A primary haemangiopericytoma of the lacrimal sac in a 45-year-old male is reported. The neoplasm, highly unusual in this location, recurred twice before being correctly identified. The relationship between bullous pemphigoid (BP) and neoplasms has been debated in the medical literature. Although numerous case reports have described the coexistence of BP with various neoplasms, case-control studies have yielded conflicting results. We present the case of a male patient who developed BP shortly after being diagnosed with mycosis fungoides (MF). He was a 77-year-old man with a history of type 2 diabetes mellitus and hypertension who was diagnosed with MF. Subsequently, he developed blisters after sun exposure, and was diagnosed with BP through histopathology and direct immunofluorescence. The patient was treated with prednisone and methotrexate, with favorable evolution without recurrence of MF or appearance of new blisters. The association between cutaneous T-cell lymphoma and autoimmune blistering disease is rare, although similar cases have been reported, some associated with phototherapy. In this case, the onset of BP after sun exposure suggests a potential connection. The coexistence of BP and MF remains controversial, and this case highlights the importance of considering autoimmune blistering diseases in patients with oncohematological neoplasms.

Darier-White Disease (DWD) is an autosomal dominant disorder, described by Darier and White in 1889, characterized by abnormal keratinization in the epidermis, nails and mucosae. We present the case of a 16-year-old, male patient, with clinically and histologically confirmed DWD treated with oral isotretinoin (0.7 mg/kg/day) for six months, with good response. We recommend an optional therapeutic treatment for chronic, itchy, recalcitrant lesions with oral retinoids.

Purpose: To examine the impact of switching glaucoma patients from timolol 0.5% to brinzolamide 1%/timolol 0.5% fixed combination (Brinz/Tim FC) on quality of life and on ocular surface status; to assess efficacy after the switch., Methods: 6-month, multicenter, open-label, prospective, switch study in 119 early to moderate glaucoma patients. Intraocular pressure (IOP), tear film break-up time (TF-BUT), fluorescein staining and Glaucoma Symptom Scale (GSS) questionnaire were recorded at baseline and after 6 months., Results: Median (interquartile range) IOP significantly decreased from 20 to 16 mmHg, independent of sex and age. Most patients (95.8%) reached an IOP < 18 mmHg. TF-BUT improved, with a negative weak correlation to age at baseline and at 6 months. The percentage of patients with no fluorescein staining improved. The quality of life recorded by GSS also improved, being related both to age and corneal staining., Conclusion: Brinz/Tim FC is effective and well tolerated. In this study, patients switched to Brinz/Tim FC obtained further reduction in IOP with no effects on ocular surface status and improved quality of life perception: a better quality of life could determine a better adherence to prescribed therapy.

Purpose: To ascertain the best surgical technique for fixating an intraocular lens (IOL) in the posterior chamber in the absence of capsular support., Setting: From the Institute of Ophthalmology, Pavia University, Pavia, Italy., Methods: Using the SFVT System, the phases of IOL fixation were evaluated in cadaver eyes. In addition, IOLs were implanted in the posterior chamber in 340 eyes of patients without capsular support. One of several IOL fixation methods was used; IOL power was calculated using the same parameters as when the lens is positioned in the sulcus. Mean follow-up was 3.5 years (range 2 months to 5 years)., Results: Intraocular lens placement was accurate only when the loops were positioned in the sulcus, with the best approach being internal with suspension sutures placed at 3 and 9 o'clock. In most cases, refractive results were good; 152 eyes were within +/- 1.00 diopter of the predicted value., Conclusion: In eyes without capsular support, IOL implantation in the sulcus was possible only when sutures were placed exactly through the sulcus. An internal approach using suspension sutures at 3 and 9 o'clock gave the best results.
