17. Quality of life in cemiplimab-treated patients with locally advanced basal cell carcinoma in a Phase II clinical trial.

Abstract

Aim: To evaluate health-related quality of life (HRQoL) in cemiplimab-treated patients with locally advanced basal cell carcinoma (laBCC). Materials & methods: Eighty-four patients with laBCC received cemiplimab 350 mg every 3 weeks (up to 9 cycles). HRQoL was assessed using the European Organization for Research and Treatment of Cancer Quality of Life Core 30 (QLQ-C30) and Skindex-16 questionnaires at baseline and each cycle. Mixed-effects repeated-measures models evaluated change from baseline across cycles. Results: Clinically meaningful improvement or maintenance was reported by 62–90% of patients on QLQ-C30 scales and by approximately 80% on Skindex-16 scales at Cycle 2, with consistent results at Cycle 9 except fatigue. Conclusion: Most cemiplimab-treated patients with laBCC reported improvement or maintenance of HRQoL with low symptom burden except fatigue. Plain Language Summary Locally advanced basal cell carcinoma (laBCC) is a type of skin cancer that has the potential to invade surrounding tissues including bone, cartilage, nerve and muscle. Cemiplimab-rwlc is approved in the US for patients with laBCC following a therapy called hedgehog inhibitor (HHI) treatment or for whom HHIs are not appropriate. In a Phase II clinical trial, intravenous (in the vein) cemiplimab 350 mg every 3 weeks for up to nine treatment cycles resulted in clinically meaningful antitumor activity in patients with laBCC who progressed on or were intolerant to HHIs. This analysis evaluated health-related quality of life, symptom burden, emotions and functional status in these patients using the European Organization for Research and Treatment of Cancer Quality of Life Core 30 (QLQ-C30) and Skindex-16 questionnaires. Baseline scores (scores at the start of the clinical trial) showed moderate to high levels of functioning and low symptom burden that, except for fatigue, were maintained or improved over the course of cemiplimab treatment. These results show that despite the presence of fatigue, health-related quality of life and functional status were maintained with cemiplimab across the study duration. Since an important component in cancer clinical trials is the measurement of patient-centric outcomes, this analysis evaluated patient-reported health-related quality of life (HRQoL) among participants in the clinical trial. HRQoL was evaluated using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (QLQ-C30), which consists of a global health status/quality of life (GHS/QoL) scale, five functional scales and eight symptom scales; and the Skindex-16, which assesses the impact of skin disease on HRQoL based on symptom, emotional and functional subscales. Mixed-effects repeated-measures models were used to estimate overall least-squares (LS) mean (95% confidence interval [CI]) change from baseline across Cycles 2–9; absolute changes ≥10 points from baseline were considered clinically meaningful. Overall changes from baseline indicated maintenance for QLQ-C30 GHS/QoL, functioning and symptom scales, except for clinically meaningful worsening of fatigue (LS mean change: 12.0; 95% CI: 4.3–19.6). Clinically meaningful improvement or maintenance on QLQ-C30 GHS/QoL, functioning and symptom scales were reported at Cycle 2 by 88%, 69–81% and 62–90% of patients, respectively, with similar proportions at Cycle 6 (∼1 year of treatment) and consistent results at Cycle 9 except for fatigue. On the Skindex-16, overall change showed clinically meaningful improvement on the emotional subscale (LS mean change, -13.8; 95% CI: -21.4 to -6.2), with maintenance on symptom and functional subscales; clinically meaningful improvement or maintenance across all three subscales was reported by approximately 80% of patients at Cycle 2 that was generally maintained at Cycles 6 and 9. These results show that most patients with laBCC treated with cemiplimab reported clinically meaningful improvement or maintenance in GHS/QoL and functioning while maintaining low symptom burden, except for fatigue. Abstract

Background: Moderate to severe (M2S) atopic dermatitis (AD) is a chronic condition impacting individuals, society, and healthcare systems. Considering the changing M2S-AD treatment landscape, this study assesses the M2S-AD burden in patients reaching referral centers in Greece. Methods: This was a multicenter, cross-sectional study. Patients aged 12 years or older with clinically diagnosed M2S-AD were enrolled. Data collected included clinical practice assessments and the following validated patient-reported instruments: Dermatology Life Quality Index (DLQI); EuroQol-5 Dimensions-3 Level scale (EQ-5D-3L); Patient Oriented Eczema Measure (POEM); Peak Pruritus Numerical Rating Scale (PP-NRS); and Work Productivity and Activity Impairment: General Health (WPAI:GH). A pain frequency/intensity/cause questionnaire and a sleep disturbance scale were also used. Results: Outcomes of 184 adults (51.1% female) with M2S-AD based on the Eczema Area and Severity Index (EASI) are presented (n = 117 moderate; n = 67 severe). Among the patients, 14.8% were obese, 59.2% had allergic comorbidities, and 88.0% were receiving AD-specific therapy (systemic: 38.6%). The median age, disease duration, body surface area, and total EASI scores were 38.8 years, 11.8 years, 30.0%, and 16.9, respectively. The median DLQI score was 12.0, with 'symptoms/feelings' being the most affected domain. EQ-5D dimensions 'anxiety/depression' and 'pain/discomfort' were also affected (65.2% and 64.1% reporting problems, respectively). The median POEM score was 17.0. Pain, severe pruritus (PP-NRS ≥ 7), and sleep disturbance were reported by 80.4%, 62.0%, and 88.5%, respectively. The median WPAI:GH 'work productivity loss' and 'activity impairment' scores were 23.8% and 30.0%, respectively. Conclusions: Both moderate and severe AD patients reaching Greek specialized centers experience significant symptom burden and impairments in quality of life, sleep, work, and daily activities. 21. Crannogs in north-east Scotland : understanding the resource

Abstract

Crannogs are artificial island dwellings known in Scotland and Ireland. They are an enigmatic, but important, part of the archaeological record in Scotland dating from as early as the Neolithic and in use as late as just a few hundred years ago. These sites hold immense archaeological potential in their well-preserved waterlogged and submerged contexts in lochs (lakes). This thesis has examined crannogs in north-east Scotland, a region where no modern investigation of crannogs has taken place. Although crannogs are normally considered an underwater archaeology, much of what we know about them comes from work carried out on drained sites. Therefore, a key question of this thesis has been to quantify the impacts of historic loch drainage, in shaping the existing record of crannogs in the study area. Through historic map analysis, the thesis demonstrates how 18th and 19th century loch drainage has been the defining element in the creation of the archaeological record in lake environments in north-east Scotland, most notably impacting crannogs. Moreover, the quantification of historic drainage has also strongly suggested that the different histories of drainage across Scotland have shaped the trajectory of crannog research, further exacerbating the existing focus in crannog research on sites in western Scotland. A major result of this research has been to investigate areas of loch drainage and suggest new likely and possible crannogs that have been misidentified as terrestrial site types. The data quantifying historic loch drainage also has revealed important patterns in Improvement period land-use changes that have hitherto been unrecognised. This thesis establishes a dataset for crannogs in north-east Scotland that builds a framework for understanding their preservation, morphology, chronology and distribution. In testing a newly identified possible crannog site, the Houff, Aberdeenshire, the results of this thesis have demonstrated a proof-of-concept that misidentified crannogs can be found in areas of loch drainage. Along with a number of further newly identified sites, it is argued that the distribution of crannogs across Scotland is far more uniform than previously considered. Furthermore, the results of the fieldwork in these areas suggests that crannogs in north-east Scotland have been severely damaged by 18th and 19th century drainage programmes, and at a higher rate than other regions of Scotland, notably the south-west. Better preserved crannog sites were also targeted for evaluative excavation such as the Loch of Leys crannog, Aberdeenshire. Excavation here revealed evidence for phases of use of this site in the Roman Iron Age and in the 9th-10th century AD in addition to its historically attested use as a medieval lordly dwelling in the 12th-15th centuries AD. The best preserved crannog sites in north-east Scotland at Loch Kinord, Aberdeenshire were also targeted to build this baseline of data on the artificial islands of the study area. Underwater survey and sampling of the organic structural components of the crannogs has revealed evidence for use of Castle Island in Early Iron Age and also in the 10th century AD. Excavation on the interior of the island demonstrated good evidence for its use as a motte-type castle from the 11th-12th centuries AD. At Prison Island, Loch Kinord timber elements and occupational deposits on the crannog were dated to the 8th-9th century AD and the 12th century AD. This contemporaneous activity at both Castle and Prison Island is paralleled at various crannog sites across the country. Using the newly established dataset, discussion on how north-east Scottish crannogs fit within wider frameworks for the understanding of crannogs more widely is undertaken. This focuses on the main descriptive themes which have concerned most previous research on crannogs. Importantly, this work in north-east Scotland has revealed evidence that suggests crannogs are more ubiquitous than previously considered and appear across Scotland from the Early Iron Age rather than seeing a western phenomenon adopted latterly in the east. Dating evidence recovered from crannogs in north-east Scotland from the 9th-10th centuries AD fills a previously noted lacuna in crannog construction which demonstrates a further reason why crannog research cannot only focus on limited geographical regions. More interpretative frameworks for how we might understand crannogs are explored in the discussion following lines of evidence thrown up by the newly created dataset for crannogs in north-east Scotland. These include a discussion of the conceptual origins of crannogs and their role in wider social networks and settlement landscapes.

Abstract

Background: There is no population-based evidence on the prevalence and impact of skin diseases in Greece. Objectives: To describe the prevalence of 12 main skin diseases and their impact on quality of life (QoL) and feelings of stigmatization in the Greek population. Methods: A population-based survey in an adult Greek representative sample was carried out as part of the multinational ‘EADV burden of skin diseases study’. Quality of life (QOL) was measured using the Dermatology Life Quality Index (DLQI) and the Visual Analogue Scale (VAS) from the EuroQol-5D (EQ. 5D) questionnaires. Results: In 1010 participants, 47.8% (n = 483) declared at least one skin disease or condition or skin-related unpleasant sensation in the last 12 months. Fungal skin infection was reported with the higher prevalence (7.5%), followed by alopecia (5.7%), atopic dermatitis (5%), acne (3.6%), sexually transmitted diseases (2.7%), psoriasis (2.2%) chronic urticaria (1.5%), rosacea (1.3%), nonmelanoma skin cancer (1.5%), vitiligo (0.6%), cutaneous melanoma (0.6%) and hidradenitis suppurativa (0.3%). Mean VAS-EQ. 5D and DLQI scores were 77.8 and 2.2, respectively. Among those reporting at least one skin problem, 68.1% reported an impact of the skin condition on their personal life and 51% reported an impact on time to take care of themselves. Regarding the impact on work-life decisions, 22.2% reoriented their professional activity, 13% were refused a professional offer, 22.2% did not get the job they hoped to and 16.7% chose their work with their skin problem in mind. Regarding feelings of stigmatization, 12% felt left out/rejected by others, 6.6% were refused access to leisure facilities and 9.8% reported the impression to be looked at with disgust. Conclusions: Our population-based study provides new information on the prevalence of skin diseases in the Southern European country of Greece and highlights the impact of prevalent skin disease on life-altering decisions and stigmatization.

Abstract

Background: A phase 2 cemiplimab study (NCT03132636) demonstrated a 24.1% objective response rate in patients diagnosed with metastatic basal cell carcinoma (mBCC) who were not candidates for continued hedgehog inhibitor (HHI) therapy due to intolerance to previous HHI therapy, disease progression while receiving HHI therapy, or having not better than stable disease on HHI therapy after 9 months. Here, health‐related quality of life (QoL) for this patient population is reported. Methods: Adult patients with mBCC were treated with intravenous cemiplimab at a dose of 350 mg every 3 weeks for 5 treatment cycles of 9 weeks/cycle then 4 treatment cycles of 12 weeks/cycle. Patients completed the European Organisation for Research and Treatment of Cancer Quality of Life‐Core 30 (QLQ‐C30) and Skindex‐16 questionnaires at baseline and Day 1 of each cycle. Across Cycles 2 to 9, the overall change from baseline was analyzed using a mixed model with repeated measures. Responder analyses determined clinically meaningful improvement or deterioration (changes ≥10 points) or maintenance across all scales. Results: Patients reported low symptom burden and moderate‐to‐high functioning at baseline. Maintenance for QLQ‐C30 global health status (GHS)/QoL and across all functioning and symptom scales was indicated by overall mean changes from baseline. Clinically meaningful improvement or maintenance was reported at Cycle 2 for GHS/QoL (77%), functioning scales (77% to 86%), and symptom scales (70% to 93%), with similar proportions of improvement or maintenance at Cycles 6 and 9, excluding fatigue. On the Skindex‐16, clinically meaningful improvement or maintenance was reported across the emotional, symptom, and functional subscales, in 76%–88% of patients at Cycle 2, which were generally maintained at Cycles 6 and 9. Overall mean changes from baseline showed maintenance across these subscales. Conclusions: The majority of patients treated with cemiplimab reported improvement or maintenance in GHS/QoL and functioning while maintaining a low symptom burden. [ABSTRACT FROM AUTHOR]

Abstract

Background: The introduction of cyclin-dependent kinase inhibitors (CDK4/6i) was a great advance in therapeutics for patients with estrogen receptor+/human epidermal growth factor receptor (HER2) locally advanced and metastatic breast cancer. Despite the increasing use of these agents, their adverse drug-related events have not yet been fully characterized. We describe the spectrum of cutaneous adverse reactions occurring in advanced breast cancer patients treated with cyclin-dependent kinase inhibitors, analyzing types, severity, time to onset, and possible treatment outcomes. Methods: We performed a multicentric retrospective study including patients with advanced breast cancer who developed cutaneous lesions during treatment with CDK4/6i in the period from June 2020 to June 2021. Patients > 18 years were recruited at eleven onco-dermatology units located in Albania (1), Argentina (1), France (1), Greece (3), Italy (3), and Spain (2). We evaluated patients’ epidemiological and clinical characteristics, types of cutaneous adverse events, their time to onset, and treatment outcomes. The severity of the skin reactions was assessed using the Common Terminology Criteria for Adverse Events (CTCAE) version 5.0 score. Results: Seventy-nine patients (median age: 62.3 years; range 39–83 years) were included in the study, and, collectively, we recorded a total of 165 cutaneous adverse events during follow-up visits. The most frequent cutaneous reactions were pruritus (49/79 patients), alopecia (25/79), and eczematous lesions (24/79). Cutaneous toxicities were usually mild in severity (>65%) and occurred after a median of 6.5 months. Only four patients (5%) required treatment discontinuation due to the severity of the skin lesions. The majority of the skin reactions were managed with topical treatments. Conclusions: To the best of our knowledge, we present the largest case series of cutaneous adverse events developing in advanced breast cancer patients treated with CDK4

Abstract

Background: In the 2022 mpox (monkeypox) outbreak, 79,000 global cases have been reported. Yet, limited dermatologic data have been published regarding lesion morphology and progression. Objective: The objective of this study was to characterize skin lesion morphology, symptomatology, and outcomes of mpox infection over time. Methods: The American Academy of Dermatology/International League of Dermatological Societies Dermatology COVID-19, Mpox, and Emerging Infections Registry captured deidentified patient cases of mpox entered by health care professionals. Results: From August 4 to November 13, 2022, 101 cases from 13 countries were entered, primarily by dermatologists (92%). Thirty-nine percent had fewer than 5 lesions. In 54% of cases, skin lesions were the first sign of infection. In the first 1-5 days of infection, papules (36%), vesicles (17%), and pustules (20%) predominated. By days 6-10, pustules (36%) were most common, followed by erosions/ulcers (27%) and crusts/scabs (24%). Crusts/scabs were the predominant morphology after day 11. Ten cases of morbilliform rash were reported. Scarring occurred in 13% of the cases. Limitations: Registry-reported data cannot address incidence. There is a potential reporting bias from the predilection to report cases with greater clinical severity. Abstract

This article discusses a study conducted in Greece on familial melanoma, a type of skin cancer that runs in families. The researchers aimed to identify genetic variations associated with familial melanoma in a Greek cohort and to understand how these variations relate to family and patient characteristics. The study found that the CDKN2A gene was the most common gene implicated in familial melanoma in Greece, with a higher frequency of pathogenic variations compared to other Mediterranean populations. Carriers of these variations were more likely to develop multiple primary melanomas at a younger age and had an increased number of nevi and atypical nevi.
