INTRODUCTION Intensity-modulated radiotherapy (IMRT) improves planning target volume (PTV) coverage and organ at risk (OAR) avoidance in the brain compared to three-dimensional conformal radiotherapy (3-D CRT). However, IMRT is time consuming with considerable resource implications. Volumetric modulated arc therapy (VMAT) offers a quicker method of delivering an IMRT plan and requires less monitor units. Planning studies have shown VMAT compares well to IMRT in the brain but there is little data regarding clinical experience, particularly in fractionated schedules. METHODS Prospectively collected data (treatment details, delivery parameters, dosimetry, time, toxicity, and quality assurance) from the first 10 patients treated with RapidArc (a VMAT product) for brain tumors at our institution was analyzed in an observational study to establish whether the theoretical advantages of VMAT in the treatment of brain tumors translate into clinical practice. RESULTS Patients were selected for RapidArc due to the proximity of PTV to OAR and complex geometric tumor volumes. These included a range of tumor sites, histology, treatment volumes, and dose schedules. In all cases the PTV received >95% prescribed dose and dose to OAR was within tolerance. One patient required a second noncoplanar arc. Quality assurance parameters were easily met. Treatment time and monitor units were approximately half that taken to deliver IMRT. Patients tolerated treatment well and experienced minimal alopecia. CONCLUSION In practice RapidArc provides excellent radiotherapy plan conformity for a wide range of brain tumors with reduced treatment times and monitor units compared to IMRT. When asked to comment about the high national imprisonment rate, Garside refers to a national embarrassment. He mentions that the financial aspect is the biggest problem in the prison system. He also discusses his perspective as an abolitionist regarding prison reforms.

An interview with Jo Wells, manager at HMP Dorchester prison located in England is presented. Topics discussed include reaction of prisoners after the announcement of closer of the prison, proposal for prison reformation and the response of the trade unions of the prison on the same. He further discusses the lesson which he took from the closer.

An interview with George Pearce, father of a serving prisoner, is presented. When asked to his views regarding the consequences when prisons are full, Pearce refers to the burden for tax payers. He talks about how prison has held his son rehabilitate from drug usage. He also discusses the need to lower the number of years for prison sentences to achieve remission.

An interview with Janet Brookers, a mother of a serving prisoner, is presented. When asked about her views regarding the consequences when prisons are full, Brookers refers to the inability to get on the course. She believes that imprisonment makes it harder for prisoners to make positive changes to their lives. She emphasizes the sense of stigma as a family member is one aspect of being the relative of a prisoner that people outside are least aware of.

The article discusses the work of the charity, Centre for Crime and Justice Studies in establishing a link between the criminal justice system and mentions how it contributes to issues relating to poverty. Topics discussed include the impact of conviction and imprisonment on the poverty, examination of the links between criminal records and poverty, and the failure of the prison-based interventions such as job preparation and employment skills in addressing issues relating to poverty.

Purpose: The purpose of this study was to provide guidance on quality management for electronic brachytherapy., Materials and Methods: The task group used the risk-assessment approach of Task Group 100 of the American Association of Physicists in Medicine. Because the quality management program for a device is intimately tied to the procedure in which it is used, the task group first designed quality interventions for intracavitary brachytherapy for both commercial electronic brachytherapy units in the setting of accelerated partial-breast irradiation. To demonstrate the methodology to extend an existing risk analysis for a different application, the task group modified the analysis for the case of post-hysterectomy, vaginal cuff irradiation for one of the devices., Results: The analysis illustrated how the TG-100 methodology can lead to interventions to reduce risks and improve quality for each unit and procedure addressed., Conclusion: This report provides a model to guide facilities establishing a quality management program for electronic brachytherapy., (© 2019 American Association of Physicists in Medicine.)
