This paper summarized the nursing management for a case of hyperemesis gravidarum treated by acupoint pressing combined with acupoint Application of Traditional Chinese Medicine (TCM). Key issues of nursing mainly included acupoint selection, pressing technique, Chinese herbal medicine ointment making and precautions during treatment. In addition, life guidance, diet care, social support and psychological intervention were carried out jointly to reduce the frequency of vomiting and improve patient’s quality of life. Primary cutaneous aspergillosis (PCA) is a rare opportunistic infection caused by Aspergillus that can be life‐threatening. PCA is mainly reported in immunocompromised hosts such as patients with AIDS, those with hematologic malignancy, or infants with occlusive dressings. However, no study has previously reported PCA associated with toxic epidermal necrolysis (TEN). This study reports four cases of TEN complicated with PCA, presenting with discrete gray or black spots over newly formed epithelia. Risk factors of PCA in patients with TEN include host factors, iatrogenic factors, indoor environment, and wound care. Two of the four cases eventually died, highlighting the importance of further exploring PCA in patients with TEN. [ABSTRACT FROM AUTHOR]

Kimura's disease, also known as eosinophilic hyperplastic lymphogranuloma, is a benign chronic lymphoproliferative inflammatory disorder with unknown etiology. Due to its rare and high recurrence rate, there is currently no consensus on the optimal treatment approach. Currently, traditional treatment for this condition mainly including glucocorticoids, surgery and radiotherapy. In recent years, the rapid development of biologies and small molecule drugs has provided new options for this disease. Background: Chromoblastomycosis is the World Health Organization (WHO)-recognized fungal implantation disease that eventually leads to severe mutilation. Cladophialophora carrionii (C. carrionii) is one of the agents. However, the pathogenesis of C. carrionii is not fully investigated yet. Methods: We investigated the pathogenic potential of the fungus in a Galleria mellonella (G. mellonella) larvae infection model. Six strains of C. carrionii, and three of its environmental relative C. yegresii were tested. The G. mellonella model was also applied to determine antifungal efficacy of amphotericin B, itraconazole, voriconazole, posaconazole, and terbinafine. Results: All strains were able to infect the larvae, but virulence potentials were strain-specific and showed no correlation with clinical background of the respective isolate. Survival of larvae also varied with infection dose, and with growth speed and melanization of the fungus. Posaconazole and voriconazole exhibited best activity against Cladophialophora, followed by itraconazole and terbinafine, while limited efficacy was seen for amphotericin B. Conclusion: Infection behavior deviates significantly between strains. In vitro antifungal susceptibility of tested strains only partly explained the limited treatment efficacy in vivo. Steady-state interfacial waves under a one-dimensional (1-D) class-IV exact triad resonance are investigated in a two-layer fluid with a free surface upper boundary. Four groups (G1–G4) of convergent series solutions are obtained by the homotopy analysis method. Though the harmonic resonance conditions are contained in the class-IV resonance criteria, the influences of 1:2 harmonic resonances on the energy spectrum could be neglected. Unlike former steady-state resonant interfacial wave spectrum where all the components joining the resonance are significant, the energy of one primary component can be ignored for two groups (G2 and G4) of wave solutions obtained in this paper. It is found that a little energy induced from the external environment might greatly change the energy spectrum for G1. However, the energy introduced from the outside cannot vary the wave energy distribution for G2. The reason for the extremely high crests on the instantaneous profiles of free surfaces of G2 and G4 is that the peaks of the class-IV exactly resonant and some trivial components momentarily overlap at some special horizontal positions. The class-IV exact triad resonance curve could be divided into four pieces containing the existence ranges of G3 and G4 and two regions with no solution found. One of the regions without convergent solutions results from an infinite number of singularities or small divisors caused by infinite exact or near resonances. Our results indicate that steady-state interfacial waves with class-IV triad resonance interactions among one surface and two internal wave modes could exist. [ABSTRACT FROM AUTHOR]

Two new oleanane-type triterpene glycosides, phytolasides A (1) and B (2), and six known ones (3–8), were isolated from Phytolacca acinosa fruit fermentation broth. Their structures were elucidated by HR-ESI-MS and 1 D- and 2 D-NMR spectroscopic methods. Antiproliferation of compounds 1 and 2 against HepG2 cells was examined by using CCK8 assays. Aim/Background: We investigated the composition and structure of intracellular polysaccharides in cucumber seeds (CSs) and their effect on Sprague-Dawley (SD) rats and potential activity for fracture healing. Materials and Methods: Polysaccharides are its main water-soluble active ingredient. The SD rat tibia fracture model was established by surgical method, and the healing effect of cucumber seed polysaccharides (CSPs) on SD rat tibia fracture was preliminarily explored. Through histopathological observation, determination of rat serum indicators alkaline phosphatase (AKP), acid phosphatase (ACP), BGP, X-ray imaging observation, and micro-Computed Tomography (CT) imaging observation at different periods. Results and Conclusion: The results showed that CSP enhanced the activity of osteoblasts, increased the metabolic rate of osteoclasts, increased the secretion of AKP, BGP, and ACP, shortened the fracture healing time of SD rats and effectively promoted the healing of tibial fractures in SD rats. In this paper, ABAQUS is used to establish a 3D finite element analysis model and to analyze the fire performance of steel-concrete composite structures. A thermo-mechanical coupling constitutive model of steel considering high temperature creep, and a thermo-mechanical coupling constitutive model of concrete considering transient thermal strain and short-term high temperature creep were adopted. The temperature field and fire resistance of concrete-filled steel tubular (CFT) columns, steel-concrete composite beams, joints and single-story composite plane frames under fire were analyzed, and the calculated results were verified by experimental results. On this basis, a three-story an three-bay steel-concrete composite plane frame model was established and ABAQUS was used to investigate two failure modes of the plane frame under local fire. Fire resistance of the plane frame, deformation of beams and columns, and internal force at column base were studied to discuss the necessity of considering the influence on frame beam fire performance from the interaction between concrete slab and steel beam. Under beam failure mode, fire performance was compared among frame beam, simply supported beam and fixed beam. Under column failure mode, fire performance was compared between CFT frame columns and CFT single columns with fixed ends. The analytical results show that under beam failure mode, considering the interaction between steel beam and concrete slab can make full use of its fire performance for the steel-concrete composite beams; fire performance of frame beam is superior to that of simply supported beam and is slightly lower than that of fixed beam; under column failure mode, with two sides suffered from fire, the fire performance of CFT single side columns is superior to the rest of frame side columns, yet with both two and four sides suffering fire, the fire performance of CFT single columns is close to that of frame interior columns. Background: Negative emotions are a major comorbidity of atopic dermatitis (AD). Evidence that supports the effectiveness of digital cognitive behavioral therapy (dCBT) as an adjuvant therapy for AD remains limited., Objective: To investigate the preliminary efficacy of additional dCBT and potential neurotransmitter biomarkers for AD accompanied by negative emotions., Methods: Thirty-two patients with AD were recruited and examined for clinical severity and negative emotions including insomnia, anxiety, and depression. Patients with mild-to-moderate negative emotions were divided into two groups that received standard care (N = 9) or mobile app-delivered CBT plus standard care (N = 11) for 12 weeks. Plasma levels of 40 neurotransmitters were determined using liquid chromatography tandem mass spectrometry pre- and post-treatment., Results: Skin lesions, itch, and insomnia were significantly improved in both treatment groups. Improvements of itch (P = 0.0449) and insomnia (P = 0.0089) were more robust in the combination treatment group than those in the standard treatment group. Neurotransmitters that involve tryptophan, dopamine, and histidine pathways were markedly altered in patients with AD compared with healthy controls. Taurine levels were selectively increased following dCBT plus standard care (P = 0.0259). Baseline levels of L-tyrosine were negatively correlated with the reduction of skin lesions (r = -0.9073, P = 0.0334) and itch intensity (r = -0.9322, P = 0.0210) in the combination therapy group., Conclusions: dCBT provides an efficacious supplementary approach for AD accompanied by negative emotions. Emotion-related neurotransmitters may contribute to AD and serve as indicators for treatment effects.

Triazole resistance in A. fumigatus is an increasing worldwide problem that causes major challenges in the management of aspergillosis. New antifungal drugs are needed with novel targets, that are effective in triazole-resistant infection. In this study, we retrospectively evaluated potency of the novel drug olorofim compared to contemporary antifungal agents against 111 clinical A. fumigatus isolates collected from Huashan Hospital, Shanghai, China, using EUCAST methodology, and reviewed the literature on triazole resistant A. fumigatus published between 1966 and 2020 in China. Olorofim was active in vitro against all tested A. fumigatus isolates with MIC 90 of 0.031mg/L (range 0.008-0.062 mg/L). For 4 triazole-resistant A. fumigatus (TRAF) isolates, the olorofim MIC ranged between 0.016-0.062mg/L. The reported rates of TRAF in China is 2.5% - 5.56% for clinical isolates, and 0-1.4% for environmental isolates.TR 34 /L98H/S297T/F495I is the predominant resistance mechanism, followed by TR 34 /L98H. Non TR-mediated TRAF isolates, mostly harboring a cyp51A single point mutation, showed greater genetic diversity than TR-mediated resistant isolates. Resistance due toTR 34 /L98H and TR 34 /L98H/S297T/F495I mutations among TRAF isolates might have evolved from separate local isolates in China.
