1. Early hematological parameters as predictors for outcomes in children with dengue in northern India: A retrospective analysis

Abstract

Abstract INTRODUCTION: Dengue presents with a variable clinical course, ranging from mild illness to potentially fatal hemorrhage and shock. We aimed to evaluate the capabilities of various hematological parameters observed early in the course of illness for predicting the clinical outcomes of illness. METHODS: We retrospectively analyzed the records of children admitted in the pediatric inpatient services of the institute with dengue between 2017 and 2019. We determined the relationships between the hematological parameters observed during the first evaluation and the various clinical outcomes. RESULTS: We evaluated data from 613 patients (age range, 26 days to 17 years). Of these, 29.85% exhibited fever with warning signs, and 8.97% had severe dengue. Lower values of hemoglobin, platelet count, mean corpuscular volume, mean corpuscular hemoglobin concentration, and mean platelet volume, and higher values of total leukocyte count (TLC), hematocrit, and red cell distribution width variably correlated with numerous clinical outcomes-duration of hospital stay, development of complications, requirement of blood component transfusion, inotropic support, and mortality. Among the parameters, TLC ≥20,000/mL and initial platelet count ≤20,000/mL significantly associated with mortality, with odds ratios (95% confidence interval) of 11.81 (4.21-33.80) and 5.53 (1.90-16.09), respectively. CONCLUSIONS: Hematological parameters observed early during dengue infection may predict its clinical outcomes in infected children. Initial high TLC and low platelet count are potential predictors of fatal outcomes in the course of disease.

2. Spina ventosa: An often missed diagnosis

Abstract

Rare and varied presentations of tuberculosis make it difficult for treating clinicians to arrive at the diagnosis. An adolescent female presented to the orthopedic outpatient department with slowly increasing swelling over the dorsum of the hand near the base of the third digit for 5 months. With multiple consultations, she was being treated with antibiotics as a case of abscess. On examination, the swelling was soft bulging with whitish watery discharge. Plain radiography revealed periosteal elevation with bony destruction of the proximal phalanx. Magnetic resonance imaging revealed signal intensity changes with collection suggestive of infection. Blood investigations were within the normal limits, except slightly raised erythrocyte sedimentation rate. A differential diagnosis of chronic osteomyelitis was performed. Since the swelling was growing with the overlying skin likely to give way, it was treated with incision and drainage. Cytology with Gram's and auramine staining helped in confirming the diagnosis of spina ventosa. Biopsy is the gold standard for diagnosis, and antitubercular therapy forms the mainstay of treatment.

6. Retrospective Analysis of Urinary Tract Infection in the Pediatric Population at a Tertiary Care Centre

Abstract

Urinary tract infection (UTI) is among the most common infections occurring during childhood. It is caused by both gram-negative and gram-positive bacteria and Data of all pediatric patients in the age group of 6 months to 18 years with urinary tract infection were taken for analysis. Urine samples were collected and cultured on the cystine lactose electrolyte-deficient medium. The presence of bacteria was identified using biochemicals, and the antimicrobial test was performed using the Kirby-Bauer test or the VITEK 2 compact system (bioMérieux, Inc., France).The prevalence of UTI was 23.5%. In total, 614 specimens tested positive with significant bacteriuria. The male-to-female ratio was 1:2.3. Approximately 54% patients presented with urinary symptoms alone. Culture positivity was significantly associated with pyuria (p0.0001).Nitrofurantoin and amikacin can be used as empirical therapy for gram-negative and gram-positive bacteria. Because resistance to various commonly used antibiotics is found to be increasing, treatment must be guided by antibiotic susceptibility reports.

11. ORAL CANDIDA CARRIAGE INCLUDING SPECIATION, BIOFILM PRODUCTION AND CORRELATION WITH OCCURRENCE OF DENTAL CARIES IN STUDENT POPULATION

Abstract

The study was done to establish oral carriage of Candida, speciation, assay biofilm production and correlate the oral Candida carriage with occurrence of dental caries in students. Swab samples from oral cavity were collected from 206 students and were cultured for yeasts. Isolated yeasts were identified and detected for Biofilm production. Results: Overall Candida carriage was found in 90 of the 206 students (75 with caries). The rate of Candida carriage in students with caries (64%) was significantly higher as compared to overall carriage in students (43.7%). A total of 123 Candida strains were isolated including C albicans (63%), C tropicalis (23 %), C. parapsilosis (6 %), C. glabrata (4 %) and C. krusei (4 %). Biofilm production by C albicans was less frequent (42. 85%) than that by non-C albicans (63.33 %). and was significantly higher in students with caries than from without caries. Conclusion: We found a significant risk of dental caries with oral Candida carriage rates in student population. Candida albicans was the most common species isolated. Biofilm production was implicated in Candida virulence and was more important for non- C.albicans than C.albicans strains to establish infections.

During the 6-year study using the Centers for Disease Control and Prevention's (CDC) U.S. National Healthcare Safety Network (NHSN) definitions for device-associated health care-associated infection (DA-HAI), we collected prospective data from 605,310 patients hospitalized in the INICC's ICUs for an aggregate of 3,338,396 days. Although device utilization in the INICC's ICUs was similar to that reported from ICUs in the U.S. in the CDC's NHSN, rates of device-associated nosocomial infection were higher in the ICUs of the INICC hospitals: the pooled rate of central line-associated bloodstream infection in the INICC's ICUs, 4.9 per 1,000 central line days, is nearly 5-fold higher than the 0.9 per 1,000 central line days reported from comparable U. S. ICUs. The overall rate of ventilator-associated pneumonia was also higher (16.8 vs 1.1 per 1,000 ventilator days) as was the rate of catheter-associated urinary tract infection (5.5 vs 1.3 per 1,000 catheter days). Frequencies of resistance of Pseudomonas isolates to amikacin (42.8% vs 10%) and imipenem (42.4% vs 26.1%) and Klebsiella pneumoniae isolates to ceftazidime (71.2% vs 28.8%) and imipenem (19.6% vs 12.8%) were also higher in the INICC's ICUs compared with the ICUs of the CDC's NHSN. 13. Study of biomedical waste management practices in a private hospital and evaluation of the benefits after implementing remedial measures for the same

Abstract

A study has been carried out to ascertain, whether biomedical Waste generated in private hospitals being segregated and managed properly? The study was carried out in a private tertiary care 620 bedded hospital located in an urban area in Delhi, India to assess the awareness and attitude of the hospital staff, to document the ongoing practices, enlisting the deficiencies, to identify the root causes and to suggest remedial measures for proper biomedical waste management and assess the benefits of implementing them. A process flow chart was made of the existing waste system of the hospital. An anonymous questionnaire survey was conducted to determine the awareness about the policies and practices. A training programme was organised and a manual for waste management was made and distributed in the hospital. The quantum of waste generated per day in the hospital was 610 kg with 150 kg being biomedical waste. It was observed that although the waste generated was being disinfected properly before disposal the hospital staff was not segregating the waste properly, with delays in lifting of waste compounded with improper disposal. The hospital has its own incinerator but it was underutilized. After the remedial measures including training and distribution of manuals, an improvement was found in the segregation process resulting in decrease in amount of infectious waste load to 50%. The survey shows that no appropriate strategy exists and there is an urgent need to increase awareness about rules, regulations and procedures regarding this vital issue.

14. Prevalence of Candidiasis, Trichomoniasis and Bacterial Vaginosis among Women of Reproductive Age Group

Abstract

Background: Vaginal symptoms are leading reason for patients to visit gynecologist. Few data are available on prevalence of different causes of vaginitis. Objective: To find out prevalence of Candidiasis, Trichomoniasis and bacterial Vaginosis among the women of reproductive age group. Method: 400 sexually active married women attended gynecology OPD and ANC clinic at Santosh Hospital, Ghaziabad included in study. Participants were interviewed and structured questionnaire were filled. Two high vaginal swabs were collected from each woman. One swab was used for making smear and other used for culture on SDA. Candidiasis was detected by presence of curdy white discharge, Gram stain and culture on SDA. Trichomoniasis was detected by presence of green yellow frothy discharge, Wet mount and Papanicolaou stain. Bacterial Vaginosis was detected by presence of homogenous white or grey discharge, Amsel method and Gram stain. Results: Over all prevalence of vaginitis was 47.75 %. The prevalence of Candidiasis was 54.5% by Gram stain and 21.5% by culture method. The prevalence of Trichomoniasis was 12% by Wet mount and 8.75% by Pap stain. The prevalence of bacterial Vaginosis was 24% by Amsel criteria and 29% by Gram stain. Conclusions: This study raised an awareness regarding high vulnerability of women in reproductive age group for vaginitis. The study reveals that prevalence of candidiasis is 21.5%, trichomoniasis 8.75% and bacterial vaginosis 29% .
