Aim: We conducted a retrospective study with the aim of determining the prevalence of lipoprotein lipase (LPL) mutation in patients with severe hypertriglyceridemia (HTG) and to study differences in characteristic features of HTG induced acute pancreatitis (AP). Materials and Methods: Seventy adults with a serum triglyceride (TG) level ?500 mg/dL were included in the study. Baseline characteristics, LPL mutation and risk factors between those with and without HTG-AP were compared. Results: The mean age was 43 ± 12 years, and males accounted for 55.7%. Amaç: Varis dışı üst gastrointestinal sistem (VDÜGİS) kanama tanısı ile takip edilen olgularımızın özelliklerinin değerlendirilmesi amaçlanmıştır. Yöntem: Tepecik Eğitim ve Araştırma Hastanesi, Acil Tıp Kliniğine 01.08.2013 ile 01.08.2014 tarihleri arasında başvuran ve İç Hastalıkları ile Gastroenteroloji Kliniğinde yatarak izlenen VDÜGİSK 141 hasta demografik özellikleri, nedensellik ilişkileri, risk faktörleri açısından ele alınmıştır. Bulgular: Doksan yedisi (%68,8) erkek, 44’ü (%31,2) kadın olan 141 olgunun yaş ortalaması 63,2+17,3 yıl (20-103) idi. Olguların 36’sında (%25,5) sigara kullanımı, 21’inde (%14.9) alkol alımı, 39’unda (%27,7) aspirin, 51’inde (%36,2) nonsteroid anti-inflamatuvar ilaç (NSAİİ), 13’ü (%9,2) klopidogrel, 13’ü (%9,2) warfarin, 3’ü (%2,1) yeni nesil oral antikoagulan ve 8’inde (%5,7) proton pompa inhibitörü (PPİ) alımı mevcut idi. Eşlik eden komorbidite sıralamasında hipertansiyon (HT) ilk sırayı 55 (%39,0) olgu ile alırken, koroner arter hastalığı (KAH) 33 (%23,4) olguda, diyabetes mellitus (DM) 27 (%19,1) olguda, serebrovasküler hastalık (SVH) hikayesi 16 (%11,3) olguda, kronik böbrek yetersizliği (KBY) 13 (%9,2) olguda, maliğnite 13 (%9,2) olguda belirlendi. Yapılan üst gis endoskopi sonucunda görülen kanama nedenlerinde duodenum ülseri (%45) ilk sırayı alırken, sırası ile diğerleri mide ülseri (%25) ve antral gastrit (%19) olarak raporlandı. Sonuç: Ülkemizde endoskopi ile raporlanan, VDÜGİSK’nin ilk sıra görülme nedeninin duodenal ülser olduğu belirlenmiştir. Kanayan olgularda yaşın ileri olması, komorbidite ve kullanılan ilaçlar (NSAİİ vs) ölüm oranlarınds artış olarak yansımaktadır., Objective: To assess the characteristics of patients with upper non-variceal gastrointestinal system bleeding (NVUGISB). Methods: Hundred and forty-one inpatients with NVUGISB who were referred to the Tepecik Research and Training Hospital, between 08.01.2013 ile 08.01.2014 and examined in the Gastroenterolgy and Internal Medicine Department were assessed regarding their demographic characteristics, causal relationship and risk factors Results: the mean age of the 141 patients was 63.2±17.3 (20-103) years. the study population consisted of 97 (68.8%) male and 44 (31.2%) female patients. Thirty-six (25.5%) patients were smokers, and 21 (14.9%) of them used alcohol. They had also used acetyl salicylic acid (ASA) (n=39: 27.7%), non-steroidal anti-inflammatory drugs (NSAIDs) (n=51: 36.2%) clopidogrel (n=13: 9.2%), warfarin (n=13: 9.2%), new generation oral anticoagulants (n=3: 2.1%), and proton pump inhibitors (n=8: 5.7%). Comorbid diseases of the patients were as follows; hypertension (n=55: 39%), coronary artery disease (n=33: 23.4%), diabetes mellitus (n=27: 19.1%), chronic renal failure cerebrovascular accident history (n=16: 11.3%), and malignancy (n=13: 9.2%) were determined. the first three most common causes of bleeding detected by endoscopy were duodenal ulcer (45%), gastric ulcer (25%) and antral gastritis (19%). Conclusion: Duodenal ulcer was found as the most common cause of NVUGISB in this study. Bleeding is seen in the elderly more commonly and advanced age, co-morbid diseases and drugs used (NSAID etc) increase the mortality.

Giriş ve Amaç: Bülent Ecevit Üniversitesi Tıp Fakültesi Gastorenteroloji Kliniğinde tedavi edilen kronik hepatit B olgularımızı retrospektif olarak değerlendirerek uygulanan ajanların; antiviral etkinliklerinin ve ilaç yan etkilerinin araştırılması planlanmıştır. Gereç ve Yöntem: Çalışmamızda 2001-2010 yılları arasında gastroenteroloji kliniğinde tedavi edilen 241 hastanın; tedaviye yanıt ve direnç oranları ile ilaç yan etkileri incelen - di. Bulgular: Karakteristik özellikler açısından aralarında fark olmayan; entekavir kullanan 35 ve tenofovir kullanan 20 naiv hastanın, bir yıllık tedavi sonunda antiviral etkinlikleri benzer saptandı. Lamivudin direnci saptanan 2 hastada tenofovir monoterapisiyle HBV DNA saptanabilir düzeyin altına indi. İnterferon ve lamivudin ikili tedavi sonrası tenofovir monoterapisine geçilen 14 hastada HBV DNA’nın saptanabilir düzeyin altına inmesi oranı sırasıyla ilk yılda %81, 2. yılda %85’ti. Lamivudin son - rası, adefovir tedavisine geçilen ancak HBV DNA düzeyi 1 yıllık tedavi ile 300 kopya/ml’nin üzerinde olan 5 hastada tenofovir monoterapisine geçildi. 12 ve 24 aylık tedavi sonunda bu hastaların tamamında HBV DNA saptanabilir düzeyin altına indi. Sonuç: Entekavir ve tenofovir; naiv hastalarda antiviral etkinlik, yan etki ve komplians bakımından benzer özelliktedir. Tenofovir, lamivudin dirençli hastalarda yüksek etkinlikte olup ilk seçenek ilaçtır. Lamivudine direnç varlığı tenofovir cevabını et- kilememektedir. Tenofovir monoterapisi; adefovire yanıtsız, suboptimal yanıtlı hastalarda dahi yüksek etkinliktedir., Background and Aims: In this study, patients with chronic hepatitis B, treated in Bülent Ecevit University School of Medicine, Department of Gastroenterology, were evaluated retrospectively with respect to anti - viral efficacy and adverse effects. Materials and Methods: Treatment responses, resistance rates and side effects of 241 patients treated be - tween 2001 and 2010 in the gastroenterology clinic were examined retrospectively. Results: There were no statistically significant differ- ences between 35 naive patients treated with entecavir and 20 naive patients treated with tenofovir regarding demographic characteristics. After a one-year period of treatment, similar antiviral activity was de - tected between these two groups. Two patients who had lamivudine resistance were switched to tenofovir, and their HBV DNA level be - came undetectable. For 14 patients with tenofovir treatment, which followed interferon and lamivudine combination treatment, the ratio of undetectable levels of HBV DNA was 81% at the first year and 85% at the second year. Five patients, treated with adefovir after lamivudine treatment and whose HBV DNA levels reached more than 300 cop - ies/ml following one year with adefovir monotherapy, were switched to tenofovir monotherapy. Following 12 and 24 months’ treatment, HBV DNA levels became undetectable in all five patients. Conclusions: Entecavir and tenofovir are similar in terms of antiviral activity, side effects and compliance in naive patients. Tenofovir is highly effective in lamivudine-resistant patients and should be the first choice in these patients. Lamivudine resistance does not affect the tenofovir treatment response. Tenofovir monotherapy is highly active in the adefovir- refrac - tory or suboptimal response patients.
