematically increase the proportion of vegetable protein, nutritional adequacy increased less, but the price increased less, the GHGE decreased and deaths avoided increased.; Contexte. Nos régimes alimentaires comportent moins de protéines animales aujourd'hui, en ligne avec la durabilité des régimes alimentaires. Compte tenu des différences de profil nutritionnel entre les sources de protéines, l'évolution de la consommation, même marginale, pose la question de la qualité nutritionnelle des régimes.Objectifs. Notre objectif était d'identifier les conséquences, sur les paramètres de durabilité, des réarrangements de l'apport en protéines visant à améliorer l'adéquation nutritionnelle, selon que nous ayons ou non limité l'augmentation de la proportion de protéines végétales.Méthodes. Sur la base des données de consommation de l’étude INCA2 (2006-2007), nous avons simulé, pour chaque individu, toutes les substitutions possibles d’une portion d’un aliment protéiné par une portion d’un autre aliment protéique, et avons sélectionné la substitution qui augmentait le plus le score PANDiet probabiliste. Les légumineuses (+ 225%), les poissons gras (+ 151%) et le poulet maigre (+ 82%) en azote et les légumineuses (+ 502%), les pizzas et les quiches (+190 %) et les poissons gras (+ 102%) dans P. Le PANDiet a augmenté légèrement plus dans N (+7,5) que dans P (+6,2) en raison de probabilités plus élevées d’adéquation en EPA + DHA, fer, iode, potassium, zinc, riboflavine, vitamines B-6 et B-12 malgré des probabilités plus faibles d’adéquation des fibres et des folates. Les GES ont augmenté dans N et diminué dans P. Le coût de l'alimentation a augmenté davantage dans N que dans P. Risques liés à l'exposition à l'arsenic inorganique, les dioxines et les furannes et nickel a augmenté dans N et P. Les modifications identifiées pourraient entraîner 1 700 et 2 200 décès prématurés évités / année en N et P, respectivement.Conclusion. Nous avons identifié de simples modifications de l'apport en protéines qui ont considérablement augmentées l'adéquation nutritionnelle des régimes. L'augmentation systématiquement de la proportion de protéines végétales a légèrement augmenté l'adéquation nutritionnelle et le prix, a diminué les émissions de GES et a augmenté le nombre de décès évités.

Context. Food provides the nutrients which enable the body to function on a physiological level as well as delivering contaminants that may affect health. From a public health point of view, both aspects must be considered in order to determine optimum food consumption levels. Seafood is an interesting example as it provides n-3 long chain polyunsaturated fatty acids (n-3 LC-PUFA), vitamins and minerals but also methylmercury, arsenic and persistent organic pollutants. Objective. The aim of this thesis was to determine in what quantities seafood consumption would provide nutritional benefits while minimising the risks linked to food contaminants. Methods. Analyses were mainly based on data from the Calipso survey conducted among 996 high seafood consumers (at least twice a week) aged 18 and over. Their nutritional intakes and their exposure to contaminants were assessed through a food frequency questionnaire and through biomarker data. This made it possible to select nutrients and contaminants of interest, since seafood contributes significantly to both the intake of nutritional requirements and exposure to nutritional and health risks. The risk/benefit analysis consisted in using an optimization model with constraints to calculate optimum seafood consumption levels. The goal was to optimize nutritional intakes as well as contaminant exposure with the condition being to attain recommended nutritional intakes without exceeding tolerable upper intakes for contaminants and nutrients, while taking into account background intakes. Results. Among high seafood consumers, the health risk linked to methylmercury and arsenic appeared to be significant. Seafood also appeared to contribute largely to the intake of vitamin D and certain minerals. Moreover, consumption higher than 200 g/week of high fat fish does not seem to imply a higher benefit in terms of cardiovascular health biomarkers when compared with lower consumption. Taking this data into account, an optimum consumption level was calculated that minimises inorganic arsenic exposure and increases vitamin D intake in the general population. In parallel, this consumption level guarantees that the consumer attains the recommended intake for n-3 LC-PUFA, selenium and iodine, while remaining below the tolerable upper intakes for methylmercury, cadmium, dioxins, polychlorobiphenyls, zinc, calcium, and copper. This consumption level is approximately 200 g/week of certain fatty fish species and approximately 50 g/week of lean fish, molluscs, and crustaceans. Conclusion. Optimum seafood consumption levels providing both nutritional benefit and controlled risk have been calculated. This study highlights the necessity of reducing arsenic exposure and refining results using the model for certain subgroups of the population while taking into account the specificities of these populations., Contexte. Chez ces mêmes consommateurs, les produits de la mer apparaissent également comme contributeurs majeurs d'apport en vitamine D et certains minéraux. Par ailleurs, une consommation de plus de 200 g/sem de poissons très gras n'entraine pas de bénéfice supplémentaire en termes de biomarqueur de santé cardiovasculaire. Tenant compte de ces éléments, il a été déterminé une consommation dite optimale, permettant à la fois de minimiser l'exposition à l'arsenic inorganique et d'augmenter l'apport de vitamine D en population générale, tout en garantissant l'atteinte de l'ANC pour les AGPI LC n-3, le sélénium et l'iode, le non dépassement des VTR établies pour le méthylmercure, le cadmium, les dioxines et polychlorobiphényles, ainsi que le non dépassement des limites de sécurité établies pour le zinc, le calcium et le cuivre. Cette consommation est de 200 g/semaine environ de certaines espèces de poissons gras et de 50 g/semaine environ de poissons maigres, mollusques ou crustacés. Conclusion. Une consommation optimale de produits de la mer entrainant à la fois un bénéfice nutritionnel et un risque sanitaire maitrisé a pu être définie. Il ressort de ce travail la nécessité de réduire les expositions à l'arsenic et d'affiner les résultats en utilisant le modèle pour certaines populations, en tenant compte des spécificités de ces populations.

International audience; As first described in the 1980s, the core food intake model allows a precise assessment of dietary nutrient intake and dietary exposure to contaminants insofar as it reflects the eating habits of a target population and covers the most important foods in term of consumption, selected nutrient and contaminant contribution. This model has been used to set up the sampling strategy of the second French Total Diet Study (TDS) which aims to obtain a realistic panorama of nutrient intakes and contaminant exposure for the whole population, useful for quantitative risk assessment. Data on consumption trends and eating habits from the second French individual food consumption survey (INCA2) as well as data from a 2004 purchase panel of French households (SECODIP) were used to identify the core foods to be sampled. 116 core foods on a national scale and 70 core foods on a regional scale were selected according to (i) the consumption data for adults and children, (ii) their consumer rates, and (iii) their high contribution to exposure to one or more contaminants of interest. Foods were collected in 8 French regions (36 cities) and prepared “as-consumed” to be analysed for their nutritional composition and contamination levels. 20,280 different food products were purchased to make up the 1,352 composite samples of core foods to be analysed for additives, environmental contaminants, pesticide residues, trace elements and minerals, mycotoxins and acrylamid. The establishment of such a sampling plan is essential for effective, high-quality monitoring of dietary exposure from a public health point of view.
