Introduction Access to COVID-19-related care during the pandemic deserve attention and study to inform future strategies to deal with similar health emergencies in the future. We aimed to describe access to care for COVID-19-related reasons during the first 9 weeks of the pandemic in Portugal, to quantify the frequency of remote and in-person contacts with healthcare for COVID-19-related reasons and to assess the association between individual and context characteristics and contacts with health services.Methods We conducted an internet-based open cohort study with a non-probabilistic sample of 12 006 persons aged 16 years or more. One questionnaire was sent daily from 23 March 2020 for 9 weeks.Results General practitioners (GPs) were the main points of contact of patients with health services for COVID-19-related reasons. In our population, the main drivers found for any difference in the probability of contact with health services for COVID-19-related reasons were perceived high-risk contacts, followed by the existence of COVID-19 main symptoms. There were 17% more (0.17, 95% CI 0.15 to 0.18) contacts with health services among participants who reported personal contact with a confirmed case, 7% more (0.07, 95%CI 0.06 to 0.07) contacts if the contact had been with a suspected case and 6% more (0.06, 95%CI 0.05 to 0.07) contacts among participants who reported COVID-19 main symptoms in the previous 24 hours. Sociodemographic and household factors were not associated with major differences in healthcare contacts. The probability of contact with any health service for COVID-19-related reasons was highest at the beginning of the pandemic.Conclusion Most contacts with health services for COVID-19-related reasons during the first wave of the pandemic were with GPs, highlighting the role of first-contact care with these clinicians in Portuguese Primary Care, particularly in the context of the COVID-19 pandemic. Sociodemographic and household factors were not associated with major risk differences, suggesting that universal coverage and equity worked in our study sample at this pandemic stage.

Objective The objective of this study is to characterise the self-reported first contact with the health system and the reasons stated for each choice, testing associations with population characteristics.Design Cross-sectional survey.Setting Primary care department of a local health unit in northern Portugal.Participants Random sample of 4286 persons, retrieved from all registered adults.Outcomes Participants who stated they usually see the same doctor when a health problem arises were considered to adopt first-contact care and were asked to identify their regular doctor. Participants were asked why they adopt first-contact care or why they choose to do otherwise. Associations between personal characteristics and the adoption of first-contact care were tested using logistic regression.Results There were 808 valid questionnaires received (19% response rate). The mean age of respondents was 53 years, 58% were women and 60% had a high school or higher degree. Most (71%) stated always seeing the same doctor when facing a health problem. This was a general practitioner (GP) in 84%. The main reasons were previous knowledge and trust in the doctor. When this doctor was not a GP, the main reason was the need to obtain an appointment quickly. Participants who chose first-contact care were less likely to have university degrees than those who did not (OR 0.31; 95% CI 0.13 to 0.76). Being registered with the same GP for over 1 year increased the odds of adopting first-contact care: twice as likely for those registered for 1–4 years with the same GP (2.07; 95% CI 1.04 to 4.11), and three times more likely for those registered for over 10 years (3.21; 95% CI 1.70 to 6.08).Conclusions The high adoption of first-contact care and the reasons given for this suggest a strong belief in primary care in this population. The longer patients experience continuity, the more they adopt first-contact care. The preferences of higher-educated patients regarding first-contact care deserve reflection.

Abstract Background The Portuguese National Health System (NHS) provides universal coverage and near-free health care, but the population has high out-of-pocket expenses and unmet care needs. This suggests impaired accessibility, a key dimension of primary care. The COVID-19 pandemic has further affected access to health care. Understanding General Practitioners’ (GP) experiences during the pandemic is necessary to reconfigure post-pandemic service delivery and to plan for future emergencies. This study aimed to assess accessibility to GPs, from their perspective, evaluating determinants of accessibility during the second pandemic year in Portugal. Methods All GPs working in NHS Family Practices in continental Portugal were invited to participate in a survey in 2021. A structured online self-administered anonymous questionnaire was used. Accessibility was assessed through waiting times for consultations and remote contacts and provision of remote access. NHS standards were used to assess waiting times. Descriptive statistics were used to characterize the study sample. Associations between categorical variables were tested using the χ2 statistic and the Student t-test was used to compare means of continuous variables. Results A total of 420 GPs were included (7% of the target population). Median weekly working hours was 49.0 h (interquartile range 42.0–56.8), although only 14% reported a contracted weekly schedule over 40 h. Access to in-person consultations and remote contacts was reported by most GPs to occur within NHS time standards. Younger GPs more often reported waiting times over these standards. Most GPs considered that they do not have enough time for non-urgent consultations or for remote contacts with patients. Conclusions Most GPs reported compliance with standards for waiting times for most in-person consultations and remote contacts, but they do so at the expense of work overload. A persistent excess of regular and unpaid working hours by GPs needs confirmation. If unpaid overtime is necessary to meet the regular demands of work, then workload and specific allocated tasks warrant review. Future research should focus on younger GPs, as they seem vulnerable to restricted accessibility. GPs’ preferences for more in-person care than was feasible during the pandemic must be considered when planning for the post-pandemic reconfiguration of service delivery.

The Douro Demarcated Region (DDR) has peculiar edaphoclimatic characteristics that provide a suitable terroir for premium wine production. As climate change effects continue to emerge, ensuring productivity and quality becomes increasingly important for viticulturists, as those directly determine their profits. Cultural approaches, such as the use of biostimulants, are actively being developed to mitigate abiotic stress. The main objective of this work was to assess the effect of foliar sprays of a seaweed (Ascophyllum nodosum)-based extract (ANE) and glycine betaine (GB) on grape berry quality, bioactive compounds, and antioxidant activity. A trial was installed in a commercial vineyard (cv. ‘Touriga Franca’) in the Douro Superior (Upper Douro) sub-region of the Douro Demarcated Region. In 2020 and 2021, three foliar sprayings were performed during the growing season, namely at pea size, bunch closure, and veraison. There was a positive effect of both biostimulants (ANE and GB) on the physiological and biochemical performance of cv. ‘Touriga Franca’ exposed to summer stress. In general, the GB 0.2% spraying was the most promising treatment for this grape cultivar, as it increased berry quality, the concentration of bioactive compounds (total phenolics, flavonoids, and ortho-diphenols), and the antioxidant activity. These results revealed the efficacy of biostimulant sprayings as a sustainable viticultural practice, improving berry quality under summer stress conditions.

Atrial fibrillation is highly prevalent in patients with end -stage kidney disease. For patients with atrial fibrillation but normal kidney function, the net clinical benefit of oral anticoagulation with vitamin K antagonists and non -vitamin K oral anticoagulants in preventing stroke against the hemorrhagic risk is well -known. However, dialysis patients present unique risk factors that increase ischemic stroke and bleeding events that are not included in the traditional risk scores. These points, in addition to the pharmacokinetic issues related to the use of oral anticoagulants in end -stage kidney disease, raise the question of the safety of these drugs, making it difficult to predict whether the benefit outweighs the risk. Data on this topic in chronic kidney disease stages 4, 5, and 5D patients is sparse and the clinical guidelines are also inconsistent in which is the best approach for these patients. This review gives an overview of the state of the art on this topic, specifically the recommendations of the newest guidelines, then discusses the various risk scores and their pitfalls, and finally summarizes the current knowledge on the possible benefits of Non -vitamin K oral anticoagulants versus Vitamin K antagonists in dialysis patients.

BackgroundIn 2020, Portugal had high levels of unmet health care needs. Primary Care was reported as the main source of unmet needs.ObjectivesTo describe face-to-face and remote access to GPs in Portugal during the COVID-19 pandemic. To discover patient experiences and attitudes to access to care. To identify determinants of access to care.MethodsA survey of a random sample of 4,286 adults registered in a group of Family Practices was conducted in 2021. Paper questionnaires were sent by post to patients who had no e-mail address registered with the practice. Patients with an e-mail address were sent a link to an online questionnaire. Outcomes were reported waiting times for face-to-face and remote contacts with GPs, dichotomized to ascertain compliance with standards. Associations between participant characteristics and outcome variables were tested using logistic regression.ResultsWaiting times for face-to-face consultations with GPs during the pandemic often exceeded the maximum waiting times (MWT) set by the National Health Service. Remote contacts were mostly conducted within acceptable standards. Waiting times for speaking with the GP over the phone were rated as 'poor' by 40% and 27% reported requests for these calls as unmet. The odds of getting care over MWT increased for participants who reported poorer digital skills. Participants were less likely to get non-urgent consultations over MWT if they found it easy to use the online patient portal to book appointments (odds ratio 0.24; 99% confidence intervals 0.09-0.61), request prescriptions (0.18; 0.04-0.74) or insert personal data (0.18; 0.04-0.95).ConclusionPatient reported access to GPs during the pandemic was uneven in Portugal. Obtaining non-urgent consultations and remote contacts over MWT affected mainly those patients with poor digital skills. Telephone access to GPs received the worse ratings. Access through traditional pathways must remain available, to prevent the widening of inequities.

Abstract Background Hospital characteristics have been recognized as potential risk factors for surgical site infection for over 20 years. However, most research has focused on patient and procedural risk factors. Understanding how structural and process variables influence infection is vital to identify targets for effective interventions and to optimize healthcare services. The aim of this study was to systematically review the association between hospital characteristics and surgical site infection in colorectal surgery. Main body A systematic literature search was conducted using PubMed, Scopus and Web of Science databases until the 31st of May, 2021. The search strategy followed the Participants, Exposure/Intervention, Comparison, Outcomes and Study design. The primary outcome of interest was surgical site infection rate after colorectal surgery. Studies were grouped into nine risk factor typologies: hospital size, ownership affiliation, being an oncological hospital, safety-net burden, hospital volume, surgeon caseload, discharge destination and time since implementation of surveillance. The STROBE statement was used for evaluating the methodological quality. A total of 4703 records were identified, of which 172 were reviewed and 16 were included. Studies were published between 2008 and 2021, and referred to data collected between 1996 and 2016. Surgical site infection incidence ranged from 3.2 to 27.6%. Two out of five studies evaluating hospital size adjusted the analysis to patient and procedure-related risk factors, and showed that larger hospitals were either positively associated or had no association with SSI. Public hospitals did not present significantly different infection rates than private or non-profit ones. Medical school affiliation and higher safety-net burden were associated with higher surgical site infection (crude estimates), while oncological hospitals were associated with higher incidence independently of other variables. Hospital caseload showed mixed results, while surgeon caseload and surveillance time since implementation appear to be associated with fewer infections. Conclusions Although there are few studies addressing hospital-level factors on surgical site infection, surgeon experience and the implementation of a surveillance system appear to be associated with better outcomes. For hospitals and services to be efficiently optimized, more studies addressing these variables are needed that take into account the confounding effect of patient case mix.

Consumption of antimicrobials is an important driver of antimicrobial resistance. There is limited knowledge of the key determinants of antimicrobial prescribing behavior in hospitals. An understanding of these determinants is required for the successful design, adoption, and implementation of quality improvement interventions in Antimicrobial Stewardship Programs (ASP). This study aimed to describe the main factors that influence the doctor’s decision on antimicrobials prescribing and to identify the behaviors that drive physicians’ decision making. A structured web-based questionnaire focused on behavioral components of antimicrobial prescription was applied to the medical staff of three different departments—Internal Medicine, General Surgery, and Intensive Care Medicine—of a university hospital. All doctors agreed that inadequate use of antimicrobials increases AMR. A total of 77% of the surgeons and 100% of the internists and intensivists perceived antimicrobial prescription as a priority in the department. Full autonomy in antimicrobial prescription was preferred by internists (64%) but not by surgeons (18%) and intensivists (24%). Most physicians were keen to have ASP advice, but most did not want advice from colleagues of the same service. Almost all surgeons ask for advice when prescribing, but only 68% of the internists do it. Less than half of all physicians and only 25% of the surgeons felt free to prescribe contrary to guidelines. Most physicians, particularly in Intensive Care Medicine (94%), adopt the “wait and see” strategy when no microbiologic confirmation is available, but 27% of the surgeons start empirical therapy. In conclusion, the context of antimicrobial prescription, autonomy, and confidence in antimicrobial prescription demonstrated heterogeneity between the three departments and this should be considered when planning ASP.

Objectives: To assess whether electronic records data could improve the efficiency, exhaustiveness, and representativeness of SSI surveillance by selecting a group of high-risk patients for manual review. Methods: Colorectal surgeries (2016–2018) and cholecystectomies (2017–2018) were selected. Post-surgical antibiotic use, positive culture, C-reactive protein (CRP) values, body temperature, leukocyte count, surgical re-intervention, admission to the emergency room, and hospital readmission were retrieved. For representativeness, procedures registered in HAI-Net were compared with non-included procedures, and the validity of each variable (or combination) was tested considering the presence of SSI as the gold standard. The proportion of procedures flagged for manual review by each criterion was estimated. Results: Little more than 50% of procedures were included in HAI-Net (SSI risk: 10.6% for colorectal and 2.9% for cholecystectomies). Non-included procedures showed higher proportions of infection markers. Antibiotic use and CRP >100 mg/dl presented the highest sensitivity for both surgical groups, while antibiotic use achieved the highest positive predictive value in both groups (22% and 21%, respectively) and flagged fewer colorectal procedures (47.7%). Conclusions: Current SSI surveillance has major limitations. Thus, the reported incidence seems unreliable and underestimated. Antibiotic use appears to be the best criterion to select a sub-sample of procedures for manual review, improving the exhaustiveness and efficiency of the system.

Abstract Introduction Antimicrobial resistance is a major public health threat. Antimicrobial stewardship (AMS) is one of the key strategies to overcome resistance, but robust evidence on the effect of specific interventions is lacking. We report an interrupted time series (ITS) analysis of a persuasive AMS intervention implemented during a KPC producing Klebsiella pneumoniae outbreak. Methods A controlled ITS for carbapenem consumption, total antibiotic consumption and antibiotic-free days, between January 2012 and May 2018 was performed, using segmented regression analysis. The AMS intervention was implemented in the Vascular Surgery ward starting on April 2016 in the context of a KPC outbreak. The General Surgery ward was taken as a control group. Data were aggregated by month for both wards, including 51 pre-intervention and 26 intervention points. Results The AMS intervention produced a level change in carbapenem consumption of − 11.14 DDDs/100 patient-days accompanied by a decreasing trend of total antibiotic consumption and stable rate of antibiotic-free days in Vascular Surgery ward. These differences were not apparent in the control group. No differences in mortality or readmission rates between pre-intervention and intervention periods were noticed in any of the groups. Conclusion Persuasive AMS interventions on top of previously implemented restrictive interventions can reduce carbapenem consumption without increasing total antibiotic consumption. Starting persuasive AMS interventions in an outbreak setting does not compromise the sustainability of the intervention.

ABSTRACT Introduction: It has been suggested that cystatin C levels are modified by obesity and inflammation. Furthermore, cystatin C has been associated with cardiovascular events and mortality outcomes. Aim: To study the association of cystatin C with the metabolic profile and cardiovascular disease of peritoneal dialysis patients. Methods: Data collected included clinical, laboratorial, and multifrequency bioimpedance assessment of 52 stable peritoneal dialysis patients. Minimal residual renal function was defined as > 2mL/min/1.73m2. Results: Serum cystatin C was not significantly associated with peritoneal or urinary cystatin C excretion. Negative correlation of cystatin C with normalized protein catabolic rate (rho -0.33, p = 0.02) and a trend towards positive correlation with relative body fat (rho 0.27, p = 0.05) were not independent from residual renal function. Cystatin C was not significantly associated with cardiovascular disease (p = 0.28), nor with glycated hemoglobin (p = 0.19) or c-reactive protein (p = 0.56). In the multivariate model, both age and diabetes were the strongest predictors of cardiovascular disease (odds ratio 1.09, p = 0.029 and odds ratio 29.95, p = 0.016, respectively), while relative body fat was negatively associated with cardiovascular disease (p = 0.038); neither cystatin C (p = 0.096) nor minimal residual renal function (p = 0.756) reached a significant association with cardiovascular disease. Conclusions: In this group of peritoneal dialysis patients, cystatin C did not correlate with the metabolic or inflammatory status, nor cardiovascular disease, after adjustment for residual renal function.

Aim: Mortality in end-stage renal disease (ESRD) remains high, particularly among elderly, who represents the most rapidly growing segment of the ESRD population in wealthier countries. We developed and validated a risk score in elderly patients to predict 6-month mortality after dialysis initiation. Methods: We used data from a cohort of 421 patients, aged 65 years and over who started dialysis between 2009 and 2016, in our Nephrology department. The predictive score was developed using a multivariable logistic regression analysis. A bootstrapping technique was used for internal validation. Results: The overall mortality within 6 months was 14.0%. Introdução: A síndrome de polipose adenomatosa familiar (PAF), na sua forma clássica, é autossómica dominante. Os indivíduos afetados podem permanecer assintomáticos por vários anos, pelo que se deve conhecer a história familiar e manter um certo grau de suspeição clínica. Assim, este relato de caso pretende reforçar o papel do médico de família na abordagem desta patologia rara e realçar a necessidade de um trabalho em equipa e de cuidados de saúde perfeitamente coordenados, tendo em conta as dificuldades existentes no seu diagnóstico atempado. Descrição do caso: Utente com 20 anos observada pela sua médica de família por história de vómitos, dejeções diarreicas frequentes e perda ponderal de 7kg, com duas semanas de evolução. Objetivamente, com dor à palpação e sinal de blumberg presente na transição da região peri-umbilical para o hipocôndrio esquerdo. Através da entrevista clínica e realização do genograma familiar verificou-se a existência de um padrão de transmissão hereditária de patologia do trato gastrointestinal. Foi enviada para o serviço de urgência hospitalar, onde se excluiu a existência de patologia cirúrgica abdominal e/ou ginecológica, tendo tido alta referenciada para consulta de Medicina Interna. Após realização de colonoscopia total, ficou internada para resseção cirúrgica de adenocarcinoma do cólon sigmóide. Comentário: Este caso realça simultaneamente a importância do genograma familiar e a limitação temporal à sua aplicação pelo médico de família. Reforça, ainda, a importância da integração e coordenação de cuidados de saúde nesta patologia. O médico de família deverá conhecer as possíveis complicações da intervenção cirúrgica realizada, bem como outras patologias associadas à PAF e qual o tipo de acompanhamento que deve ser feito nestes utentes. Neste caso há, ainda, que ter em consideração a necessidade de rastreio de outros familiares afetados e a importância da consulta pré-concecional e/ou de diagnóstico pré-natal.

Strongyloides stercoralis is a nematode, endemic in tropical and subtropical areas. Strongyloidiasis has been reported in recipients of hematopoietic stem cells, kidney, liver, heart, intestine, and pancreas, eventually presenting as disseminated strongyloidiasis and hyperinfection syndrome (SHS) which is associated with high mortality. We report one case of a recent renal transplant recipient, who presented with gastrointestinal and respiratory symptoms, evolving into shock. The identification of Strongyloides stercoralis in the bronchoalveolar lavage (BAL) lead us to the diagnosis of SHS. Treatment with subcutaneous ivermectin was started, however the patient did not survive. Retrospective serum donor analysis allowed us to identify the donor as the source of infection. Abstract Background Antibiotic resistance (AMR) is a growing public health problem worldwide, in part related to inadequate antibiotic use. A better knowledge of physicians’ motivations, attitudes and practice about AMR and prescribing should enable the design and implementation of effective antibiotic stewardship programs (ASPs). The objective of the study was to assess attitudes and perceptions concerning AMR and use of antibiotics among surgeons who regularly perform emergency or trauma surgery. Methods A cross-sectional web-based survey was conducted contacting 4904 individuals belonging to a mailing list provided by the World Society of Emergency Surgery. Participation was voluntary and anonymous. The survey was open for 5 weeks (from May 3, 2017, to June 6, 2017), within which two reminders were sent. The self-administered questionnaire was developed by a multidisciplinary team; reliability and validity were assessed. Results The overall response rate was 12.5%. Almost all participants considered AMR an important worldwide problem, but 45.6% of them underrated the problem in their own hospitals. Surgeons provided with periodic reports on local AMR demonstrated a lower underrating in their hospital. Only 66.3% of the surgeons stated to receive periodic reports on local AMR data, and among them, 56.2% had consulted them to select an antibiotic in the previous month. Availability of systematic reports about AMR, availability of guidelines for therapy of infections, and advice from an infectious diseases specialist were considered very helpful measures to improve antibiotic prescribing by 68.0, 65.7, and 64.9%, respectively. Persuasive and restrictive ASPs were both considered helpful measures by 64.5%. Moreover, 86.3% considered locally developed guidelines more useful than national ones. Only 21.9% received formal training in antibiotic prescribing in the previous year; among them, 86.6% declared to be interested in receiving more training. Conclusions Availability of periodic reports on local AMR data was considered an important tool to guide surgeons in choosing the correct antibiotic and to increase awareness of the problem of AMR. Local guidelines for therapy of infections should be implemented in every emergency surgery setting, and developed by a multidisciplinary team directly involving surgeons, infectious diseases specialists, and microbiologists, and formally established in an ASP.

The deep-sea constitutes a true unexplored frontier and a potential source of innovative drug scaffolds. Here, we present the genome sequence of two novel marine actinobacterial strains, MA3_2.13 and S07_1.15, isolated from deep-sea samples (sediments and sponge) and collected at Madeira archipelago (NE Atlantic Ocean; Portugal). The de novo assembly of both genomes was achieved using a hybrid strategy that combines short-reads (Illumina) and long-reads (PacBio) sequencing data. Phylogenetic analyses showed that strain MA3_2.13 is a new species of the Streptomyces genus, whereas strain S07_1.15 is closely related to the type strain of Streptomyces xinghaiensis. In silico analysis revealed that the total length of predicted biosynthetic gene clusters (BGCs) accounted for a high percentage of the MA3_2.13 genome, with several potential new metabolites identified. Strain S07_1.15 had, with a few exceptions, a predicted metabolic profile similar to S. xinghaiensis. In this work, we implemented a straightforward approach for generating high-quality genomes of new bacterial isolates and analyse in silico their potential to produce novel NPs. The inclusion of these in silico dereplication steps allows to minimize the rediscovery rates of traditional natural products screening methodologies and expedite the drug discovery process.

As the world develops and population increases, so too does the demand for higher agricultural output with lower resources. Plant biostimulants appear to be one of the more prominent sustainable solutions, given their natural origin and their potential to substitute conventional methods in agriculture. Classified based on their source rather than constitution, biostimulants such as humic substances (HS), protein hydrolysates (PHs), seaweed extracts (SWE) and microorganisms have a proven potential in improving plant growth, increasing crop production and quality, as well as ameliorating stress effects. However, the multi-molecular nature and varying composition of commercially available biostimulants presents challenges when attempting to elucidate their underlying mechanisms. While most research has focused on the broad effects of biostimulants in crops, recent studies at the molecular level have started to unravel the pathways triggered by certain products at the cellular and gene level. Understanding the molecular influences involved could lead to further refinement of these treatments. This review comprises the most recent findings regarding the use of biostimulants in plants, with particular focus on reports of their molecular influence.

Calciphylaxis is a life threatening disease for which patients with chronic kidney disease, particular those on dialysis, are at increased risk. As it is a rare condition, treatment options are limited and duration of treatment is not clear, especially when it comes to patients on renal replacement treatment with peritoneal dialysis. We report a case of a 55 year-old woman with end stage renal disease on renal replacement treatment with peritoneal dialysis for five months that presented with severe calciphylaxis of both legs. She had started haemodialysis at the age of 31 and 3 later she received a cadaveric renal transplant that lasted for 20 years. She was treated with warfarin due to atrial fibrillation diagnosis. The treatment approach was multimodal and multidisciplinary and included treatment with sodium thiosulfate and hyperbaric oxygen, warfarin suspension and maintenance of optimized peritoneal dialysis prescription. Additionally, wound dressings were done every other day and antibiotics started whenever there were signs of infection and pain was managed with high doses of opiates and pain adjuvants. There was significant improvement and the patient kept peritoneal dialysis treatment with preserved residual renal function. This raises the question, if there is a need to transition calciphylaxis patients on peritoneal dialysis to haemodialysis, as it is possible to ensure a high efficiency dialysis program with automated peritoneal dialysis.

Abstract The occurrence of ascites after Renal Transplant (RT) is infrequent, and may be a consequence of surgical or medical complications. Case report: 61 year-old, male, history of arterial hypertension, tongue carcinoma and alcoholic habits 12-20g/day. He had chronic kidney disease secondary to autosomal dominant polycystic kidney disease, without hepatic polycystic disease. He underwent cadaver donor RT in September 2017. He had delayed graft function by surgically corrected renal artery stenosis. He was admitted in January 2018 for ascites de novo, with no response to diuretics. HE had visible abdominal collateral circulation. Graft dysfunction, adequate tacrolinemia, Innocent urinary sediment, mild anemia, without thrombocytopenia. Serum albumin 4.0g / dL. Normal hepatic biochemistry. Peritoneal fluid with transudate characteristics and serum albumin gradient > 1.1. Ultrasound showed hepatomegaly, permeable vascular axes, without splenomegaly. Mycophenolate mofetil was suspended, with reduced remaining immunosuppression. He maintained refractory ascites: excluded infectious, metabolic, autoimmune and neoplastic etiologies. No nephrotic proteinuria and no heart failure. MRI: micronodules compatible with bile cysts. Upper Digestive Tract Endoscopy did not show gastroesophageal varicose veins. Normal abdominal lymphoscintigraphy. He underwent exploratory laparoscopy with liver biopsy: incomplete septal cirrhosis of probable vascular etiology some dilated bile ducts. He maintained progressive RT dysfunction and restarted hemodialysis. The proposed direct measurement of portal pressure was delayed by ascites resolution. There was further recovery of the graft function. Discussion: Incomplete septal cirrhosis is an uncommon cause of non-cirrhotic portal hypertension. Its definition is not well known, morphological and pathophysiological. We have not found published cases of post-RT ascites secondary to this pathology, described as possibly associated with drugs, immune alterations, infections, hypercoagulability and genetic predisposition.

ABSTRACT Introduction: Chronic kidney disease (CKD) is an independent risk factor for several unfavorable outcomes including cardiovascular disease (CVD), particularly in the elderly, who represent the most rapidly growing segment of the end-stage kidney disease (ESKD) population. Portugal has the highest European unadjusted incidence and prevalence rates of ESKD. In 2012, we started to follow a cohort of elderly CKD patients, we describe their baseline characteristics, risk profile, and cardiovascular disease burden. Methods: All CKD patients aged 65 years and older referred to our department during 2012 were enrolled. Baseline data included: demographic, CKD stage, medication, comorbid conditions. Estimated glomerular filtration rate (eGFR) was calculated by the CKD-EPI formula. Results: A total of 416 patients, 50% referred by primary care physicians, aged 77 ± 7 years, 52% male, with a median eGFR of 32 mL/min/1.73m2 participated in the study. Fifty percent had diabetes (DM), 85% dyslipidemia, 96% hypertension; 26% were current/former smokers, and 24% had a body mass index > 30 kg/m2. The prevalence of CVD was 62% and higher in stage 4-5 patients; in diabetics, it gradually increased with CKD progression (stage 3a < stage 3b < stage 4-5) (39, 58, 82%; p < 0.001). Conclusions: At baseline, our CKD elderly cohort had a higher burden of CVD. The prevalence of CVD was greater than in other European CKD cohorts. Lower level of eGFR was associated with a greater burden of CVD and was more pronounced in diabetics, highlighting the importance of strategically targeting cardiovascular risk reduction in these patients.

Background: The COVID-19 pandemic poses novel challenges in antimicrobial consumption metrics and stewardship strategies. COVID-19 patients became the major cause of hospital admission during the first wave of the pandemic, often leading to an antimicrobial prescription upon admission or treatment for superinfections. The aim of this study was to understand how antimicrobial consumption was impacted at the beginning of the pandemic in a tertiary care hospital, a reference center for COVID-19. Materials and Methods: A retrospective before-and-after study was done. Descriptive statistics of discharges, patient-days, and antimicrobial use indicators (defined daily doses (DDD)/100 discharges, DDD/100 patient-days) for various groups were calculated for the first three months of the pandemic (March, April, and May 2020) as a quarterly value, and for each year in 2011–2019, and their annual percentage changes were used to estimate 95% confidence intervals. The indicators were compared to patient type (medical/surgical), type of admission (urgent/elective), and age groups using Spearman’s correlation coefficient. Results: Statistically significant increases occurred in 2020 for total antibacterials, macrolides, cephalosporins, amoxicillin/clavulanic acid, carbapenems, meropenem, and third-generation cephalosporins, while a reduction was seen in cefazolin/cefoxitin. A correlation was found between antibacterial consumption and patient or admission type. In 2020, unlike in pre-pandemic years, there was a different impact in DDD/100 discharges and DDD/100 patient-days due to increased lengths-of-stay and longer antimicrobial therapy. Conclusions: The COVID-19 pandemic led to an increase in antimicrobial consumption with a different impact in DDD/100 discharges and DDD/100 patient-days. This highlights the need to use both indicators simultaneously to better understand the causes of antimicrobial consumption variation and improve the design of effective antimicrobial stewardship interventions.

Cracking of sweet cherry (Prunus avium L.) fruits is caused by rain events close to harvest. This problem has occurred in most cherry growing regions with significant economic losses. Several orchard management practices have been applied to reduce the severity of this disorder, like the foliar application of minerals or growth regulators. In the present study, we hypothesized that preharvest spray treatments improve the physiological performance of sweet cherry trees and could also mitigate environmental stressful conditions. Effects of repeated foliar spraying of calcium (Ca), gibberellic acid (GA3), abscisic acid (ABA), salicylic acid (SA), glycine betaine (GB), and the biostimulant Ascophyllum nodosum (AN) on the physiological and biochemical performance of ‘Skeena’ sweet cherry trees during two consecutive years (without Ca in 2015 and in 2016 with addition of Ca) were studied. Results showed that in general spray treatments improved the physiological performance and water status of the trees. AN and ABA sprays were demonstrated to be the best compounds for increasing yield and reducing cherry cracking as well as improving photosynthetic performance and leaf metabolites content. In conclusion, AN and ABA might be promising tools in the fruit production system.

Sweet cherries are attractive fruits due to their taste, color, nutritional value, and beneficial health effects. Sweet cherry is a highly perishable fruit and all quality attributes and the level of health promoting compounds are affected by growth conditions, picking, packing, transport, and storage. During production, the correct combination of scion × rootstock will produce fruits with higher firmness, weight, sugars, vitamins, and phenolic compounds that boost the fruit antioxidant activity. Orchard management, such as applying drip irrigation and summer pruning, will increase fruit sugar levels and total phenolic content, while application of growth regulators can result in improved storability, increased red coloring, increased fruit size, and reduced cracking. Salicylic acid, oxalic acid, acetylsalicylic acid, and methyl salicylate are promising growth regulators as they also increase total phenolics, anthocyanins, and induce higher activity of antioxidant enzymes. These growth regulators are now also applied as fruit coatings that improve shelf-life with higher antioxidant enzyme activities and total phenolics. Optimizing storage and transport conditions, such as hydro cooling with added CaCl2, chain temperature and relative humidity control, are crucial for slowing down decay of quality attributes and increasing the antioxidant capacity. Application of controlled atmosphere during storage is successful in delaying quality attributes, but lowers ascorbic acid levels. The combination of low temperature storage in combination with modified atmosphere packaging (MAP) is successful in reducing the incidence of fruit decay, while preserving taste attributes and stem color with a higher antioxidant capacity. A new trend in MAP is the use of biodegradable films such as micro-perforated polylactic acid film that combine significant retention of quality attributes, high consumer acceptability, and a reduced environmental footprint. Another trend is to replace MAP with fruit edible coatings. Edible coatings, such as various lipid composite coatings, have advantages in retaining quality attributes and increasing the antioxidant activity (chitosan) and are regarded as approved food additives, although studies regarding consumer acceptance are needed. The recent publication of the sweet cherry genome will likely increase the identification of more candidate genes involved in growing and maintaining health related compounds and quality attributes.

Introdução: A enurese noturna (EN) é um problema comum em idade pediátrica, atingindo cerca de 15% das crianças entre os cinco e os seis anos de idade. Pela sua elevada prevalência, várias modalidades terapêuticas têm surgido no sentido de atenuar o seu impacto, muitas vezes estigmatizante para a criança e família. Entre as opções disponíveis para o seu tratamento sobressai o alarme noturno (AN), tratamento comportamental com efeito condicionado a nível do sono. Objetivo: Rever a evidência científica existente quanto à eficácia do alarme noturno como opção terapêutica no tratamento da enurese noturna. Metodologia: Foi efetuada uma pesquisa bibliográfica nas principais bases de dados internacionais, utilizando os termos MeSH: Nocturnal enuresis AND Clinical alarms. Foi igualmente consultado o Índex RMP usando os termos de pesquisa (DeCS) “Enurese noturna” e “Alarme”. Como critérios de inclusão foram selecionados: meta-análises, revisões sistemáticas, ensaios clínicos aleatorizados, estudos observacionais e orientações clínicas, tendo sido incluídos artigos em língua inglesa e portuguesa, de janeiro de 2005 a janeiro de 2015. Para atribuição do nível de evidência (NE) foi utilizada a escala Oxford Centre for Evidence-Based Medicine 2011. Resultados: Depois de analisados os 52 artigos resultantes da pesquisa, foram incluídos nove artigos: três revisões sistemáticas, três estudos originais e três normas de orientação clínica. Na maioria da literatura analisada foi identificado benefício acrescido do AN quando comparado com as restantes terapêuticas não farmacológicas. Nos estudos que estabeleceram comparação com a desmopressina, o alarme noturno mostrou resultados equiparáveis, excetuando-se menor risco de recaída com o AN e melhor res-posta na EN com capacidade vesical noturna diminuída. Como aspetos menos positivos enumeram-se os resultados mais tardios do AN, a necessidade de colaboração por parte dos pais e o risco de menor adesão ao tratamento. No que respeita às orientações clínicas elaboradas por entidades reconhecidas, as recomendações são concordantes com os resultados dos restantes estudos. Discussão/Conclusão: Considerando os resultados promissores do alarme noturno (Nível de evidência 1, Força de recomendação A), importa que o médico de família considere a sua potencial indicação nos casos de EN resistentes às medidas iniciais, evitando-se a necessidade de intervenção farmacológica e/ou referenciação hospitalar precoce.

As we approach the third decade since the WHO started addressing the eradication of poliomyelitis and leprosy, a reflection of the previous campaigns efficacy and an evaluation of further elimination feasibility is important to adapt and intensify the next steps. We performed a critical review of the poliomyelitis and leprosy eradication campaigns to evaluate their technical and operational feasibilities. Vaccination and active case search are highly effective tools against poliomyelitis. If political stability and good vaccination coverage is achieved, poliomyelitis will be an easy target for eradication. Leprosy, on the other hand, faces many barriers towards elimination. The lack of a high efficacy vaccine, the long asymptomatic but infective period, the lack of screening tests and a poorly established elimination target, prevents this disease from being eliminated. In a world where resources and funding are limited, it is apparent that poliomyelitis is a more feasible target for elimination than leprosy.

In recent years the consumption of small fruits has increased continuously and knowledge about the more suitable production requirements is essential. This study aims to evaluate the influence of harvest year and altitude on chemical composition of four blueberry cultivars, in order to create rentable opportunities for producers and minimize eventual losses in quality fruits. Dry mass, protein, fat, energy, free sugars, organic acids, and vitamin C contents were determined using HPLC-UV-DAD and spectrophotometric methods. Differences (p0.05) influence. Citric acid was the main organic acid and fructose the most abundant sugar in blueberries. Fruits of “Ozarkblue” had the highest mass and volume whilst the “Bluecrop” was the cultivar with highest crude protein and fat contents. “Goldtraube” showed the highest content of sucrose and organic acids and “Duke” had the highest content of fructose.

Objectives: To assess the yield of medical record review to recover missing data originally collected by questionnaire, to analyze the agreement between these two data sources and to determine interobserver variability in clinical record review. Methods: We analyzed data from a birth cohort of 8,127 women who were consecutively recruited after giving birth from 2005-2006. Recruitment was conducted at all public maternity units of Porto, Portugal. We reviewed the medical records of 3,657 women with missing data in the baseline questionnaire and assessed agreement between these two sources by using information from participants with data from both sources. Interobserver variability was assessed by using 400 randomly selected clinical records. Results: Data on pregnancy complications and maternal anthropometric parameters were successfully recovered. Agreement between the questionnaire and records in family history data was fair, particularly for cardiovascular disease [k=0.27; 95% confidence interval (95%CI): 0.23-0.32]. The highest agreement was observed for personal history of diabetes (k=0.82; 95%CI 0.70-0.93), while agreement for hypertension was moderate (k=0.60; 95%CI 0.50-0.69). Discrepancies in prepregnancy body mass index classes were observed in 10.3% women. Data were highly consistent between the two reviewers, with the highest agreement found for gestational diabetes (k=1.00) and birth weight (99.5% concordance). Conclusion: Data from the medical records and questionnaire were concordant with regard to pregnancy and well-known risk factors. The low interobserver variability did not threaten the precision of our data.Objetivo: Evaluar el rendimiento de la revisión de registros médicos para completar datos originalmente recogidos por cuestionario, y analizar la concordancia entre ambas fuentes de datos y la variabilidad interobservador en la revisión de registros médicos. Métodos: Cohorte de nacimiento con 8.127 mujeres reclutadas de forma consecutiva después del parto en todas las maternidades públicas de Porto, Portugal (2005-2006). Se revisaron los registros médicos de 3.657 mujeres con datos incompletos en el cuestionario inicial, y se evaluó la concordancia entre ambas fuentes. La variabilidad interobservador se evaluó en 400 historias clínicas seleccionadas aleatoriamente. Resultados: La información sobre complicaciones patológicas del embarazo y la antropometría de las madres se recuperó con éxito. La concordancia entre el cuestionario y los registros con respecto a los antecedentes familiares era débil, especialmente para las enfermedades cardiovasculares (k=0,27, intervalo de confianza del 95% [IC95%]: 0,23-0,32). La concordancia máxima se observó en los antecedentes personales de diabetes (k=0,82, IC95%: 0,70-0,93), mientras que para la hipertensión fue moderada (k=0,60, IC95%: 0,50-0,69). Se observaron discrepancias en las categorías de índice de masa corporal antes del embarazo en el 10,3% de las mujeres. Los datos fueron muy concordantes entre los revisores, con el máximo nivel de concordancia para la diabetes gestacional (k=1,00), seguida del peso al nacer (99,5% concordantes). Conclusión: Los registros médicos y la información del cuestionario fueron concordantes para los datos relacionados con el embarazo y los factores de riesgo conocidos. La baja variabilidad interobservador no pone en peligro la precisión de los datos.

This study aimed to quantify the association between adequacy of prenatal care and prevalence of folic acid, iron, and multivitamin intake during pregnancy. Data were obtained on socio-demographics, prenatal care, pregnancy complications, and use of vitamin/mineral supplements for 836 women, using a postpartum interview. Associations with the use of vitamin/mineral supplements were quantified with risk ratios (RR), computed by generalized binomial regression. A high proportion of women reported the use of folic acid (81.9%), iron (55.4%), and multivitamins (76.2%) as supplements during pregnancy. Use of supplements was independently associated with adequacy of prenatal care (adequate vs. inadequate: folic acid, RR = 2.28; 95%CI: 1.58-3.29; iron, RR = 1.99; 95%CI: 1.57-2.52, multivitamins, RR = 1.97; 95%CI: 1.54-2.51). Higher schooling was also associated with increased use of folic acid (RR = 1.42; 95%CI: 1.18-1.70), but not multivitamins (RR = 0.87; 95%CI: 0.77-0.98). Use of folic acid was less prevalent in single women (RR = 0.67; 95%CI: 0.48-0.95) and during unplanned pregnancies (RR = 0.81; 95%CI: 0.71-0.92). Adequacy of prenatal care is a major determinant of vitamin/mineral intake during pregnancy.Quantificar a associação entre a adequação dos cuidados pré-natais e a prevalência de utilização de ácido fólico, ferro e vitaminas durante a gravidez. Após o parto, 836 mulheres foram questionadas relativamente a características sócio-demográficas, utilização dos cuidados pré-natais, complicações durante a gravidez e utilização de suplementos vitamínicos/minerais. A associação entre as variáveis foi quantificada por meio de riscos relativos (RR) calculados por regressão binomial generalizada. Uma elevada proporção de mulheres reportou ter tomado ácido fólico (81,9%), ferro (55,4%) e multivitaminas (76,2%) durante a gravidez. A utilização de suplementos esteve independentemente associada à adequação dos cuidados pré-natais (adequado vs. inadequado: ácido fólico, RR = 2,28; IC95%: 1,58-3,29; ferro, RR = 1,99; IC95%: 1,57-2,52; multivitaminas, RR = 1,97; IC95%: 1,54-2,51). O elevado nível de escolaridade associou-se ao uso de ácido fólico (RR = 1,42; IC95%: 1,18-1,70), mas não de multivitaminas (RR = 0,87; IC95%: 0,77-0,98). A utilização de ácido fólico foi menos prevalente em mulheres que viviam sozinhas (RR = 0,67; IC95%: 0,48-0,95) e cuja gravidez não foi planeada (RR = 0,81; IC95%: 0,71-0,92). A adequação dos cuidados pré-natais é um determinante importante da utilização de vitaminas/minerais durante a gravidez.

Objetivos: Avaliar a demonstração prática e a descrição oral da utilização do inalador pressurizado e inalador em pó seco de médicos e profissionais de farmácia. Tipo de estudo: Observacional, multicêntrico, analítico, transversal. Local: Centros de saúde, serviços de pediatria e medicina interna e farmácias de quatro ilhas dos Açores. População: Médicos e profissionais de farmácia. Métodos: Nos médicos foi efetuado um censo da população, enquanto nos profissionais de farmácia foi feita uma amostra de conveniência. Os profissionais simularam o uso de dois dispositivos inalatórios e os autores verificaram o cumprimento de cada etapa numa checklist. Para a análise dos dados foi utilizada a regressão logística binária. Resultados: Participaram no estudo 45 profissionais de farmácia, 91 médicos de família, 26 internistas e 19 pediatras com idade média de 40,8 anos (±12,2), 66,9% (121) eram mulheres. Cinco (2,8%) demonstraram corretamente a técnica inalatória de ambos os dispositivos. As etapas em que mais erraram foram: a expiração máxima antes da inalação, suster a respiração pelo menos cinco segundos em ambos os inaladores, lavar a boca com água e agitar o inalador pressurizado antes de usar. No inalador pressurizado, um conhecimento pobre da técnica inalatória está relacionado com o facto de ser profissional de farmácia (OR=4,94), idade entre 51 e 65 anos (OR=5,95) e explicar “às vezes, nem sempre ou nunca” a técnica (OR=3,63). Relativamente ao inalador pó seco observou-se que ter um conhecimento pobre da técnica inalatória está associado com o facto de ser profissional de farmácia (OR=3,51), internista (OR=6,55), idade entre 36 e 50 anos (OR=3,68), 51 e 65 anos (OR=11,44) e exercer na ilha do Faial (OR=15,98). Conclusões: A maioria dos profissionais de saúde falha na explicação de várias etapas essenciais para o uso correto dos dispositivos inalatórios.

Objectivos: Determinar a efectividade e segurança da administração da vacina anti-pneumocócia polissacárida em idosos. Fontes de Dados: National Guideline Clearing House, Canadian Medical Association, Cochrane Library, TRIP database, Pubmed, Bandolier, Dare e Índex de Revistas Médicas Portuguesas. Métodos de revisão: Pesquisa de normas de orientação clínicas, metanálises, revisões sistemáticas e ensaios clínicos aleatorizados e controlados publicados entre Maio de 2002 e Julho de 2012 nas línguas inglesa, portuguesa e espanhola. Foram utilizados os termos MeSH pneumococcal vaccines e aged. Na avaliação dos estudos e atribuição dos níveis de evidência e forças de recomendação foi utilizada a escala Strength of Recommendation Taxonomy (SORT) da American Family Physician. Resultados: Foram encontrados 517 artigos. Destes, apenas 12 preencheram os critérios de inclusão, dos quais cinco normas de orientação clínica, três metanálises, três revisões sistemáticas e um ensaio clínico controlado e aleatorizado. Apesar da segurança da vacina, não existe consenso sobre a efectividade da mesma nos idosos. Os estudos observacionais revelaram uma efectividade de 55% a 59% na prevenção da doença pneumocócica invasiva, ao contrário dos ensaios clínicos aleatorizados e controlados incluídos nas metanálises e revisões sistemáticas avaliadas. Conclusões: Perante a evidência disponível não foi possível determinar a efectividade da vacina anti-pneumocócica no idoso (Força de Recomendação B). São necessários ensaios clínicos controlados, aleatorizados, de elevada qualidade, de grandes dimensões, cuja amostra seja representativa da população de idosos para quem a vacina é recomendada, para obter conclusões firmes sobre a efectividade da mesma.

OBJECTIVE: To evaluate how the country of origin affects the probability of being delivered by cesarean section when giving birth at public Portuguese hospitals. STUDY DESIGN: Women delivered of a singleton birth (n = 8228), recruited from five public level III maternities (April 2005-August 2006) during the procedure of assembling a birth cohort, were classified according to the country of origin and her migration status as Portuguese (n = 7908), non-Portuguese European (n = 84), African (n = 77) and Brazilian (n = 159). A Poisson model was used to evaluate the association between country of birth and cesarean section that was measured by adjusted prevalence ratio (PR) and respective 95% confidence intervals (95%CI). RESULTS: The cesarean section rate varied from 32.1% in non-Portuguese European to 48.4% in Brazilian women (p = 0.008). After adjustment for potential confounders and compared to Portuguese women as a reference, Brazilian women presented significantly higher prevalence of cesarean section (PR = 1.26; 95%CI: 1.08-1.47). The effect was more evident among multiparous women (PR = 1.39; 95%CI: 1.12-1.73) and it was observed when cesarean section was performed either before labor (PR = 1.43; 95%CI: 0.99-2.06) or during labor (PR = 1.30; 95%CI: 1.07-1.58). CONCLUSIONS: The rate of cesarean section was significantly higher among Brazilian women and it was independent of the presence of any known risk factors or usual clinical indications, suggesting that cultural background influences the mode of delivery overcoming the expected standard of care and outcomes in public health services.

BACKGROUND: Secondary bone marrow (BM) myelodysplastic syndromes (MDS) are increasingly common, as a result of radio or chemotherapy administered to a majority of cancer patients. Patients with secondary MDS have increased BM cell apoptosis, which results in BM dysfunction (cytopenias), and an increased risk of developing fatal acute leukemias. In the present study we asked whether TNF-alpha, known to regulate cell apoptosis, could modulate the onset of secondary MDS. PRINCIPAL FINDINGS: We show that TNF-alpha is induced by irradiation and regulates BM cells apoptosis in vitro and in vivo. In contrast to irradiated wild type (WT) mice, TNF-alpha deficient (TNF-alpha KO) mice or WT mice treated with a TNF-alpha-neutralizing antibody were partially protected from the apoptotic effects of irradiation. Next we established a 3-cycle irradiation protocol, in which mice were sub-lethally irradiated once monthly over a 3 month period. In this model, irradiated WT mice presented loss of microsatellite markers on BM cells, low white blood cell (WBC) counts, reduced megakaryocyte (MK) and platelet levels (thrombocytopenia) and macrocytic anemia, phenoypes that suggest the irradiation protocol resulted in BM dysfunction with clinical features of MDS. In contrast, TNF-alpha KO mice were protected from the irradiation effects: BM cell apoptosis following irradiation was significantly reduced, concomitant with sustained BM MK numbers and absence of other cytopenias. Moreover, irradiated WT mice with long term (> or = 5 months) BM dysfunction had increased BM angiogenesis, MMPs and VEGF and NFkB p65, suggestive of disease progression. CONCLUSION: Taken together, our data shows that TNF-alpha induction following irradiation modulates BM cell apoptosis and is a crucial event in BM dysfunction, secondary MDS onset and progression.

Several studies have stressed the outcomes for families who are supported by Early Intervention in the following dimensions: knowledge/information; development of competencies; well-being; and quality of life. As a result, this study assessed 999 families to examine the outcomes of Early Intervention in Portugal. Our results show that families supported by Early Intervention benefit in dimensions related to the development of competencies in the child and the family, and to the gain of information and knowledge about support and community resources, which results in perceived control and co-responsibility. The analysis of the outcomes of Early Intervention also shows that parents' education and occupation levels are determining variables for a family to benefit from the positive impact of Early Intervention. In light of such results, we present recommendations for practices in the area of Early Intervention with the aim of adapting the practices of professionals to the concerns and priorities of Portuguese families.
