Aim: We are reporting a case of an unusual presentation of granulomatosis with polyangiitis (GPA) with liver involvement. Case presentation: A 45-year-old male patient presented with erythematous plaques on the face and bilateral nasal obstruction. On physical examination, the patient had a ring-shaped squamous plaque on the face. The laboratory findings revealed an accelerated erythrocyte sedimentation rate at 100 mm/h, an elevated C-reactive protein at 66 mg/L, hyper gamma globulinemia 16 g/L and an elevated alkaline phosphatase (twice the upper normal limit). The craniofacial and thoracoabdominal computed tomography (CT) -scans showed ethmoid and maxillary sinusitis, low facial bone density, multiple mediastinal and hilar lymphadenopathy, diffuse small pulmonary nodules, and hepatomegaly. A cutaneous lesion biopsy, the nasal mucosa, and the liver showed a chronic inflammatory granulomatosis process with necrosis. Serum anti-neutrophil cytoplasmic antibody (ANCA) against PR3 was positive. The clinical, biological, radiological, and histological findings substantiated the diagnosis of GPA. The patient received systemic steroids combined with cyclophosphamide pulses on days 1, 14 and 28 and then he was lost to follow-up. Two-years later, he presented with a cardiac failure and skin ulcer in the right lower limb. A nasal endoscopic exam showed nasal septum cartilage perforation with resorption of the middle and inferior nasal concha. Two weeks later, he developed a diffuse alveolar hemorrhage and was therefore transferred to the intensive care unit but died of respiratory failure 3 days later. Conclusion: Clinicians should be aware of GPA atypical clinical manifestations.

The Drug Rash with Eosinophilia and Systemic Symptoms (DRESS) syndrome can be potentially life-threatening. The diagnosis is sometimes difficult since the clinical manifestations may be incomplete or non-specific. Insulinoma is a rare functioning neuroendocrine tumor (NET) of the pancreas. Medical therapy may be needed when surgery is contraindicated, delayed or refused. Diazoxide is widely used to control hypoglycemia in patients with insulinoma. We report a clinical case of an insulinoma in a 85-year-old patient treated with diazoxide with a fatal outcome due to a delayed diagnosis of a DRESS syndrome. This is the first case of DRESS syndrome reported after using diazoxide for insulinoma treatment in our knowledge.

The purpose of this research article was to determine the contribution of performing laparoscopy first in the etiological diagnosis of isolated exudative ascites. Our retrospective descriptive study had been performed over 15 years and included 46 patients who had undergone exploratory laparoscopy for exudative ascites. The average age of the patients was 52 years. Biological and morphological examinations contributed to making the etiological diagnosis. Therefore, diagnostic laparoscopy was indicated. Peritoneal carcinomatosis and tuberculosis were the most common causes seen in 34.2% and respectively 65.8% of cases. Visual laparoscopic diagnosis was peritoneal carcinomatosis in 16 cases and peritoneal tuberculosis in 28 cases, indecisive in one case and biliary ascites in one case. The histological diagnosis was peritoneal tuberculosis in 24 cases and peritoneal carcinomatosis in 12 cases and there were 9 cases of rare diagnoses. Peritoneal biopsies were negative in 2 cases for which the etiological diagnoses were kidney failure in one case and hypothyroidism in the other case. The postoperative recovery was simple in 42 cases (91.3%). The complications were: an accidental rupture of the left diaphragmatic dome in one case (2.1%), gastrointestinal bleeding in the immediate postoperative period in two cases (4.2%) and infected ascites in one case (2.1%). The post-operative scapular pain was reported in 4.2% of our patients. Operative mortality in our patients was nil. The overall care had an average cost of 1685.7 dinars for each of our patients, with extremes ranging from 1055.7 to 3605.7 dinars. For patients with isolated exudative ascites that should have been explored through laparoscopy first, the average cost would be 130.1 dinars, with a material gain for each of 1555.6 dinars. This attitude could also allow a reduction in the mean hospital stay (which was 27 days to few days) associated with early treatment. Our study confirms the interest in laparoscopy first with peritoneal biopsies as part of the etiological diagnosis of isolated exudative ascites.

The small bowel is the least common site for diverticula in the entire gastro-intestinal tract. Chronic upper intestinal obstruction due to diverticula is very rare. We report a case of multiple small bowel diverticula causing mechanical obstruction of the duodenojejunal flexure.
