Introduction: The etiology of esophageal squamous papilloma (ESP) is largely unknown. Previous studies have shown a variable association with human papillomavirus (HPV) with conflicting data. The aim of this study was to further investigate the possible association of HPV in our ESP series using RNA in-situ hybridization (ISH) and compare study groups from the United States of America and China. Methods: Demographic and clinical data of patients with ESP were retrieved from the University of California Los Angeles (UCLA) (1/2016-3/2019) and Peking Union Medical College Hospital (PUMCH) (9/2014-3/2019) pathology databases. Hematoxylin and eosin slides were reexamined. Confirmed cases were examined by high- and low-risk HPV RNA ISH. Results: For the UCLA cohort, 13 429 upper endoscopies were performed and 78 biopsies from 72 patients were identified as ESP (F:M = 45:27, 66.7% > 45 years). Seventy-four (94.9%) biopsies were designated as polyps or nodules and 46.6% were located in the mid-esophagus. Other abnormal findings included gastroesophageal reflux disease (48.6%), hiatal hernia (38.9%), and esophagitis (36.1%). For the PUMCH cohort, 63 754 upper endoscopies were performed and 73 biopsies from 71 patients were identified as ESP (F:M = 48:23, 71.8% > 45 years). Sixty-four (87.7%) biopsies were designated as polyps or nodules and 57.5% were located in the mid-esophagus. Other abnormal findings included esophagitis (19.7%), and hiatal hernia (8.5%). No features of conventional cytologic dysplasia or viral cytopathic change were found. None of the cases was associated with squamous cell carcinoma, and none showed positive HPV RNA ISH results. Conclusions: No association was found between ESP and active HPV infection in our 2 cohorts. Other etiopathogenetic mechanisms, such as aging, might contribute to the development of these innocent lesions.

This case report details the unusual presentation of a 6-year-old male child who had an asymptomatic, well-defined, brown raised lesion over the vertex area of the scalp since birth. His parents noticed a new growth with finger-like projections over existing old lesions for 1 year. With an initial diagnosis of growth of verruca over nevus sebaceous (NS), a sample for histopathology study was sent which revealed findings consistent with squamous papilloma occurring on NS. This is reported to emphasize that any growth over NS should be confirmed histopathologically due to the rarity of this association and to reduce the possibility of its malignant potential. The article emphasizes how crucial histological analysis is to clarifying mistaken cutaneous lesions and refining an accurate diagnosis.

The clinical appearance of eyelid tumors many times may not concur with the final histopathological diagnosis. A 61-year-old female presented with a hyperpigmented mass in the right upper eyelid of 2-month duration. Clinically, it appeared to be squamous papilloma, a benign lesion. She underwent complete surgical excision of the mass. Histopathologically, the lesion was found to be squamous cell carcinoma, a malignant lesion. There was no recurrence of the mass until the last follow-up. This case report highlights the role of histopathology in aiding the diagnosis of eyelid swellings.

Human papillomavirus (HPV) infection has emerged as an etiologic factor of squamous papilloma (SP). The oropharynx and larynx are common sites of SP, but studies on the prevalence of HPV infection in these sites are lacking. This study aimed to evaluate and compare the prevalence and characteristics of HPV infection in oropharyngeal SP (OPSP) and laryngeal SP (LSP). HPV detection and genotyping data of patients with pathologically confirmed OPSP and LSP were retrospectively analyzed. A total of 119 patients were enrolled, consisting of 93 patients with OPSP and 26 patients with LSP. Of those patients, 13 patients with OPSP and 14 patients with LSP were positive for HPV infection, accounting for a prevalence of 14.0% and 53.8%, respectively (p < 0.001). The most prevalent genotype was HPV16 in OPSP and HPV6 in LSP. Over two-thirds (69.2%) of HPV(+)-OPSP infections were high-risk types compared with 14.3% of HPV(+)-LSP infections (p = 0.004). The prevalence of HPV infection in patients with OPSP and LSP demonstrated no differences in terms of age, sex, and smoking status. These results could provide a better understanding of HPV infection in OPSP and LSP and serve as a background for the epidemiology of HPV-related tumorigenesis of the oropharynx and larynx. We present a case of a 68-year-old lady with an insidious progressive history of otorrhea, otalgia, and reduced hearing for 3 months. She was treated for malignant otitis externa and was treated with systemic antibiotic but showed subtherapeutic response to treatment. She underwent examination under anaesthesia and biopsy with tumour debulking of the right ear via posterior auricular approach and histopathological examination revealed squamous papilloma of the external auditory canal. It has been suggested that CD34 may be related to invasive potential. Aim: To evaluate the expression of the CD34 antigen in the stroma of benign and malignant breast lesions using the Immunohistochemistry (IHC) method. Postsurgical specimens, including biopsies and mastectomies, were fixed in 10% neutral buffered formalin. The initial diagnosis was made by Haematoxylin and Eosin (H&E)- stained tissue sections. Sections from the same block were subjected to IHC staining using a monoclonal antibody to the CD34 antigen, and the intensity of expression in stromal cells was graded from 0 to 3+ based on nuclear positivity: up to 5% stromal cells immunoreactive (grade-0), >5% and up to 25% (grade-1), >25% and up to 50% (grade-2), and > 50% (grade-3). The data was entered into Microsoft Excel 2007 and analysed using Statistical Package for Social Sciences (SPSS) statistical software version 19.0. The inferential statistics included the use of the Chi-square test. Results: Out of the total 50 cases evaluated, 25 (50%) were categorised as benign, and rest 25 (50%) were categorised as malignant lesions. Among the benign lesions, the most common lesion was fibroadenoma, accounting for 9 (36%) cases, followed by benign phyllode tumour, with 6 (24%) cases. Among malignant lesions, the most common lesion was Invasive Ductal Carcinoma (IDC), accounting for 23 (92%) cases. The intensity of CD34 expression was found to be significantly higher in benign as compared to malignant breast lesions. Out of the 25 benign breast cases, the maximum number of cases (19) showed Grade-3+ positivity, 5 cases showed Grade-2+, and 1 case showed Grade-0 positivity. However, among malignant lesions, maximum cases (23, 92%) showed Grade-0 on CD34 immunostaining. Conclusion: This study was undertaken to evaluate the CD34 expression in benign and malignant breast lesions which might be able to distinguish between the benign and malignant breast lesions. The most benign breast lesions express CD34 in the stroma, while it is almost totally lost in all malignant neoplasms. Loss of CD34 is associated with the tumour’s capacity for invasion. However, to determine its function as a therapeutic target in cases of breast cancer, additional research can also be done.

Background: Recurrent respiratory papillomatosis (RRP) is the most common benign mesenchymal neoplasm of the larynx caused by the human papillomavirus (HPV). This condition has a childhood-onset and adult-onset type, which historically has an inclination toward malignant transformation. Case description: A detailed case discussion of two patients with two clinically distinct and dissimilar lesions on two different subsites of the larynx. Both patients were clinically diagnosed to have laryngeal papilloma, with a second malignant lesion. In case 1, the final histopathology of the malignant lesion was found to have papillomatous fronds with evidence of severe dysplasia. In case 2, histopathology of the malignant lesion revealed invasive squamous cell carcinoma with features of papilloma. Both patients revealed "no type" in viral typing. Conclusion: Malignant transformation in adult laryngeal papilloma may occur as a relatively acute phenomenon that may present at the first surgical intervention. It is often associated with the "no type" HPV entity. The presence of squamous papilloma and carcinoma of the larynx, even at two different subsites, should be histologically confirmed and considered a malignant transformation of the laryngeal papilloma. Introduction: Squamous papillomas (SPs) are benign epithelial neoplastic lesions commonly found in skin, oral mucosa, upper aerodigestive tract and genitals. Involvement of external auditory canal (EAC) by SP is extremely rare and whatever scarce literature is available is in the form of case reports. The exact incidence of this entity is still unknown. Consequently, its etiology, histopathological features, differential diagnoses and optimal management merit a better understanding by the Otolaryngologists. Case report: We present a case of EAC SP in a 15 year old girl hailing from north-eastern India who presented to us with a mass in the EAC which was acutely painful. Contrast Enhanced Computed Tomography remained the gold standard and imaging study of choice for evaluation. However, the diagnosis was clinched only by histopathology. We also discuss our strategy of management along with a brief review of literature. Conclusion: Squamous papilloma has rarely been reported to affect the EAC. Human papilloma virus remains the most consistent etiological agent. Advanced imaging like contrast enhanced high resolution computed tomogram remains gold standard to evaluate any mass seen in EAC. Complete excision with cold steel technique or coblation with good post-operative care and follow up is the standard of care.

Simple Summary: Studies on human papillomavirus (HPV) infection in oropharyngeal squamous papilloma (OPSP) are lacking, although HPV is a critical oncogenic virus for the development of oropharyngeal cancer. This study evaluated the prevalence and characteristics of HPV infection in OPSP and showed a 14.5% overall prevalence. High-risk HPV accounts for 75% of all HPV infections, with HPV16 being the most prevalent genotype, accounting for 58.3% of all HPV infections. There was a trend toward a higher prevalence of high-risk HPV infection in patients with OPSP aged ≤45 years, never-smokers, and those with multifocal diseases, which corresponded with the clinicodemographic profiles of HPV-associated oropharyngeal cancer. These findings could enhance our understanding of HPV infection in OPSP and could be used as valuable epidemiological data for the management of HPV-associated OPSP. A recent article in the International Journal of Surgical Pathology discusses the detection of human papillomavirus (HPV) in esophageal squamous papillomas (ESPs). The authors conducted an analysis using RNA in situ hybridization (ISH) and found that none of the cases tested positive for HPV RNA ISH, leading them to conclude that HPV is unlikely to be involved in the development of ESPs. However, the authors also noted the presence of RAS mutations in ESPs, suggesting that oncogenic driver mutations may play a role in their occurrence. AIM: To evaluate the demographic features, location, and histopathologic results in primary eyelid tumors. METHODS: We retrospectively reviewed the records of patients with primary eyelid tumor diagnosed and treated between November 1997 and June 2016 on our service. RESULTS: Nine hundred and eleven lesions from 874 patients were included in this study. Four hundred and forty-six (51.0%) of the patients were females and 428 (49.0%) were males. The mean age was 51.0y (range: 3mo-94y). The lesions were located in the upper eyelid (418 cases, 45.9%), lower eyelid (378 cases, 41.5%), medial canthus (89 cases, 9.8%), and lateral canthus (26 cases, 2.8%). Four hundred and seventy (51.6%) lesions were located on the right side and 441 (48.4%) on the left. Of the eyelid lesions, 666 (73.1%) were benign, 230 (25.2%) malignant, and 15 (1.6%) premalignant. When the eyelid tumors were classified according to their tissue or cell of origin, 527 (57.8%) of the lesions were found to be epidermal, 171 (18.8%) adnexal, 131 (14.4%) inflammatory and infectious, and 65 (7.1%) stromal. The most common benign lesions were squamous cell papilloma (139 lesions, 15.2%), intradermal nevus (97 lesions, 10.6%), epidermoid inclusion cysts (78 lesions, 8.6%), seborrheic keratosis (60 lesions, 6.6%), and inflammatory masses (59 lesions, 6.5%). Basal cell carcinoma was the most common malignant tumor (191 lesions, 21.0%) followed by squamous cell carcinoma (16 lesions, 1.8%), sebaceous carcinoma (14 lesions, 1.5%), and malignant melanoma (5 lesions, 0.5%). CONCLUSION: In this study, 73.1% of eyelid lesions are benign and the remaining 26.9% are premalignant and malignant. Basal cell carcinoma is the most common tumor among all histopathological diagnosis followed by squamous papilloma.

No such case has previously been reported in the literature. Conclusion: This report highlights the need for histopathological examination of all eyelid lesions to enable surgeons to prognosticate the disease.

Intraoral squamous papilloma is an uncommon lesion which presents as a verrucous, exophytic or papillary growth of the oral mucosa. Human Papilloma Virus (HPV) is often detected associated with this condition but HPV may be an incidental finding. In this report we present the surgical management of squamous papilloma of hard palate and discuss its association with HPV.

A 45-year-old male patient went to our observation for a white lesion in the left lower gum. At the anamnesis he reported previous diagnosis of oral erosive and genital lichen planus (discovered 2 and 12 years before respectively) and the excision of 2 small oral squamous papillomas 2 year ago. A PCR-real time test using a saliva sample was carried out a year ago, but it did not confirm the presence of the HPV DNA. During the clinical examination we observed a white not scrapable lesion of 2.2cm in the vestibular gum around the teeth 3.6-3.7. The lesion is not linked with a traumatic event and we decided to make an incisional biopsy to verify if it was another injury linked to the spread of oral erosive lichen planus or a new entity. The histological examination confirmed a diagnosis of oral HPV lesion, precisely squamous papilloma, but in-situ hybridation did not find HPV DNA. The presence of a mild dysplasia with unaffected margins was also confirmed. At our first knowledge, this is the first documented case in literature of an oral squamous papilloma with a keratin-like appearance, rather than exophytic and/or cauliflower form. The differential diagnosis could be with oral lichen planus, leukoplakia or verruca vulgaris. The patient was advised of the risk of malignancy connected with the simultaneous presence of oral erosive lichen planus and HPV lesions.

Oral squamous papillomas are benign proliferating lesions induced by human papillomavirus. These lesions are painless and slowly-growing masses. As an oral lesion, it raises concern because of its clinical appearance. These lesions commonly occur between age 30 and 50 years, and sometimes can occur before the age of 10 years. Oral squamous papilloma accounts for 8% of all oral tumors in children. Common site predilection for the lesion is the tongue and palate and may occur on any other surface of the oral cavity such as the uvula and vermilion of the lip. Here, we are presenting a case of squamous papilloma on the palate in a 9-year-old child.

Objectives: In the larynx, differentiating squamous papillomas from de-novo papillary squamous dysplasias or squamous cell carcinomas (SCC) has significant consequences for management. Overlapping clinical presentations and cytologic changes across the spectrum of papillary lesions presents diagnostic challenges for otolaryngologists and pathologists. In this study, we evaluate whether ribonucleic acid (RNA) in-situ hybridization (ISH) for low-risk and high-risk human papillomavirus (HPV) can help distinguish these lesions.Methods: We constructed tissue microarrays from 97 papillary laryngeal lesions, including 61 squamous papillomas, two papillomas with dysplasia, two SCCs-ex papilloma, 14 papillary squamous dysplasias, and 18 papillary SCCs identified at the Johns Hopkins Hospital between 2000 and 2017. We performed RNA ISH using probes for low-risk and high-risk HPV types.Results: Low-risk HPV RNA was identified in 55 benign papillomas (90%), two papillomas with dysplasia (100%), and two SCCs-ex papilloma (100%) but was absent in de-novo papillary dysplasias and SCCs (0%). High-risk HPV RNA ISH was positive only in four papillary SCC (22%). Overall, low-risk HPV RNA ISH was 90% sensitive and 89% specific for benign papillomas with a positive predictive value of 93% and negative predictive value of 84%. In contrast, high-risk HPV was 20% sensitive for SCC.Conclusion: Low-risk HPV RNA ISH is a useful diagnostic adjunct for distinguishing laryngeal squamous papillomas from papillary squamous dysplasia and SCC. However, it is not entirely specific for benign processes as it is also retained in papillomas with dysplasia and SCCs-ex papilloma. Epulis fissuratum also called as Denture-induced hyperplasia (DIH) is a tumor-like hyperplasia of fibrous connective tissue, which develops in association with an ill-fitting complete or partial denture. Virchoft first invented the term "Epulis" and the meaning is "over the gums". However the use of this term is not appropriate as it emphasizes only the actual site of the lesion. In most cases, the affected mucosa is usually the oral mucosa of the vestibular sulcus or the palatal region. Therefore, "Denture-induced fibrous hyperplasia" was considered to be a much-preferred term. Ocular surface squamous neoplasia (OSSN) may have atypical or unusual presentations and may attain large sizes especially in cases of delayed presentation resulting in late diagnosis, treatment, and eventual guarded visual prognosis. We are reporting an interesting cases series of OSSN with variable clinical presentations to highlight the importance of the pre-operative clinical judgment and tissue diagnosis. Six patients (4 females and 2 males; mean age 59 years; range 42–79 years) were included with suspicious conjunctival lesions. The maximum dimension of the lesions was 17 mm. The initial suspected pre-operative clinical diagnosis -other than OSSN- included pterygium/pinguecula (n = 2), benign squamous papilloma (n = 1), cyst versus pyogenic granuloma (n = 1), and lymphoma (n = 1). This work has been reported in line with the PROCESS criteria. The final histopathological diagnosis was unexpectedly invasive squamous cell carcinoma (SCC) in 4, one SCC in-situ, and squamous dysplasia in one. The primary treatment included MMC 0.02 % for 2 cycles for chemo-reduction in one of the cases where OSSN was suspected. Excisional biopsy was performed eventually for all lesions with application of one or more of the following modalities: MMC 0.02 %, absolute alcohol 99 %, and cryotherapy to the conjunctival margin. No tumor recurrence was noted in any of the patients after an average period of follow-up of 26 months. Even though the outcome was good, tumor-related morbidity and delay in the referral by general ophthalmologists are to be considered. Conjunctival OSSN has wide presentation clinically and can be challenging in terms of diagnosis. Histopathological evaluation is essential for a definitive diagnosis and treatment. Accurate clinical diagnosis might affect the management plan with consideration for topical therapeutic modalities, however, these cases are best managed by wide excision using the no-touch technique and double-freeze-thaw cryotherapy to the conjunctiva with consideration of topical chemotherapy. • Ocular surface squamous neoplasia (OSSN) may have variable and often atypical clinical presentation. • Histopathological evaluation is essential for a definitive diagnosis and treatment. • We present six conjunctival OSSN lesions with diagnostic challenges at initial presentation. • Wide excision of OSSN lesions with local therapy (cryotherapy and/or topical chemotherapy) is advisable. Oral squamous papilloma is a benign tumor of unknown origin which arises from stratified squamous epithelium and appears as a cauliflower-like growth with finger-like projections seen on the tongue, palate, and other parts of oral cavity. The treatment modalities include surgery, cold steel excision, cryosurgery, and laser ablation. Here, we report a case of squamous papilloma on buccal mucosa treated with a diode laser.

Human papillomaviruses (HPVs) are causative of a group of clinically papillary lesions. The HPV-related lesions of the oral cavity include squamous papilloma, condyloma acuminatum, verruca vulgaris, and multifocal epithelial hyperplasia. Benign entities, such as verruciform xanthoma or giant cell fibroma, as well as malignancies, such as papillary squamous cell carcinoma and verrucous carcinoma, may be considered in the clinical and/or histologic differential diagnoses of these lesions. Mechanisms of infection, epidemiology, clinical presentations, histologic features, and differential diagnoses of the HPV-related oral pathologies are discussed. Current concepts of viral transmission, especially as pertaining to lesions in pediatric patients, and the impacts of HPV vaccination are reviewed. [ABSTRACT FROM AUTHOR]

Canalicular papillomatosis is a rare disorder characterized by a mass lesion arising from the epithelium as a stalk from one of the canalicular walls. Traditionally, they have been treated with an open canaliculotomy and excision biopsy with or without additional cryotherapy. A patient with upper canalicular squamous papillomas treated with dacryoendoscopy-guided transcanalicular intralesional and topical interferon alpha 2b is presented, and the ineffectiveness of interferons in this case is discussed. We report a rare case of extensive esophageal squamous papillomas (ESPs) involving the entire esophagus and florid cardiac gland hyperplasia involving only the lower esophagus in a 39-year-old woman with heartburn and epigastric distress for the past 2 years. Previous esophagogastroduodenoscopy showed multiple ESPs involving the entire esophagus extending 38 cm from the esophageal orifice to the esophagogastric junction (EGJ). Additionally, prominent cardiac gland hyperplasia over the esophageal posterior wall was exhibited extending 12 cm from the mid-esophagus to the EGJ. A biopsy obtained from the ESP area showed typical squamous papillomas and cardiac gland hyperplasia with no evidence of koilocytosis or malignancy. Polymerase chain reaction was negative for a variety of human papilloma virus DNAs. Subsequently, Siewert type II gastric cancer with submucosal elevation of the stomach was detected at the EGJ. Endoscopy showed a 20-mm-thick lesion appearing to extend to the muscularis propria; subsequent biopsy showed invasive adenocarcinoma. Total gastrectomy with D2 lymph node dissection, splenectomy, and Roux-en-Y reconstruction were performed for the EGJ cancer. The patient died from widespread multiorgan metastasis within 2 years following surgery.
