Introduction: Primary Sclerosing Cholangitis (PSC) is a rare chronic inflammatory disease of the bile ducts. It can lead to cholangiocarcinoma which results in a high mortality. The etiology is multifactorial. There is only symptomatic treatment which usually consists of weight-adapted prescription of ursodeoxycholic acid and biliary interventions in case of dominant bile duct strictures. Eventually only a liver transplantation can save the lives of patients with aggressive disease progression. Dysfunctional acetylcholine-mediated pathways might play a role in pathogenesis of chronic bile duct inflammation through the bile duct specific mAChR3 that alters the bicarbonate umbrella which protects the biliary epithelial barrier from toxic effect of bile acids. In previous studies, inhibitory auto-antibodies have been shown in patients with auto-immune diseases. The aim of this study was to show the prevalence and significance of auto-antibodies against the muscarinic acetylcholine recptor type 3 (anti-mAChR3) in patients from the interdisciplinary liver center of Charité Medical School, Campus Virchow Clinic. and from the outpatient clinic for liver transplantation of the Charité Medical School, Campus Virchow Clinic.
