Abstract

With the establishment of the Plant Health Panel in 2006, EFSA became the body responsible for risk assessment in the plant health area for the European Union (EU). Since then more than 70 outputs have been produced dealing with the full range of organisms harmful to plant health across all crop types and plants in the environment. There has been an increasing trend towards producing scientific opinions which are full pest risk assessments for the whole EU territory. In its work, and as a contribution to the wider development of risk assessment methodology, the Panel has developed a series of guidance documents. These deal with the peer review of existing pest risk assessments, a framework for conducting risk assessments which harmonise standards set by the International Plant Protection Convention and the legislative requirements of the EU, and extension of this framework to include environmental risk assessment and the evaluation of risk reducing options. Quantitative approaches have become increasingly important during this time. The Panel has developed such methods in climatic mapping (in association with the Joint Research Councils), application of spatial spread models, re-evaluation of quantitative pathway analyses, and in statistical modelling of experimental data. A Plant Health Network has been established to facilitate interaction with EU Member States, especially in relation to data collection and co-ordination of risk assessment activities. At the current time a revision of the EU Plant Health Regime is being formulated. The legislative consequences of the revision will be of considerable significance for the work of the Plant Health Panel.

Abstract

The present work aims to identify the predictors of hemodynamic failure (HF) developed during pediatric intensive care unit (PICU) stay testing a set of machine learning techniques (MLTs), comparing their ability to predict the outcome of interest. The study involved patients admitted to PICUs between 2010 and 2020. Data were extracted from the Italian Network of Pediatric Intensive Care Units (TIPNet) registry. The algorithms considered were generalized linear model (GLM), recursive partition tree (RPART), random forest (RF), neural networks models, and extreme gradient boosting (XGB). Since the outcome is rare, upsampling and downsampling algorithms have been applied for imbalance control. For each approach, the main performance measures were reported. Among an overall sample of 29,494 subjects, only 399 developed HF during the PICU stay. The median age was about two years, and the male gender was the most prevalent. The XGB algorithm outperformed other MLTs in predicting HF development, with a median ROC measure of 0.780 (IQR 0.770–0.793). PIM 3, age, and base excess were found to be the strongest predictors of outcome. The present work provides insights for the prediction of HF development during PICU stay using machine-learning algorithms.

8. An updated assessment of the risks to plant health posed Xylella fastidiosain the EU territory

Abstract

Trabajo presentado en la 2nd European conference on Xylella fastidiosa (how research can support solutions), celebrada en Ajaccio el 29 y 30 de octubre de 2019., EFSA received a request from the European Commission to update its 2015 Pest Risk Assessment (PRA) for Xylella fastidiosa in the EU territory. The update focused on potential establishment, short and long-range spread, the length of the asymptomatic period, the impact of X. fastidiosaand an update on risk reduction options, accounting for the different subspecies and Sequence Types of X. fastidiosaduring the risk assessment where data availability allowed. Here we summarise the main conclusions of the updated PRA, the uncertainties associated with the conclusions, and recommended futuredirections for research and data collection to reduce these uncertainties. The quantitative PRA provided new insights on containing existing outbreaks and preventing further spread in the EU. For example, it was demonstrated that most of the EU has potential climatic suitability for Xylella but that the southern EU is most at risk;however,X. fastidiosasubsp. multiplexdemonstrated areas of potential establishment further north in Europe compared with other subspecies. Modelling was also used to assess data from a wide range of studies on the length of the asymptomatic period, as well as to simulate the spread of the disease and management measures aimed at eradicating or containing the pathogen. The assessment showed the importance of disease control measures, including the use of buffer zones and plant removal, but also the need to couple these measures with effective vector control, rapid implementation of management measures following disease discovery, and early detection of new positive cases. The latter issue is a particular challenge for Xylellagiven the length of the asymptomatic period, particularly for some host and subspecies combinations. In a separate mandate EFSA is developing new surveillance guidelines for Xylellato enable for more effective and targeted detection surveys for the disease.

Abstract

The European Food Safety Authority (EFSA) has been requested by the European Commission to prepare the pest categorisation for two large groups of non-EU viruses and virus-like organisms, i) those infecting the genera Cydonia, Fragaria, Malus, Prunus, Pyrus, Ribes, Rubus and Vitis (fruit plants) and ii) those infecting potatoes. To accomplish these tasks, the EFSA Scientific Panel on Plant Health has established two Working Groups (WGs), which are respectively supported by two Framework Partnership Agreements, with the Institute for Sustainable Plant Protection-CNR, Italy (for the pest categorisation of viruses and viroids of fruit plants) and the Netherlands Food and Consumer Product Safety Authority (for the viruses and viroids of potatoes). The methodology adopted by both WGs to perform a pest categorization of these two large groups of pathogens includes the following steps: i) extensive literature searches to list the viruses and viroids infecting one or more of the host genera under consideration and to collect evidence on their geographic distribution; ii) identification of the viruses/viroids not known to occur in the European Union (EU) or occurring only in restricted areas of the EU (non-EU viruses) and of the viruses/viroids associated with inconclusive information to support their inclusion in the list of non-EU viruses (viruses with undetermined standing), iii) consultation with EU Member States to amend, if needed, the lists of viruses/viroids; iv) further categorization of viruses and viroids determined to be non-EU or with undetermined standing to evaluate whether they meet the requirements to potentially qualify as European Union quarantine pests. In the first step of this process, about 200 and 80 viruses/viroids infecting fruit plants and potatoes were identified, respectively. The lists of non-EU viruses/viroids and of viruses/viroids with undetermined standing for fruit plants and potatoes will be presented and an example for the pest categorisation of those of Cydonia, Malus and Pyrus will be discussed in more details. The most critical uncertainties in the pest categorization process regard information on distribution, host range, transmission, impact, and intraspecific diversity, particularly for those viruses recently discovered by high-throughput sequencing. The importance of these data for the pest categorization process will be discussed.

A Framework Partnership Agreements has been signed between EFSA and the Institute for Sustainable Plant Protection-CNR (Italy) to jointly perform the pest categorization of this large group of pathogens. To this aim, a specific strategy has been developed including the following: a preliminary step, in which a list of viruses/viroids that must be categorized was generated; such a list includes viruses/viroids that infect the reported plant genera and that are not known to occur in the EU or known to occur only in restricted areas of the EU, and those associated with inconclusive information on their presence in the EU. After a consultation phase with EU Member States, the categorization of viruses of interest started. The pest categorization of these viruses will be presented in several EFSA scientific opinions addressing the non-EU viruses infecting the reported host genera. The state of the art and the most critical uncertainties in the pest categorization process will be briefly presented.

Trabajo presentado en la 2nd European conference on Xylella fastidiosa (how research can support solutions), celebrada en Ajaccio el 29 y 30 de octubre de 2019., Early detection of the new outbreaks of Xylella fastidiosaand knowledge of the disease dynamics are key elements for an effective management of new foci. These elements where explored in the Update of the Scientific Opinion on the risks to plant health posed by Xylella fastidiosain the EU territory, recentlypublished by EFSA. In this opinion,a short-range spread model was used to investigate the spatial dynamics of a new outbreak in a free area and to comparatively assess the control measures aimed at local eradication of the disease. A process-based approach was used to describe the bacterial growth in a plant in relation to the symptoms/disease progression, the population dynamics of the spittlebug and the vector-mediated transmission. The model parameters were derived from the data acquired on the spread of X. fastidiosasubsp. paucain olive groves in the Apulia region. Four epidemiological scenarios were considered combining host susceptibility and vector abundance. Four management options were considered to account for the timeline for the detection and for the implementation of control measures, efficacy of vector control and plant removal. Simulation results showed that the spread rate of the disease increases over time with anon-linear pattern depending on the scenario components. High efficacy of nymph and adult vector control and short delay in detection and implementation of control measures are key factors for the successful eradication of an outbreak in a free area. Model structure and flexibility make it possible to explore a wide range of conditions to account for different vector species, bacterial strains, vegetation components, landscape structures (homogeneous, heterogeneous but continuous, patchy) and combinations of management options. This makes the model a suitable tool to support decision making for the drafting and management of emergency plans related to new outbreaks.

The Scientific Panel on Plant Health (PLH) of the European Food Safety Authority conducts risk assessment of plant pests and pathogens, providing scientific advice to the Commission, Parliament and Member States of the European Union (EU) and supporting science-based decisions on plant health and quarantine issues. In 2011 and 2012 the PLH panel conducted pest risk assessments for the EU of pospiviroids: Potato spindle tuber viroid, Citrus exocortis viroid, Columnea latent viroid, Mexican papita viroid, Tomato apical stunt viroid, Tomato chlorotic dwarf viroid, Tomato planta macho viroid, Chrysanthemum stunt viroid and Pepper chat fruit viroid. The PLH Panel also identified and evaluated risk reduction options. The key biological and epidemiological features of pospiviroids, including a relatively broad host range, the long distance spread via vegetatively propagated crops and, at least in some instances, seed- and/or insect-mediated transmission, have been analysed to assess the likelihood of introduction and spread as well as the potential impact of pospiviroids on solanaceous crops and on other ornamental species in the EU. The results of the assessments are presented and discussed in terms of how these features of pospiviroids determine their quarantine risk.

Abstract

A multicentric Italian investigation on legionnaires' disease is in course to clarify host factors as well as pathogen associated characteristics involved in the infection/disease. The main goal of the research plan is to account for some critical aspects concerning identification and prevention of legionellosis. To improve knowledge on factors associated with Legionella spp colonisation in hot waters, to detect cases and to characterize risk factors in subjects which develop pneumonia are specific objectives of the research programme. Preliminary results show that hot waters of houses and hotels are frequently contaminated (22.6% and 54.6%, respectively), mainly by L. pneumophila. Microbial concentrations were low in domestic waters (1.000 ufc/l), but higher in samples from the hotels (geom. mean 1.85 x 10(3) ufc/l). Warming system, age of the plant, type of building were risk factors significantly associated with Legionella spp positivity. The active surveillance on patients affected by pneumonia with search for Legionella urinary antigen allowed the identification of 34 cases, 3 of which of nosocomial origin, corresponding to 4.2% of the screened pneumonia. After informed consent, 26 subjects were recruited for a case-control-study to clarify risk factors for the disease.
