Interest in joint and segment contributions to pitched ball velocity has been dominated by inverse dynamic solutions, which is limited in ascertaining complex muscle/joint interactions. Our purpose was to use induced velocity analysis to investigate which joint(s) made the largest contribution to the velocity of a pitched ball. Pitching data were collected from six elite high school-aged pitchers with no history of arm injury. Participants threw a fastball pitch from the windup on flat ground. Data were collected using seven Vicon 612 cameras (250 Hz) and three AMTI force platforms (1000 Hz). A 14-segment biomechanical model (feet, legs, thighs, pelvis, a combined thorax-abdomen-head, i.e., trunk, upper arms, forearms, and hands) was implemented in Visual3D as a dynamic link library built using SD/Fast (PTC) software. Model-generated induced velocity of the ball was validated against ball velocity obtained from a calibrated radar gun. Velocity induced torques at the shoulder just prior to release, and elbow during the cocking phase, contributed 31.0% and 18.1%, respectively, to forward ball velocity. The centripetal/Coriolis effects from the upper arm and forearm velocities made the largest contribution to ball velocity (average 57.8%), but the source of these effects are unknown. The lower extremities and trunk made little direct contribution to pitched ball velocity. An objective understanding of human foot and ankle function can drive innovations of bio-inspired wearable devices. Specifically, knowledge regarding how mechanical force and work are produced within the human foot-ankle structures can help determine what type of materials or components are required to engineer devices. In this study, we characterized the combined functions of the foot and ankle structures during walking by synthesizing the total force, displacement, and work profiles from structures distal to the shank. Eleven healthy adults walked at four scaled speeds. We quantified the ground reaction force and center-of-pressure displacement in the shank's coordinate system during stance phase and the total mechanical work done by these structures. This comprehensive analysis revealed emergent properties of foot-ankle structures that are analogous to passive springs: these structures compressed and recoiled along the longitudinal axis of the shank, and performed near zero or negative net mechanical work across a range of walking speeds. Moreover, the subject-to-subject variability in peak force, total displacement, and work were well explained by three simple factors: body height, mass, and walking speed. We created a regression-based model of stance phase mechanics that can inform the design and customization of wearable devices that may have biomimetic or non-biomimetic structures., Competing Interests: The authors have declared that no competing interests exist.

A real-time locating system (RTLS) can be used to track the walking activity of institutionalized older adults in long-term care who are at risk for wandering behaviors. The benefits of a RTLS are objective and continuous measurements of activity. Self-report methods of activity, especially wandering, by health care staff are vulnerable to floor effects and recall bias, and continuous clinical or research observation over the long-term can be time-consuming and expensive. Health care staff also fail to recognize the onset and/or duration of wandering behaviors, which are associated with a variety of adverse health outcomes in this population but amenable to intervention. RTLS technologies can measure the walking activity of institutionalized residents with cognitive impairment over time with a high degree of accuracy. This is particularly useful for the study of wandering, defined as walking for at least 60 seconds with few (if any) breaks in activity. Wandering is associated with disease progression, hospitalizations, falls and death. Previous work suggests older adults with poor balance ability and high sustained walking activity may be particularly susceptible to poor health outcomes. RTLS's are used to assess cognitive impairment and factors associated with gait and balance; however, supplemental paper and pencil gait/balance tools may be used to further refine risk profiles. This project discusses the use of a RTLS to measure walking activity and also gait quality and balance ability measures on this population.

Purpose of Study: The purpose of this study was to determine the influence of cognitive impairment (CI), 1 gait quality, and balance ability on walking distance and speed in an assisted living facility., Materials and Methods: This was a longitudinal cohort study of institutionalized older adults (N = 26; 555 observations) followed for up to 8 months. Hierarchical linear modeling statistical techniques were used to examine the effects of gait quality and balance ability (using the Tinetti Gait and Balance Test) and cognitive status (using the Montreal Cognitive Assessment) on walking activity (distance, sustained distance, sustained speed). The latter were measured objectively and continuously by a real-time locating system (RTLS)., Results: A one-point increase in balance ability was associated with an 8% increase in sustained walking distance (p = 0.03) and a 4% increase in sustained gait speed (p = 0.00). Gait quality was associated with decreased sustained gait speed (p = 0.03). Residents with moderate (ERR = 2.34;p = 0.01) or severe CI (trend with an ERR = 1.62; p = 0.06) had longer sustained walking distances at slower speeds when compared to residents with no CI., Conclusions: After accounting for cognitive status, it was balance ability, not gait quality, that was a determinant of sustained walking distances and speeds. Therefore, balance interventions for older adults in assisted living may enable sustained walking activity. Given that CI was associated with more sustained walking, limiting sustained walking in the form of wandering behavior, especially for those with balance impairments, may prevent adverse events, including fall-related injury., (Published by Elsevier B.V.)

Segmental power is used in human movement analyses to indicate the source and net rate of energy transfer between the rigid bodies of biomechanical models. Segmental power calculations are performed using segment endpoint dynamics (kinetic method). A theoretically equivalent method is to measure the rate of change in a segment's mechanical energy state (kinematic method). However, these two methods have not produced experimentally equivalent results for segments proximal to the foot, with the difference in methods deemed the "power imbalance." In a 6 degree-of-freedom model, segments move independently, resulting in relative segment endpoint displacement and non-equivalent segment endpoint velocities at a joint. In the kinetic method, a segment's distal end translational velocity may be defined either at the anatomical end of the segment or at the location of the joint center (defined here as the proximal end of the adjacent distal segment). Our mathematical derivations revealed the power imbalance between the kinetic method using the anatomical definition and the kinematic method can be explained by power due to relative segment endpoint displacement. In this study, we tested this analytical prediction through experimental gait data from nine healthy subjects walking at a typical speed. The average absolute segmental power imbalance was reduced from 0.023 to 0.046 W/kg using the anatomical definition to ≤0.001 W/kg using the joint center definition in the kinetic method (95.56-98.39% reduction). Work can reveal the mechanism by which movements occur. However, work is less physically intuitive than more common clinical variables such as joint angles, and are scalar quantities which do not have a direction. Therefore, there is a need for a clearly reported and comprehensively calculated approach to easily visualize and facilitate the interpretation of work variables in a clinical setting. We propose the Constituent Lower Extremity Work (CLEW) approach, a general methodology to visualize and interpret cyclic tasks performed by the lower limbs. Using six degree-of-freedom power calculations, we calculated the relative work of the four lower limb constituents (hip, knee, ankle, and distal foot). In a single pie chart, the CLEW approach details the mechanical cost-of-transport, the percentage of positive and negative work performed in stance phase and swing phase, and the individual contributions of positive and negative work from each constituent. This approach can be used to compare the constituent-level adaptations occurring between limbs of individuals with impairments, or within a limb at different gait intensities. In this article, we outline how to generate and interpret the CLEW pie charts in a clinical report. As an example of the utility of the approach, we created a CLEW report using average reference data from eight unimpaired adult subjects walking on a treadmill at 0.8 statures/s (1.4m/s) compared with data from the intact and prosthetic limbs of an individual with a unilateral amputation walking with an above-knee passive prosthesis., (Copyright © 2017 Elsevier B.V. The modulation of walking speed results in adaptations to the lower limbs which can be quantified using mechanical work. A 6 degree-of-freedom (DOF) power analysis, which includes additional translations as compared to the 3 DOF (all rotational) approach, is a comprehensive approach for quantifying lower limb work during gait. The purpose of this study was to quantify the speed-related 6 DOF joint and distal foot work adaptations of all the lower extremity limb constituents (hip, knee, ankle, and distal foot) in healthy individuals. Relative constituent 6 DOF work, the amount of constituent work relative to absolute limb work, was calculated during the stance and swing phases of gait. Eight unimpaired adults walked on an instrumented split-belt treadmill at slow, moderate, and typical walking speeds (0.4, 0.6, and 0.8 statures/s, respectively). Using motion capture and force data, 6 DOF powers were calculated for each constituent. Contrary to previously published results, 6 DOF positive relative ankle work and negative relative distal foot work increased significantly with increased speed during stance phase (p<0.05). Similar to previous rotational DOF results in the sagittal plane, negative relative ankle work decreased significantly with increased speed during stance phase (p<0.05). Scientifically, these findings provide new insight into how healthy individuals adapt to increased walking speed and suggest limitations of the rotational DOF approach for quantifying limb work. The Bridging Advanced Developments for Exceptional Rehabilitation (BADER) Consortium began in September 2011 as a cooperative agreement with the Department of Defense (DoD) Congressionally Directed Medical Research Programs Peer Reviewed Orthopaedic Research Program. A partnership was formed with DoD Military Treatment Facilities (MTFs), U.S. Department of Veterans Affairs (VA) Centers, the National Institutes of Health (NIH), academia, and industry to rapidly conduct innovative, high-impact, and sustainable clinically relevant research. The BADER Consortium has a unique research capacity-building focus that creates infrastructures and strategically connects and supports research teams to conduct multiteam research initiatives primarily led by MTF and VA investigators.BADER relies on strong partnerships with these agencies to strengthen and support orthopaedic rehabilitation research. Its focus is on the rapid forming and execution of projects focused on obtaining optimal functional outcomes for patients with limb loss and limb injuries. The Consortium is based on an NIH research capacity-building model that comprises essential research support components that are anchored by a set of BADER-funded and initiative-launching studies. Through a partnership with the DoD/VA Extremity Trauma and Amputation Center of Excellence, the BADER Consortium's research initiative-launching program has directly supported the identification and establishment of eight BADER-funded clinical studies. BADER's Clinical Research Core (CRC) staff, who are embedded within each of the MTFs, have supported an additional 37 non-BADER Consortium-funded projects. Additional key research support infrastructures that expedite the process for conducting multisite clinical trials include an omnibus Cooperative Research and Development Agreement and the NIH Clinical Trials Database. Background: Passive-dynamic ankle-foot orthosis characteristics, including bending stiffness, should be customized for individuals. However, while conventions for customizing passive-dynamic ankle-foot orthosis characteristics are often described and implemented in clinical practice, there is little evidence to explain their biomechanical rationale., Objectives: To develop and combine a model of a customized passive-dynamic ankle-foot orthosis with a healthy musculoskeletal model and use simulation tools to explore the influence of passive-dynamic ankle-foot orthosis bending stiffness on plantar flexor function during gait., Study Design: Dual case study., Methods: The customized passive-dynamic ankle-foot orthosis characteristics were integrated into a healthy musculoskeletal model available in OpenSim. Quasi-static forward dynamic simulations tracked experimental gait data under several passive-dynamic ankle-foot orthosis conditions. Predicted muscle activations were calculated through a computed muscle control optimization scheme., Results: Simulations predicted that the passive-dynamic ankle-foot orthoses substituted for soleus but not gastrocnemius function. Induced acceleration analyses revealed the passive-dynamic ankle-foot orthosis acts like a uniarticular plantar flexor by inducing knee extension accelerations, which are counterproductive to natural knee kinematics in early midstance., Conclusion: These passive-dynamic ankle-foot orthoses can provide plantar flexion moments during mid and late stance to supplement insufficient plantar flexor strength. However, the passive-dynamic ankle-foot orthoses negatively influenced knee kinematics in early midstance., Clinical Relevance: Identifying the role of passive-dynamic ankle-foot orthosis stiffness during gait provides biomechanical rationale for how to customize passive-dynamic ankle-foot orthoses for patients. The aims of the current study were to explore the pattern of the force-velocity (F-V) relationship of leg muscles, evaluate the reliability and concurrent validity of the obtained parameters, and explore the load associated changes in the muscle work and power output. Subjects performed maximum vertical countermovement jumps with a vest ranging 0-40% of their body mass. The ground reaction force and leg joint kinematics and kinetics were recorded. The data revealed a strong and approximately linear F-V relationship (individual correlation coefficients ranged from 0.78-0.93). The relationship slopes, F- and V-intercepts, and the calculated power were moderately to highly reliable (0.67 < ICC < 0.91), while the concurrent validity F- and V-intercepts, and power with respect to the directly measured values, was (on average) moderate. Despite that a load increase was associated with a decrease in both the countermovement depth and absolute power, the absolute work done increased, as well as the relative contribution of the knee work. The obtained findings generally suggest that the loaded vertical jumps could not only be developed into a routine method for testing the capacities of leg muscles, but also reveal the mechanisms of adaptation of multijoint movements to different loading conditions.

Background: With the recent technological advancements of prosthetic lower limbs, there is currently a great desire to objectively evaluate existing prostheses. Using a novel biomechanical analysis, the purpose of this case study was to compare the mechanical energy profiles of anatomical and two disparate prostheses: a passive prosthesis and an active prosthesis., Case Description and Methods: An individual with a transtibial amputation who customarily wears a passive prosthesis (Elation, Össur) and an active prosthesis (BiOM, iWalk, Inc.) and 11 healthy subjects participated in an instrumented gait analysis. The total mechanical power and work of below-knee structures during stance were quantified using a unified deformable segment power analysis., Findings and Outcomes: Active prosthesis generated greater peak power and total positive work than passive prosthesis and healthy anatomical limbs., Conclusion: The case study will enhance future efforts to objectively evaluate prosthetic functions during gait in individuals with transtibial amputations., Clinical Relevance: A prosthetic limb should closely replicate the mechanical energy profiles of anatomical limbs. Bending stiffness of passive-dynamic ankle-foot orthoses (PD-AFOs) is a functional characteristic thought to restore lost ankle function due to weakened plantar flexors. However, lower extremity impairment profiles of patients are seldom limited to plantar flexion weakness, and PD-AFO characteristics often influence gait in other ways. Combined, all PD-AFO characteristics and patient impairments likely mask the main effect of PD-AFO bending stiffness and complicate the PD-AFO bending stiffness prescription process. In this study, we propose a biomechanical probing paradigm, where customized PD-AFOs with a range of precise stiffness values are worn by healthy subjects, to experimentally test a PD-AFO strength substitution hypothesis while simultaneously documenting gait adaptations to PD-AFO use. Two healthy subjects walked at a scaled velocity while wearing a series of three PD-AFOs that ranged in bending stiffness levels. Supporting the strength substitution hypothesis, peak ankle plantar flexion moments remained unchanged across PD-AFO stiffness conditions. Further biomechanical analyses documented a complex series of ankle related kinematic and kinetic adaptive movement strategies due to PD-AFO use. This study demonstrated the utility of the biomechanical probing paradigm to help understand the contribution of PD-AFO stiffness to ankle strength and its secondary effects on ankle biomechanics.

Passive-dynamic ankle-foot orthosis (PD-AFO) bending stiffness is a key functional characteristic for achieving enhanced gait function. However, current orthosis customization methods inhibit objective premanufacture tuning of the PD-AFO bending stiffness, making optimization of orthosis function challenging. We have developed a novel virtual functional prototyping (VFP) process, which harnesses the strengths of computer aided design (CAD) model parameterization and finite element analysis, to quantitatively tune and predict the functional characteristics of a PD-AFO, which is rapidly manufactured via fused deposition modeling (FDM). The purpose of this study was to assess the VFP process for PD-AFO bending stiffness. A PD-AFO CAD model was customized for a healthy subject and tuned to four bending stiffness values via VFP. Two sets of each tuned model were fabricated via FDM using medical-grade polycarbonate (PC-ISO). Dimensional accuracy of the fabricated orthoses was excellent (average 0.51 ± 0.39 mm). Manufacturing precision ranged from 0.0 to 0.74 Nm/deg (average 0.30 ± 0.36 Nm/deg). Bending stiffness prediction accuracy was within 1 Nm/deg using the manufacturer provided PC-ISO elastic modulus (average 0.48 ± 0.35 Nm/deg). Using an experimentally derived PC-ISO elastic modulus improved the optimized bending stiffness prediction accuracy (average 0.29 ± 0.57 Nm/deg). Robustness of the derived modulus was tested by carrying out the VFP process for a disparate subject, tuning the PD-AFO model to five bending stiffness values. For this disparate subject, bending stiffness prediction accuracy was strong (average 0.20 ± 0.14 Nm/deg). Overall, the VFP process had excellent dimensional accuracy, good manufacturing precision, and strong prediction accuracy with the derived modulus. Implementing VFP as part of our PD-AFO customization and manufacturing framework, which also includes fit customization, provides a novel and powerful method to predictably tune and precisely manufacture orthoses with objectively customized fit and functional characteristics.

Over the last half-century, the field of prosthetic engineering has continuously evolved with much attention being dedicated to restoring the mechanical energy properties of ankle joint musculatures during gait. However, the contributions of 'distal foot structures' (e.g., foot muscles, plantar soft tissue) have been overlooked. Therefore, the purpose of this study was to quantify the total mechanical energy profiles (e.g., power, work, and work-ratio) of the natural ankle-foot system (NAFS) by combining the contributions of the ankle joint and all distal foot structures during stance in level-ground steady state walking across various speeds (0.4, 0.6, 0.8 and 1.0 statures/s). The results from eleven healthy subjects walking barefoot indicated ankle joint and distal foot structures generally performed opposing roles: the ankle joint performed net positive work that systematically increased its energy generation with faster walking speeds, while the distal foot performed net negative work that systematically increased its energy absorption with faster walking speeds. Accounting for these simultaneous effects, the combined ankle-foot system exhibited increased work-ratios with faster walking. Most notably, the work-ratio was not significantly greater than 1.0 during the normal walking speed of 0.8 statures/s. Therefore, a prosthetic design that strategically exploits passive-dynamic properties (e.g., elastic energy storage and return) has the potential to replicate the mechanical energy profiles of the NAFS during level-ground steady-state walking., (Copyright © 2013 Elsevier B.V. We investigated the effect of simultaneous changes in body-weight-support level and walking speed on mean peak internal joint moments at the ankle, knee and hip. We hypothesized that observed changes in these joint moments would be approximately linear with both body-weight-support and walking speed and would be similar across joints. Kinematic and kinetic data were collected from 8 unimpaired adult subjects walking on an instrumented treadmill while wearing a dynamically controlled overhead support harness. Subjects walked with four levels of body-weight-support (0%, 20%, 40%, and 60% of bodyweight) at three walking speeds (0.4, 0.6, and 0.8 statures/s, ranging on average from 0.7 to 1.4m/s). Data were used to calculate mean peak joint moments across subjects for each condition. In general, subjects' mean peak joint moments decreased linearly with decreasing walking speed and with increasing body-weight-support, except the knee extension moment, which showed a quadratic relationship with walking speed and no significant change with body-weight-support. All joint moments, with the exception of knee extension, showed a significant interaction effect between walking speed and body-weight-support, indicating that the sensitivity of these joint moments to changes in these variables was interdependent. In most cases, the ankle and hip extension moments showed the largest sensitivity to walking speed. The ankle moment was observed to have the greatest sensitivity to body-weight-support. This finding, that altering walking speed and body-weight-support level results in non-uniform changes in peak moments across joints, suggests that further research is warranted to establish the set of combined speed and support conditions that produce motor patterns supportive of normal gait retraining., (Copyright © 2013 Elsevier Ltd. Background: Drugs can treat human diseases through chemical interactions between the ingredients and intended targets in the human body. However, the ingredients could unexpectedly interact with off-targets, which may cause adverse drug side effects. Notifying patients and physicians of potential drug effects is an important step in improving healthcare quality and delivery., Objective: With the increasing popularity of Web 2.0 applications, more and more patients start discussing drug side effects in many online sources. These online discussions form a valuable source for mining interesting knowledge about side effects. The main goal of this paper is to investigate the feasibility of exploiting these discussions to discover unrecognized drug side effects., Methods: We propose methods that can 1) build a knowledge base for drug side effects by automatically integrating the information related to drug side effects from different sources; and 2) monitor online discussions about drugs and discover potential unrecognized drug side effects., Results: Experiment results show that the online discussions indeed provide useful information discovering unrecognized drug side effects. We find that the integrated knowledge base contains more information than individual online sources. Moreover, both proposed detection methods can identify the side effects related to the four recently recalled drugs, and the information from online discussions makes it possible to make the detection much earlier than official announcements. Finally, the proposed generative modeling method is shown to be more effective than the discriminative method., Conclusions: We find that it is possible to monitor online discussions to detect unrecognized drug side effects. The developed system is expected to serve as a complementary tool for drug companies and FDA to receive feedbacks from the patients, and it has the potentials to expedite the discovery process of unrecognized drug side effects and to improve the quality of healthcare.

Anatomically-relevant (AR) biomechanical models are traditionally used to quantify joint powers and segmental energies of lower extremity structures during gait. While AR models contain a series of rigid body segments linked together via mechanical joints, prosthetic below-knee structures are often deformable objects without a definable ankle joint. Consequently, the application of AR models for the study of prosthetic limbs has been problematic. The purpose of this study was to develop and validate a unified deformable (UD) segment model for quantifying the total power of below-knee structures. Estimates of total below-knee power derived via the UD segment model were compared to those derived via an AR model during stance in gait of eleven healthy subjects. The UD segment model achieved similar results to the AR model. Differences in peak power, total positive work, and total negative work were 1.91±0.31%, 3.97±0.49%, and 1.39±0.33%, relative to the AR model estimates. The main advantage of the UD segment model is that it does not require the definition of an ankle joint or foot structures. Therefore, this technique may be valuable for facilitating direct comparisons between anatomical and disparate prosthetic below-knee structures in future studies., (Copyright © 2012 Elsevier Ltd. Passive-dynamic ankle-foot orthoses (PD-AFOs) constitute a class of ankle braces that rely on material properties and physical features to establish functional characteristics such as bending or rotational stiffness. We have developed a novel framework that combines a fully parameterized PD-AFO computer-aided design (CAD) model and free-form fabrication to rapidly manufacture customized PD-AFOs. The three-dimensional locations of select anatomic landmarks serve to fit customize the PD-AFO CAD model. A virtual orthopedic alignment process and selection of discrete design parameter values further customize the orthosis, which is fabricated via selective laser sintering. CAD models were customized and full-scale orthoses were manufactured for two nondisabled subjects. The surface of one half-scale CAD model was marked with 3 mm hemispherical dimples, and four orthoses were manufactured in different build orientations and positions. Dimensional accuracy was determined by calculating discrepancies between corresponding CAD and fabricated orthoses interdimple distances. Subjective evaluations of the full-scale PD-AFOs following use in gait were positive. Dimension discrepancies were well under a 2 mm tolerance for the four half-scale orthoses. Mean foot plate, strut, and cuff component discrepancies were 0.31 +/- 0.28, 0.34 +/- 0.08, 0.52 +/- 0.39 mm, respectively, and 0.29 +/- 0.23 mm for the overall orthosis. Dimensional accuracy of the rapid customization and manufacturing framework was well within tolerances suggested in the literature.

Persons with a unilateral, trans-tibial amputation have an increased risk of developing osteoarthritis (OA) in the knee of their intact limb. Between-side strength discrepancies observed in individuals with an amputation may indicate overuse of the intact limb and increased osteoarthritis risk; however, the relationship between lower extremity strength and gait mechanics has not been addressed in previous literature. It was hypothesized that amputee subjects' strength and gait would be more asymmetrical than controls, and that strength asymmetry would positively correlate with gait variable asymmetry and intact side gait variables associated with osteoarthritis risk. Eight persons with unilateral, trans-tibial amputation and eight able-bodied control subjects participated. Three gait variables related to osteoarthritis risk (knee external adduction moment, knee adduction moment load rate, and vertical ground reaction force load rate) were measured bilaterally, along with three strength measures (hip abductors, knee extensors, and knee flexors). Four of the six variables were more asymmetrical in the amputee group than the control group (p<0.05 and/or effect size greater than 0.70). Knee extension strength asymmetry was significantly related to knee adduction moment load rate asymmetry (rho=0.714), and knee flexion strength asymmetry was moderately related to the vertical ground reaction force on the intact limb (rho=0.643). Results suggest that strength asymmetry in unilateral trans-tibial amputees has a moderate relationship with osteoarthritis risk, and may be a useful way to assess gait ability and the need for rehabilitation in this population., (Copyright © 2010. The purpose of this study was to determine if gastrocnemius elongation or shortening and direction and velocity of knee movement influenced knee kinesthesia. Healthy volunteers sat with their knee flexed (20 degrees) and was then passively rotated (flexion or extension) at three velocities (0.5, 2, or 10 degrees/s) while the ankle was either fixed or rotated (dorsiflexed or plantar flexed at 0.17, 0.65, or 3.3 degrees/s) creating gastrocnemius elongation or shortening. Subjects activated a thumb switch, stopping motion once they detected onset and direction of the motion. Detection of passive movement sense (DPMS) was the angular movement before activation of a thumb-switch. Significant differences (P=0.003) in the rate of change in DPMS across a variety of movement velocities was observed but shortening or elongation of the gastrocnemius did not affect DPMS. Gastrocnemius elongation/shortening did not affect knee DPMS, simple reaction time plays an important role in testing kinesthesia especially at faster movements. While feedback from the gastrocnemius muscle plays a limited role in healthy subjects, differences in testing velocities may incorporate higher levels of central nervous system processing. Clinical measures of kinesthesia can be affected by both movement direction and movement velocity that are speed dependent.

It has been suggested that feedforward planning of gait and posture is diminished in older adults. Motor adaptation is one mechanism by which feedforward commands can be updated or fine-tuned. Thus, if feedforward mechanisms are diminished in older adults, motor adaptation is also likely to be limited. The purpose of the study was to compare the ability of healthy older versus young adults in generating a voluntary stepping motor adaptation in response to a novel visual sensory perturbation. We recorded stepping movements from 18 healthy older and 18 young adults during baseline and adaptation stepping blocks. During baseline, the stepping target remained stationary; in adaptation, a visual perturbation was introduced by shifting the target laterally during mid-step. We compared adaptation between groups, measured by improvements in endpoint accuracy and movement duration. Older adults adapted stepping accuracy similarly to young adults (accuracy improvement: 29.7 + or - 27.6% vs. 37.3 + or - 22.9%, older vs. young group respectively, p = 0.375), but showed significant slowness during movement. Thus older adults were able to achieve accuracy levels nearly equivalent to younger adults, but only at the expense of movement speed, at least during the early adaptation period (movement duration: 1143.7 + or - 170.6 ms vs. 956.0 + or - 74.6 ms, p < 0.001). With practice, however, they were able to reduce movement times and gain speed and accuracy to levels similar to young adults. These findings suggest older adults may retain the ability for stepping adaptations to environmental changes or novel demands, given sufficient practice., (Copyright 2009 Elsevier B.V. We increased the accuracy of an instrumented treadmill's measurement of center of pressure and force data by calibrating in situ and optimizing the transformation between the motion capture and treadmill force plate coordinate systems. We calibrated the device in situ by applying known vertical and shear loads at known locations across the tread surface and calculating a 6 x 6 calibration matrix for the 6 output forces and moments. To optimize the transformation, we first estimated the transformation based on a locating jig and then measured center-of-pressure error across the treadmill force plate using the CalTester tool. We input these data into an optimization scheme to find the transformation between the motion capture and treadmill force plate coordinate systems that minimized the error in the center-of-pressure measurements derived from force plate and motion capture sources. When the calibration and transformation optimizations were made, the average measured error in the center of pressure was reduced to approximately 1 mm when the treadmill was stationary and to less than 3 mm when moving. Using bilateral gait data, we show the importance of calibrating these devices in situ and performing transformation optimizations.

Objective: To determine whether Adaptive Physical Activity (APA-stroke), a community-based exercise program for participants with hemiparetic stroke, improves function in the community., Methods: Nonrandomized controlled study in Tuscany, Italy, of participants with mild to moderate hemiparesis at least 9 months after stroke. Forty-nine participants in a geographic health authority (Empoli) were offered APA-stroke (40 completed the study). Forty-four control participants in neighboring health authorities (Florence and Pisa) received usual care (38 completed the study). The APA intervention was a community-based progressive group exercise regimen that included walking, strength, and balance training for 1 hour, thrice a week, in local gyms, supervised by gym instructors. No serious adverse clinical events occurred during the exercise intervention. Outcome measures included the following: 6-month change in gait velocity (6-Minute Timed Walk), Short Physical Performance Battery (SPPB), Berg Balance Scale, Stroke Impact Scale (SIS), Barthel Index, Hamilton Rating Scale for Depression, and Index of Caregivers Strain., Results: After 6 months, the intervention group improved whereas controls declined in gait velocity, balance, SPPB, and SIS social participation domains. These between-group comparisons were statistically significant at P<.00015. Individuals with depressive symptoms at baseline improved whereas controls were unchanged (P<.003). Oral glucose tolerance tests were performed on a subset of participants in the intervention group. For these individuals, insulin secretion declined 29% after 6 months (P=.01)., Conclusion: APA-stroke appears to be safe, feasible, and efficacious in a community setting.

Subject-specific musculoskeletal models are essential to biomedical research and clinical applications, such as customized joint replacement, computer-aided surgical planning, gait analysis and automated segmentation. Generating these models from CT or magnetic resonance imaging (MRI) is time and resource intensive, requiring special skills. Therefore, in many studies individual bone models are approximated by scaling a generic template. Thus, the primary goal of this study was to determine a set of clinically available parameters (palpable measures and demographic data) that could improve the prediction of femoral dimensions, as compared to predicting these variables using uniform scaling based on palpable length. Similar to previous non-homogenous anthropometric scaling methods, the non-homogenous scaling method proposed in this study improved the prediction over uniform scaling of five key femoral measures. Homogenous scaling forces all dimensions of an object to be scaled equally, whereas non-homogenous scaling allows the dimensions to be scaled independently. The largest improvement was in femoral depth, where the coefficient of determination (r(2)) improved from 0.22 (homogenous) to 0.60 (non-homogeneous). In general, the major advantage of this non-homogenous scaling method is its ability to support the accurate and rapid generation of subject-specific femoral models since all parameters can be collected clinically, without imaging or invasive methods.

Background: The ability to redirect the path of the foot during walking is critical for responding to perturbations and maintaining upright stability. The purpose of the current study was to compare mechanisms of reactive stepping adjustments in young versus older adults when responding to an unexpected perturbation during voluntary step initiation., Methods: We tested 13 healthy community-dwelling older adults and an equal number of young control participants performing stepping movements onto a visual target on the floor. In some trials, perturbations were introduced by unexpectedly shifting the target, at various time points, from its usual location to a new location 20 cm to the right. We measured ground reaction forces under the supporting leg and three-dimensional kinematics of the stepping leg in baseline and target shift trials., Results: During target shift trials, that is, when reactive adjustments were required, older adults demonstrated the following: delayed responses in modifying the lateral propulsive forces under the supporting foot, reduced rates of lateral force production, delayed responses in modifying the stepping foot trajectory, and prolonged movement execution times., Conclusions: The current study quantitatively distinguishes between healthy older and young adults in generating reactive stepping adjustments to an unpredictable shift of a visual target. The decreased capability for rapidly planning and executing an effective voluntary step modification could reveal one potential cause for the increased risk of falls in the older population.

Joint position sense (JPS), one method to assess proprioception, is the ability to replicate a target limb position. Feedback is commonly used to improve motor performance but it has not been demonstrated to improve JPS. The purpose of this study was to determine if feedback decreases error associated with knee JPS at three movement velocities. Healthy volunteers sat with their hip and knees flexed. The knee was passively extended at three velocities (0.5, 2, and 10 degrees/s). Subjects were instructed to stop knee motion, via a thumb switch, at a 20 degrees knee flexion target. Following movement, each subject received visual and proprioceptive feedback indicating final leg position relative to the target position. Movement velocities and times (4 s, 5 s, 6 s) were randomly presented so subjects could not predict the target position. Measures of JPS included constant error (CE), absolute error (AE), variable error (VE), and percent correct (%CORR). Significant decreases in CE, AE, and VE as well as an increase in %CORR were demonstrated. The majority of JPS improvement (85%) occurred by the tenth trial. Short-term improvements in JPS may be the result of temporary CNS adaptations via feedback that was provided to subjects. Long-term learning of JPS enhancement needs further investigation.

Predicting muscle fascicle length changes during passive movements may lead to a better understanding of muscle function. The purpose of this study was to experimentally compare fascicle length changes in the gastrocnemius during two-joint passive movements with a previously derived kinematic model based on anatomical measures from a cadaver. The ratio of passive ankle to knee motion was manipulated to generate medial gastrocnemius fascicle elongation and lateral gastrocnemius fascicle shortening. Ultrasound images from both heads of the gastrocnemius fascicles were acquired at 10 degrees knee flexion increments and compared with this kinematic model. Our results suggest that the two-joint kinematic model from which we originally based our knee and ankle movements did not adequately reflect fascicle length changes during any of the movement conditions in this study. From our data, we propose that for every degree of ankle motion the medial and lateral gastrocnemius changes 0.42 mm and 0.96 mm, respectively, whereas changes of 0.14 mm and 0.22 mm are observed for the medial and lateral gastrocnemius, respectively, during knee movements.

Ankle-foot orthosis (AFO) designs vary in size, shape, and functional characteristics depending on the desired clinical application. Passive Dynamic (PD) Response ankle-foot orthoses (PD-AFOs) constitute a design that seeks to improve walking ability for persons with various neuromuscular disorders by passively (like a spring) providing variable levels of support during the stance phase of gait. Current PD-AFO manufacturing technology is either labor intensive or not well suited for the detailed refinement of PD-AFO bending stiffness characteristics. The primary objective of this study was to explore the feasibility of using a rapid freeform prototyping technique, selective laser sintering (SLS), as a PD-AFO manufacturing process. Feasibility was determined by replicating the shape and functional characteristics of a carbon fiber AFO (CF-AFO). The study showed that a SLS-based framework is ideally suited for this application. A second objective was to determine the optimal SLS material for PD-AFOs to store and release elastic energy; considering minimizing energy dissipation through internal friction is a desired material characteristic. This study compared the mechanical damping of the CF-AFO to PD-AFOs manufactured by SLS using three different materials. Mechanical damping evaluation ranked the materials as Rilsan D80 (best), followed by DuraForm PA and DuraForm GF. In addition, Rilsan D80 was the only SLS material able to withstand large deformations.

Objectives: To test the assumption of geometrically similar growth by developing experimentally derived models of human body growth during the age interval of 5 to 18 years; to use these derived growth models to establish a new human body shape index (HBSI) based on natural age-related changes in human body shape (HBS); and to compare various metrics of relative body weight (body mass index [BMI], ponderal index [PI], and HBSI) in a sample of 5- to 18-year-old children., Study Design: Nondisabled Polish children (n = 847) participated in this descriptive study. To model growth, the best fit between body height (H) and body mass (M) was calculated for each sex using the allometric equation M = m(i) H(chi). HBSI was calculated separately for girls and boys, using sex-specific values for chi and a general HBSI from combined data. The customary BMI and PI were calculated and compared with HBSI values., Results: The models of growth were M = 13.11H(2.84) (R2 = 0.90) for girls and M = 13.64H(2.68) (R2 = 0.91) for boys. HBSI values contained less inherent variability and were less influenced by growth (age and height) compared with BMI and PI., Conclusions: Age-related growth during childhood is sex-specific and not geometrically similar. Therefore, indices of HBS formulated from experimentally derived models of human growth are superior to customary geometric similarity-based indices for characterizing HBS in children during the formative growth years.

Background: The purpose of this case series was to quantify different strategies used to compensate in gait for hip muscle weakness., Methods: An instrumented gait analysis was performed of three females diagnosed with idiopathic inflammatory myopathies and compared to a healthy unimpaired subject. Lower extremity joint moments obtained from the gait analysis were used to drive an induced acceleration model which determined each moment's contribution to upright support, forward progression, and hip joint acceleration., Findings: Results showed that after midstance, the ankle plantar flexors normally provide upright support and forward progression while producing hip extension acceleration. In normal gait, the hip flexors eccentrically resist hip extension, but the hip flexor muscles of the impaired subjects (S1-3) were too weak to control extension. Instead S1-3 altered joint positions and muscle function to produce forward progression while minimizing hip extension acceleration. S1 increased knee flexion angle to decrease the hip extension effect of the ankle plantar flexors. S2 and S3 used either a knee flexor moment or gravity to produce forward progression, which had the advantage of accelerating the hip into flexion rather than extension, and decreased the demand on the hip flexors., Interpretation: Results showed how gait compensations for hip muscle weakness can produce independent (i.e. successful) ambulation, although at a reduced speed as compared to normal gait. Knowledge of these successful strategies can assist the rehabilitation of patients with hip muscle weakness who are unable to ambulate and potentially be used to reduce their disability.

The purpose of this case series was to quantify the effectiveness of different compensatory strategies used by individuals with muscle weakness to produce knee extension during the stance phase gait. Subjects were three males with less than anti-gravity strength in the quadriceps femoris and a variable pattern of weakness elsewhere in the lower extremity. They walked independently at a self-selected speed without assistive devices. Gait analysis of the pelvis and bilateral lower extremity segments was performed with a six-camera, two force platform motion capture system. Joint angles and net internal moments were computed from the motion data. Induced acceleration analysis was performed to quantify the relative ability of each lower extremity joint moment and gravity to produce knee angular acceleration. Results showed that a variety of adaptive strategies both within and across limbs can control knee position during gait. One subject generated knee extension almost exclusively via the hip extensor moment. Another relied less on the hip extensor moment (39%) and more on the ankle plantar flexor moment (61%) to create knee extension. The third subject used the ipsilateral hip extensor moment (24%) and the contralateral ankle plantar flexor moment (67%) to assist knee extension. The strategy selected by each subject likely was influenced by both their total pattern of impairments and the effectiveness of the available compensations. This case series demonstrates how an induced acceleration analysis can augment a traditional gait analysis to expand and enhance our understanding of compensatory movement control strategies.

Objective: To test the suitability of using biomechanic measures associated with a minimum measured input model (MMIM) approach to assess mobility of people with chronic stroke during the execution of a sit-to-stand (STS) to upright posture motor task., Design: Single group, observational., Setting: Institutional settings in the United States and Italy., Participants: Twenty-nine subjects with chronic unilateral lower-limb impairments and resultant mobility limitations secondary to stroke., Interventions: Not applicable., Main Outcome Measures: Manual measurement of lower-limb strength; performance-based tests including repeated chair standing, walking speed, and standing balance; and ground reactions measured with a force platform during STS and upright posture. The ground reactions were fed to a telescopic inverted pendulum model of the musculoskeletal system. Parameters representing the model outputs were compared with performance-based and strength measures to assess, respectively, motor ability and impairment-related changes in subjects' motor strategies., Results: The parameters derived from the model effectively differentiated between motor strategies associated with different performance-based scores, and allowed the identification of relevant difficulties encountered in STS execution. These difficulties could be associated with different strength scores. This was also true for subjects scoring the maximum in both performance-based and strength tests., Conclusions: The MMIM is a relatively inexpensive and noninvasive approach that enhances mobility assessment of hemiparetic subjects with different motor ability levels. It provides information that correlates well with performance-based and strength scores and, in addition, it allows for subject-specific motor strategy identification.

Relative motion plots are the most prevalent method for displaying interjoint coupling. The method, however, is limited when amplitude and timing comparisons of like data are of interest. Another limitation of relative motion plots is that the second parameter (e.g., angle) is included at the expense of a continuous time reference. In this paper, we present a novel method for displaying three-dimensional movement pattern deviations. Parameter-parameter-time data (e.g., knee and hip angle as a function of time) are color-coded based on the magnitude and direction of the deviation. The color-coded deviations are mapped to an individual's three-dimensional parameter-parameter-time trajectory, resulting in a multi-color, three-dimensional curve depicting how an individual's parameter-parameter-time pattern differs relative to a reference pattern. The algorithmic development of the color-coded parameter-parameter-time display is presented and comparative patient and normative data are reported.

The source of patellofemoral pain is a common orthopaedic complaint that often is difficult to determine because of the lack of correlation between symptoms and specific clinical measurements. Excessive joint contact stresses resulting from patellofemoral malalignment and pathologic femoral shape often are associated with this pain. These measures are likely sensitive to the limb position (orientation and position relative to the imaging system with which they are quantified). Because of this sensitivity, the measures have large variations and do not show correlations with subjective symptoms. The purpose of this study was to determine if varying limb position resulted in significant changes in standard clinical measures of patellofemoral alignment and femoral shape. This dependence was investigated by simulating alterations in limb position through resectioning of three-dimensional magnetic resonance image sets (20 healthy knees) to create axial images with altered orientation (eight images) or location (four images) relative to a fixed reference. By quantifying the variability of the clinical measures across all images, it was determined that simulated alterations in limb position produced greater variability in femoral shape and patellofemoral alignment measures than the variability seen across control subjects. This indicated that a standardized method for establishing limb position relative to the imager is warranted.

Objectives: To explore the association between an individual's functional status, movement task difficulty, and effectiveness of compensatory movement strategies within a sit-to-stand (STS) paradigm., Design: Cross-sectional study., Settings: Rehabilitation unit of the Istituto Nazionale Riposo e Cura Anziani Geriatric Hospital of Florence, Italy., Participants: A convenience sample (131 subjects) of the outpatient clinic and day-hospital population., Measurements: A performance-based test (repeated chair standing) was used to divide the subjects into five functional groups. Subjects performed a series of single STS tasks across a range of five descending seat heights. They were instructed to stand without using arms or compensatory strategies. If unable, swinging the arms was allowed, and if the inability persisted, subjects could push with their arms during subsequent attempts. The strategy or inability to stand formed the dependent measures., Results: Subjects within the two highest functional groups could complete the single STS task at all seat heights, with a slight increased use of compensatory strategies at the lowest seat height. The effectiveness of the compensatory strategies decreased rapidly as a function of seat height and functional status. One-third (35.5%) of the subjects in the middle functional group swung their arms at the lower seat heights. Across the three least functional groups, 11.8%, 30.6%, and 83.3% of the subjects, respectively, were unable to stand at the lowest seat height., Conclusion: The individual's functional status and difficulty of the task influenced the effectiveness of a compensatory strategy to maintain the ability to stand, supporting the idea that disability depends on the interplay between environmental demands and physical ability.

Plotting kinematic and kinetic data of a patient's movement patterns relative to normative values (i.e., mean and +/-1 S.D.) is a common method used by clinicians to visually assess deviations and interpret the patient's gait analysis data. This method of data interpretation is often time consuming and complex, especially when the process requires the inspection of a plethora of line graphs for numerous variables that are displayed across several report pages. In this paper we propose an alternate method for displaying movement pattern deviations relative to normative data by color-coding the magnitude and the direction of the deviation. An advantage of this approach is that a single page summary of all the deviation magnitudes can be displayed simultaneously, in a manner that is concise, visually effective and reduces complexity. The purpose of this paper is to describe the algorithmic development of the color-coding method.

Purpose: To define the femoral anatomic region that provides the most reliable reference for measuring femoral orientation, from which patellofemoral and tibiofemoral orientation can be measured., Materials and Methods: After a three-dimensional image-based osteo-alignment procedure, two independent estimates of distal femoral orientation, the anterior (AFA) and posterior femoral angles (PFA), were acquired. These sets were comprised of 31 axial femoral orientation estimates each (obtained across a region 10 mm above to 20 mm below the patellar center) from 42 normal axial magnetic resonance (MR) image sets. An analysis of variance was conducted to determine the influence of image plane location, sex, and side on femoral orientation measures., Results: The AFA presented greater variability across subjects (average SD = 3.13 degrees) as compared to the PFA (average SD = 1.78 degrees ). There were significant differences in the AFA based on image plane location and sex. In contrast, no differences were found between sides, sexes, or image plane locations for the PFA., Conclusion: In total, the posterior femoral condylar surface is more reliable than the anterior condylar surface for measuring femoral orientation and is best measured at least 8 mm below the patellar center., (Copyright 2004 Wiley-Liss, Inc.)

The study purpose was to estimate the ability of joint moments to transfer mechanical energy through the leg and trunk during gait. A segmental power analysis of five healthy adult subjects revealed that internal joint extensor moments removed energy from the leg and added energy to the trunk, while flexor moments and gravity produced the opposite effects. The only exception to this pattern was during the push off phase of gait when the ankle plantar flexor moment added energy to both the leg and the trunk. Pairs of joint moments with opposite energetic effects (knee extensor vs gravity, hip flexor vs ankle plantar flexor) worked together to balance energy flows through the segments. This intralimb coordination suggests that moments with contradictory effects are generated simultaneously to control mechanical energy flow within the body during walking.

Objectives: To investigate the relationships between the severity of flexed posture (FP), skeletal fragility, and functional status level in elderly women., Design: Cross-sectional study., Setting: Geriatric rehabilitation research hospital., Participants: Sixty elderly women (aged 70-93) with FP referred to a geriatric rehabilitation department for chronic back pain without apparent comorbid conditions., Measurements: Multidimensional clinical assessment included the severity of FP (standing occiput-to-wall distance) demographic (age) and anthropometric (height, weight) data, clinical profile (number of falls, pain assessment, Mini-Mental State Examination, Comorbidity Severity Index, Geriatric Depression Scale, Multidimensional Fatigue Inventory), measures of skeletal fragility (number of vertebral fractures by spine radiograph, bone mineral density (BMD), and T-score of lumbar spine and proximal femur), muscular impairment assessment (muscle strength and length), motor performance (Short Physical Performance Battery, Performance Oriented Mobility Assessment, instrumented gait analysis), and evaluation of disability (Barthel Index, Nottingham Extended Activities of Daily Living Index)., Results: The severity of FP was classified as mild in 11, moderate in 28, and severe in 21 patients. Although there were no differences between FP groups on the skeletal fragility measurements, the moderate and severe FP groups were significantly different from the mild FP group for greater pain at the level of the cervical and lumbar spine. The severe FP group was also significantly different from the mild but not the moderate FP group in the following categories: clinical profile (greater depression, reduced motivation), muscle impairment (weaker spine extensor, ankle plantarflexor, and dorsiflexor muscles; shorter pectoralis and hip flexor muscles), the motor function performance-based tests (lower scores in the balance and gait subsets of the Performance Oriented Mobility Assessment), the instrumented gait analysis (slower and wider base of support), and disability (lower score on the Nottingham Extended Activities of Daily Living Index). The total number of vertebral fractures was not associated with differences in severity of FP, demographic and anthropometric characteristics, clinical profile, muscular function, performance-based and instrumental measures of motor function, and disability, but it was associated with reduced proximal femur and lumbar spine BMD., Conclusion: The severity of FP in elderly female patients (without apparent comorbid conditions) is related to the severity of vertebral pain, emotional status, muscular impairments, and motor function but not to osteoporosis, and FP has a measurable effect on disability. In contrast, the presence of vertebral fractures in patients with FP is associated with lower BMD but not patients' clinical and functional status. Therefore, FP, back pain, and mobility problems can occur without osteoporosis. Older women with FP and vertebral pain may be candidates for rehabilitation interventions that address muscular impairments, posture, and behavior modification. Randomized controlled trials are needed to support these conclusions.

This paper describes a testing methodology and resultant set of four variables that can be used to quickly and easily document the correct installation, configuration, and combined working status of force platform (FP) and three-dimensional (3D) motion capture components of a clinical movement analysis (CMA) laboratory. Using a rigid, rod-shaped testing device, CMA laboratory data are collected simultaneously from the FP and motion capture components (typically, video-based kinematic measurements) as the device is manually loaded while being pivoted broadly about a point on the FP. Using a computational method based on static equilibrium, it is possible to independently measure the rod's orientation and tip position during the moving trial, using FP derived data exclusively, and to compare these estimates to rod orientation and tip position estimates derived exclusively from the motion capture component. The motion laboratory accreditation test (MLAT) variables include: the difference (angle) between the orientation of the long axis of the testing device as independently determined from kinematic measures (motion capture component) and the FP derived data; and the difference (x, y, z) between the center of pressure position (FP derived) and the position of the testing device tip (motion capture derived) that loads the FP. A numerical dynamics model was explored to evaluate the appropriateness of the static equilibrium-based FP data model and to determine guidelines for testing device movement frequency and FP loading. The MLAT technique provides a simple means of detecting the combined presence of errors from many sources, several of which are explored in this paper. The MLAT has been developed to help meet one criteria of the CMA laboratory accreditation process, and to serve as a routine quality assessment tool.

Objective: To investigate retest reliability and concurrent validity of the fundamental measurements made of a posturographic protocol that employs quiet standing to quantify the severity and the nature of patients' postural disturbances., Study Design: Retrospective complete block design., Setting: Geriatric rehabilitation department., Participants: Thirty-six participants (age range, 67 to 86 yrs) having normal, moderate, or severe levels of disequilibrium., Methods: Quiet standing was evaluated on three occasions using a three-dimensional motion analysis system and a force platform. Eight testing conditions, designed to vary task difficulty by controlling the contributions of vision, foot proprioception, and base-of-support width, were administered., Main Outcome Measures: Retest reliability of body sway, joint alignment, body position, and motor coordination indicators were evaluated by intraclass correlation coefficients (ICCs). Concurrent validity of protocol measures was evaluated by the prediction of disequilibrium from a stepwise linear discriminant analysis., Results: ICCs indicated high level of retest reliability for all variables but those of motor coordination, which was not influenced by testing conditions. Discriminant analysis resulted in a four-factor discriminator that included measures of body sway, position, alignment, and motor coordination. The derived linear discriminate function correctly classified 96% of the patients' level of disequilibrium., Conclusions: The posturographic protocol has the potential to be a useful tool for evaluating severity and nature of postural instability and the effects of pharmacologic and rehabilitative treatment. Results also indicate that combining direct body measurements with force-plate data has the potential to expose the underlying impairments that cause disequilibrium, determine their pathogenesis, and evaluate compensatory strategies.

Objective: To evaluate how painful metatarsal arthritis affects foot and ankle mechanics and mobility., Methods: We studied 16 symptomatic forefeet in 10 patients with rheumatoid arthritis (RA) and compared them with 14 asymptomatic forefeet in 7 nonarthritic subjects. RA limbs with significant disease at other locations were excluded. We measured pain and deformity of the foot using a visual analog scale and a modified articular index. A video based 3 dimensional gait analysis system and force platform were used to collect data on subjects walking barefoot at a self-selected pace according to an established protocol. Mobility level was quantified using the Sickness Impact Profile (SIP) ambulation subscale., Results: We observed considerable pain and deformity of the forefeet of RA subjects. During gait, motion and force measures revealed that RA subjects significantly (p < 0.005) delayed and reduced forefoot loading, which minimized use of the foot as a rigid level for push off. As a result, stride lengths were shorter and gait was slower compared to nonarthritic subjects. SIP scores revealed that these changes in gait resulted in moderate disability in RA subjects (p=0.05)., Conclusion: Impairments of the forefoot due to RA include pain and deformity, which produce characteristic stance phase abnormalities in foot function, a slow walking speed, and moderate disability.

A three-dimensional musculoskeletal database of the lower extremities has been developed for use in human musculoskeletal models. The locations of idealized muscle attachments on the pelvis, both femurs, both tibias and fibulas, and both feet were accurately digitized for 52 dried skeletal specimens. The mean specimen heights were 177.5 cm (male) and 166.2 cm (female) and the mean specimen age at the time of death was 48.8 yr. Statistical accumulation and scaling techniques were used to generate highly representative normative models, which were divided into groups and tested for differences based on gender and race. From the test results, the pelvis was divided into a male model (RMS = 8.6 mm), a black female model (RMS = 7.0 mm) and a white female model (RMS = 7.3 mm). The foot was separated into black (RMS = 3.7 mm) and white models (RMS = 3.6 mm). Single models were used for the femur (RMS = 6.5 mm) and the tibia/fibula (RMS = 3.7). Containing over 12000 anatomical landmarks digitized from 52 dried skeletons, this study represents an improvement over previous databases by an order of magnitude.

To test the sensitivity of knee flexion-extension moment patterns to variation in estimates of the knee center location (KCL), inverse dynamics analysis was performed using three different KCLs for data collected from 18 healthy adult subjects who walked at five different speeds (25%, 50%, 75%, 100% and 125% of 0.785 statures.s< ?? opnbop ?? ?-1). The KCL, which was determined using data from static subject calibration trials, was varied in software by plus or minus 10 mm in the anteroposterior direction. The effect of this KCL variation on knee moments was similar across subjects and at all five walking speeds. The relative knee center variation effect, expressed as a percentage of the mean knee moment, was progressively greater at slower walking speeds, up to 123% of the first extensor peak at the slowest speed. The implication of these results is that, while KCL variation does not affect the general shape of the moment pattern at more natural walking speeds, it can change the sign of the moment-and thus the interpretation-when the moment magnitude is small (e.g. at the very slow speeds used by some patients). When knee moments are of magnitudes less than or equal to the KCL variation effect, one cannot confidently interpret them as representing either a net flexor or extensor knee control strategy. OBJECTIVE: Changes in patterns of knee joint flexion-extension rotations, moments and powers were investigated during the stance phase across a wide range of walking speeds that includes the very slow speeds used by many patients referred for gait analysis. DESIGN: Each subject walked at 25, 50, 75, 100, and 125% of a scaled natural speed of 0.785 statures.s(-1). BACKGROUND: The results of earlier studies suggest that control subjects, as they walk at progressively slower speeds, increasingly use a large internal knee flexor moment in the early stance phase. METHODS: Eighteen healthy adult subjects (nine male and nine female) were tested. RESULTS: The shapes of the mean patterns were similar for the three fastest, most natural walking speeds. At the two slowest speeds, however, subjects used little knee flexion and small knee moments through mid-stance, resulting in negligible joint power through the first 80% of the stance phase. At these speeds, most subjects had moment patterns that could not be classified with confidence as 'normal' or as predominantly flexor or extensor. Only four subjects had knee flexor moment patterns at the slowest speeds, and these included much smaller flexor moments in the early stance phase than in some previous reports. RELEVANCE: Many patients walk at very slow speeds for which 'normal' joint kinetic patterns have not been described. The knee joint kinetic data provided here from healthy subjects is useful in helping to distinguish 'abnormal' patterns caused by neuromuscular pathologies or adaptations from patterns reflecting the effects of slow walking speed alone.

In standing humans, it is not certain whether anticipatory postural adjustments associated with rapid, voluntary elbow flexion movements (focal movements) originate as a selection from preset synergies or as the result of specific planning of motor commands. We studied these muscle recruitment patterns when the same focal movement was made under behavioral conditions of a self-paced task (SPT) and a reaction-time task (RTT). While standing still, eight normal subjects performed focal movements under the SPT and RTT behavioral conditions and under three different biomechanical conditions: (1) unloaded-upright, (2) loaded-upright (holding a 3800-g metal bar), and (3) unloaded-forward leaning. Anticipatory postural adjustments were quantified using the latency and duration of electromyographic (EMG) data and the center of pressure (COP) displacement data. Postural-kinematic [joint and body center of gravity (COG) motion] data were used to quantify the anticipatory postural program's effectiveness at preventing postural movement. Focal movement kinematics and associated EMG activity differed due to biomechanical but not behavioral (SPT vs. RTT) conditions. The maximum and net displacement of the body COG measurements did not differ between the behavioral conditions. The amplitude, timing, and net movements of lower extremity joints were influenced by the behavioral conditions. However, the behavioral conditions significantly affected the phasing (including order of activation) and duration of anticipatory postural EMG activity and the phasing of COP displacements under certain biomechanical conditions. These findings support the theory that anticipatory postural adjustments are planned in detail.

A technique to measure foot function during the stance phase of gait is described. Advantages of the method include its three-dimensional approach with anatomically based segment coordinate systems. This allows variables such as ground reaction forces and center of pressure location to be expressed in a local foot coordinate system, which gives more anatomical meaning to the interpretation of results. Application of the measurement technique to case examples of patients with rheumatoid arthritis demonstrated its ability to discriminate normal from various levels of pathological function. Future studies will utilize this technique to study the impact of pathology and treatment on foot function.

The purpose of this paper is to describe the concept of Functional Movement Analysis (FMA) and review the recent advancements pertaining to the clinical application of FMA. After a brief introduction to the concept of FMA, the role computers have played in FMA development is presented. A generalized movement measurement technique is introduced along with the resulting benefits associated with the implementation of body segment based coordinate systems. Finally, the role of mechanical power estimates and movement synthesis techniques in the future maturation of FMA is considered.
