4. Association of Aromatase Inhibitor-Induced Musculoskeletal Symptoms with Central Sensitization-Related Symptoms: A Cross-Sectional Study.

Abstract

Introduction: Aromatase inhibitor (AI)-induced musculoskeletal symptoms (AIMSS) can decrease health-related quality of life and lead to discontinuation of AI therapy for postmenopausal women with breast cancer (BC). Although central sensitization (CS) may contribute to AIMSS, the relevance of CS-related symptoms to AIMSS has not been fully clarified. This study aimed to investigate the relationship between AIMSS and CS-related symptoms in women with BC who received AI therapy. Methods: This cross-sectional study recruited women who underwent BC surgery before at least 1 year and were taking AI for at least 6 months. Participants were assessed for joint pain and CS-related symptoms using the central sensitization inventory (CSI). The severity of CS-related symptoms was classified into three groups, and the prevalence of AIMSS was calculated. Multiple logistic regression analysis was used to assess the relationship between AIMSS and factors of possible relevance to AIMSS, including CSI severity. Results: Of the 73 women who were included in this study, 31 (42.4%) were categorized into the AIMSS group and 42 (57.6%) into the non-AIMSS group. Participants with a history of chemotherapy and higher CSI score were significantly more likely to have AIMSS. Multiple logistic regression analysis showed that a history of chemotherapy (odds ratio = 4.21) and higher CSI severity (odds ratio = 13.43) had significantly associated with AIMSS. Conclusion: CS-related symptoms assessed using CSI may be strongly associated with AIMSS. Further longitudinal studies to investigate the causal relationship and effectiveness of CS-targeted interventions are needed to prevent and treat AIMSS effectively. [ABSTRACT FROM AUTHOR]

Abstract

Persistent infection of human papillomavirus (HPV) is suggested to be a risk factor for non- melanomal skin cancer, including Bowen’s disease, however the epidemiologic evidences are not sufficient at present. To clarify the role of HPV infection in the morphogenesis of Bowen’s disease, we examined whether there are differences between HPV-positive and HPV-negative cases with respect to characteristic histologic features of the disease, including hyperkeratosis, parakeratosis, acanthosis, papillomatosis, and cell vacuolation. In 40 histopathologically diagnosed cases of Bowen’s disease, the presence of HPV in tumor cells was examined by in situ hybridization. HPV DNA was detected in four (10%) of these cases, and papillomatosis was recognized in 18 (45%). While papillomatosis was severe in one, moderate in five and slight in nine of the 36 HPV-negative cases, papillomatosis was severe in two and moderate in one of the four HPV-positive cases. With respect to the degree of papillomatosis, there was a statistically significant difference between HPV-positive cases and HPV-negative cases (p=0.036). However, there was no statistically significant relation between HPV infection and the severity of hyperkeratosis, parakeratosis, acanthosis, and cell vacuolation. These data indicate that severity of papillomatosis in Bowen’s lesions is statistically related to the presence of HPV infection, and the results suggest that HPV infection may be associated with morphogenesis in Bowen’s disease

Abstract

BACKGROUND: For locally advanced breast cancer, neoadjuvant chemotherapy (NAC) is the standard of care for downstaging the tumor prior to surgery, and improved prognoses that are associated with pathological complete responses (pCR) have been noted, particularly in patients with human epidermal growth factor receptor 2 (HER2)-positive and triple negative (TN) tumors. OBJECTIVE: The aim of this study was to assess the differences in pathological responses among intrinsic subtypes of local lesions and status of axillary lymph nodes (Ax LN). METHODS: A consecutive series of 134 patients with locally advanced breast cancer who were treated with NAC was analyzed. Tumors were classified into the following 5 subtypes according to immunohistochemical staining results: Luminal A type, Luminal B type (HER2-negative and HER2-positive), HER2 type, and TN type. RESULTS: In Luminal A, Luminal B (HER2-negative), Luminal B (HER2-positive), HER2, and TN tumors, the pCR rates were 10% (4 of 40), 19% (8 of 42), 42% (8 of 19), 59% (10 of 17), and 38% (6 of 16), respectively. HER2-positive tumors showed good therapeutic effects, while Luminal A tumors showed less therapeutic effects. CONCLUSIONS: Strategies that are determined according to intrinsic subtypes are becoming very important in the treatment of locally advanced breast cancer. [ABSTRACT FROM AUTHOR]

Abstract

We evaluated the effect of capecitabine for metastatic breast carcinoma. Of 18 metastatic breast carcinoma patients experienced in our institution from November 2002 to July 2007, all patients had resisted the anthracycline or taxanes therapy before. Of these patients, 7 had liver, 6 had bone, and 5 had lung metastases. The capecitabine response in these 18 patients was evaluated as follows: PR in 4, long SD in 6, SD in 3, and PD in 5. In particular, 3 patients with liver metastases showed remarkable tumor regression. Compared to the 3rd-and 4th-line, 2nd-line capecitabine proved more effective. Hand-foot syndrome was found in 4 patients. Only one patient discontinued the therapy due to nausea. These results showed that capecitabine may be a useful treatment regimen for chemotherapy-resistant metastatic breast carcinoma patients.

Abstract

We evaluated the effect of combination therapy of trastuzumab and paclitaxel for metastatic breast carcinoma. Among the 23 metastatic breast carcinoma patients treated in our institution from September 2001 to December 2002, 10 (43%) patients were immunohistochemically positive for the HER2 protein. Four of these patients had bone, 3 had lung, 2 had liver and 1 had supraclavicular lymph node metastases. The combination chemotherapy for these 10 patients was evaluated as follows: CR in 1 patient, PR in 4, NC in 2, and PD in 3. Two patients with liver metastases showed remarkable tumor regression. Combination therapy did not have to be stopped in any of the patients due to side effects. These results show that combination therapy of trastuzumab and paclitaxel may be a useful treatment regimen for chemotherapy-resistant metastatic breast carcinoma patients.
