Abstract Educational Leadership in Teaching Excellence (EnLITE) is an 11-month faculty development program at the University of Guelph, Ontario. Created and led by faculty members and educational developers, EnLITE is designed to engage participants in the principles, practice and theory of teaching and learning in higher education and to promote a learner-centred approach to teaching. Participants critically examine and discuss scholarly topics on teaching and learning and in their own disciplines; collaborate with one or more teaching mentors; engage in peer classroom observation; and participate in other teaching-related activities informed by their individual learning plans. Our objective was to determine the perceived impact of EnLITE on participants' teaching-related practices and experiences. We collected pre-, post- and one-year post-program quantitative and qualitative survey responses from each of the 2014-2015, 2015-2016 and 2016-2017 EnLITE cohorts (N = 17 participants representing a variety of disciplines; 71% female). There were significant improvements in participants' perceived teaching practices related to critical self-reflection (13% increase from pre- to one-year post-program), student engagement (+28.2%), collaborative learning (+31%) and learner-centred pedagogy (+22.9%, all p < 0.05). There was little to no change in use of technology, student assessment, leadership, participation in communities of practice, or dissemination of teaching-related scholarship. These results provide empirical evidence of the effectiveness of a peer-driven faculty development program in promoting a learning-centred approach to teaching. Future research should determine whether these changes translate into improved student learning, and whether such programs demonstrate longer term improvements in engagement in teaching-related leadership, communities of practice and dissemination.

Partnerships with students are considered one of the principles of good Scholarship of Teaching and Learning (SoTL) practice. However, not all partnerships are equally successful. What characteristics are common to successful partnerships and what preparatory elements can lead toward more successful partnerships? In this article, our team of graduate students, educational developers, and faculty members engage in detailed self-reflection on our past and ongoing SoTL projects as an inquiry into what it means to be in a successful student-faculty/staff partnership. Using thematic analysis, we identify and describe four distinct domains that can shape partnerships: (1) motivations to participate, (2) challenges, (3) power, and (4) definitions of success. The article concludes with a set of questions to stimulate initial and ongoing conversations between partners to guide new partnerships in defining the parameters for success in their proposed collaboration.

Background: α-Fetoprotein (AFP) is conventionally absent in testicular classical seminoma (TCS). However, moderate AFP elevations can occur in TCS patients, as observed at this and other centres, which can be challenging to diagnostic and management practices. Methods: This retrospective cohort study considered AFP concentration in the context of germ-cell tumour diagnosis and characterisation at baseline (BL), disease status during chemotherapy, and long-term surveillance. The study considered patients with histologically diagnosed stage 1 TCS requiring chemotherapy over six years. For those with AFP above the reference interval at BL, histological imaging, case notes, and biochemical data were reviewed from BL to surveillance completion. Outcomes included AFP changes, diagnoses, therapy, disease progression, and death. Results: Of the 175 patients included, eight (4.6%) had elevated AFP at BL. Of these, two showed statistically but not clinically significant AFP changes during therapy, while six had moderate, stable AFP elevations with no changes in diagnosis during follow-up. During therapy, one patient developed metastases, and one died of causes likely unrelated to their TCS. Conclusions: Mild elevations of AFP in TCS may lead to diagnostic uncertainty or inappropriate management and investigation. However, AFP changes, alongside imaging, did not affect diagnosis, therapy, or follow-up at this centre for any of the patients examined. The COVID-19 pandemic provided lessons for health policies across the globe. We assessed the knowledge, attitudes and practices of vulnerable populations in an informal settlement in Mumbai, India, during the pandemic. We discuss the viability and effectiveness of communication strategies based on these results and offer suggestions for policy modifications regarding awareness generation and behavioral change during health emergencies. The study was conducted through a telephone survey among 460 respondents (233 men and 226 women) from April to May 2021. Television (94%) and family (70%) were the most common sources of information, but they were not the most trusted. Most respondents were aware of the causes and preventive measures of COVID-19, including vaccination. However, this awareness did not always result in changes in attitudes and behavior- 66% perceived low or no risk to COVID-19, while 93% did not consider it essential to wear masks in workspaces and 78% did not sanitize their hands before entering workspaces. More respondents were concerned about loss of employment (53%) than about their physical (10%) or mental (4%) health. The study highlights the need for context-specific communication strategies for vulnerable populations. This includes providing reliable and accessible sources of information, emphasizing accuracy and detail, and adopting a holistic and multidimensional approach to awareness and information sharing. Background: Human papillomavirus (HPV) vaccination coverage is characterized by geographic disparities in the United States, with national studies finding lower coverage in rural versus nonrural areas. To direct quality improvement efforts in North Carolina, we sought to understand how different rurality measures characterize these disparities. Methods: We used separate negative binomial regression models to test associations between five dichotomized county-level rurality measures and HPV vaccination coverage (=1 dose) among North Carolina adolescents, aged 11 to 12 years (n = 326,345). Rurality measures were derived from: Office of Management and Budget's Metropolitan Statistical Areas, Rural-Urban Continuum Codes, Index of Relative Rurality, US Census Bureau classifications, and North Carolina Rural Center classifications. Models controlled for Social Vulnerability Index (SVI) percentile and rate of pediatricians per county. Vaccination data came from the North Carolina Immunization Registry. Results: HPV vaccination coverage was 29% across North Carolina's 100 counties (range: 13%, 46%). Agreement between rurality measures ranged from 54% to 93% of counties. In adjusted analyses, none of the five rurality measures were correlated with HPV vaccination coverage, but higher SVI and higher rate of pediatricians were positively associated with coverage (P < 0.01). Exploratory moderation analyses suggested regional variation in the relationship between rurality and coverage, with a positive association in one region, a negative association in one region, and no association in four regions. Conclusions: County-level rurality measures did not identify disparities in HPV vaccination coverage in North Carolina. Impact: Measures related to social vulnerability and access to pediatricians may be better suited for understanding and addressing the state's substantial county-level vaccination disparities. Simple Summary: Producers maintain cows in their herd based on breed type, lactation number, and individual productivity characteristics. However, there are questions about the effect of management decisions regarding cow behavior and productivity on cow welfare and the economic viability of dairy farms in Central Texas, which is not an ideal environment for dairy production. Data from this study confirm that breed and lactation numbers influence milk yield, milk urea nitrogen concentrations, lying time, and the incidence of lameness. Also, a higher lameness incidence was found in high-producing cows, with lower milk urea nitrogen concentrations. It is suggested that management decisions regarding the retention of cows, breed types, and lactation number influence the incidence of lameness, with subsequent effects on cow behavior and productivity. The objective of this study was to evaluate the effects of breed, lactation number, and lameness on lying time, milk yield, milk urea nitrogen concentration (MUN), progesterone concentration (P4), and the calving-to-conception interval (CCI) of lactating dairy cows in Central Texas. A total of 84 lactating dairy cows (Holsteins, Jerseys, and crossbreeds) from a commercial dairy farm in Central Texas were randomly selected and enrolled in this study from October 2023 to February 2024. Cows (60 ± 7 DIM) were enrolled in cohorts weekly for five weeks and were randomly fitted with an IceQube pedometer (IceRobotics, Edinburgh, UK) to track lying time. Lameness and body condition scores (BCS) were recorded, and blood samples were collected once a week. Parameters of reproductive performance included insemination rate, conception rate, pregnancy rate, and the CCI. Monthly dairy herd improvement association (DHIA) testing included milk yield and MUN concentrations. Breed and lactation number had a significant effect on milk yield, MUN concentration, lying time, BCS, and lameness (p < 0.001). Lactation number had a significant effect on P4 concentrations (p < 0.001). There was a positive correlation between lameness and milk yield (p = 0.014) and a negative correlation between lameness and MUN concentrations (p = 0.038). [ABSTRACT FROM AUTHOR]

Using data from 88 countries, we test hypotheses linking a country's economic freedom and cultural values with the propensity and timing of decisions to impose stringent policies to combat the spread of Covid-19, as well as society's compliance with those restrictive measures. Our analysis supports hypotheses that a country's economic freedom and cultural dimensions of individualism and masculinity predict early implementation of stringent policies. After accounting for endogeneity, we find that individualism also helps explain residents' compliance with stringent measures. Sexual minority adults—including lesbian, gay, bisexual, queer/questioning, and other sexual minorities (LGBQ+)—face demonstrated barriers to employment, health insurance, and healthcare access. We sought to assess the association between sexual orientation, unmet medical needs, and telehealth use due to the COVID-19 pandemic using the National Health Interview Survey (2020). Using multivariable analyses, we assessed the likelihood of having unmet medical needs and telehealth use in LGBQ+ adults and heterosexual adults, adjusting for socioeconomic, demographic, and clinical factors. We found that LGBQ+ individuals had a significantly higher likelihood of having any unmet medical needs or unmet non–COVID-related medical needs due to the pandemic compared with heterosexual individuals. The LGBQ+ individuals were more likely to use telehealth than their heterosexual counterparts—for both general and pandemic-related reasons. Our findings indicate that disastrous events such as the COVID-19 pandemic likely exacerbate existing healthcare access disparities faced by LGBQ+ individuals. This warrants further research to explore the interventional potential of telehealth for reducing healthcare access disparities, especially for those living in geographic regions lacking culturally competent providers. PURPOSE Financial navigation services support patients with cancer and address the direct and indirect financial burden of cancer diagnosis and treatment. These services are commonly delivered through a variety of frontline oncology support personnel (FOSP) including navigators, social workers, supportive care providers, and other clinic staff, but the perspective of FOSPs is largely absent from current literature on financial burden in oncology. We surveyed a national sample of FOSPs to understand their perspectives on patient financial burden, resource availability, and barriers and facilitators to assisting patients with cancer-related financial burden. METHODS We used Qualtrics online survey software and recruited participants using multiple professional society and interest group mailing lists. Categorical responses were described using frequencies, distributions of numeric survey responses were described using the median and IQR, and two open-ended survey questions were categorized thematically using a priori themes, allowing additional emergent themes. RESULTS Two hundred fourteen FOSPs completed this national survey. Respondents reported a high awareness of patient financial burden and felt comfortable speaking to patients about financial concerns. Patient assistance resources were commonly available, but only 15% described resources as sufficient for the observed needs. A substantial portion of respondents reported moral distress related to this lack of resources. CONCLUSION FOSPs, who already have requisite knowledge and comfort in discussing patient financial needs, are a critical resource for mitigating cancer-related financial burden. Interventions should leverage this resource but prioritize transparency and efficiency to reduce the administrative and emotional toll on the FOSP workforce and reduce the risk of burnout.
