What HRR criterion at 1-minute after a 6-minute walk test (6MWT) best defines pulmonary impairment?.Study Design and Methods: A total of 5008 phase 2 COPDGene (NCT00608764) participants with smoking history were included. A total of 2127 had COPD and, of these, 385 were followed-up 5-years later. Lung surgery, transplant, bronchiectasis, atrial fibrillation, heart failure and pacemakers were exclusionary. HR was measured from pulse oximetry at end-walk and after 1-min seated recovery. A receiver operator characteristic (ROC) identified optimal HRR cut-off. Generalized linear regression determined HRR association with spirometry, chest CT, symptoms and exacerbations.Results: HRR after 6MWT (bt/min) was categorized in quintiles: ≤ 5 (23.0% of participants), 6– 10 (20.7%), 11– 15 (18.9%), 16– 22 (18.5%) and ≥ 23 (18.9%). Compared to HRR≤ 5, HRR≥ 11 was associated with (p< 0.001): lower pre-walk HR and 1-min post HR; greater end-walk HR; greater 6MWD; greater FEV1%pred; lower airway wall area and wall thickness. HRR was positively associated with FEV1%pred and negatively associated with airway wall thickness. An optimal HRR ≤ 10 bt/min yielded an area under the ROC curve of 0.62 (95% CI 0.58– 0.66) for identifying FEV1< 30%pred. HRR≥ 11 bt/min was the lowest HRR associated with consistently less impairment in 6MWT, spirometry and CT variables. In COPD, HRR≤ 10 bt/min was associated with (p< 0.001): ≥ 2 exacerbations in the previous year (OR=1.76[1.33– 2.34]); CAT≥ 10 (OR=1.42[1.18– 1.71]); mMRC≥ 2 (OR=1.42[1.19– 1.69]); GOLD 4 (OR=1.98[1.44– 2.73]) and GOLD D (OR=1.51[1.18– 1.95]). HRR≤ 10 bt/min was predicted COPD exacerbations at 5-year follow-up (RR=1.83[1.07– 3.12], P=0.027).Conclusion: HRR≤ 10 bt/min after 6MWT in COPD is associated with more severe expiratory flow limitation, airway wall thickening, worse dyspnoea and quality of life, and future exacerbations, suggesting that an abnormal HRR≤ 10 bt/min after a 6MWT may be used in a comprehensive assessment in COPD for risk of severity, symptoms and future exacerbations.Keywords: autonomic dysfunction, chest computed tomography, COPD exacerbation, exercise, spirometry

Abstract

Despite the effectiveness of HAART in controlling HIV-1 replication, the emergence of drug-resistant viruses in infected patients and the severe side effects caused by the currently used drug regimens and the lack of an effective vaccine necessitate the continued search for new therapeutic strategies for prevention and therapy of HIV disease. Previously we reported that natural autoantibodies, recognizing peptide FTDNAKTI (peptide NTM1) derived from the C2 domain of HIV-1 gp120, contribute to the control of HIV disease. Here we demonstrated that sera from well-trained athletic (HIV-negative) subjects showed high reactivity with peptide NTM1. This result confirms that aerobic exercise training stimulates production of natural autoantibodies, which recognize peptide NTM1. Bioinformatics analysis indicates that these natural autoantibodies could slow down disease progression by blocking the superantigenic site on HIV-1 gp120. The results suggest that aerobic exercise training may be a promising non-toxic and inexpensive adjunctive anti-HIV therapy.

24. Application of satellite remote-sensing data to land selection and management

Abstract

A pilot project conducted to demonstrate the utility and economy of satellite data in preparing thematic maps of a wilderness area emphasizing those resources of greatest interest to the potential owner is described. Vegetation maps delineating potential commercial timber and maps of suggested mineral prospecting areas of seven scattered regions were prepared by interpretation of LANDSAT images, coupled with a limited amount of ground truth. Images acquired both in winter and summer seasons were registered to township maps and used in making interpretations of the areal extent of commercial timber potentials. The amount of snow cover visible through the forest canopies was found to be a useful indicator of timber potentials. Identification was made of characteristic topographic features which are typical of flood plain deposits or of the well developed trellis drainage patterns which can indicate the strike of structural grain of underlying Cretaceous sedimentary rocks. The presence of igneous and mixed igneous and metamorphic rocks were indicated by combinations of spectral differences and anomalous interruptions of local radial drainage patterns.

27. LANDSAT survey of near-shore ice conditions along the Arctic coast of Alaska

Abstract

The author has identified the following significant results. Winter and spring near-shore ice conditions were analyzed for the Beaufort Sea 1973-77, and the Chukchi Sea 1973-76. LANDSAT imagery was utilized to map major ice features related to regional ice morphology. Significant features from individual LANDSAT image maps were combined to yield regional maps of major ice ridge systems for each year of study and maps of flaw lead systems for representative seasons during each year. These regional maps were, in turn, used to prepare seasonal ice morphology maps. These maps showed, in terms of a zonal analysis, regions of statistically uniform ice behavior. The behavioral characteristics of each zone were described in terms of coastal processes and bathymetric configuration.

Abstract

The growth mechanism of the grounded ice feature on Hanna's Shoal, located at approximately 162 deg W, 72 deg N in the Chukchi Sea, has been analyzed using Landsat I and II satellite imagery. The primary mechanism of growth was deduced to be the formation of large wedges of ice against the upstream edge of the feature. These wedges of piled ice form in three stages and may or may not become permanently affixed to the core of the feature. Remains of these growth patterns can sometimes be seen in the interior of the feature on the satellite imagery.

29. Reindeer range inventory in western Alaska from computer-aided digital classification of LANDSAT data

Abstract

An inventory of reindeer-range resources was conducted for the USDA Soil Conservation Service of 1.6 million hectares of wildlands in western Alaska using clustering techniques with digital Landsat data. Computer-aided digital analysis produced a provisional map of rangeland types which was used to design the field collection of vegetation and soil types data. This field data facilitated refinement of the inventory map and was used to describe the map units. The informational classes important to range resources were wet, moist and alpine tundra, tidal marsh, brush and open spruce forest. A significant feature of the study was the extraction of acreage figures by administrative boundaries within the study area. Abstract

The stable xenon CT method of measuring cerebral blood flow has been investigated in research studies for over 10 years. Recently, it has been gaining clinical acceptance, primarily owing to a combination of several unique advantages it holds over other cerebral blood flow measurement techniques. The accuracy of this technique in quantifying low cerebral blood flow gives it a unique application in cases of brain death and acute stroke and it can be repeated after an interval of 20 min. making it possible to evaluate autoregulation and cerebrovascular reserve. Furthermore, cerebral blood flow information is directly coupled to CT anatomy. Although it is more difficult to administer than a standard CT scan, careful monitoring can ensure patient safety during the examination. In this article we review the physiologic and technical bases for the clinical application of xenon CT-derived quantitative cerebral blood flow information and discuss the advantages and disadvantages of the technique. We also describe its current clinical applications, including its usefulness in the evaluation of acute stroke, occlusive vascular disease, carotid occlusion testing, vasospasm, arteriovenous malformations, and head trauma management.

Abstract

Digital analysis of AVHRR (Advanced Very High Resolution Radiometer) thermal infrared band imagery has yielded sea ice surface temperatures which were used to calculate daily net surface heat loss and accompanying ice production and salt rejection from strips of thin ice within the Chukchi Polynya. 50. The role of passive immunization in hiv-positive patients : a case report.

Abstract

An HIV-positive patient presented with pulmonary tuberculosis as her AIDS-defining diagnosis in 1993 and was effectively treated with 12 months of standard antituberculosis medications (isoniazide, rifampin, and pyrazinamide for 2 months). She received zidovudine for 6 weeks at the time of her diagnosis; however, because of patient preference, she has not received subsequent standard HIV medications (7 years). Her CD4 count at the time of diagnosis (1993) was 297/microL. Monthly passive immunotherapy was administered (fresh frozen plasma from HIV-negative blood donors with a significant titer for the anti-vasoactive intestinal peptide [VIP]/NTM antibody) from December 1993 to June 1994. Her CD4 count increased to > 400/microL during the passive immunotherapy and has remained stable for the past 6 years. The rational for the use of anti-VIP/NTM antibodies preparations in HIV, the possible mode of action of anti-VIP/NTM antibodies, the use of Ig preparations, and the role of exercise as a natural source of anti-VIP/NTM antibodies are discussed. This case report supports the potential therapeutic use of anti-VIP antibodies for treatment of HIV disease. [ABSTRACT FROM AUTHOR]
