- Abstract
Background: The extent to which health care systems have adapted to the COVID-19 pandemic to provide necessary cardiac diagnostic services is unknown. Objectives: The aim of this study was to determine the impact of the pandemic on cardiac testing practices, volumes and types of diagnostic services, and perceived psychological stress to health care providers worldwide. Methods: The International Atomic Energy Agency conducted a worldwide survey assessing alterations from baseline in cardiovascular diagnostic care at the pandemic's onset and 1 year later. Multivariable regression was used to determine factors associated with procedure volume recovery. Results: Surveys were submitted from 669 centers in 107 countries. Worldwide reduction in cardiac procedure volumes of 64% from March 2019 to April 2020 recovered by April 2021 in high- and upper middle-income countries (recovery rates of 108% and 99%) but remained depressed in lower middle- and low-income countries (46% and 30% recovery). Although stress testing was used 12% less frequently in 2021 than in 2019, coronary computed tomographic angiography was used 14% more, a trend also seen for other advanced cardiac imaging modalities (positron emission tomography and magnetic resonance; 22%-25% increases). Pandemic-related psychological stress was estimated to have affected nearly 40% of staff, impacting patient care at 78% of sites. In multivariable regression, only lower-income status and physicians’ psychological stress were significant in predicting recovery of cardiac testing. Conclusions: Cardiac diagnostic testing has yet to recover to prepandemic levels in lower-income countries. Worldwide, the decrease in standard stress testing is offset by greater use of advanced cardiac imaging modalities. Pandemic-related psychological stress among providers is widespread and associated with poor recovery of cardiac testing.

Incarcerated inguinal hernia is a common diagnosis in patients presenting a painful and nonreducible groin mass. Although the diagnosis is usually made by physical examination, the content of the hernia sac and the extent of the surgical operation may vary and can require multimodal imaging integration (e.g., ultrasonography, computed tomography); the usual finding is a segment of small bowel and, less commonly, large bowel. We present an extremely rare case of a sigmoid cancer incarcerated in a left inguinal hernia and infiltrating the spermatic cord. The patient underwent whole-body computed tomography (CT) with contrast agent injection for staging, followed by a left hemicolectomy paralleled by a unilateral orchiectomy.

10. Effects of the Teach-Model-Coach-Review Instructional Approach on Caregiver Use of Language Support Strategies and Children's Expressive Language Skills

- Abstract
Purpose: In this study, the authors examined the effects of the Teach-Model-Coach-Review instructional approach on caregivers' use of four enhanced milieu teaching (EMT) language support strategies and on their children's use of expressive language. Method: Four caregiver-child dyads participated in a single-subject, multiple-baseline study. Children were between 24 and 42 months of age and had language impairment. Interventionists used the Teach-Model-Coach-Review instructional approach to teach caregivers to use matched turns, expansions, time delays, and milieu teaching prompts during 24 individualized clinic sessions. Caregiver use of each EMT language support strategy and child use of communication targets were the dependent variables. Results: The caregivers demonstrated increases in their use of each EMT language support strategy after instruction. Generalization and maintenance of strategy use to the home was limited, indicating that teaching across routines is necessary to achieve maximal outcomes. All children demonstrated gains in their use of communication targets and in their performance on norm-referenced measures of language. Conclusion: The results indicate that the Teach-Model-Coach-Review instructional approach resulted in increased use of EMT language support strategies by caregivers. Caregiver use of these strategies was associated with positive changes in child language skills.

BACKGROUND: The extent to which health care systems have adapted to the COVID-19 pandemic to provide necessary cardiac diagnostic services is unknown.OBJECTIVES: The aim of this study was to determine the impact of the pandemic on cardiac testing practices, volumes and types of diagnostic services, and perceived psychological stress to health care providers worldwide.METHODS: The International Atomic Energy Agency conducted a worldwide survey assessing alterations from baseline in cardiovascular diagnostic care at the pandemic's onset and 1 year later. Multivariable regression was used to determine factors associated with procedure volume recovery.RESULTS: Surveys were submitted from 669 centers in 107 countries. Worldwide reduction in cardiac procedure volumes of 64% from March 2019 to April 2020 recovered by April 2021 in high- and upper middle-income countries (recovery rates of 108% and 99%) but remained depressed in lower middle- and low-income countries (46% and 30% recovery). Although stress testing was used 12% less frequently in 2021 than in 2019, coronary computed tomographic angiography was used 14% more, a trend also seen for other advanced cardiac imaging modalities (positron emission tomography and magnetic resonance; 22%-25% increases). Pandemic-related psychological stress was estimated to have affected nearly 40% of staff, impacting patient care at 78% of sites. In multivariable regression, only lower-income status and physicians' psychological stress were significant in predicting recovery of cardiac testing.CONCLUSIONS: Cardiac diagnostic testing has yet to recover to prepandemic levels in lower-income countries. Worldwide, the decrease in standard stress testing is offset by greater use of advanced cardiac imaging modalities. Pandemic-related psychological stress among providers is widespread and associated with poor recovery of cardiac testing.

As sea levels rise, coastal wetlands are encountering physical barriers to inland migration in a phenomenon known as “coastal squeeze.” Specifically, wetlands are being squeezed between sea-level rise on one side and human development on the other preventing their natural ability to adapt by moving to higher ground. State and local coastal governments have legal and policy tools available to adapt to sea-level rise and limit the impacts of coastal squeeze on migrating wetlands. This article presents legal and policy tools and considerations governments could evaluate to facilitate long-term wetland conservation and migration to maximize benefits for people, the environment, and economies. This article first provides legal background on the law and federal, state, and local actors that could impact state and local decisions. This article then identifies five primary components of a comprehensive wetland migration strategy for state and local coastal governments: (1.) data; (2.) planning; (3.) voluntary land acquisitions; (4.) legal tools; and (5.) community engagement. This article also includes case study examples. Decisionmakers could potentially integrate any of these five components into existing coastal, environmental, land-use, and climate adaptation efforts to align policy objectives to protect wetlands across different programs and projects.

A new chiral Mn–salen catalyst, functionalized with a long aliphatic chain and a choline group, able to act as surfactant catalyst for green epoxidation in water, is here described. This catalyst was employed with a commercial surfactant (CTABr) leading to a nanoreactor for the enantioselective epoxidation of some selected alkenes in water, using NaClO as oxidant. This is the first example of a nanoreactor for enantioselective epoxidation of non-functionalized alkenes in water.

Collaborative robotic configurations for monitoring and tracking human targets have attracted interest in the fourth industrial revolution. The fusion of different types of sensors embedded in collaborative robotic systems achieves high-quality information and contributes to significantly improve robotic perception. However, current methods have not deeply explored the capabilities of thermal multisensory configurations in human-oriented tasks. In this paper, we propose thermal multisensor fusion (TMF) for collaborative robots to overcome the limitations of stand-alone robots. Thermal vision helps to utilize the heat signature of the human body for human-oriented tracking. An omnidirectional (O-D) infrared (IR) sensor provides a wide field of view (FOV) to detect human targets, and Stereo IR helps determine the distance of the human target in the oriented direction. The fusion of O-D IR and Stereo IR also creates a multisensor stereo for an additional determination of the distance to the target. The fusion of thermal and O-D sensors brings their limited prediction accuracy with their advantages. The maximum a posteriori method is used to predict the distance of the target with high accuracy by using the distance results of TMF stereo from multiple platforms according to the reliability of the sensors rather than its usage of visible-band-based tracking methods. The proposed method tracks the distance calculation of each sensor instead of target trajectory tracking as in visible-band methods. We proved that TMF increases the perception of robots by offering a wide FOV and provides precise target localization for collaborative robots.

Increasingly frequent and intense extreme weather events are pushing communities to confront difficult decisions in order to protect people and infrastructure sitting in harm’s way. One decision is around managed retreat or the voluntary movement and transition of individuals and communities away from coastal and other climate-vulnerable areas. “Receiving communities” and other low-risk areas where people may choose to relocate must adopt a multi-pronged strategy that addresses the full breadth of services and resources that relocating residents will need in order to adapt to their new communities, such as in areas of health, education, and workforce development. A critical consideration for receiving communities is the provision of adequate and affordable housing for new residents, many of whom have been driven to relocate due to housing insecurity caused by climate change impacts. Climate change can contribute to housing insecurity in multiple ways, such as when insurance rates increase for low-income homeowners living in flood-prone areas, or when disasters destroy existing affordable housing stock. These impacts can trigger the displacement of individual households or entire communities, requiring local governments to plan ahead for community-centered housing solutions. This article (1) explores the affordable housing crisis in the USA and the compounding impacts of climate change; (2) analyzes the concept of receiving communities and the unique considerations for preparing and investing in affordable housing; and (3) recommends how local governments can use new and existing planning, funding, and legal tools to ensure an effective and inclusive housing strategy for receiving communities.

En este trabajo se analiza una empresa cuyo objeto social es la producción y comercialización de medicamentos para uso humano. Este laboratorio se encuentra en la Provincia de Córdoba, República Argentina, y fue fundado en 1993. Comercializa sus productos en el país, y desde el año 2005, mediante exportaciones, a países de Latinoamérica, habiendo creado en ese momento un área especialmente abocada a dicho objetivo. Desde el año 2018, la empresa decidió profundizar el desarrollo comercial internacional, lo cual se suma al crecimiento de ésta en el territorio nacional, generando la necesidad de financiar dicho incremento de actividad. Por lo expuesto, se activa la iniciativa de realizar el presente trabajo, cuyo objetivo es conocer la estructura óptima de capital para la empresa, y evaluar las fuentes de financiamiento existentes en el mercado. Por lo tanto, se realiza el cálculo y el análisis de la estructura de capital por nivel de endeudamiento, teniendo en cuenta el impacto de los distintos niveles en el resultado a obtener del Economic Value Added (EVA), llegando así a la estructura óptima de capital. El objetivo general de este trabajo consiste en elaborar estrategias financieras adecuadas para las Pymes industriales en el nuevo contexto económico mundial (COVID-19) en relación a la posición competitiva que requieren las Pymes industriales para su desarrollo y supervivencia. Es de suma importancia tener estrategias financieras claras en la pequeña y mediana empresa industrial (PYME industrial) para no caer en lo que ninguna empresa quiere, la quiebra. A su vez es importante saber que de llevarse a cabo e implementarse buenas estrategias financieras y aplicarlas en la PYME industrial de manera correcta, se pueden lograr resultados muy favorables. Otra serie de factores que son básicos para aplicar estrategias financieras son los estados financieros, las proyecciones financieras, y las razones financieras. Conociendo lo que cada uno indica se pueden de esta manera llevar a cabo proyecciones financieras precisas y poder manejar cualquier situación que se pueda presentar. El problema de la investigación es que gran cantidad de Pymes no toman las decisiones acertadas en cuestión financiera. Introduction: Single coronary artery is a rare condition characterized by the origin of a coronary that supplies the entire heart from a single coronary ostium. Patient concerns: A 45-year-old woman with an altered exercise testing was addressed to a computed tomography coronary angiography (CTCA) to rule out coronary artery disease (CAD). Diagnosis: CTCA examination showed the absence of the right coronary artery (RCA). The left anterior descending artery and the left circumflex artery (LCX) presented regular origin and course and LCX provided the posterior interventricular artery and the posterolateral artery. Interventions: As CTCA highlighted the absence of potentially life-threatening features related to coronary anomaly, no surgical treatment was advised. Outcomes: The patient was dismissed, kept under pharmacological control and monitored over time. Conclusion: CTCA is the first-choice imaging modality in patients with ECG abnormalities properly allowing the differential diagnosis between CAD and congenital heart disease.

Incarcerated inguinal hernia is a common diagnosis in patients presenting a painful and nonreducible groin mass. Although the diagnosis is usually made by physical examination, the content of the hernia sac and the extent of the surgical operation may vary and can require multimodal imaging integration (e.g., ultrasonography, computed tomography); the usual finding is a segment of small bowel and, less commonly, large bowel. We present an extremely rare case of a sigmoid cancer incarcerated in a left inguinal hernia and infiltrating the spermatic cord. The patient underwent whole-body computed tomography (CT) with contrast agent injection for staging, followed by a left hemicolectomy paralleled by a unilateral orchiectomy.

A new chiral Mn-salen catalyst, functionalized with a long aliphatic chain and a choline group, able to act as surfactant catalyst for green epoxidation in water, is here described. This catalyst was employed with a commercial surfactant (CTABr) leading to a nanoreactor for the enantioselective epoxidation of some selected alkenes in water, using NaClO as oxidant. This is the first example of nanoreactor for enantioselective epoxidation of non-functionalized alkenes in water.

Objective : To analyse the conditioning effects of localised acute muscle pain on power output during ipsi- and contra-lateral ballistic arm extensions. Methods : Eight male subjects performed right arm (ipsilateral) and left arm (contra-lateral) bench press movements. The power output (and force and velocity) of the concentric phase of movement was measured before and during muscle pain induced by a standardised intramuscular injection of levo-ascorbic acid in the right pectoralis major muscle (prime mover muscle) and in the lateral head of the right triceps brachii muscle (synergist). Results : The power output of ipsi- and contra-lateral arm bench press movements was significantly decreased during pain of the right pectoralis major muscle, but not during pain of the right lateral triceps muscle. The velocity and force were both affected and contributed to the decrease in power output. Conclusions : Acute muscle pain of a prime mover muscle reduces ipsi- and contra-lateral motor performance of ballistic upper limb extension. This is not a generalised or non-specific inhibitory effect on the motor system, since pain failed to modify motor performance when applied to a synergist, non-prime mover, muscle. A basically bilateral executive cortical plan for ballistic upper limb extension could explain the conditioning effect of muscle pain on both ipsi- and contra-lateral arm. Significance : The results presented here contributes to the understanding of the conditioning effects of muscle pain on movement performance.

Objective: It is known that tonic muscle pain induced by a Levo-Ascorbic (L-AS) solution injected in a foot muscle can transiently modify both regional proprioception and stimulus perception. These findings are paralleled by changes of middle-latency lower-limb somatosensory evoked potentials (SEPs). However, little is known on the behaviourally relevant aspect whether eventual SEP pain-induced changes could be partly due to a sort of ‘motor strategy’ of subjects in the frame of a self-protective reaction towards the noxious stimulus. Movement and imagery of movements are in fact known to reduce mainly pre-central SEP amplitude (i.e. gating effect). Methods: Low-threshold afferents ulnar SEPs, psychophysical pain ratings and fingers’ position sense were monitored in the time-course during L-AS injection in the right first dorsal interosseous muscle. Control experiments included SEPs (either following prevalent ulnar nerve low-threshold afferent stimulation or more conventional mixed nerve stimulation) during actual movements execution and imagery of movements of the right hand. Results: Tonic pain induced a significant reduction of the post-central N20-P25-N33 complex and a significant increase of the N18 wave. These changes, that were paralleled by distortion of the finger position sense, were delayed 2–5 min with respect to the maximal subjective pain sensation. Conversely, movement imagery tasks lead to a significant, selective, reduction of the pre-central N30 complex. This wave was even more reduced during actual movements, in combination with a reduction of those post-central components peaking after the first activation of the primary sensory cortex. Conclusions: Early sensory processing at cortical level is changed during tonic muscle pain, mainly for those components which may be theoretically involved in proprioceptive afferent elaboration. These changes are likely not due to subconscious or voluntary motor strategies of the subjects in the frame of a self-protective aversive reaction towards the noxious stimulus.

Eight right-handed male subjects were asked to use each hand to point accurately toward four points in the anterior trunk midline solely on the basis of the mental representation of their position along the trunk while standing upright and while lying supine. It was found that subjects were more accurate when performing in the upright posture. However, statistical analysis of the mean constant transverse errors of pointing showed that neither posture nor target factors had any significant effect on pointing accuracy. On the contrary, analysis of the mean constant longitudinal errors showed that both the posture and target factors were significant. Subjects pointed caudally to the target locations and made the largest errors, which were on the order of centimeters, when performing in the horizontal posture. These findings indicate that gravitational cues are critical in accurately localising mentally represented cutaneous targets.

Twelve right-handed male subjects were asked to use each hand to point accurately toward the center of their forehead, determining the correct position according to their mental representation, while the head was either aligned with the trunk or tilted 30 degrees to the right or left. Analysis of end-positions of pointing revealed that the right hand exhibited a slight leftward bias with respect to the putative head midline passing through the center of the glabella and the center of the fissure between the two upper central incisors, regardless of the head-on-body orientation, whereas the left hand proved very accurate when the head was aligned with the trunk but, when the head was tilted either way, it deviated to the opposite side. These results lead to the conclusion that the head midline appears to act as a reliable reference frame for encoding head-on-body orientation when pointing is carried out by the dominant hand.

The effects of selective transection of the rostralmost portion of the corpus callosum, which contains fibers interconnecting the motor cortices of the two hemispheres, on interocular temporal coupling in the initiation of learned symmetrical blinking were examined in 5 cats trained to blink in response to a 500-ms tone paired with 100-ms airpuffs randomly delivered to either eye (alternate airpuff; 3 animals) or simultaneously directed to both eyes (bilateral airpuff; 2 animals) 400 ms after tone onset. Lesioning had no effect on ability to respond, but did cause a significant increase in the mean conditioned response (CR) latencies of both eyes in all subjects. In the intact cats, a statistically significant difference between the mean CR latencies of the two eyes (hereafter called side superiority) was found in three animals, two of which exhibited superiority of the left eye. After lesioning, it was found that the right eye proved superior to the left eye in four subjects. Both before and after lesioning, side superiority was not due to the ability of one eye to give CRs consistently shorter than those of the other eye, but it crucially depended on the higher proportion of trials in which the superior eye led. A linear correlation between CR latencies of the right and left eye was found in all animals both before and after lesioning. It is suggested that subsequent to lesioning aimed at callosal disconnection of the motor cortex, the effects on initiation of learned symmetrical blinking are the consequences of withdrawal of both bilaterally balanced modulatory influences to both eyes and lateralized modulatory influences to the left eye.

This study was carry out to obtain direct evidence that the body midline actually is perceived and to assess some psychophysical properties of this line. Twelve normal, right-handed male subjects were asked to make accurate pointing movements toward the midline of the anterior trunk on the basis of their mental representation of this line. Each hand was used to point while the head was either aligned with the trunk or tilted 30° to the right or left. Analysis of end-positions of pointing on trunk images acquired by an image analysis system indicated that the trunk midline indeed is perceived as a straight line. Three putative trunk midlines were taken into consideration on the basis of anatomic landmarks, and it was found that the mental representation of the trunk midline came nearest to the line orthogonal to the intermammary line crossing its midpoint. The performing hand and the position of the head relative to the trunk both had an effect on the mental representation of the trunk midline. These findings suggest that somatosensory signals from the trunk, as well as proprioceptive input from the neck, contribute to the elaboration of the subject's mental representation of the trunk midline.

In order to study interocular temporal coupling in the initiation of learned symmetrical blinking, experiments were carried out on cats trained to blink in response to a 500-ms tone paired with 100-ms airpuffs randomly delivered to either eye (alternate airpuff; 6 animals) or simultaneously directed to both eyes (bilateral airpuff; 4 animals) 400 ms after tone onset. In spite of the fact that differences in conditioned response (CR) latencies of the right and left eye varied in a wide range between positive and negative values in all subjects, a statistically significant difference between the mean CR latencies of the two eyes (further called side superiority) was found in 6 animals, of which 4 were trained by alternate airpuff and 2 by bilateral airpuff. Superiority of the right eye was found in 3 animals and the opposite was observed in the other 3. Analysis of the differences between CR latencies of the two eyes showed that side superiority was not due to the ability of one eye to give CRs consistently shorter than those of the other eye, but it crucially depended on the higher proportion of trials in which the superior eye led. The ability to give simultaneous CRs by the two eyes was found inversely related to the mean CR latency per session and subject. Regression analysis showed that power equations best described these relationships. In all animals, the frequency distribution of simultaneous CRs paralleled the frequency distribution of all CRs. In spite of a considerable trial-by-trial variability in the temporal relationships between CR latencies of the two eyes, clear-cut linear correlations were found by plotting the mean CR latencies of the right and left eye per both session and subject. The results reviewed in this paper are best accounted for by suggesting that blink onset of the two eyes is independently controlled by two distinct command signals and is modulated by bilaterally-balanced and lateralized influences.

Purpose In this study, the authors examined the effects of the Teach-Model-Coach-Review instructional approach on caregivers' use of four enhanced milieu teaching (EMT) language support strategies and on their children's use of expressive language. Method Four caregiver–child dyads participated in a single-subject, multiple-baseline study. Children were between 24 and 42 months of age and had language impairment. Interventionists used the Teach-Model-Coach-Review instructional approach to teach caregivers to use matched turns, expansions, time delays, and milieu teaching prompts during 24 individualized clinic sessions. Caregiver use of each EMT language support strategy and child use of communication targets were the dependent variables. Results The caregivers demonstrated increases in their use of each EMT language support strategy after instruction. Generalization and maintenance of strategy use to the home was limited, indicating that teaching across routines is necessary to achieve maximal outcomes. All children demonstrated gains in their use of communication targets and in their performance on norm-referenced measures of language. Conclusion The results indicate that the Teach-Model-Coach-Review instructional approach resulted in increased use of EMT language support strategies by caregivers. Caregiver use of these strategies was associated with positive changes in child language skills.
