Coda-Q is used to estimate the attenuation and scattering properties of the Earth. So far focus has been on earthquake data at frequencies above 1 Hz, as the high noise level in the first and second microseismic peak, and possibly lower scattering coefficient, hinder stable measurements at lower frequencies. In this work, we measure and map coda-Q in the period bands 2.5–5 s, 5–10 s and 10–20 s in the greater Alpine region using noise cross-correlations between station pairs, based on data from permanent seismic stations and from the temporary AlpArray experiment. The observed coda-Q for short interstation distances is independent of azimuth so there is no indication of influence of the directivity of the incoming noise field on our measurements. In the 2.5–5 s and 5–10 s period bands, our measurements are self-consistent, and we observe stable geographic patterns of low and high coda-Q in the period bands 2.5–5 s and 5–10 s. In the period band 10–20 s, the dispersion of our measurements increases and geographic patterns become speculative. The coda-Q maps show that major features are observed with high resolution, with a very good geographical resolution of for example low coda-Q in the Po Plain. There is a sharp contrast between the Po Plain and the Alps and Apennines where coda-Q is high, with the exception a small area in the Swiss Alps which may be contaminated by the low coda-Q of the Po Plain. The coda of the correlations is too short to make independent measurements at different times within the coda, so we cannot distinguish between intrinsic and scattering Q. Measurements on more severely selected data sets and longer time-series result in identical geographical patterns but lower numerical values. Therefore, high coda-Q values may be overestimated, but the geographic distribution between high and low coda-Q areas is respected. Patients and methods: APOLLO-1 (NCT02815709) was a phase III, double-blind, randomized trial in patients with moderate-to-severe pain following bunionectomy. Patients received a loading dose of either placebo, oliceridine (1.5 mg), or morphine (4 mg), followed by demand doses via patient-controlled analgesia (0.1, 0.35, or 0.5 mg oliceridine, 1 mg morphine, or placebo). The primary endpoint compared the proportion of treatment responders through 48 hours for oliceridine regimens and placebo. Secondary outcomes included a composite measure of respiratory safety burden (RSB, representing the cumulative duration of respiratory safety events) and the proportion of treatment responders vs morphine. The brown marmorated stink bug, Halyomorpha halys (Sta° l), feeds on a variety of fruits and vegetables, and is an economically important invasive hemipteran pest. Trap cropping of H. halys was examined at the Pennsylvania State University Southeast Agriculture Research and Extension Center (SEAREC) in Lancaster Co., PA, from 2012 to 2013, with sunflowers used as a trap crop to protect bell pepper. H. halys were observed frequently on sunflowers planted surrounding the pepper field, and in both years of this experiment significantly more H. halys were observed in sunflowers than peppers. Both adults and nymphs were observed with equal frequency, with higher numbers of both observed in September. A 2:1 ratio of females to males was observed throughout both years. While sunflowers were attractive to H. halys, no difference in fruit damage was observed in peppers surrounded by the sunflower trap crop versus those peppers surrounded by peppers. While sunflowers present an interesting potential trap crop for H. halys, future research is needed to clarify the feasibility of this crop protection technique. [ABSTRACT FROM AUTHOR]

The authors describe user interface tools based on search histories to support legal information seekers. The design of the tools was informed by the results of a user study (Komlodi, 2002a) that examined the use of human memory, external memory aids, and search histories in legal information seeking and derived interface design recommendations for information storage and retrieval systems. The data collected were analyzed to identify potential task areas where search histories can support information seeking and use. The results show that many information-seeking tasks can take advantage of automatically and manually recorded history information. These findings encouraged the design of user interface tools building on search history information: direct search history displays, history-enabled scratchpad facilities, and organized results collection tools. This randomized, double-blind, patient-controlled analgesia Phase IIb study was conducted to investigate the efficacy, safety, and tolerability of oliceridine compared with morphine and placebo in patients with moderate to severe pain following abdominoplasty (NCT02335294; oliceridine is an investigational agent not yet approved by the US Food and Drug Administration). Methods: Patients were randomized to receive postoperative regimens of intravenous oliceridine (loading/patient-controlled demand doses [mg/mg]: 1.5/0.10 [regimen A]; 1.5/0.35 [regimen B]), morphine (4.0/1.0), or placebo with treatment initiated within 4 hours of surgery and continued as needed for 24 hours. Results: Two hundred patients were treated (n=39, n=39, n=83, and n=39 in the oliceridine regimen A, oliceridine regimen B, morphine, and placebo groups, respectively). Patients were predominantly female (n=198 [99%]) and had a mean age of 38.2 years, weight of 71.2 kg, and baseline pain score of 7.7 (on 11-point numeric pain rating scale). Background and Aims: Heart failure endpoints in cardiovascular outcome trials are commonly identified through centralized adjudication of investigator-reported events. It remains unclear whether central adjudication improves the accuracy of treatment effect estimates in terms of log[hazard ratios (HR)]., Methods: Data from seven cardiovascular outcome trials with >1000 patients that included centrally adjudicated heart failure outcomes were utilized to assess (i) the concordance between investigator-reported and centrally adjudicated heart failure and cardiovascular death events; (ii) rates of subsequent all-cause mortality following positively vs. negatively adjudicated heart failure events; and (iii) the correlation of log(HR) based on centrally adjudicated vs. investigator-reported events., Results: Positive adjudication rates for investigator-reported events varied widely across trials, but were generally higher for cardiovascular death (range: 87.9%-99.2%) than for heart failure hospitalization (range: 61.6%-88.0%). The risk for subsequent all-cause death was similar for positively and negatively adjudicated heart failure hospitalizations. Log(HR) correlated well for cardiovascular death [R2 = .80, 95% credible interval (CrI): 0.53-0.93] and the composite of cardiovascular death or heart failure hospitalization (R2 = .79, 95% CrI: 0.46-0.93), but less for heart failure hospitalization (R2 = .57, 95% CrI: 0.10-0.83)., Conclusions: Positive adjudication rates were lower for heart failure events than cardiovascular death, but even negatively adjudicated heart failure events are prognostically important. Central adjudication of events did not alter the results (precision or estimated log(HR)), though some variation was observed, depending on the indication. The translational research community, in general, and the Clinical and Translational Science Awards (CTSA) community, in particular, share the vision of repurposing EHRs for research that will improve the quality of clinical practice. Many members of these communities are also aware that electronic health records (EHRs) suffer limitations of data becoming poorly structured, biased, and unusable out of original context. This creates obstacles to the continuity of care, utility, quality improvement, and translational research. Analogous limitations to sharing objective data in other areas of the natural sciences have been successfully overcome by developing and using common ontologies. This White Paper presents the authors' rationale for the use of ontologies with computable semantics for the improvement of clinical data quality and EHR usability formulated for researchers with a stake in clinical and translational science and who are advocates for the use of information technology in medicine but at the same time are concerned by current major shortfalls. Hypertension is the most common and preventable risk factor for cardiovascular disease (CVD), accounting for 20% of deaths worldwide. However, 2/3 of people with hypertension are undiagnosed, untreated, or under treated. A multi-pronged approach is needed to improve hypertension management. Elevated blood pressure (BP) in childhood is a predictor of hypertension and CVD in adulthood; therefore, screening and education programmes should start early and continue throughout the lifespan. Home BP monitoring can be used to engage patients and improve BP control rates. Progress in imaging technology allows for the detection of preclinical disease, which may help identify patients who are at greatest risk of CV events. There is a need to optimize the use of current BP control strategies including lifestyle modifications, antihypertensive agents, and devices. Reducing the complexity of pharmacological therapy using single-pill combinations can improve patient adherence and BP control and may reduce physician inertia. Other strategies that can improve patient adherence include education and reassurance to address misconceptions, engaging patients in management decisions, and using digital tools. Strategies to improve physician therapeutic inertia, such as reminders, education, physician-peer visits, and task-sharing may improve BP control rates. Digital health technologies, such as telemonitoring, wearables, and other mobile health platforms, are becoming frequently adopted tools in hypertension management, particularly those that have undergone regulatory approval. Finally, to fight the consequences of hypertension on a global scale, healthcare system approaches to cardiovascular risk factor management are needed. We present a novel approach to recommending articles from the medical literature that support clinical diagnostic decision-making, giving detailed descriptions of the associated ideas and principles. The specific goal is to retrieve biomedical articles that help answer questions of a specified type about a particular case. Based on the filtered keywords, MeSH(Medical Subject Headings) lexicon and the automatically extracted acronyms, the relationship between keywords and articles was built. The paper gives a detailed description of the process of by which keywords were measured and relevant articles identified based on link analysis in a weighted keywords network. Some important challenges identified in this study include the extraction of diagnosis-related keywords and a collection of valid sentences based on the keyword co-occurrence analysis and existing descriptions of symptoms. All data were taken from medical articles provided in the TREC (Text Retrieval Conference) clinical decision support track 2015. Ten standard topics and one demonstration topic were tested. In each case, a maximum of five articles with the highest relevance were returned. The total user satisfaction of 3.98 was 33% higher than average. The results also suggested that the smaller the number of results, the higher the average satisfaction. However, a few shortcomings were also revealed since medical literature recommendation for clinical diagnostic decision support is so complex a topic that it cannot be fully addressed through the semantic information carried solely by keywords in existing descriptions of symptoms. Nevertheless, the fact that these articles are actually relevant will no doubt inspire future research.

Aims: The aims of the present study were to investigate the maternal, fetal and neonatal safety and tolerability, pharmacodynamics and pharmacokinetics of intravenous (IV) retosiban in pregnant women with spontaneous preterm labour (PTL) between 34 0/7 and 35 6/7  weeks' gestation., Methods: In parts A and B of a three-part, double-blind, placebo-controlled, multicentre study, women were randomized 3:1 (Part A) or 2:1 (Part B) to either 12-h IV retosiban followed by a single dose of oral placebo (R-P) or 12-h IV placebo followed by single-dose oral retosiban (P-R)., Results: A total of 29 women were randomized; 20 to R-P and nine to P-R. An integrated analysis found that adverse events were infrequent in mothers/newborns and consistent with events expected in the population under study or associated with confounding factors. Retosiban was rapidly absorbed after oral administration, with an observed half-life of 1.45 h. Efficacy analyses included 19 women. While not statistically significant, those receiving R-P more frequently achieved uterine quiescence in 6 h (R-P, 63%; 95% credible interval [CrI]: 38, 84; P-R, 43%; 95% CrI: 12, 78) and more achieved a reduction of ≥50% in uterine contractions in 6 h (R-P, 63%; 95% CrI: 38, 84; P-R, 29%; 95% CrI: 4, 64). The number of days to delivery was increased in women receiving R-P (median 26 days for R-P vs. 13 days for P-R)., Conclusions: Intravenous retosiban has a favourable safety and tolerability profile and might prolong pregnancies in women with PTL. This paper presents the beginnings of a comprehensive ontology for organizing information about metrics and its potential application to defining and managing metrics in the CTSA (Clinical and Translational Science Award) project. The aim is to support an integrated database of all metrics used by CTSA components. The ontology is given as an entity-relationship conceptual data schema. Its completion should draw on metrics definition templates that can be found in many places.

The aim of this study was to identify the underlying semantics of health consumers' questions and physicians' answers in order to analyze the semantic patterns within these texts. We manually identified semantic relationships within question-answer pairs from Ask-the-Doctor Web sites. Identification of the semantic relationship instances within the texts was based on the relationship classes and structure of the Unified Medical Language System (UMLS) Semantic Network. We calculated the frequency of occurrence of each semantic relationship class, and conceptual graphs were generated, joining concepts together through the semantic relationships identified. We then analyzed whether representations of physician's answers exactly matched the form of the question representations. Lastly, we examined characteristics of the answer conceptual graphs. We identified 97 semantic relationship instances in the questions and 334 instances in the answers. The most frequently identified semantic relationship in both questions and answers was brings_about (causal). We found that the semantic relationship propositions identified in answers that most frequently contain a concept also expressed in the question were: brings_about, isa, co_occurs_with, diagnoses, and treats. Using extracted semantic relationships from real-life questions and answers can produce a valuable analysis of the characteristics of these texts. This can lead to clues for creating semantic-based retrieval techniques that guide users to further information. For example, we determined that both consumers and physicians often express causative relationships and these play a key role in leading to further related concepts.

Healthcare consumers need to find, comprehend, and interpret health information before making informed decisions. Recent work by others and our own work suggest that mis-matches in representations of health information used by consumers and professionals occur at different levels of knowledge representation, such as terminology (i.e., form or surface structure and concept or meaning) and semantic relation-ships. A challenge for consumer health informatics research is to devise a comprehensive strategy to bridge the gap between consumer understanding and biomedical knowledge at all levels. We propose a framework to inform the design of an "interpretive layer" to "mediate" between lay (illness model) and professional (disease model) perspectives, at all levels. In our view, the goal is to assist consumers in identifying terms to describe their needs, finding and understanding relevant information, and applying that knowledge for informed healthcare decision making.

Healthcare consumers often have difficulty expressing and understanding medical concepts. The goal of this study is to identify and characterize medical expressions or "terms" (linguistic forms and associated concepts) used by consumers and health mediators. In particular, these terms were characterized according to the degree to which they mapped to professional medical vocabularies. Lay participants identified approximately 100,000 term tokens from online discussion forum postings and print media articles. Of the over 81,000 extracted term tokens reviewed, more than 75% were mapped as synonyms or quasi-synonyms to the Unified Medical Language System (UMLS) Metathesaurus. While 80% conceptual overlap was found between closely mapped lay (consumer and mediator) and technical (professional) medical terms, about half of these overlapping concepts contained lay forms different from technical forms. This study raises questions about the nature of consumer health vocabularies that we believe have theoretical and practical implications for bridging the medical vocabulary gap between consumers and professionals.

Cardiac excitation-contraction (E-C) coupling is impaired at the myofilament level in the reversible postischemic dysfunction known as "stunned" myocardium. We characterized tension development and calcium cycling in intact isolated trabeculae from transgenic (TG) mice expressing the major proteolytic degradation fragment of troponin I (TnI) found in stunned myocardium (TnI(1-193)) and determined the ATPase activity of myofibrils extracted from TG and non-TG mouse hearts. The phenotype of these mice at baseline recapitulates that of stunning. Here, we address the question of whether contractile reserve is preserved in these mice, as it is in genuine stunned myocardium. During twitch contractions, calcium cycling was normal, whereas tension was greatly reduced, compared with non-TG controls. A decrease in maximum Ca2+-activated tension and Ca2+ desensitization of the myofilaments accounted for this contractile dysfunction. The decrease in maximum tension was paralleled by an equivalent decrease in maximum Ca2+-activated myofibrillar ATPase activity. Exposure to high calcium or isoproterenol recruited a sizable contractile reserve in TG muscles, which was proportionately similar to that in control muscles but scaled downward in amplitude. These results suggest that calcium regulatory pathways and beta-adrenergic signal transduction remain intact in isolated trabeculae from stunned TG mice, further recapitulating key features of genuine stunned myocardium.

In differentiated HL-60 cells the amphiphilic peptide mastoparan induces a dose-dependent stimulation of phosphoinositide breakdown with an EC50 value of 9 microM. Such stimulation can be markedly reduced by pretreatment of the cells with pertussis toxin (100 ng/ml, 2 h). In membranes obtained from differentiated HL-60 cells, guanine nucleotides stimulate the formation of IP2 and IP3. Calcium ions also induce phosphoinositide breakdown in this preparation independent of the presence of guanine nucleotides. In HL-60 cell membranes, mastoparan inhibited GTP gamma S-stimulation of phosphoinositide breakdown with an IC50 value of 3 microM. Such inhibitory activity of mastoparan also was present in membranes from cells pretreated with pertussis toxin. Calcium-induced stimulation of phosphoinositide breakdown was not significantly inhibited by mastoparan. The analogs mastoparan-X and polistes mastoparan had similar inhibitory activity, whereas the analog des-Ile1-Asn2-mastoparan was inactive. In permeabilized HL-60 cells mastoparan also inhibited phosphoinositide breakdown. Another amphiphilic peptide, melittin, was inactive in HL-60 intact cells, but similar to mastoparan, inhibited guanine nucleotide-induced phosphoinositide breakdown in HL-60 cell membranes and permeabilized cells. Thus, mastoparan peptides can stimulate phosphoinositide breakdown in intact HL-60 cells, probably through the interaction with a guanine nucleotide binding protein. In permeabilized cells and in cell membranes, mastoparan induces inhibition of guanine nucleotide-mediated phosphoinositide breakdown presumably through an interaction with an intracellular site. The inhibitory action of mastoparan and melittin is probably related to the amphiphilic character of these peptides.

The Food and Drug Administration's Factored Food Vocabulary (FFV) uses standardized language to describe characteristics of food products that are important for food safety and nutritional quality. Each food product is described by a set of descriptors from the following factors: product type; food source; part of plant or animal; physical state, shape, or form; degree of preparation; cooking method; treatment applied; preservation method; packing medium; container or wrapping; food contact surface; and user group. The purpose of the vocabulary is to facilitate retrieval of food composition, food consumption, food contamination, and other food-related data relative to these factors. The major advantages of this system are flexibility with specificity, enhanced searchability with economy, and ease of change and updating.
