We aimed to determine the association of evidence-based processes of care for acute ischemic stroke (AIS) and clinical outcome of patients who participated in the HEADPOST (Head Positioning in Acute Stroke Trial), a multicenter cluster crossover trial of lying flat versus sitting up, head positioning in acute stroke. Methods and Results-Use of 8 AIS processes of care were considered: reperfusion therapy in eligible patients; acute stroke unit care; antihypertensive, antiplatelet, statin, and anticoagulation for atrial fibrillation; dysphagia assessment; and physiotherapist review. Hierarchical, mixed, logistic regression models were performed to determine associations with good outcome (modified Rankin Scale scores 0-2) at 90 days, adjusted for patient and hospital variables. Among 9485 patients with AIS, implementation of all processes of care in eligible patients, or defect-free care, was associated with improved outcome (odds ratio, 1.40; 95% CI, 1.18-1.65) and better survival (odds ratio, 2.23; 95% CI, 1.62-3.09). Defect-free stroke care was also significantly associated with excellent outcome (modified Rankin Scale score 0-1) (odds ratio, 1.22; 95% CI, 1.04-1.43). No hospital characteristic was independently predictive of outcome. Only 1445 (15%) of eligible patients with AIS received all processes of care, with significant regional variations in overall and individual rates. Conclusions-Use of evidence-based care is associated with improved clinical outcome in AIS. Strategies are required to address regional variation in the use of proven AIS treatments. Anxiety and fatigue adversely affect women's postpartum recovery, but few effective non-pharmacological interventions are known to relieve these two common and unpleasant symptoms.To examine the efficacy of an auricular acupressure intervention provided during early postpartum in reducing anxiety, fatigue levels, cortisol levels, blood pressure, and heart rate.A single-blind, randomised controlled trial.A 40-bed postpartum unit.Women who underwent caesarean section were randomly allocated to two groups: intervention (auricular pressure, n=40), and control (usual care, n=40).The intervention group received auricular acupressure on the shenmen acupoint twice a day (9 AM and 5 PM), and the control group received usual postpartum care. Serum cortisol levels were assessed by immunochemiluminescence, with blood pressure and heart rate assessed by electric sphygmomanometer. Anxiety and fatigue symptoms were assessed using the State Anxiety subscale of the State-Trait Anxiety Inventory and the Fatigue Continuum Form, respectively.Of the 76 women who completed the study, those who received auricular acupressure had significantly lower mean cortisol levels (mean difference=4μg/dl, p0.05), heart rate (mean difference=9.2 beats/min, p0.001), anxiety symptoms (mean difference=3.8, p0.01), and fatigue symptoms (mean difference=7.1, p0.01) than women in the control group at 5 days postpartum.Auricular acupressure is an effective non-pharmacological method for reducing cortisol levels, heart rate, anxiety, and fatigue in early postpartum after caesarean section.

Fatigue is not only a distressful symptom that is prevalent during labor; it may also interfere with the ability of laboring women to adjust to pain and bear down in the second labor stage as well as hinder maternal recovery and impair postpartum maternal-role performance. Childbirth-related fatigue is a frequent cause of physical and mental discomfort during labor and increases the likelihood of medical interventions such as instrumental delivery and cesarean section. Even so, as this fatigue is widely regarded as an inevitable part of the labor experience, little attention has been given to mediative interventions. In light of the above, this paper illustrates the definition and characteristics of childbirth-related fatigue during labor, analyzes the factors that affect this fatigue, and summarizes relevant nursing-care principles. It is hoped this paper increases nurse awareness of childbirth-related fatigue during labor and helps nurses take appropriate early prevention and intervention measures to reduce the adverse effects of fatigue and improve intrapartum care quality.生產疲憊：一個重要卻常被忽略的症狀疲憊不僅是常見於生產過程的不適症，亦為干擾產婦調適產痛、第二產程用力、產後復舊以及承擔母性任務等能力的重要因素。是已知易影響其身心舒適，甚至是提高器械及剖腹產機率的產時健康問題。雖然如此，卻常被視為待產必然的經歷，而未受到應有的重視。基於此，本文首先闡釋生產疲憊的定義與特性、分析影響生產疲憊的因素，並綜整生產疲憊的照護要點，以增加護理人員對生產過程疲憊的認識，進而能及早採取適當的預防和處理措施，減輕疲憊的不良影響，提昇產時照護品質。

Over-involvement in patient trauma and loss in clinical settings negatively affects nurses and may cause compassion fatigue. Compassion fatigue is associated with prolonged exposure to trauma cases, over-involvement in patient situations and over-extending caring energies. Nurses unable to adapt and cope may suffer physically and psychologically, reduce their quality of care, cause medical care costs to increase, and ultimately become frustrated and decide to leave the nursing profession. While current approaches to addressing compassion fatigue focus on evaluating individual nurse situations, adjusting attitudes, holding education programs, and bolstering support systems, little research attention has been directed toward the consequences of such on patient care or the origins of compassion fatigue in Taiwan nurses. This paper defines compassion fatigue, examines its importance and consequences, and delineates common nursing coping strategies in Taiwan. Findings expand domestic research on this phenomenon and suggest relevant theories and effective interventions to achieve remediation.
