12. Early Life Environmental Factors Associated with Autism Spectrum Disorder Symptoms in Children at Age 2 Years: A Birth Cohort Study

Abstract

Mounting evidence finds that early life environmental factors increased the probability of autism spectrum disorder. We estimated prospective associations between early life environmental factors and autism spectrum disorder symptoms in children at the age of 2 years in a population-derived birth cohort, the Barwon Infant Study. Autism spectrum disorder symptoms at the age of 2 years strongly predicted autism spectrum disorder diagnosis by the age of 4 years (area under curve = 0.93; 95% CI (0.82, 1.00)). After adjusting for child's sex and age at the time of behavioural assessment, markers of socioeconomic disadvantage, such as lower household income and lone parental status; maternal health factors, including younger maternal age, maternal pre-pregnancy body mass index, higher gestational weight gain and prenatal maternal stress; prenatal alcohol; environmental air pollutant exposures, including particulate matter < 2.5 [mu]m at birth, child secondhand tobacco smoke exposure at 12 months, dampness/mould and home heating with oil, kerosene or diesel heaters at 2 years postnatal. Lower socioeconomic indexes for area, later birth order, higher maternal prenatal depression, and maternal smoking frequency had a dose-response relationship with autism spectrum disorder symptoms. Future studies on environmental factors and autism spectrum disorder should consider the reasons for the socioeconomic disparity and the combined impact of multiple environmental factors through common mechanistic pathways. 22. Performance of variable and function selection methods for estimating the non-linear health effects of correlated chemical mixtures: a simulation study

Abstract

Statistical methods for identifying harmful chemicals in a correlated mixture often assume linearity in exposure-response relationships. Non-monotonic relationships are increasingly recognised (e.g., for endocrine-disrupting chemicals); however, the impact of non-monotonicity on exposure selection has not been evaluated. In a simulation study, we assessed the performance of Bayesian kernel machine regression (BKMR), Bayesian additive regression trees (BART), Bayesian structured additive regression with spike-slab priors (BSTARSS), and lasso penalised regression. We used data on exposure to 12 phthalates and phenols in pregnant women from the U.S. National Health and Nutrition Examination Survey to simulate realistic exposure data using a multivariate copula. We simulated datasets of size N = 250 and compared methods across 32 scenarios, varying by model size and sparsity, signal-to-noise ratio, correlation structure, and exposure-response relationship shapes. We compared methods in terms of their sensitivity, specificity, and estimation accuracy. In most scenarios, BKMR and BSTARSS achieved moderate to high specificity (0.56--0.91 and 0.57--0.96, respectively) and sensitivity (0.49--0.98 and 0.25--0.97, respectively). BART achieved high specificity ($\geq$ 0.96), but low to moderate sensitivity (0.13--0.66). Lasso was highly sensitive (0.75--0.99), except for symmetric inverse-U-shaped relationships ($\leq$ 0.2). Performance was affected by the signal-to-noise ratio, but not substantially by the correlation structure. Penalised regression methods that assume linearity, such as lasso, may not be suitable for studies of environmental chemicals hypothesised to have non-monotonic relationships with outcomes. Instead, BKMR and BSTARSS are attractive methods for flexibly estimating the shapes of exposure-response relationships and selecting among correlated exposures.

Abstract

Background/Objectives: The presence of antibiotic residues (ARs) in animal products such as milk can be an important driver of antimicrobial resistance in commensal and pathogenic bacteria. Previous studies on ARs in Nepal have demonstrated the presence of ARs in milk samples but without further characterization of the samples for risk factor analysis. This study aimed to quantify the prevalence and risk factors for the presence of ARs in 140 peri-urban dairy farms in Kathmandu, Nepal, included in a cross-sectional survey in 2019 to estimate farm-level AR prevalence. Results: Our results reveal the presence of ARs of sulfamethazine (61%), sulfamethoxazole (53%), ciprofloxacin (46%), and enrofloxacin (42%). Furthermore, of those samples positive for sulfamethazine, sulfamethoxazole, and ciprofloxacin, 81%, 42%, and 42%, respectively, exceeded the maximum residue limit (MRL). While samples taken from farms where staff administered antibiotics were less likely to have single drug residues and multidrug residues (two, three, and four drugs), farms with more workers were more likely to have single residues. Moreover, samples from farms with a higher number of calves and milking cows were more likely to contain single and multiple residues exceeding the MRL, while milk from farms with higher numbers of dry cows and farmers reported by a visiting chemist were less likely to have multidrug residues exceeding the MRL. Methods: High-performance liquid chromatography was conducted on bulk milk samples from farms for an AR analysis, revealing positive results. Additionally, a structured questionnaire and direct farmer interviews were used to collect data on farm biosecurity and farming practices, animal health and hygiene, antibiotic usage (AMU), and attitudes and practices towards antibiotic stewardship. Conclusions: Ultimately, this study provides evidence on the role of modifiable ARs risk factors in the peri-urban milk industry of Kathmandu, which can serve as a foundation for developing improved antimicrobial stewardship guidelines and designing intervention measures to reduce the public health risk posed by ARs in milk sold in Kathmandu, Nepal. While environmental factors such as ambient temperature extremes have been associated with preterm births, little is known of the environmental associations with EPT births. The objective of this study is to explore whether ambient temperature is associated with increased risk of EPT birth. Methods Birth records for 315,226 infants born in Queensland Australia (2007-2015) were matched to average maximum and minimum temperature for the last month of pregnancy. Odds ratios and 95% confidence intervals were calculated using a generalised linear model. Population attributable risk was calculated for a 5% reduction in maximum temperature. Results Each one degree increase in maximum [aOR 1.03 (95% CI 1.01, 1.05)] and minimum temperature [aOR 1.02 (95% CI 1.01, 1.04)] was associated with an increase in odds for EPT birth. Increased odds for EPT births was found for maternal smoking [aOR 1.46 (95% CI 1.23, 1.72)], increasing plurality [OR 6.38 (95% CI 5.48, 7.42)] and stillbirth [aOR 342.99 (95% CI 295.53, 398.06)]. When stratified by birth status, the association was only found for live births. Discussion Higher temperatures are associated with small increases in the odds of delivering an infant in the EPT period. The risk may be enhanced for women who smoke during pregnancy. Women at an increased risk of preterm births should be counselled around methods to reduce their exposure to excessive heat.

Introduction: This consensus statement recommends eight high‐level trackable policy actions most likely to significantly improve health and wellbeing for children and young people by 2030. These policy actions include an overarching policy action and span seven interconnected domains that need to be adequately resourced for every young person to thrive: Material basics; Valued, loved and safe; Positive sense of identity and culture; Learning and employment pathways; Healthy; Participating; and Environments and sustainable futures. Main recommendations: Provide financial support to invest in families with young children and address poverty and material deprivation in the first 2000 days of life.Establish a national investment fund to provide sustained, culturally relevant, maternal and child health and development home visiting services for the first 2000 days of life for all children facing structural disadvantage and/or adversity.Implement a dedicated funding model for Aboriginal and Torres Strait Islander community‐controlled early years services across the country to ensure these services are fully resourced to provide quality early learning and integrated services grounded in culture and community.Properly fund public schools, starting by providing full and accountable Schooling Resource Standard funding for all schools, with immediate effect for schools in communities facing structural disadvantage.Establish legislation and regulation to protect children and young people aged under 18 years from the marketing of unhealthy and harmful products.Amend the electoral act to extend the compulsory voting age to 16 years.Legislate an immediate end to all new fossil fuel projects in Australia.Establish a federal Future Generations Commission with legislated powers to protect the interests of future generations. Abstract

• Sensitive, non-invasive biomarkers are needed to track airway inflammatory activity in CF. • Urinary GSA levels correlate well with levels in BAL and other markers of inflammation. • Paired admission-discharge samples were available for 53 children and 60 adults. • A significant difference was observed between admission-discharge for children and adults. • Urinary GSA may be responsive to the resolution of an acute pulmonary exacerbation. There is an urgent need to develop sensitive, non-invasive biomarkers that can track airway inflammatory activity for patients with cystic fibrosis (CF). Urinary glutathione sulfonamide (GSA) levels correlate well with GSA levels in BAL samples and other markers of neutrophilic inflammation, suggesting that this biomarker may be suitable for tracking disease activity in this population. We recruited 102 children (median 11.5 years-old) and 64 adults (median 32.5 years-old) who were admitted to hospital for management of an acute pulmonary exacerbation and/or eradication of infectious agents such as Pseudomonas aeruginosa or Staphylococcus aureus. Our aim was to explore how urinary GSA levels changed across admission timepoints. Urine samples were collected at admission and discharge, and GSA measured by liquid chromatography with mass spectrometry. Paired admission-discharge results were compared using Wilcoxon signed-rank test. Paired admission-discharge samples were available for 53 children and 60 adults. A statistically significant difference was observed between admission-discharge for children and adults. Spearman's correlation analysis identified a correlation between urinary GSA levels and sex and S. aureus infection for children only. Our preliminary findings suggest that urinary GSA is responsive to the resolution of an acute pulmonary exacerbation and therefore warrants further studies in this population. [ABSTRACT FROM AUTHOR]
