Purpose: We aimed to compare the retinal nerve fiber layer, ganglion cell layer, inner plexiform layer, and the choroid thickness between patients with Behcet disease and healthy subjects by using spectral domain optical coherence tomography (SD-OCT). Methods: Ninety eyes of 45 healthy subjects and 104 eyes of 52 patients with Behcet disease were included in this study. Rheumatoid factor and C-reactive protein levels were measured by blood testing in the patients. Results: The mean thickness of the retinal nerve fiber layer, ganglion cells layer, and inner plexiform layer were significantly lower in patients with Behcet's disease than in the healthy subjects. The mean choroidal thickness was significantly higher in the patients than in the healthy subjects. Conclusions: SD-OCT was a useful and non-invasive tool for the detection of retinal nerve degeneration and choroidal changes in patients with Behcet disease even in the absence of ocular involvement. RESUMO Objetivo: Comparar a camada de fibras nervosas da retina, a camada de células ganglionares, a camada plexiforme interna e a espessura coróide entre os pacientes com doença de Behçet e indivíduos saudáveis usando tomografia de coerência óptica (OCT) de domínio espectral (SD). Métodos: Noventa olhos de 45 indivíduos saudáveis e 104 olhos de 52 pacientes com doença de Behcet foram incluídos no estudo. O fator reumatoide e os níveis de proteína C-reativa foram medidos por exames de sangue em pacientes com doença de Behcet. Resultados: As médias de espessura da camada de fibras nervosas da retina, da camada de células ganglionares e da camada plexiforme interna dos pacientes com Doença de Behcet foram significativamente menores do que o grupo controle. As medidas de espessura coróide média dos pacientes com doença de Behcet foram significativamente mais elevadas do que o grupo controle. Conclusões: Tomografia de coerência óptica é uma ferramenta útil e não invasiva para acompanhar a degeneração nervosa retiniana e as alterações coroidais em pacientes com doença de Behcet, mesmo sem envolvimento ocular.

Purpose: To evaluate the visual acuity, fluorescein angiography, optic coherence tomography and life quality of patients diagnosed with exudative age-related macular degeneration and administered with intravitreal Ranibizumab injection. Material and Methods: This study included of 48 different patients who were diagnosed as exudative age-related macular degeneration and administered with ranibizumab injection. In this study, demographic characteristics, pre- and post-injection corrected visual acuity, angiography, optic coherence tomography alteration and the scores of quality of life questionnaire were prospectively analyzed. Results: The patients were followed up for 20±1 months on average. After ranibizumab injection, 12 patients (25%) gained ≥3 lines of visual acuity, 28 patients (58.3%) gained ≤3 lines of visual acuity, 6 patients (12.5%) lost ≤3 lines of visual acuity and 2 patients (4.2%) lost ≥3 lines of visual acuity. The increase in Early Treatment Diabetic Retinopathy Study was lower in patients with Hypertension and positive family history In this study, it was determined that The National Eye Institute Visual Function Questionnaire score increased in patients with improving visual acuity after ranibizumab injection and the difference was statistically significant. Conclusion: Visual acuity was found to improve in patients with exudative age-related macular degeneration and treated with intravitreal ranibizumab injection. The National Eye Institute Visual Function Questionnaire provided reliable results in patients with age-related macular degeneration and the questionnaire score was determined to increase following the treatment., Amaç: Eksudatif tip yaşa bağlı makula dejenerasyonu saptanan ve intravitreal Ranibizumab enjeksiyonu yapılan hastalarda görme keskinlikleri, floresein anjiyografi, optik koherens tomografi ve yaşam kalitesi değişimini değerlendirmek. Gereç ve Yöntem: Bu çalışma eksudatif tip yaşa bağlı makula dejenerasyonu olan ve ranibizumab enjeksiyonu yapılan 48 hastayı içermektedir. Bu çalışmada hastaların demografik özellikleri, enjeksiyon öncesi ve sonrası düzeltilmiş görme keskinlikleri, anjiyografi, optik koherens tomografi değişiklikleri ve yaşam kalitesi anketi skorları prospektif olarak değerlendirildi. Bulgular: Hastalar ortalama 20±1 ay süreyle takip edildi. Ranibizumab enjeksiyonu sonrası görme keskinliğinde, 3 sıra ve üzerinde kazanımı olan 12 hasta (%25), 3 sıradan az kazanımı olan 28 hasta (%58.3), 3 sıradan az kaybı olan 6 hasta (%12.5) ve 3 sıradan fazla kaybı olan 2 hasta (%4.2)’dır. Yüksek tansiyon ve aile hikayesi pozitif olan hastalarda Erken Tedavi Diyabetik Retinopati Çalışmasındaki artış daha düşük oldu. Bu çalışmada, ranibizumab enjeksiyonu sonrası görme keskinliği artan hastalarda Ulusal Göz Enstitüsü Görsel Fonksiyon Anketi yaşam kalitesi değerlerinin arttığı tespit edildi ve bu fark istatistiksel olarak anlamlı bulundu. Sonuç: İntravitreal ranibizumab enjeksiyonu ile tedavi edilen eksudatif tip yaşa bağlı makula dejenerasyonlu olguların görme keskinliğinde artış saptandı. Yaşa bağlı makula dejenerasyonu olan hastalarda Ulusal Göz Enstitüsü Görsel Fonksiyon Anketi güvenilir sonuçlar verdi ve işlem sonrası ankette puan artışı saptandı.

ÖZET ÖZET Otoimmün oküler yüzey ve uvea hastalıklarında korneal hasar oluşumunu açıklayabilmek amacıyla, üveitlerde kornea epiteline karşı antikor oluşumunu ön segmentteki vasküler konjesyonla açıklayan tezden yola çıkarak, Anabilimdalımız Uvea-İmmünoloji biriminde takip edilen immünolojik kökenli sistemik veya lokal hastalığı 50 olguluk hasta grubu ve rastgele seçilmiş 20 olguluk kontrol grubunda indirekt immünfloresan yöntemiyle kornea epiteline karşı antikorlar, PEG presipitasyon yöntemiyle dolaşan immünkompleksler ve RİD yöntemiyle immünglobülinler araştırıldı. Hasta grubunda kornea epiteline karşı antikor oluşumunun (%48), kontrol grubuna (%10) oranla anlamlı olarak farklılık gösterdiği saptandı. Dolaşan immünkompleksler ve immünglobülinlerde hasta grubunda anlamlı olarak yüksek bulundu. Çalışma kapsamına 29 Behçet, 5 kronik siklit, 5 idiopatik anterior üveit, 1 romatoid artrit (anterior üveit), 2 keratoüveit, 1 posterior üveit, 1 SLE (keratokonjonktivitis sikka), 1 episklerit, 1 panüveit, 1 ankilozan spondilit (episklerit), 1 diffüz episklerit, 1 posterior sklerit, 1 nekrotizan skleritli olgu alındı. Kornea epiteline karşı antikor oluşumunun yaş dışında hiçbir faktörle korelasyon ve ilişkisi saptanamadı. Antikor oluşumunun dolaşan immünkompleks ve immünglobülin değerleriyle de korelasyon göstermediği görüldü. Olgularımızda, antikor pozitifliğini üveitlerde oluşan ön segment hiperemisi ve gözün değişik yapılan arasındaki çarpraz reaktivite ile açıklayan görüşleri destekler bulgular saptanmadı. Çalışmamızda elde edilen bulgularla epitele karşı antikor oluşumunun genel otoimmün bozukluğun bir neticesi olabileceği, otoimmün bozukluğu olan olgularda kanda birden çok antikor mevcut olduğu ve bu antikorların vücudun değişik antijenik yapı gösteren dokularında ayrı ayrı reaksiyon verebilecekleri sonucuna varıldı. 29 32

WOS: 000079878100009 PubMed ID: 10202292 In the present study the effects of P-adrenergic antagonist and a-adrenergic agonist drugs on rabbit corneas were evaluated in vivo by using transmission electron microscopy. Twenty-four New Zealand albino rabbits were divided into six groups according to the drug applied. The rabbits to which only balanced salt solution (BSS) or BSS and benzalkonium chloride (BAC) were applied were taken as the control groups. The other four groups consisted of the rabbits to which Timoptic 0.5%, Betagan 0.5%, Betoptic 0.5% and lopidine 1% were applied, respectively. All of drugs were instilled topically twice dal ly for 6 weeks. In the BSS group, a II layers of the cornea were ultrastructurally normal. In the BSS and BAC group slight epithelial and endothelial changes were found. However, in the other groups, loss of microvilli, increase in glycogen particles, nuclear indentation, widening of the intercellular spaces and cytoplasmic vacuolization in epithelium were observed. No significant abnormality was found in the basal lamina, stroma and Descemet's membrane. Slight ultrastructural changes were noted in the endothelium such as vacuolization due to dilatation of the endoplasmic reticulum cisternae and focal cytoplasmic lytic areas. The results of this study indicate that various ultrastructural changes occur in groups treated with antiglaucomatous drug and that topical treatment with timolol and apraclonidine for 6 weeks is more toxic to the rabbit cornea than levobunolol and betaxolol.

Amaç: Yaşa bağlı makula dejenerasyonu ile ilişkili koroid neovaskülarizasyonlu seçilmiş olgularda verteporfin kullanılarak yapılan fotodinamik tedavinin sonuçlarını değerlendirmekGereç ve Yöntem: ÇÜTF Göz Hastalıkları Anabilim Dalı Retina Birimi'ne Ekim 2004 ve Mayıs 2008 tarihleri arasında başvuran ve FDT uygulanan 300 hasta dosyası retrospektif olarak incelendi. Sadece YBMD'a ikincil KNV gelişen ve en az 1 yıllık takibi olan 188 hastanın 216 gözü çalışmaya alındı. Bu olgulardan 55' ine takiplerde ya da ilk muayenede intravitreal bevacizumab ile FDT kombine uygulandı.Bulgular: YBMD'ye ikincil KNV gelişmiş 188 hastanın 216 gözü çalışmaya alındı. Hastaların 73'ü kadın 115'i erkek olup, yaş ortalaması 67,96±9,97 ve ortalama FDT sayısı 1,65±0,8 idi.Hastaların görme keskinliği Snellen ve ETDRS eşelleri ile alındı. FDT sonrası görme keskinliğinde, 101'inde (% 47) en az 3 sıra kayıp, 75'inde (% 34) en az 3 sıra artış görüldü ve 40'ında (% 19) görme stabil seyretti.Subfoveal yerleşimli gruptaki sıra kaybı, jukstafoveal yerleşimli gruba göre daha az olduğu saptandı ve istatistiksel olarak anlamlı bulundu.Olguların 55'ine (% 25,5) intravitreal bevacizumab enjeksiyonu ile FDT kombine olarak uygulandı. Son kontrol muayenesinde, olguların % 40'ında görme artışı sağlanırken, % 43'ünde 3 sıradan daha az görme kaybı ve % 17'sinde ise GK' nin stabil kaldığı belirlendi.Sonuç: Verteporfin ile yapılan FDT, YBMD'ye bağlı gelişen KNV'lerde görme kaybı riskini güvenilir bir şekilde azaltabilmektedir. Son dönemlerde yapılan çalışmalar, intravitreal anti-VEGF ilaç enjeksiyonu ile FDT'nin kombine olarak uygulanmasının görme keskinliğinde artışla sonuçlandığını göstermektedir. Purpose: To evaluate results of treatment of choroidal neovascularization with photodynamic therapy (PDT) using verteporfin in eyes age-related macular degeneration (AMD).Methods: A retrospective case series in Department of Ophthalmology, Cukurova University evaluated patients choroidal neovascularization due to AMD. Two hundred-sixteen eyes of one hundred-eighty eight patients with neovascular AMD were enrolled this trial. Fifty five patients recieved intravitreal bevacizumab combined with verteporfin PDT (combined therapy).Results: Two hundred-sixteen eyes of one hundred-eighty eight patients with neovascular AMD were evaluated. Seventy three were women and 115 were men. The average age was 67.96±9.97 years. The average of 1.65±0.8 teratments were given. The visual acuity (VA) was measured with Snellen and ETDRS charts. At the end of the year, there was no change in VA in 40 (19%) patients, 3 lines less decrease in 101 (47%) patients, and 3 lines more increase in 75 (34%) patients. VA improvement was subfoveal CNV than better juxtafoveal CNV.55 of patients (25.5%) was treatment with combined therapy. At last visit, there was no change in VA in 17 % patients, 3 lines less decrease in 43 % patients and 3 lines more increase in 40 % patients.Conclusion: Verteporfin therapy of chroidal neovascularization from AMD can safely reduce the risk of vision loss. Current studies, the combined therapy with PDT and intravitreal bevacizumab is a safe and effective treatment for neovascular AMD. 65

ÖZET DİABETİK RETÎNOPATİ'DE OKSİDATİF STRES ve HEPATOSİT BÜYÜME FAKTÖRÜ'NÜN ETKİNLİĞİ Yaşam süresinin uzamasıyla birlikte son yıllarda büyük artış gösteren ve gittikçe artacağı düşünülen diyabetik retinopati insidansı, diyabetik retinopatinin gelişimi ve ilerleyişi hakkında araştırmaların yoğunlaşmasına neden olmuştur. Bu çalışmada, diyabetik retinopati gelişme sürecinde oksidatif stres ile Hepatosit Büyüme Faktörü' nün etkinliği ve birbirleriyle olan ilişkilerini araştırmak amacıyla, her biri 32 hastadan oluşan, proliferatif diyabetik retinopati ( PDR ), nonproliferatif diyabetik retinopati ( NPDR ) ve retinopatisi olmayan diyabetik hasta ( Kontrol ) gruplarının tam kanda glutatyon ve hemoglobin Alc ( HbAlc ), plazma malondialdehit ( MDA ), serum Hepatosit Büyüme Faktörü ( HGF ) düzeyleri karşılaştırılmıştır. Randomize tek -kör olarak yapılan çalışmada; glutatyon spektrofotometrik olarak, malondialdehit düzeyi tiyobarbitürik asitle reaksyon veren maddelerin (TBARS ) ölçümü prensibiyle ve Hepatosit Büyüme Faktörü de enzim bağlantılı immünosorban yöntemiyle (ELISA) Ölçüldü. Ortalama MDA ve HGF düzeyleri, PDR grubunda NPDR ve Kontrol grubuna göre istatistiksel olarak anlamlı derecede yüksek saptandı ( p< 0,001 ). NPDR ile Kontrol grubu arasında anlamlı bir fark görülmedi ( p> 0,1 ). Benzer şekilde HbAlc seviyesi, PDR ile NPDR ve PDR ile Kontrol grubu arasında anlamlı derecede farklı ( p 0,5 ). Glutatyon düzeyi, PDR grubunda NPDR grubuna g0re anlamlı derecede düşük ( p< 0,01 VI), NPDR grubunda Kontrol grubuna kıyasla anlamlı derecede yüksek saptanırken ( p< 0,05 ), PDR ile Kontrol grubu arasında anlamlı bir fark görülmedi ( p> 0,5 ). HGF ile MDA ve HbAlc arasında aynı yönde, glutatyon ile MDA arasında ters yönde anlamlı korelasyon bulundu. Elde edilen sonuçlar, diyabetes mellitusta bozulan oksidan-antioksidan denge ile HGF' nin, diyabetik retinopatinin ilerlemesinde birlikte rol aldıklarım işaret etmektedir. This study aimed to investigate progression of diabetic retinopathy with oxidative stress and Hepatocyte Growth Factor ( HGF ) activity and relation between them. Each groups have consisted by 32 patients. These are proliferative diabetic retinopathy ( PDR ), nonproliferative diabetic retinopathy ( NPDR ) and without retinopathy ( Control ). In this case complete blood glutathion, glycated hemoglobin ( HbAlc ), plasma malondialdehyde ( MDA ) and serum HGF levels were compared. In the one blind randomized study glutathion has measured as spectrophptometrically, the level of MDA has measured by the principal of the substances which reacts to thiobarbituric acid and HGF has measured by enzyme-linked immunosorbent assay (ELISA ). On the PDR group, average levels of MDA and HGF have been found highly statistically significant ( p< 0.001 ) versus NPDR and control group, between NPDR and control group differences weren't found ( p> 0,1 ). Similarly the levels of HbAlc was significantly different (p < 0,01) both PDR and NPDR also PDR and control group. On NPDR and control group wasn't statistically difference( p > 0.5 ). Glutathion levels were found significantly low in PDR ( p < 0,01 ) cpmpared with NPDR, in NPDR glutathion levels were found significantly high compared with control group ( p < 0.05 ), in PDR compared with control group wasn't found different ( p > 0,5 ). Possitive correlation has vmbeen found on HGF and MDA also HbAlc, opposite side correlation has been found between glutathion and MDA. As a result increased levels of oxidative stress in diabetic patients made structural and functional changes in retina which caused to leading release of HGF these two factors playing role together in the progression of diabetic retinopathy. ÖZET Çalışmamızda, bir non-oküler antijen kullanılarak tavşanlarda, primer ve sekonder üveit oluşturuldu ve bu model üzerinde histopatolojik ve ultrastrüktürel inceleme yapıldı. Burada amaç deneysel üveit oluşturmak ve bu model üzerinde deneysel üveitin patogenezini araştırmak idi. Ayrıca klinik özellikleri, histopatolojisi ve ultrastrüktürel özellikleri tanımlanan bu modelin daha ileri çalışmalarda basamak oluşturabileceği düşünüldü. Bu amaçla 12 adet albino tavşan çalışma kapsamına alındı. Tavşanların tümünün söl gözlerine intravitreal olarak `bovine serum albumine` (BSA), sağ gözlerine ise kontrol grubu oluşturmak amacı steril distile su enjekte edildi. 2 olgu çeşitli nedenlerle çalışma dışı bırakıldı. Kalan 10 tavşandan 5 'inin gözleri, gelişen primer üveitin en şiddetli olduğu dönemde (1. grup), 2' sinin gözleri üveit bulguları geriledikten sonra enüklee edildi (2. grup). Kalan 3 tavşana üveit bulguları iyice geriledikten sonra BSA intravenöz olarak enjekte edildi ve sekonder üveitin geliştiği ilk 12 saat içinde gözleri enüklee edildi (3. grup). Enüklee edilen gözler hematoksilen eozinle boyanıp ışık mikroskobik olarak ve uranil asetat-kurşun sitrat ile boyanıp elektron mikroskobik olarak incelendi. Primer üveitde etkin hücrelerin lenfoplazmositer hücreler olduğu gözlendi. Sekonder üveitde ise olaya polimorfonükleer lökositlerin (özellikle eozinofiller) de katıldığı gözlendi. Elektron mikroskobik olarak primer üveitli olgularda, uveal dokuda damar endotel hücrelerinde dejeneratif değişiklikler, bazal lamina hasan, sekonder üveitli olgularda ise endotelyal bileşkelerde yer yer ayrılma gözlendi. Bu bulgularla primer üveitde etkin hücrelerin lenfoplazmositer hücreler olduğu, sekonder üveitli olgularda üveitin akut fazında, polimorfonükleer lökositlerin özellikle de eozmofillerin önemli rol aldığı düşünüldü. Ayrıca deneysel üveitde uveal dokuda, inflamatuar hücrelerin damar dışına çıkışının, elektron mikroskobik inceleme ile saptanan bazal lamina ve endotelyal bileşke hasan aracılığıyla olduğu düşünüldü. IV ABSTRACT HISTOPATHOLOGIC AND ULTRASTRUCTURAL CHANGES IN AN EXPERIMENTAL UVEITIS MODEL In the present study, primary and secondary uveitis were induced in rabbits by using a non-ocular antigen and histopathological and ultrastructural examination were performed in this model. This study was performed in order to induce an experimental uveitis model, and to define the pathogenesis of uveitis in this model. In addition it was thought that this model in which clinical, histopathological and ultrastructural features were defined, could constitute a step for following studies. The study group consisted of 12 albino rabbits. Bovine serum albumine (BSA) was injected intravitreally to the left eyes. The right eyes were used as control, and sterile distilled water was injected in these eyes. Two cases were excluded from the study because of various causes. Among remaining 10 rabbits, eyes of the five rabbits were enuclated during the period that primary uveitis was most severe (the first group). The eyes of the two rabbits were enuclated after uveitis findings were regressed (the second group). BSA was injected intravenously to the remaining three rabbits after the uveitis findings regressed completely, and than the eyes were enuclated in the first twelve hours in which secondary uveitis was developed (the third group). The enuclated eyes, were examined by light microscopy after staining with hematoxylin-eosin; and by electron microscopy after staining with uranyl asetate-lead sitrate. It was found out that the effective cells were lymphocytes and plasma cells in primary uveitis. However in secondary uveitis it was observed that polymorphonuclear leukocytes (especially eosinophils) also accompanied the reaction. On electron microscopy, basal lamina destruction in vessel walls, degenerative changes in vascular endothelial cells in uvea in primary uveitis, and separation at endothelial junction in some areas in some of the secondary uveitis were detected. Through these findings it was suggested that the effective cells were lymphocytes and plasma cells in primary uveitis and polymorphonuclear leukocytes (especially eosinophils) in acute phase of secondary uveitis. Destruction of basal lamina and endothelial junction which were detected by electron microscopy can lead to migration of inflammatory cells through the vascular compartment in experimental uveitis.
