Aims The aim of the current study was to compare cancellations or postponement of medical care among older adults during the COVID-19 pandemic between 2021 and 2020. Methods Data of respondents aged ≥ 62 years were used from the longitudinal aging study Amsterdam (LASA), collected in 2020 and 2021, directly after the main COVID-19 waves in the Netherlands. A questionnaire assessed cancellations of medical care and postponed help-seeking behavior. Descriptive analyses were performed. Results Overall, cancellations declined from 35% in 2020 (sample n = 1128) to 17% in 2021 (sample n = 1020). Healthcare-initiated cancellations declined from 29 to 8%. Respondent-initiated cancellations declined from 12 to 7%. Postponed help-seeking remained around 8%. Conclusions In 2021, less cancellations were reported compared to just after the first wave of the pandemic in 2020, while postponed help-seeking remained the same. It is important to investigate how cancellations and postponed help-seeking can be prevented in future pandemics.

Background Intrinsic capacity (IC) defined by the WHO refers to the composite of five domains of capacities. So far, developing and validating a standardized overall score of the concept have been challenging partly because its conceptual framework has been unclear. We consider that a person’s IC is determined by its domain-specific indicators suggesting a formative measurement model. Aims To develop an IC score applying a formative approach and assess its validity. Methods The study sample (n = 1908) consisted of 57–88-year-old participants from the Longitudinal Aging Study Amsterdam (LASA). We used logistic regression models to select the indicators to the IC score with 6-year functional decline as an outcome. An IC score (range 0–100) was constructed for each participant. We examined the known-groups’ validity of the IC score by comparing groups based on age and number of chronic diseases. The criterion validity of the IC score was assessed with 6-year functional decline and 10-year mortality as outcomes. Results The constructed IC score included seven indicators covering all five domains of the construct. The mean IC score was 66.7 (SD 10.3). The scores were higher among younger participants and those who had lower number of chronic diseases. After adjustment for sociodemographic indicators, chronic diseases, and BMI, a one-point higher IC score was associated with a 7% decreased risk for 6-year functional decline and a 2% decreased risk for 10-year mortality. Conclusions The developed IC score demonstrated discriminative ability according to age and health status and is associated with subsequent functional decline and mortality.

Background Physical activity may be both a risk and protective factor for falls and fall-related fractures. Despite its positive effects on muscle and bone health, physical activity also increases exposure to situations where falls and fractures occur. This paradox could possibly be explained by frailty status. Therefore, the aim of this study was to investigate the associations between physical activity and both falls and fractures, and to determine whether frailty modifies the association of physical activity with falls, and fractures. Methods Data of 311 community-dwelling participants aged 75 years or older from the Longitudinal Aging Study Amsterdam, who participated in a three-year longitudinal study with five nine-monthly measurements between 2015/2016 and 2018/2019. Their mean age was 81.1 (SD 4.8) years and frailty was present in 30.9% of the participants. Physical activity in minutes per day was objectively assessed with an inertial sensor (Actigraph) for seven consecutive days. Falls and fractures were assessed every nine months using self-report during an interview over a follow-up period of three years. Frailty was determined at baseline using the frailty index. Associations were estimated using longitudinal logistic regression analyses based on generalized estimating equations. Results No association between physical activity and falls was found (OR = 1.00, 95% CI: 0.99–1.00). Fall risk was higher in frail compared to non-frail adults (OR = 2.21, 95% CI: 1.33–3.68), but no effect modification was seen of frailty on the association between physical activity and falls. Also no relation between physical activity and fractures was found (OR = 1.00, 95% CI: 0.99–1.01). Fracture risk was higher in frail compared to non-frail adults (OR = 2.81, 95% CI: 1.02–7.75), but also no effect modification of frailty was present in the association between physical activity and fractures. Conclusions No association between physical activity and neither falls nor fractures was found, and frailty appeared not to be an effect modifier. However, frailty was a risk factor for falls and fractures in this population of older adults. Our findings suggest that physical activity can be safely recommended in non-frail and frail populations for general health benefits, without increasing the risk of falls.

ObjectivesThe aim of this study was to develop an index to measure older adults’ exposure to the COVID-19 pandemic and to study its association with various domains of functioning.DesignCross-sectional study.SettingThe Longitudinal Aging Study Amsterdam (LASA), a cohort study in the Netherlands.ParticipantsCommunity-dwelling older adults aged 62–102 years (n=1089) who participated in the LASA COVID-19 study (June–September 2020), just after the first wave of the pandemic.Primary outcome measuresA 35-item COVID-19 exposure index with a score ranging between 0 and 1 was developed, including items that assess the extent to which the COVID-19 situation affected daily lives of older adults. Descriptive characteristics of the index were studied, stratified by several sociodemographic factors. Logistic regression analyses were performed to study associations between the exposure index and several indicators of functioning (functional limitations, anxiety, depression and loneliness).ResultsThe mean COVID-19 exposure index score was 0.20 (SD 0.10). Scores were relatively high among women and in the southern region of the Netherlands. In models adjusted for sociodemographic factors and prepandemic functioning (2018–2019), those with scores in the highest tertile of the exposure index were more likely to report functional limitations (OR: 2.24; 95% CI: 1.48 to 3.38), anxiety symptoms (OR: 3.14; 95% CI: 1.82 to 5.44), depressive symptoms (OR: 2.49; 95% CI: 1.55 to 4.00) and loneliness (OR: 2.97; 95% CI: 2.08 to 4.26) than those in the lowest tertile.ConclusionsAmong older adults in the Netherlands, higher exposure to the COVID-19 pandemic was associated with worse functioning in the physical, mental and social domain. The newly developed exposure index may be used to identify persons for whom targeted interventions are needed to maintain or improve functioning during the pandemic or postpandemic.

The role of the source of discrimination in relation to minority Muslim youths’ psychosocial well-being has received remarkably little attention in the post-9/11 climate. We have examined one of the aspects of psychosocial well-being that is given prominent attention in the media and public discourse, namely externalising behaviour. The article reports whether perceived discrimination by four sources (school peers and teachers, peers, and adults outside the school) is related to externalising behaviour. Links between perceived discrimination sources and externalising behaviour among Dutch Muslim youths (n = 308, ages 14–18) were examined through surveys. The quantitative findings guided our qualitative analyses of interviews with 10 Muslim Dutch youths on their accounts of discrimination in the school context. When comparing different discrimination sources, only teacher discrimination was found to predict externalising behaviour significantly (explaining 15% of the variance). The qualitative follow-up illustrated the significance of teacher discrimination: Some Muslim youths felt that their teachers held back their school progress, while others reported receiving insults from teachers about their parents’ native country and their religion. We argue that students’ perceived powerlessness within the teacher-student relationship deserves further attention, as some Dutch Muslim youths reported painful experiences, with perceived teacher discrimination linked to higher levels of externalising behaviour.

The study of moral judgements often centres on moral dilemmas in which options consistent with deontological perspectives (that is, emphasizing rules, individual rights and duties) are in conflict with options consistent with utilitarian judgements (that is, following the greater good based on consequences). Greene et al. (2009) showed that psychological and situational factors (for example, the intent of the agent or the presence of physical contact between the agent and the victim) can play an important role in moral dilemma judgements (for example, the trolley problem). Our knowledge is limited concerning both the universality of these effects outside the United States and the impact of culture on the situational and psychological factors affecting moral judgements. Thus, we empirically tested the universality of the effects of intent and personal force on moral dilemma judgements by replicating the experiments of Greene et al. in 45 countries from all inhabited continents. We found that personal force and its interaction with intention exert influence on moral judgements in the US and Western cultural clusters, replicating and expanding the original findings. Moreover, the personal force effect was present in all cultural clusters, suggesting it is culturally universal. The evidence for the cultural universality of the interaction effect was inconclusive in the Eastern and Southern cultural clusters (depending on exclusion criteria). We found no strong association between collectivism/individualism and moral dilemma judgements. Very few studies have investigated frailty among older immigrants in Europe. The aim of the current study was to investigate inequalities in frailty in young-olds related to gender, educational level and country of origin, as well as intersections between these characteristics. Cross-sectional data were used from older Turkish and Moroccan immigrants (n = 466) and native Dutch (n = 1,020), all aged 55–65 years and participating in the Longitudinal Aging Study Amsterdam. Frailty was assessed with a 30-item frailty index, based on the deficit accumulation approach. Frailty was higher among women, lower educated, and people with a migration background. Of all groups considered, frailty levels were the highest among Turkish immigrants. No statistically significant interaction effects between gender, educational level and country of origin were found. When targeting frailty interventions, special attention should be devoted to older immigrants, as they are the most vulnerable group with the highest frailty levels.

Background and Objectives There is an urgent need to better understand frailty and its predisposing factors. Although numerous cross-sectional studies have identified various risk and protective factors of frailty, there is a limited understanding of longitudinal frailty progression. Furthermore, discrepancies in the methodologies of these studies hamper comparability of results. Here, we use a coordinated analytical approach in 5 independent cohorts to evaluate longitudinal trajectories of frailty and the effect of 3 previously identified critical risk factors: sex, age, and education. Research Design and Methods We derived a frailty index (FI) for 5 cohorts based on the accumulation of deficits approach. Four linear and quadratic growth curve models were fit in each cohort independently. Models were adjusted for sex/gender, age, years of education, and a sex/gender-by-age interaction term. Results Models describing linear progression of frailty best fit the data. Annual increases in FI ranged from 0.002 in the Invecchiare in Chianti cohort to 0.009 in the Longitudinal Aging Study Amsterdam (LASA). Women had consistently higher levels of frailty than men in all cohorts, ranging from an increase in the mean FI in women from 0.014 in the Health and Retirement Study cohort to 0.046 in the LASA cohort. However, the associations between sex/gender and rate of frailty progression were mixed. There was significant heterogeneity in within-person trajectories of frailty about the mean curves. Discussion and Implications Our findings of linear longitudinal increases in frailty highlight important avenues for future research. Specifically, we encourage further research to identify potential effect modifiers or groups that would benefit from targeted or personalized interventions.

Estimates from genome-wide association studies (GWAS) of unrelated individuals capture effects of inherited variation (direct effects), demography (population stratification, assortative mating) and relatives (indirect genetic effects). Family-based GWAS designs can control for demographic and indirect genetic effects, but large-scale family datasets have been lacking. We combined data from 178,086 siblings from 19 cohorts to generate population (between-family) and within-sibship (within-family) GWAS estimates for 25 phenotypes. Within-sibship GWAS estimates were smaller than population estimates for height, educational attainment, age at first birth, number of children, cognitive ability, depressive symptoms and smoking. Some differences were observed in downstream SNP heritability, genetic correlations and Mendelian randomization analyses. For example, the within-sibship genetic correlation between educational attainment and body mass index attenuated towards zero. In contrast, analyses of most molecular phenotypes (for example, low-density lipoprotein-cholesterol) were generally consistent. We also found within-sibship evidence of polygenic adaptation on taller height. Here, we illustrate the importance of family-based GWAS data for phenotypes influenced by demographic and indirect genetic effects. Within-sibship genome-wide association analyses using data from 178,076 siblings illustrate differences between population-based and within-sibship GWAS estimates for phenotypes influenced by demographic and indirect genetic effects.

Key summary points Aim To investigate the extent to which Dutch older adults reported cancellation or avoidance of medical care during the first months of the COVID-19 pandemic. Findings One third of the study sample reported cancellation or avoidance of medical care during the first months of the pandemic, and this was more common among those with multiple chronic conditions. Message Delay of routine care during the pandemic may impact morbidity and related adverse outcomes in the long term, which should be monitored in future research., Purpose Delay of routine medical care during the COVID-19 pandemic may have serious consequences for the health and functioning of older adults. The aim of this study was to investigate whether older adults reported cancellation or avoidance of medical care during the first months of the COVID-19 pandemic, and to explore associations with health and socio-demographic characteristics. Methods Cross-sectional data of 880 older adults aged ≥ 62 years (mean age 73.4 years, 50.3% female) were used from the COVID-19 questionnaire of the Longitudinal Aging Study Amsterdam, a cohort study among community-dwelling older adults in the Netherlands. Cancellation and avoidance of care were assessed by self-report, and covered questions on cancellation of primary care (general practitioner), cancellation of hospital outpatient care, and postponed help-seeking. Respondent characteristics included age, sex, educational level, loneliness, depression, anxiety, frailty, multimorbidity and information on quarantine. Results 35% of the sample reported cancellations due to the COVID-19 situation, either initiated by the respondent (12%) or by healthcare professionals (29%). Postponed help-seeking was reported by 8% of the sample. Multimorbidity was associated with healthcare-initiated cancellations (primary care OR = 1.92, 95% CI = 1.09–3.50; hospital OR = 1.86, 95% CI = 1.28–2.74) and respondent-initiated hospital outpatient cancellations (OR = 2.02, 95% CI = 1.04–4.12). Depressive symptoms were associated with postponed help-seeking (OR = 1.15, 95% CI = 1.06–1.24). Conclusion About one third of the study sample reported cancellation or avoidance of medical care during the first months of the pandemic, and this was more common among those with multiple chronic conditions. How this impacts outcomes in the long term should be investigated in future research.

Background The ability to accurately predict survival in older adults is crucial as it guides clinical decision making. The added value of using various health indicators as well as changes in these indicators for predicting mortality remains unclear. The aim of this study was to investigate whether changes in health indicators such as frailty and physical performance improve mortality predictions in old age. Methods This is a population based prospective cohort study on 995 community-dwelling people aged 68–92 years from the Longitudinal Aging Study Amsterdam. Two measurements at a three-year interval (1995/1996 and 1998/1999) were available for the frailty index, frailty phenotype, grip strength, walking speed, and Mini-Mental State Examination (MMSE). Cox regression was used to analyze mortality risks associated with the current health status and changes in health, with mortality data up to 2017. The extent to which these health indicators improved mortality predictions compared to models with age and sex only was assessed by the area under the receiver operating characteristic curve (AUC). Results The AUC of age and sex for five-year mortality was 72.8% (95% CI 69.0 – 76.5) and was the lowest in the oldest old (age > 80.5 years). The added AUC of the current status of health indicators ranged from 0.7 to 3.3%. The added AUC of the three-year change was lower, ranging from -0.0 to 1.1%, whereas the added AUC of three-year change and current status combined was similar to current status alone, ranging from 0.6 to 3.2%. Across age, the added AUC of current status was highest in the oldest old, however there was no such pattern using three-year change. Overall, the frailty index appeared to improve mortality predictions the most, followed by the frailty phenotype, MMSE, grip strength, and walking speed. Conclusions Current health status improved mortality predictions better than changes in health. Its contribution was highest in the oldest old, but the added value to models with age and sex only was limited.

The Longitudinal Aging Study Amsterdam (LASA) is a prospective cohort study of older adults in the Netherlands, initially based on a nationally representative sample of people aged 55-84 years. The study has been ongoing since 1992, and focuses on the determinants, trajectories and consequences of physical, cognitive, emotional and social functioning. Strengths of the LASA study include its multidisciplinary character, the availability of over 25 years of follow-up, and the cohort-sequential design that allows investigations of longitudinal changes, cohort differences and time trends in functioning. The findings from LASA have been reported in over 600 publications so far (see www.lasa-vu.nl). This article provides an update of the design of the LASA study and its methods, on the basis of recent developments. We describe additional data collections, such as additional nine-monthly measurements in-between the regular three-yearly waves that have been conducted among the oldest old during 2016-2019, and the inclusion of a cohort of older Turkish and Moroccan migrants.

This study describes how parents in academia negotiate their professional identity in relation to dominant discourses of science as a calling. Based on in-depth interviews with men and women academics in a Dutch university, five discursive strategies are distilled that reconcile contradictory claims of academia and parenthood. Parents are conforming, suffering or fighting dominant discourses, are optimistic about or pragmatically arranging academia and parenthood. These discursive strategies illustrate agency of parents, simultaneously subscribing to dominant discourses and negotiating alternative stances. Furthermore, from focus groups with leaders we distilled how the material structure of different schools, reflected in the rules and procedures regulating standards to which institutions and individuals are held, sets limits to discursive strategies that academics adopt. We identify the constraints and room for agency and argue that agency can only lead to transformation when transcending individual awareness by moving towards collective action.

BACKGROUND: Frailty is an important geriatric syndrome, but little is known about its development in the years preceding onset of the syndrome. The aim of this study was to examine the progression of frailty and compare the trajectories of each frailty component prior to frailty onset.METHODS: Repeat data were from two cohort studies: the Longitudinal Aging Study Amsterdam (n = 1440) with a 15-year follow-up and the InCHIANTI Study (n = 998) with a 9-year follow-up. Participants were classified as frail if they had >3 frailty components (exhaustion, slowness, physical inactivity, weakness, and weight loss). Transitions between frailty components were examined with multistate modeling. Trajectories of frailty components were compared among persons who subsequently developed frailty to matched nonfrail persons by using mixed effects models.RESULTS: The probabilities were 0.43, 0.40, and 0.36 for transitioning from 0 to 1 frailty component, from 1 component to 2 components, and from 2 components to 3-5 components (the frail state). The transition probability from frail to death was 0.13. Exhaustion separated frail and nonfrail groups already 9 years prior to onset of frailty (pooled risk ratio [RR] = 1.53, 95% confidence interval [CI] 1.04-2.24). Slowness (RR = 1.94, 95% CI 1.44-2.61), low activity (RR = 1.59, 95% CI 1.19-2.13), and weakness (RR = 1.39, 95% CI 1.10-1.76) separated frail and nonfrail groups 6 years prior to onset of frailty. The fifth frailty component, weight loss, separated frail and nonfrail groups only at the onset of frailty (RR = 3.36, 95% CI 2.76-4.08).CONCLUSIONS: Evidence from two cohort studies suggests that feelings of exhaustion tend to emerge early and weight loss near the onset of frailty syndrome.

Age at first sexual intercourse and age at first birth have implications for health and evolutionary fitness. In this genome-wide association study (age at first sexual intercourse, N = 387,338; age at first birth, N = 542,901), we identify 371 single-nucleotide polymorphisms, 11 sex-specific, with a 5–6% polygenic score prediction. Heritability of age at first birth shifted from 9% [CI = 4–14%] for women born in 1940 to 22% [CI = 19–25%] for those born in 1965. Signals are driven by the genetics of reproductive biology and externalising behaviour, with key genes related to follicle stimulating hormone (FSHB), implantation (ESR1), infertility and spermatid differentiation. Our findings suggest that polycystic ovarian syndrome may lead to later age at first birth, linking with infertility. Late age at first birth is associated with parental longevity and reduced incidence of type 2 diabetes and cardiovascular disease. Higher childhood socioeconomic circumstances and those in the highest polygenic score decile (90%+) experience markedly later reproductive onset. Results are relevant for improving teenage and late-life health, understanding longevity and guiding experimentation into mechanisms of infertility.

Introduction: Frailty can be seen as a continuum, from fit to frail. While many recent studies have focused on frailty, much less attention has been paid to the other end of the continuum: the group of older adults that remain (relatively) vital. Moreover, there is a lack of studies on frailty and vitality that investigate predictors from multiple domains of functioning simultaneously. The aim of this study was to identify predictors of frailty as well as vitality among older adults aged 75 years and over. Methods: We used longitudinal data from 569 adults aged ≥75 years who participated in the Longitudinal Aging Study Amsterdam. Predictors from the sociodemographic, social, psychological, lifestyle, and physical domains of functioning were measured at T1 (2008–2009). We used the frailty index (FI) to identify frail (FI ≥ 0.25) and vital (FI ≤ 0.15) respondents at follow-up, 3 years later (T2: 2011–2012). We conducted logistic regression analyses with backward stepwise selection to develop and internally validate our prediction models. Results: The prevalence of frailty in our sample at follow-up was 49.4% and the prevalence of vitality was 18.3%. Predictors of frailty and vitality partly overlapped and included age, depressive symptoms, number of chronic diseases, and self-rated health. We also found predictors that did not overlap. Male sex, moderate alcohol use, more emotional support received, and no hearing problems, were predictors of vitality. Lower cognitive functioning, polypharmacy, and pain were predictors of frailty. The final model for vitality explained 42% of the variance and the final model for frailty explained 48%. Both models had a good discriminative value (area under ROC-curve [AUC] vitality: 0.88; AUC frailty: 0.85). Conclusion: Among older adults aged 75 years and over, predictors of frailty only partially overlap with predictors of vitality. The readily accessible predictors in our models may help to identify older adults who are likely to be vital, or who are at risk of frailty.

OBJECTIVE: We examined the association between vitamin K status and physical functioning over 13 years in the Longitudinal Aging Study Amsterdam.STUDY DESIGN: Longitudinal cohort study of 633 community-dwelling adults from the Longitudinal Aging Study Amsterdam (LASA) aged 55-65 years (54% women).MAIN OUTCOME MEASURES: At baseline (2002-2003), plasma desphospho-uncarboxylated matrix Gla protein (dp-ucMGP) was measured with a sandwich ELISA as a marker of vitamin K status. The outcome measures handgrip strength, calf circumference, self-reported functional limitations and functional performance were obtained at baseline and four follow-up examinations. We used generalized estimating equations to determine the relationship between dp-ucMGP tertiles and the various outcome measurements after adjusting for potential confounders. The lowest dp-ucMGP tertile reflects a high vitamin K status and was the reference.RESULTS: Mean dp-ucMGP was 376 ± 233 pmol/L and mean follow-up was 11.1 years. Participants showed a decline in the outcome measures over time. Compared with the lowest tertile, the highest dp-ucMGP tertile had: lower handgrip strength, 1.1 kg (95% confidence interval (-2.1, -0.1; P-trend CONCLUSIONS: Low vitamin K status was associated with lower handgrip strength, smaller calf circumference, and, in women only, with poorer functional performance score. A low vitamin K status was however not related to the 13-year decline in these measures.

Background: Frailty is an important risk factor for adverse outcomes in older people. Substantial variation in frailty prevalence between socioeconomic groups exists, but longitudinal evidence for the association between socioeconomic position (SEP) and frailty is scarce. Objective: To investigate the course of socioeconomic inequalities in frailty among older adults during 10 years of follow-up. Methods: Data were used from the Longitudinal Aging Study Amsterdam (n = 1,509). Frailty was measured with the functional domains approach, based on deficiencies in four domains: physical, nutritive, cognitive, and sensory. Mixed-model analyses were performed to estimate the course of frailty and its association with SEP during a 10-year follow-up. We investigated whether similar patterns of associations held in different scenarios, comparing results of survivor analyses with those based on two imputation methods accounting for dropout due to death (substitution of first missing value and missing values imputed with a prediction model). Results: All scenarios showed a linear increase in frailty with aging (survivor analyses OR = 1.87, 95% CI = 1.66-2.11) and associations of low education and low income with frailty (adjusted OR for low education = 1.76, 95% CI = 1.05-2.97; adjusted OR for low income = 1.90, 95% CI = 1.20-3.01; both for survivor analyses). Sex-stratified analyses indicated that socioeconomic inequalities were mainly present in men, not in women. Similar patterns of associations of SEP with frailty were observed in all scenarios, but the increase in frailty prevalence over time differed substantially between the scenarios. There were no statistically significant interactions between time and SEP on frailty (all scenarios), suggesting that inequalities in frailty did not increase or decrease during follow-up. Conclusion: SEP inequalities in frailty among older adults were observed, mainly among men, and persisted during 10 years of follow-up.

The association between the metabolic syndrome (MetS) and adverse outcomes in older adults may be explained by other health conditions. This study examined the role of frailty in explaining the association between MetS and mortality, independent of comorbidity. Data were used from 1247 men and women aged ≥ 65 years of the Longitudinal Aging Study Amsterdam. MetS was assessed using the definition of the US National Cholesterol Education Program. Frailty was measured by the frailty phenotype. Mortality was monitored from 1995 until 2015. Associations of MetS with 19-year all-cause mortality were assessed using Cox proportional hazard models. MetS was present in 37% of the participants. In a model adjusted for age, sex and educational level hazard ratios of mortality were significantly higher in people with MetS (HR = 1.23, 95% CI = 1.08–1.40). After adjusting for frailty the association of MetS with mortality reduced, but remained statistically significant (HR = 1.15, 95% CI = 1.01–1.31). The presence of chronic diseases (cardiovascular diseases and diabetes) explained a larger part of the relationship between MetS and mortality (HR = 1.12, 95% CI = 0.98–1.28). These results show that physical frailty has a smaller contribution to the explanation of the association between MetS and 19-year all-cause mortality than the presence of chronic diseases.

OBJECTIVES: Very few studies looking at slow gait speed as early marker of cognitive decline investigated the competing risk of death. The current study examines associations between slow gait speed and transitions between cognitive states and death in later life.METHODS: We performed a coordinated analysis of three longitudinal studies with 9 to 25 years of follow-up. Data were used from older adults participating in H70 (Sweden; n = 441; aged ≥70 years), InCHIANTI (Italy; n = 955; aged ≥65 years), and LASA (the Netherlands; n = 2824; aged ≥55 years). Cognitive states were distinguished using the Mini-Mental State Examination. Slow gait speed was defined as the lowest sex-specific quintile at baseline. Multistate models were performed, adjusted for age, sex and education.RESULTS: Most effect estimates pointed in the same direction, with slow gait speed predicting forward transitions. In two cohort studies, slow gait speed predicted transitioning from mild to severe cognitive impairment (InCHIANTI: HR = 2.08, 95%CI = 1.40-3.07; LASA: HR = 1.33, 95%CI = 1.01-1.75) and transitioning from a cognitively healthy state to death (H70: HR = 3.30, 95%CI = 1.74-6.28; LASA: HR = 1.70, 95%CI = 1.30-2.21).CONCLUSIONS: Screening for slow gait speed may be useful for identifying older adults at risk of adverse outcomes such as cognitive decline and death. However, once in the stage of more advanced cognitive impairment, slow gait speed does not seem to predict transitioning to death anymore.

Living apart together (LAT) combines intimacy with autonomy and flexibility but, possibly, with lower commitment to exchanging support and care compared with first marriages, remarriages, and cohabitation of older adults. Data from 50- to 79-year-old respondents in the Family and Fertility Survey 2013 (Statistics Netherlands; N = 4,108) showed that older LAT partners are most often involved in exchanging emotional support. No differences were found in the receipt of daily care. Multivariate analyses showed that receiving support and care from the partner was associated with more health problems, higher quality of the partner relationship, and a broader support network. When in poor health, LAT partners were less likely to receive daily care, but not emotional support, from their partner compared with the other types. Partner relationships in later life are well equipped to provide emotional support, but partner care is facilitated largely by help from others.

Introduction: This study examines the role of educational attainment, an indicator of cognitive reserve, on transitions in later life between cognitive states (normal Mini-Mental State Examination (MMSE), mild MMSE impairment, and severe MMSE impairment) and death.Methods: Analysis of six international longitudinal studies was performed using a coordinated approach. Multistate survival models were used to estimate the transition patterns via different cognitive states. Life expectancies were estimated.Results: Across most studies, a higher level of education was associated with a lower risk of transitioning from normal MMSE to mild MMSE impairment but was not associated with other transitions. Those with higher levels of education and socioeconomic status had longer nonimpaired life expectancies.Discussion: This study highlights the importance of education in later life and that early life experiences can delay later compromised cognitive health. This study also demonstrates the feasibility and benefit in conducting coordinated analysis across multiple studies to validate findings.

This study examines a factor that has thus far received little attention in research on attitudes toward violent in-group defense, namely, the role of perceived parental ethnic socialization. We hypothesized that perceived parental ethnic socialization (i.e., cultural socialization, egalitarianism, bias/mistrust) affects attitudes toward violence in defense of the in-group by others as well as willingness to use such violence oneself via its influence on collective identity factors (in-group connectedness, collective deprivation, religious superiority, connectedness with mainstream society). We analyzed a sample of children of Turkish Muslim migrants in the Netherlands. The data came from a survey conducted among pupils at 7 secondary schools (age 14–18, N = 133). Results show that perceived parental ethnic socialization has an indirect effect on attitudes toward and willingness to use a violent in-group defense that runs via the collective identity factors. Perceived parental socialization that emphasizes equality is related to less willingness to use violent in-group defense. Perceived parental messages of mistrust of the other and preparation for bias were associated with a more positive attitude toward violent in-group defense by others and toward willingness to use such violence. Perceived cultural socialization correlates positively with attitude toward violent in-group defense by others and willingness to use violent in-group defense. The total size of the indirect effects of perceived parental ethnic socialization was modest. We did not find a direct effect of perceived parental socialization. Many topics that scientists investigate speak to people’s ideological worldviews. We report three studies—including an analysis of large-scale survey data—in which we systematically investigate the ideological antecedents of general faith in science and willingness to support science, as well as of science skepticism of climate change, vaccination, and genetic modification (GM). The main predictors are religiosity and political orientation, morality, and science understanding. Overall, science understanding is associated with vaccine and GM food acceptance, but not climate change acceptance. Importantly, different ideological predictors are related to the acceptance of different scientific findings. Political conservatism best predicts climate change skepticism. Religiosity, alongside moral purity concerns, best predicts vaccination skepticism. GM food skepticism is not fueled by religious or political ideology. Finally, religious conservatives consistently display a low faith in science and an unwillingness to support science. Thus, science acceptance and rejection have different ideological roots, depending on the topic of investigation.

textabstractLoneliness is nowadays considered one of the main problems in society. The negative experience of a discrepancy between the desired and the achieved personal network of relationships is common and affects both younger and older adults. This chapter first addresses well-established aspects and new developments in the main concepts of loneliness and social isolation, the measuring instruments and the prevalence of loneliness. This chapter continues with an overview of theoretical ideas regarding loneliness, focusing on individual-level and societal predisposing characteristics as well as on genetic/evolutionary perspectives on the onset and continuation of loneliness. The main part of this chapter is dedicated to empirical evidence from many sources and disciplines, including psychology, sociology, and epidemiological sciences. The prevention of loneliness, coping, and interventions are addressed in the final part of this chapter. Other chapters in this volume address topics related to loneliness, namely social rejection, the neuroscience of social disconnection, social networks, and relationships and health (see Leary & Acosta, Chapter 28, this volume; Cacioppo, Chapter 16, this volume; Felmlee & Sinclair, Chapter 34, this volume; and Holt-Lunstad, Chapter 33, this volume).

BACKGROUNDMost research asks whether or not cohabitation has come to rival marriage. Little is known about the meaning of living apart together (LAT) relationships, and whether LAT is an alternative to marriage and cohabitation or a dating relationship.OBJECTIVEWe examine across Europe: (1) the prevalence of LAT, (2) the reasons for LAT, and (3) the correlates of (a) LAT relationships vis-a-vis being single, married, or cohabiting, and (b) different types of LAT union.METHODSUsing Generations and Gender Survey data from ten Western and Eastern European countries, we present descriptive statistics about LATs and estimate multinominal logistic regression models to assess the correlates of being in different types of LAT unions.RESULTSLAT relationships are uncommon, but they are more common in Western than Eastern Europe. Most people in LAT unions intend to live together but are apart for practical reasons. LAT is more common among young people, those enrolled in higher education, people with liberal attitudes, highly educated people, and those who have previously cohabited or been married. Older people and divorced or widowed persons are more likely to choose LAT to maintain independence. Surprisingly, attitudinal and educational differences are more pronounced in Eastern Europe than in Western Europe.CONCLUSIONSA tentative conclusion is that LAT is more often a stage in the union formation process than an alternative to marriage and cohabitation. Yet some groups do view LAT as substituting for marriage and cohabitation, and these groups differ between East and West. In Eastern Europe a cultural, highly educated elite seems to be the first to resist traditional marriage norms and embrace LAT (and cohabitation) as alternative living arrangements, whereas this is less the case in Western Europe. In Western Europe, LAT unions are mainly an alternative for persons who have been married before or had children in a prior relationship.

This study aimed to explore the loneliness of several groups of older immigrants in Canada compared to native-born older adults. Data from the Canadian General Social Survey, Cycle 22 (N older adults = 3,692) were used. The dependent variable is the 6 item De Jong Gierveld loneliness scale. Determinants of loneliness included country of birth, ethnic background (cultural context); belongingness (community context) and social networks (social context). Results showed that only some immigrant groups are significantly lonelier than older adults born in Canada. Immigrants with similar language and culture are not lonelier; while those from countries that differ in native language/culture are significantly higher on loneliness. Multivariate analyses showed the importance of cultural background, of composition of the network of relatives and friends, and of local participation and feelings of belonging to the Canadian society in explaining loneliness of older immigrants.

We propose a typology of different meanings of cohabitation that combines cohabiters' intentions to marry with a general attitude toward marriage, using competing risk analyses to examine whether some cohabiters are more prone than others to marry or to separate. Using data (N = 1,258) from four waves of the German Family Panel (PAIRFAM) and a supplementary study (DEMODIFF), we compared eastern and western German cohabiters of the birth cohorts 1971-73 and 1981-83. Western Germans more frequently view cohabitation as a step in the marriage process, whereas eastern Germans more often cohabit as an alternative to marriage. Taking into account marital attitudes reveals that cohabiters without marriage plans differ from those with plans in their relationship careers, and also shows that cohabiters who plan to marry despite holding a less favourable view of marriage are less likely to realize their plans than cohabiters whose intentions and attitudes are more congruent.

This study considers changes in child care by grandparents between 1992 and 2006 in relation to changes in mothers' need for and grandparents' opportunity to provide child care. Data from the Longitudinal Aging Study Amsterdam are used to compare two cohorts of Dutch grandparents aged 58–68 (N1992=181; N2006=350). Multi-level regression analysis shows that the probability that grandparents care for their adult daughters' children (N1992=261; N2006=484) increased from 0.23 to 0.41. The increase can be ascribed to higher maternal employment rates, growth in single motherhood, reduced travel time and a decline in the number of adult children. The increase would have been higher if the employment rate of grandparents had not risen.

Gerotranscendence defines a shift in meta-perspective from earlier materialistic and pragmatic concerns, toward more cosmic and transcendent ones in later life. Population-based studies that have empirically examined this concept using Tornstam's gerotranscendence scale, highlight cosmic transcendence as a core component, which includes a sense of belongingness with past and future generations. Such generative concerns may increase expectations regarding the quality of the bond with one's children in later life. This study examined whether the association between emotional support exchanged with children and feelings of loneliness later in life varied by the degree of cosmic transcendence of the older parent. Data from 1,845 older parents participating in a population-based study living in The Netherlands were analyzed from the 1995/1996 cycle of the Longitudinal Aging Study Amsterdam. Interviews included self-report measures of cosmic transcendence, loneliness, emotional support exchanged with children, health indicators, and marital status. Results indicated that a negative association between loneliness and level of emotional support exchanged with children was more pronounced among older parents with higher cosmic transcendence scores, in particular among the married. It is argued that cosmic transcendence reflects a sense of generativity and an increased emotional dependency on children in later life. Under favorable social conditions (supportive relationships with children and being married) cosmic transcendent views had a positive impact on social well-being in later life. When children no longer met emotional needs of older parents, cosmic transcendence increased feelings of loneliness. © Springer-Verlag 2006.

The population of the Netherlands is aging, although it is still relatively young in comparison with the population of most other European countries. As Dutch society transitions from a welfare state to a society based more on individual responsibility, the increasingly well-educated and financially well-off elderly people wish to exert more control over their own lives. Research and education in the field of aging have grown rapidly over the past few decades, along with variety in research focus and methodology. In addition, funding organizations nowadays stress the importance of participation of older adults in research studies and the usability of research findings to society. Thus, academic and applied research is expected to thrive and contribute to the autonomy, health, and well-being of Dutch elders, while also providing insight into physical, mental, social, and financial aspects of aging. Thanks to these insights, public debate is focusing not only on the costs of health care and pensions but also on older generations' autonomy and contributions to society. © 2013 The Author. Background: Studies explaining why cohabiters are more likely to keep money separate than spouses have mainly focused on selection processes, without taking into account the heterogeneity within both union types in levels of commitment. Cross-national studies are rare and have predominantly included Northern and Western European countries, the United States, and Canada. Objective: This study explains the higher likelihood of cohabiters to keep income separate by selection as well as commitment factors and explores country differences, including countries from Central and Eastern Europe. Methods: Using data from the Generations and Gender Surveys of Bulgaria, France, Georgia, Germany, Romania, and Russia, N=41,456 cohabiting and married individuals are studied. Binary logistic regression models of the likelihood that respondents keep money separate are calculated. Results: Across countries, higher education, female labor market participation, both partners being employed, short union duration, absence of joint children, presence of separation thoughts, and (for cohabiters) a lack of marital intentions are the most persistent correlates of keeping money separate. Differences between cohabiters and married couples are reduced when selection and commitment are taken into account, but are still significant. Cross-national variation in the effect of cohabitation on keeping separate purses is persistent. Conclusions: Different money management strategies of cohabiters and spouses can be explained to some extent by selection processes and inherent differences in the level of commitment within cohabitation and marriage. Countries also differ in the socio-economic context and norms concerning the way intimate relationships are organized which might lead to persistent differences in the way cohabiting and married couples organize their income. When is an individual likely to be accepted or rejected by a group? This research investigates responses towards prospective group members depending on how they compare to the group in terms of their perceived morality or competence. Because morality is of particular importance to groups, we hypothesized that the perceived morality of prospective group members has more impact on the group's tendency to accept versus reject them than their competence. Across three experiments, employing self‐report, psychophysiological and behavioural measures, results supported this hypothesis: Immoral (vs. incompetent) individuals were perceived as more different from the group and were more likely to be rejected. Additionally, the rejection of prospective group members with perceived inferior morality (but not those with inferior competence) was mediated by the group threat they imply. Inclusion success thus seems to be mainly contingent upon how a group evaluates the individual's morality relative to the group's standards.

Human ageing is inextricably bound to the loss of physical and cognitive functions, loss of social roles, and loss of social contacts, though losses in functioning and social roles not necessarily threatens the level of well-being according to older adults themselves. This paradoxical finding has fuelled the scientific and political arena for many years, and discussions about what is successful aging has not yet subsided. To investigate to what extend changes in various domains of functioning are related to changes in older adults’ perception of well being, we estimate fourteen year trajectories in both domains and estimate to what extend changes in these domains are interrelated. Based on data from the Longitudinal Aging Study Amsterdam (n= 1257), we observed that ageing is associated with losses in multiple domains but decline in many functions is quite small. General well-being, on the other hand, does not decrease. Most striking finding was that there is only weak correlation between decline in domains of functioning generally considered as crucial for successful and decline in general well-being. Thus, aging successfully can be achieved even without doing well.

PurposeIn the context of research on the career advancement of women and men in academia, this paper aims to reflect on how deans at six schools of a Dutch arts and a Dutch sciences‐based university construct the image of the ideal academic, and on how these images are gendered.Design/methodology/approachUsing an inductive approach, the study analyzed the transcripts of semi‐structured in‐depth interviews with six deans (all men) from two different Dutch universities on the career advancement of men and women at their school.FindingsIt was expected that the images of the ideal academic would be more gendered in the sciences than in the arts university, considering the stronger male domination in the sciences university. The images of the ideal academic, while fundamentally different, regarding the expertise, the applicability of knowledge, and the visibility needed to be considered successful, were equally gendered in assuming that practicing science leaves little room for caring obligations outside work; in both places science was considered an omnipresent and greedy calling. Moreover, deans at both universities to a similar extent expected women academics not to fit to this standard. Paradoxically, in the arts university deans construct an image of women academics that in some aspects reflects a mirror image of women academics in the sciences university and vice versa.Research limitations/implicationsThe paper suggests that in this construction the process of “othering” women academics is more constant than the content of the ideal academic. They contribute to theories on the ideal worker in the field of science by arguing the construction of the ideal academic is fluid rather than fixed. Further research could investigate how the image of the ideal academic changes within the same discipline across different countries with a higher representation of women among full professors, as the findings are limited to The Netherlands.Practical implicationsThe paper argues that the fluidity of the ideal academic norm offers space for renegotiating such norms by making it more inclusive for women, which will have positive consequences for women's career advancement in academia.Originality/valueThe contribution of this paper is that constructions of the ideal academic are fluid rather than fixed, while dominant actors in organizations seem to attribute universal value to these images. Objectives: To disentangle the reciprocal effects between depressive symptoms and cognitive functioning over time and to study the association between changes in their trajectories using 13 years of follow-up. Design and Participants: Data were used from five waves of the population-based Longitudinal Aging Study Amsterdam. Subjects were included if data was present on depressive symptoms and cognitive performance on at least two occasions, which resulted in a study sample of N = 2,299. Measurements: Depressive symptoms were assessed with the Center for Epidemiologic Studies Depression Scale. Cognitive functioning was assessed using the Mini- Mental State Examination (general cognitive functioning) and timed coding task (speed of information processing). Results: Cross-domain latent change analyses showed that depression at baseline predicted both decline of general cognitive functioning and information processing speed, independent of relevant covariates. Conversely, information processing speed at baseline, but not general cognitive functioning was related to the course of depressive symptoms. The course of cognitive functioning was not significantly associated with the course of depressive symptoms. Conclusion: Depressive symptoms in older patients flag an increased likelihood of cognitive decline. This effect is considerable and may be due to several underlying mechanisms. The likelihood of the relationship reflecting either a causal effect of depression on cognitive decline, or a common cause, or both, should be estimated. Likewise, older persons with more limitations in information processing speed specifically are more vulnerable to increases in depression. © 2013 American Association for Geriatric Psychiatry.

Objective: Older couples are likely to be confronted with health problems of both spouses and these health problems may negatively influence their marital satisfaction. The present study examined these possible negative effects using a dyadic perspective. Method: Data from 78 independently living older couples were analyzed using the Actor–Partner Interdependence Model (APIM). Health problems were modeled as a latent factor of functional disability, the number of chronic diseases, and self-rated health. The couple’s health context, that is, similarity or dissimilarity, was examined with an actor–partner interaction variable. Results: For wives, spousal health problems were negatively associated with their marital satisfaction, but only under the condition that their own health was relatively good. For husbands, neither own nor spousal health problems were associated with their marital satisfaction. Discussion: Future research focusing on older couples needs to consider the couple’s health context next to health at the individual level.

Age-related decline in processing speed has long been considered a key driver of cognitive aging. While the majority of empirical evidence for the processing speed hypothesis has been obtained from analyses of between-person age differences, longitudinal studies provide a direct test of within-person change. Using recent developments in longitudinal mediation analysis, we examine the speed-mediation hypothesis at both the within-and between-person levels in two longitudinal studies, Longitudinal Aging Study Amsterdam (LASA) and Origins of Variance in the Oldest-Old (OCTO-Twin). We found significant within-person indirect effects of change in age, such that increasing age was related to lower speed, which in turn relates to lower performance across repeated measures on other cognitive outcomes. Although between-person indirect effects were also significant in LASA, they were not in OCTO-Twin which is not unexpected given the age homogeneous nature of the OCTO-Twin data. A more in-depth examination through measures of effect size suggests that, for the LASA study, the within-person indirect effects were small and between-person indirect effects were consistently larger. These differing magnitudes of direct and indirect effects across levels demonstrate the importance of separating between- and within-person effects in evaluating theoretical models of age-related change. © 2013 American Psychological Association.

Research in gerontology has demonstrated mixed effects of social support on cognitive decline and dementia: Social support has been shown to be protective in some studies, but not in others. Moreover, little is known about the underlying mechanisms between social support and cognitive functioning. We investigate one of the possible mechanisms, and argue that subjective appraisals rather than received amounts of social support affect cognitive functioning. Loneliness is seen as an unpleasant experience that occurs when a person's network of relationships is felt to be deficient in some important way. As such, loneliness describes the extent to which someone's needs are not being met and thus provides a subjective assessment of support quality. We expect that receiving instrumental and emotional support reduces loneliness, which in turn preserves cognitive functioning. Data are from the Longitudinal Aging Study Amsterdam (LASA) and include 2255 Dutch participants aged 55-85 over a period of six years. Respondents were measured every three years. Cognitive functioning was assessed with the Mini-Mental State Examination (MMSE), the Coding Task, and the Raven's Coloured Progressive Matrices. The analytical approach comprised latent growth mediation models. Frequent emotional support related to reduced feelings of loneliness and better cognitive functioning. Increases in emotional support also directly enhanced cognitive performance. The protective effect of emotional support was strongest amongst adults aged 65 years and older. Increase in instrumental support did not buffer cognitive decline, instead there were indications for faster decline. After ruling out the possibility of reversed causation, we conclude that emotional support relationships are a more powerful protector of cognitive decline than instrumental support relationships. Collective action, or the large-scale cooperation in the pursuit of public goods, has been suggested to have evolved through cultural group selection. Previous research suggests that the costly punishment of group members who do not contribute to public goods plays an important role in the resolution of collective action dilemmas. If large-scale cooperation sustained by the punishment of defectors has evolved through the mechanism of cultural group selection, two implications regarding costly punishment follow: (1) that people are more willing to punish defecting group members in a situation of intergroup competition than in a single-group social dilemma game and (2) that levels of "perverse" punishment of cooperators are not affected by intergroup competition. We find confirmation for these hypotheses. However, we find that the effect of intergroup competition on the punishment of defectors is fully explained by the stronger conditionality of punishment on expected punishment levels in the competition condition. © 2012 The Author(s).

The aim of this research is to investigate the often assumed relation between alcohol use and depressive symptoms among older men and women. For this study, a subsample of 2,119 participants of the Longitudinal Aging Study Amsterdam, aged 65 to 85 years at baseline, was followed over time and visited in their homes in 1992, 1995, 1998 and 2002. Depressive symptoms are assessed with the Centre for Epidemiologic Studies Depression Scale (CES-D). Alcohol use is measured with questions about the frequency and quantities of alcohol use. A relation between depressive symptoms and alcohol use could not be demonstraded in a population based sample of older drinkers. Only older heavily drinking men with higher levels of depressive symptoms, higher levels of anxiety, and more chronic diseases at baseline significantly reduced the number of glasses consumed per week from 26 to 14 in the ten years of follow-up. Heavily drinking women do not reduce the level of alcohol intake during follow-up. Public prevention strategies are needed to make older heavy drinking women and men who are still in relatively good health aware of the potential risks of excessive alcohol use.

Drinken van wijn wordt daarom misschien wel vaker geassocieerd met symbolen, rituelen, heroische overwinningen en romantische ontmoetingen dan met verslaving. Maar er is zoals bekend een keerzijde, en die keerzijde heeft een hele lange geschiedenis. This study examined whether grandparents perceive adult grandchildren as frequent and important contacts by analyzing network membership. It additionally examined whether this network membership is related to relationship intensity during childhood. Network membership was assessed in 1992 (397 grandparents, 1,594 adult grandchildren) and at the 2005-2006 follow-up (155 grandparents, 429 adult grandchildren) from the Longitudinal Aging Study Amsterdam. Relationship intensity during childhood was assessed in 1992. One out of 4 grandparents identified at least 1 adult grandchild in their personal network. Adult grandchildren who had an intense relationship with their grandparents during childhood were more often in grandparents' network than others. An intense relationship during childhood promotes continuation of the relationship into adulthood and might contribute to grandparent's support potential. © 2011 IARR.

The goal of this research is to test whether often observed correlates of loneliness in older age are related to onset of loneliness longitudinally. Despite the increasing number of longitudinal studies, the investigation of factors that are related to onset of loneliness is still limited. Analyses are based on data of the TamELSA study, which is a population-based prospective study in Tampere, Finland and started in 1979. For the present study 469 older adults aged between 60 and 86 years at baseline, who were not lonely at baseline, were selected and followed-up in 1989, 1999 and 2006. During the 28 years of follow-up approximately one third (N = 178) of the study population developed feelings of loneliness. Logistic regression analyses indicated that losing a partner, reduced social activities, increased physical disabilities, increased feelings of low mood, uselessness and nervousness, rather than baseline characteristics, are related to enhanced feelings of loneliness at follow-up. The higher incidence of loneliness among women can be fully explained by the unequal distribution of risk factors among men and women (e. g., women more often become widowed). Our results are in line with the cognitive approach that conceptualizes loneliness as an unpleasant feeling due to a perceived discrepancy between the desired and the achieved level of social and personal resources. © 2011 The Author(s).

Attitudes toward homosexuality are complex. To get a comprehensive view on the attitudes of people, different dimensions need to be included in research. Based on a review of the literature, we distinguish five dimensions: acceptance of homosexuality in a general sense; attitude toward equal rights; reactions to homosexuality "at close quarters"; reactions to homosexuality in public; and so-called modern homonegativity. In a study on a representative sample of Dutch Defence personnel (N = 1,607) we tested this model. Structural equation modeling of several items measuring the attitude toward homosexuality offers a six factor solution. These six factors are more or less comparable to the five dimensions we distinguished. The dimension "reactions to homosexuality at close quarters" is, however, empirically split in a dimension on affective reactions to homosexuality and homosexual persons in general and a dimension on affective reaction to homosexual friends or acquaintances.

This article questions existing findings and provides new evidence about the consequences of nonstandard work schedules on partnership quality. Using quantitative couple data from The Netherlands Kinship Panel Study (NKPS) (N = 3,016) and semistructured qualitative interviews (N = 34), we found that, for women, schedules with varying hours resulted in greater relationship dissatisfaction than for men. Men with young children who worked varying hours had less relationship conflict and spent more time with children. Parents used nonstandard schedules for tag-team parenting or to maintain perceptions of full-time motherhood. The lack of negative effects, particularly for night shifts, suggests that previous findings-largely U.S. ones-are not universal and may be attributed to wider cultural, industrial relations, and economic contexts.
