1. Efficacy of motivational-interviewing and guided opioid tapering support for patients undergoing orthopedic surgery (MI-Opioid Taper): A prospective, assessor-blind, randomized controlled pilot trial

Abstract

Background: Postoperative opioid use can lead to chronic use and misuse. Few studies have examined effective approaches to taper postoperative opioid use while maintaining adequate analgesia. Methods: This randomized, assessor-blinded, pilot trial of postoperative motivational interviewing and guided opioid tapering support (MI-Opioid Taper) added to usual care (UC) enrolled patients undergoing total hip or knee arthroplasty at a single U.S. academic medical center. MI-Opioid Taper involved weekly (to seven weeks) and monthly (to one year) phone calls until patient-reported opioid cessation. Opioid tapering involved 25% weekly dose reductions. The primary feasibility outcome was study completion in the group to which participants were randomized. The primary efficacy outcome, time to baseline opioid use, was the first of five consecutive days of return to baseline preoperative dose. Intention-to-treat analysis with Cox proportional hazards regression was adjusted for operation. ClinicalTrials.gov registration: NCT02070003. Findings: From November 26, 2014, to April 27, 2018, 209 patients were screened, and 104 patients were assigned to receive MI-Opioid Taper (49 patients) or UC only (55 patients). Study completion after randomization was similar between groups (96.4%, 53 patients receiving UC, 91.8%, 45 patients receiving MI-Opioid Taper). Patients receiving MI-Opioid Taper had a 62% increase in the rate of return to baseline opioid use after surgery (HR 1.62; 95%CI 1.06–2.46; p = 0•03). No trial-related adverse events occurred. Interpretation: In patients undergoing total joint arthroplasty, MI-Opioid Taper is feasible and future research is needed to establish the efficacy of MI-Opioid Taper to promote postoperative opioid cessation. Abstract

JUSTIFICATIVA E OBJETIVOS: A artrite reumatoide (AR) é uma doença inflamatória crônica e de etiologia desconhecida. Os pacientes com AR são reconhecidos como pessoas com redução na expectativa de vida, em comparação com a população em geral. As doenças reumáticas são numerosas e ocorrem com alta variabilidade; algumas são desenvolvidas rapidamente; outras, cronicamente, provocando incapacidades durante toda a vida. Os riscos anestésicos, em desordens osteoarticulares, envolvem, além das deformidades mecânicas causadas pela doença, os sistemas cardiovascular, respiratório, renal e digestivo. CONTEÚDO: A proposta da presente revisão foi levantar a importância das fases da doença em processo, que podem influenciar no controle da anestesia antes, durante e após a cirurgia, destacando a experiência dos autores em uma avaliação retrospectiva dos casos de pacientes portadores de artrite reumatoide juvenil (ARJ) submetidos a próteses ortopédicas, com ênfase para as técnicas de intubação. CONCLUSÕES: Pacientes com artrite reumatoide podem apresentar um bom número de problemas complexos para o anestesiologista. Isso requer uma cuidadosa avaliação pré-operatória; a anestesia requer experiência com a técnica e o cuidado pós-operatório deve ser criteriosamente escolhido para atender à necessidade específica do paciente. O procedimento demanda efetiva comunicação entre cirurgião, reumatologista e anestesiologista, para que cada membro do grupo multidisciplinar contribua com sua experiência, visando a um melhor benefício ao paciente.

4. Successful Closed Reduction of a Dislocated Constrained Total Hip Arthroplasty

Abstract

This case report and literature review examines whether closed reduction is a viable option to manage hip dislocation when the patient has a constrained liner. Introduction Dislocation after total hip arthroplasty (THA) ranges from less than 1% to 6% of primary cases [1] and from 15% to 30% of revision cases [2]. Unfortunately, the success rate of non-operative treatment of dislocation after THA can be unreliable, and a third of such patients have recurrences [1]. Surgical procedures used to treat instability and dislocation include: •Tightening the abductor musculature •Removing sources of impingement •Repositioning malaligned components •Using acetabular liners with elevated rims Such treatments fail in 30% to 50% of patients, however [3]. The use of constrained liners, which relies on a locking mechanism to capture the femoral head, has been developed to help manage this problem [4]. Despite such efforts, patients continue to be at high risk for instability, and 3% to 18% experience recurrent dislocation even after constrained components have been implanted [5,6]. Generally, open surgical reduction is thought to be the only available treatment for such cases. However, this compromises the integrity of the joint and exposes the patient to the additional risk of surgery [7]. Several authors have reported closed reduction for dislocation of a constrained liner [7-13]. Constrained acetabular liners are currently available in various designs, with differences in the locking mechanisms. To our knowledge, ours is the first reported case of a successful closed reduction of a dislocated constrained THA using a Trilogy Acetabular System Constrained liner (Zimmer, Inc, Warsaw, Indiana, USA). The study protocol adhered to the ethics guidelines of the 1975 Declaration of Helsinki, and the study was approved by the Institutional Review Board of Stanford University.

Abstract

Disorders of the synovial joint, such as osteoarthritis (OA) and rheumatoid arthritis (RA), afflict a substantial proportion of the global population. However, current clinical management has not been focused on fully restoring the native function of joints. Organ-on-chip (OoC), also called a microphysiological system, which typically accommodates multiple human cell-derived tissues/organs under physiological culture conditions, is an emerging platform that potentially overcomes the limitations of current models in developing therapeutics. Herein, we review major steps in the generation of OoCs for studying arthritis, discuss the challenges faced when these novel platforms enter the next phase of development and application, and present the potential for OoC technology to investigate the pathogenesis of joint diseases and the development of efficacious therapies.

Abstract

Osteoarthritis (OA) is a degenerative joint disease resulting in limited mobility and severe disability. Type II diabetes mellitus (T2D) is a weight-independent risk factor for OA, but a link between the two diseases has not been elucidated. Adipose stem cells (ASCs) isolated from the infrapatellar fat pad (IPFP) may be a viable regenerative cell for OA treatment. This study analyzed the expression profiles of inflammatory and adipokine-related genes in IPFP-ASCs of non-diabetic (Non-T2D), pre-diabetic (Pre-T2D), and T2D donors. Pre-T2D ASCs exhibited a substantial decrease in levels of mesenchymal markers CD90 and CD105 with no change in adipogenic differentiation compared to Non-T2D and T2D IPFP-ASCs. In addition, Cyclooxygenase-2 (COX-2), Forkhead box G1 (FOXG1) expression and prostaglandin E2 (PGE2) secretion were significantly increased in Pre-T2D IPFP-ASCs upon stimulation by interleukin-1 beta (IL-1β). Interestingly, M1 macrophages exhibited a significant reduction in expression of pro-inflammatory markers TNFα and IL-6 when co-cultured with Pre-T2D IPFP-ASCs. These data suggest that the heightened systemic inflammation associated with untreated T2D may prime the IPFP-ASCs to exhibit enhanced anti-inflammatory characteristics via suppressing the IL-6/COX-2 signaling pathway. In addition, the elevated production of PGE2 by the Pre-T2D IPFP-ASCs may also suggest the contribution of pre-diabetic conditions to the onset and progression of OA.

Item does not contain fulltext BACKGROUND: The Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) has been extensively evaluated in groups of patients with osteoarthritis, yet not in patients with a femoral neck fracture. This study aimed to determine the reliability, construct validity, and responsiveness of the WOMAC compared with the Short Form-12 (SF-12) and the EuroQol 5D (EQ-5D) questionnaires for the assessment of elderly patients with a femoral neck fracture. METHODS: Reliability was tested by assessing the Cronbach alpha. Construct validity was determined with the Pearson correlation coefficient. Change scores were calculated from ten weeks to twelve months of follow-up. Standardized response means and floor and ceiling effects were determined. Analyses were performed to compare the results for patients less than eighty years old with those for patients eighty years of age or older. RESULTS: The mean WOMAC total score was 89 points before the fracture in the younger patients and increased from 70 points at ten weeks to 81 points at two years postoperatively. In the older age group, these scores were 86, 75, and 78 points. The mean WOMAC pain scores before the fracture and at ten weeks and two years postoperatively were 92, 76, and 87 points, respectively, in the younger age group and 92, 84, and 93 points in the older age group. Function scores were 89, 68, and 79 points for the younger age group and 84, 71, and 73 points for the older age group. The Cronbach alpha for pain, stiffness, function, and the total scale ranged from 0.83 to 0.98 for the younger age group and from 0.79 to 0.97 for the older age group. Construct validity was good, with 82% and 79% of predefined hypotheses confirmed in the younger and older age groups, respectively. Responsiveness was moderate. No floor effects were found. Moderate to large ceiling effects were found for pain and stiffness scales at ten weeks and twelve months in younger patients (18% to 36%) and in the older age group (38% to 53%). CONCLUSIONS: The WOMAC showed good reliability, construct validity, and responsiveness in both age groups of elderly patients with a femoral neck fracture who had been physically and mentally fit before the fracture. The instrument is suitable for use in future clinical studies in these populations. CLINICAL RELEVANCE: The results are based on two clinical trials. The questionnaires used concern pure, clinically relevant issues (ability to walk, climb stairs, etc.). Moreover, the results can be used for future research comparing clinical outcomes (or treatments) for populations with a femoral neck fracture.

Abstract

Wear particles generated from total joint replacements can stimulate macrophages to release chemokines, such as monocyte chemoattractant protein 1 (MCP-1), which is the most important chemokine regulating systemic and local cell trafficking and infiltration of monocyte/macrophages in chronic inflammation. One possible strategy to curtail the adverse events associated with wear particles is to mitigate migration and activation of monocyte/macrophages. The purpose of this study is to modulate the adverse effects of particulate biomaterials and inflammatory stimuli such as endotoxin by interfering with the biological effects of the chemokine MCP-1. In the current study, the function of MCP-1 was inhibited by the mutant MCP-1 protein called 7ND, which blocks its receptor, the C–C chemokine receptor type 2 (CCR2) on macrophages. Addition of 7ND decreased MCP-1-induced migration of THP-1 cells in cell migration experiments in a dose-dependent manner. Conditioned media from murine macrophages exposed to clinically relevant polymethylmethacrylate (PMMA) particles with/without endotoxin [lipopolysaccharide (LPS)] had a chemotactic effect on human macrophages, which was decreased dramatically by 7ND. 7ND demonstrated no adverse effects on the viability of macrophages, and the capability of mesenchymal stem cells (MSCs) to form bone at the doses tested. Finally, proinflammatory cytokine production was mitigated when macrophages were exposed to PMMA particles with/without LPS in the presence of 7ND. Our studies confirm that the MCP-1 mutant protein 7ND can decrease macrophage migration and inflammatory cytokine release without adverse effects at the doses tested. Local delivery of 7ND at the implant site may provide a therapeutic strategy to diminish particle-associated periprosthetic inflammation and osteolysis.

Abstract

Motion at the interface between bone and implants for joint replacement may interfere with osseointegration and prosthesis stabilization. Particulate materials may cause foreign body and chronic inflammatory reactions resulting in bone resorption (osteolysis). The micromotion chamber (MC) and the bone harvest chamber (BHC) were implanted in the rabbit tibia, and the effects of micromotion and phagocytosable particulate materials on tissue formation within the chamber were assessed by studying bone ingrowth into a 1-mm pore. Using the MC, one short daily episode of motion (20 cycles/day, 0.5 mm amplitude) for three weeks decreased the amount of bone ingrowth. Using a different pore configuration, the same parameters of motion increased bone ingrowth. Increasing the amplitude of motion (from 0.5 to 0.75 mm), or the number of daily motion periods (from one to two per day) then decreased bone ingrowth. These studies suggest the existence of a window of externally applied strain: a small stimulus may facilitate and a large stimulus may discourage bone formation within the chamber. Cessation of a given set of motion parameters (producing primarily fibrous tissue) for an additional three weeks was accompanied by tissue differentiation into bone. Using the BHC, small, phagocytosable particles of bone cement, high density polyethylene and cobalt chrome alloy, at a concentration of 1.0 x 10(8) particles/mL, caused a foreign body reaction and inhibited the ingrowth of bone. Particles of titanium alloy had no effect on net bone formation. In studies using normal and immunodeficient rats, T lymphocytes were not a prerequisite for macrophages to phagocytose polyethylene particles. In the clinical situation, micromotion and particulate debris may be synergistic in producing prosthetic loosening. If an implant does not undergo osseointegration due to excessive micromotion, the fibrous tissue interface may provide a conduit for the subsequent migration of particles around the implant.

Abstract

Objective: Severe bone loss associated with failed revision total knee arthroplasty is a challenging scenario. The pedicled fibular flap is a method to obtain vascularized bone for use in knee arthrodesis after failure of a total knee arthroplasty, with substantial loss of bone. Methods: We report 2 successful knee arthrodeses using this method in patients with infected, failed multiply revised total knee arthroplasties. The failed prosthesis was removed, and the bones were aligned and stabilized. The fibular flap was then harvested, fed through a subcutaneous tunnel, and placed within the medullary canal at the arthrodesis site. The soft tissue was closed over the grafts and flaps. Results: Two elderly women presented with pain and drainage from previous total knee arthroplasties after multiple revisions. Arthrodeses were performed as described, and both patients were pain-free with the knee fused at 1 year. Conclusions: Thus, pedicled vascularized flaps are a viable alternative in the treatment of failed revision arthroplasty with large segmental bone loss.

The micromotion chamber consists of a titanium outer cylinder and a central core, which are pierced by a transverse 1-mm canal for tissue ingrowth. Six weeks after implantation in the proximal tibia in mature rabbits, the outer cylinder is osseointegrated; the central core can then be moved in relation to the fixed outer cylinder. Thus, the tissue growing through the pore, from the cylinder into the core, can be subjected to motion of a predetermined amplitude and frequency. In this study we investigate the influence of pore cross-sectional shape on tissue ingrowth in the canal. In six animals, the outer cylinder was pierced by a square 1-mm hole that was congruent with the square hole in the core; in five animals, the hole in the cylinder was round. The cross-sectional area of the square hole in the cylinder was about 21% greater than in a round hole. In all cases, the channel in the inner core was a 1 × 1 × 5-mm quadrate. All chambers underwent 20 cycles/day of micromotion for a 3-week period. The amplitude of the micromotion was 0.5 mm. Chambers containing cylinders with a round hole demonstrated less bone ingrowth as compared to cylinders with a square hole. This observation may be due to several factors including the greater cross-sectional area of the several factors including the greater cross-sectional area of the square versus the round hole in the cylinder and the enhanced congruity provided by the square outer and inner holes, versus a round outer and square inner hole. © 1993 John Wiley & Sons, Inc.

Coccidioidomycosis is a rare fungal infection caused by C immitis in endemic areas of the southwestern United States. Extrapulmonary hematogenous dissemination is a feared complication of the primary pulmonary disease. The musculoskeletal system can be involved, and disseminated musculoskeletal infections can be extremely difficult to eradicate. Surgical treatment of chronic bone and joint infections includes débridement and eventual arthrodesis for end-stage secondary osteoarthritis.

Abstract

Highly cross-linked ultrahigh molecular weight polyethylene (UHMWPE) implants have produced lower wear rates compared with conventional UHMWPE implants in laboratory testing and in clinical use1. Highly cross-linked UHMWPE has resulted in reduced osteolysis in clinical follow-up studies2. The various formulations of highly cross-linked UHWMPE used today are cross-linked with ionizing radiation and then annealed or melted to reduce or eliminate the resultant free radicals that could lead to oxidation. An alternative approach to stabilizing radiation-induced free radicals while maintaining the mechanical properties of the highly cross-linked UHMWPE is to incorporate vitamin E, an antioxidant. Laboratory studies have shown that after irradiation, vitamin E-stabilized UHMWPE has improved oxidation resistance, equivalent wear, and improved mechanical strength relative to non-vitamin E-stabilized highly cross-linked UHMWPE3-7. Nevertheless, as we describe in this case report, vitamin E-stabilized UHMWPE remains susceptible to rim fracture in a highly anteverted shell, similar to first-generation highly cross-linked UHMWPE8,9 without in vivo material property changes. The patient was informed that data concerning the case would be submitted for publication, and she provided consent. Institutional review board approval was obtained. Twelve years after a fifty-five-year-old woman with rheumatoid arthritis had undergone primary left total hip arthroplasty, she was referred to us for evaluation of episodic, activity-related left groin pain. Radiographs showed a well-fixed cementless hip arthroplasty with an eccentric femoral head, indicating polyethylene wear and osteolysis (Fig. 1-A and Fig. 2-A). Abstract

Abstract  Prosthetic cost contributes greatly to the overall expense of THA. A key question, therefore, in the selection of implant technique is whether any price difference exists between a cementless and a cemented femoral prosthesis. We evaluated the price difference between the most commonly used cemented and cementless femoral stems at three high-volume academic medical centers. Each hospital’s costs for prostheses from the manufacturers were recorded. The average cost of implanting a cementless femoral prosthesis was $296 more than the average cost of implanting a cemented femoral stem, even with the additional expense of two batches of bone cement and the accessories commonly used to achieve a third-generation cementing technique. The price difference was less variable if the cost of the prostheses from only the primary implant supplier for each institution was considered. As the number of THAs performed per year continues to escalate, implantation of a cemented femoral component remains an attractive method of fixation from an economic standpoint. Abstract

I N MODERN colloquial Japanese, the patterns of occurrence of the velar nasal [ij] are sufficiently skewed to require or elicit special techniques of phonological description. The symmetry of the Japanese phonemic system and the vague Systemzwang that can be imputed to the phonology of the language suggest an / m, n, Ij / nasal series parallel to the series of voiced and voiceless stops. Still, even a gross statistical estimate shows that the [g]i phone is limited to particular phonetic contexts and bears a very limited structural load. As a matter of phonemic theory, it is by no means unusual to find individual phonemes with infrequent occurrences in special environments in various languages, and, when synchronic and diachronic linguistic techniques have been profitably combined, such situations often indicate the emergence of an allophone to phonemic status.

Abstract

The development of safe and effective methods for the treatment of osteoarthritis(OA)-associated pain is a critical need. Recently, our team successfully modeled OA-like pathology in an in vitro multi-component joint-on-a-chip (microJoint), which integrates engineered osteochondral complex, synovium, adipose tissue and given their crucial role in joint pain, macrophages. In the present project, we introduce the sensory neurons into the established microJoint to generate the neu-microJoint, enabling the dynamic interplay between the peripheral nervous system and joint tissues. By recording activity in sensory neurons, joint integrity as well as therapeutic efficacy can be monitored in real time. The neu-microJoint is a hybrid PDMS/3D printed system where microchannels establish a diffusion barrier but enable sensory neuron innervation of joint tissues. Neural activity is assessed with micro electrode arrays (MEAs) and fura2-based calcium imaging. With the use of L4 and L5 dorsal root ganglion (DRG) neurons recovered from organ donors, the neu-microJoint can be fully humanized. DRG neurons cultured for 4 days were screened with increasing concentrations of KCl (10, 30, and 50mM) to detect spiking in the MEAs. There was little to no spontaneous activity under baseline conditions, but activity was detected in response to 30mM KCl. Rodent and human DRG neurons plated in the neu-microJoint cultured between 10 – 28 days showed substantial axonal growth through the microchannels as visualized through DiI or fura2 labeling. Moreover, electrical stimulation applied to the neurites evoked an increase in calcium in the corresponding soma. Conditioned media from the OA-modeled microJoint applied to the neurons caused an increase in calcium in a subpopulation of neurons. These data suggest the potential to apply an in vitro microphysiological system to study OA pain. These results also indicate that a subset of neurons, now undergoing specific characterization, is potentially responsible for OA pain modeled in the neu-microJoint. In rabbits, cortisone increases marrow fat cell size and intrafemoral head pressure, produces systemic fatty embolization, and probably is one of the causes of avascular necrosis of the femoral head. Clofibrate, a lipid-clearing agent, decreases the serum cholesterol level, reduces the marrow fat cell size changes, and decreases intrafemoral head pressure. This might be significant in improving femoral head venous outflow in long-term steroid-treated persons.
