1. Frequency and Type of Organisms in Gallstone Culture

Abstract

Background: Spillage of stones in peritoneal cavity is quite common in patients undergoing laparoscopiccholecystectomy. Lost stones may cause wide range of complications postoperatively which reflects theinfective potential of these stones. Objective: The aim of this study was to find out the frequency of viable bacteria in stones retrieved fromthe gallbladder after laparoscopic cholecystectomy & subjecting the stones for culture. Design: Cross-sectional study. Place &Duration: This study was conducted at Surgical Unit 4 of Civil Hospital Karachi,from July 2013 till March 2014. Materials & Metods: 80 cases of cholelithiasis , selected & operated by laparoscopic cholecystectomy wereincluded. During cholecystectomy , stones were collected , intact stones were sent in two separate containersfrom each subject, for microbiological examination. Results: This study included 80 patients, 58 patients(72.5%) were female & 22 patients (27.5%) were male.Out of these 80 patients, 27 patients (33.75%) had positive stone culture, out of these 27 patients , 16 patients(20%) with positive core culture, 11 patients (13.75%) had positive surface culture.The commonest organismwas E.coli in 13 patients (16.3%) followed by Pseudomonas in 3 patients(3.8%), Klebsiella in 3 patients(3.80%), Staphylococcus aureus in 2 patients (2.5%) & Bacteroides fragilis in 1 patient (1.3%). No salmonellaspecies were isolated. All organisms were tested for sensitivity against quinolones,penicillins, cephalosporins,aminoglycosides & carbapenems. Conclusion: This study proves the infective potential of gallstones, as they contain viable bacteria, whichmay cause post operative complications, therefore all spilled stones during surgery should be removed &peritonial cavity should be meticulously cleared.

3. The effect of intraoperative wound protector use on the risk of surgical site infections in patients undergoing pancreatoduodenectomy: a systematic review and meta-analysis

Abstract

To evaluate the effect of intraoperative wound protectors on the risk of surgical site infection (SSI) in patients undergoing pancreatoduodenectomy.In compliance with PRISMA statement standards, electronic databases were searched to identify all studies comparing wound protector use with no wound protector use in patients undergoing pancreatoduodenectomy. SSI (superficial or deep) was considered primary outcome measure. The secondary outcome measures included superficial SSI, deep SSI, and organ-space SSI. Random effects modelling was applied to calculate pooled outcome data. The certainty of evidence was assessed using GRADE system.A total of 12159 patients from four studies were included. The included populations in both groups were comparable in terms of baseline characteristics. The use of wound protector was associated with lower risk of superficial or deep SSI (OR: 0.55, 95% CI 0.43-0.70, P0.00001), superficial SSI (OR: 0.59, 95% CI 0.46-0.76, P0.0001), and organ-space SSI (OR: 0.80, 95% CI 0.72-0.90, P=0.0002). There was no difference between the two groups in terms of the risk of deep SSI (OR: 0.68, 95% CI 0.43-1.06, P=0.09) although this may be subject to type 2 error.The results of current study suggests that the use of intraoperative wound protector during pancreatoduodenectomy may reduce the risk of postoperative SSI. The quality of the available evidence is moderate with high certainty. While evidence from future randomised controlled trials could increase the robustness of our conclusions, we do not hesitate to recommend the use of wound protectors during pancreatoduodenectomy based on the current evidence.

4. FECALITH IN THE ILEUM CAUSING INTESTINAL OBSTRUCTION

Abstract

Small bowel obstruction is one of the common conditions presenting in surgical wards, however fecalith is one of the rare causes of bowel obstruction. We present here a case of 65 years old lady, who presented with sub-acute intestinal obstruction. In spite of the initial diagnosis being sigmoid volvulus, exploratory laparotomy revealed a focolith in ileum which was retrieved through an enterotomy and primary closure was done. The patient recovered uneventfully. Thus emphasizing the need of through history and workup which steer us to the correct diagnosis.
