Background The prevalence of pelvic floor dysfunction in postmenopausal women is progressively increasing, leading to a significant impact on both physical and mental well-being due to stress urinary incontinence (SUI) . Objective To investigate the impact of menopausal hormone therapy (MHT) in conjunction with Kegel exercises on pelvic floor structure and clinical symptoms in individuals experiencing mild to moderate SUI. Methods A total of 75 patients with menopausal syndrome accompanied by mild to moderate SUI who visited the Menopause Clinic at Beijing Shijitan Hospital, Capital Medical University in 2022 were selected. They were allocated into the MHT group and the control group in a 2∶1 ratio using a random number table method, and the MHT group was further randomly divided into subgroups receiving Tibolone and estrogen combined with progestogen therapy (EPT) in a 1∶1 ratio. Both the control group and the MHT group underwent pelvic floor muscle training (PFMT), commonly referred to as Kegel exercises, for 15-30 minutes per session, 2-3 times daily, and 2-3 d per weekly, over a continuous period of 12 months. The control group received Kuntai capsules orally in combination with PFMT, with 4 capsules taken per dose, 3 times a day, for 12 months. The MHT group received menopausal hormone therapy in conjunction with PFMT. The EPT subgroups include continuous combined estrogen-progestin therapy (1 mg estradiol valerate + 10 mg dydrogesterone, once daily), continuous sequential estrogen-progestin therapy (femoston 12 courses), and the Tibolone subgroup takes tibolone orally, all of which were administered continuously for 12 months. The study examined serum estradiol (E2) and follicle-stimulating hormone (FSH) levels within and between groups and subgroups before and after treatment. In addition, measurements were obtained for urethral rotation angle (URA), bladder detrusor thickness (BDT), posterior vesicourethral angle (PVUA), levator hiatus area (LHA), urinary incontinence quantification, ICI-Q-SF score, clinical efficacy, as well as changes in the modified Kupperman Menopausal Index (KMI) score, and the modified oxford staging (MOS) . Results Upon completion of the study, 7 participants from the MHT group were lost to follow-up (5 in the Tibolone subgroup and 2 in the EPT subgroup), with 3 participants from the control group also lost to follow-up. In the end, a total of 65 participants were included. After 1 year of treatment, there were no statistically significant differences in FSH, E2, and MOS between the MHT group and the control group (P>0.05). After 1 year of treatment, there were no statistically significant differences in PUVA, BDT, and URA between the MHT group and the control group (P>0.05). However, the LHA of the MHT group was significantly lower than that of the control group (P=0.028). After 1 year of treatment, there were no statistically significant differences in PUVA, BDT, URA, and LHA between the EPT and Tibolone subgroups (P>0.05). Statistically significant differences were found in the comparison of clinical efficacy between the control group and the MHT group (P=0.005). Conversely, no statistically significant differences were observed in the comparison of clinical efficacy between the EPT and Tibolone subgroups (P=0.727). After 1 year of treatment, the MHT group showed lower urinary incontinence quantity, ICI-Q-SF score, and KMI score compared to the control group (P0.05) . Conclusion The combination of MHT with PFMT yields a positive effect on the pelvic floor structure and markedly alleviates symptoms of urinary incontinence. Nevertheless, there is no significant differences between EPT and Tibolone in the improvement of pelvic floor structure and alleviation of clinical symptoms in patients.

This paper reviewed the clinical data of a patient with postherpetic neuralgia, summarized the intervention effect of five-element music therapy and acupoint massage and related nursing measures. Five-element music theory was conducted under the guidance of traditional Chinese medicine, combined with manipulation massage on selected acupoints. Comprehensive nursing interventions such as dietary guidance and emotional care were carried out to improve treatment efficacy of pain relief in patients with postherpetic neuralgia. Objective To investigate the effect of Traditional Chinese Medicine (TCM) collapse stains combined with western medicine in treating acute herpes zoster. Methods A total of 60 patients with acute herpes zoster were randomly divided into observation group and control group, with 30 cases in each group. The control group was treated with conventional western medicine, and the observation group was treated with TCM collapse stains combined with western medicine. The pain degree measured by Visual Analogue Scale (VAS) and treatment efficacy of the two groups were compared. Results The VAS score of both group decreased after treatment, and the VAS score in the observation group was significantly lower than that in the control group(P＜0. 05). The total effective rate of the observation group was 83. 33%(25/30), which was higher than 66. 67%(20/30) in the control group (P＜0. 05). Conclusion The Combination of TCM collapse stains and western medicine is more effective than single use of western medicine in relieving the pain and improve the clinical symptoms of patients with acute herpes zoster. (目的 探讨中药溻渍联合西医治疗急性期带状疱疹的临床效果。方法 选取2021年5月—2022年1月收治的急性期带状疱疹患者60例, 随机分为观察组和对照组, 各30例。对照组给予常规干预, 观察组在对照组基础上实施中药溻渍治疗。评估两组疼痛程度和临床疗效。结果 干预后, 两组视觉模拟评分法(VAS)评分均降低, 且观察组评分低于对照组, 差异有统计学意义(P＜0. 05)。观察组总有效率83. 33%(25/30), 高于对照组66. 67%(20/30), 差异有统计学意义(P＜0. 05)。结论 中药溻渍联合西医治疗急性期带状疱疹, 能有效减轻患者疼痛, 改善临床症状。)

Objective To analyze the effect of training for medical caregivers, and provide reference for the standard training for medical caregivers. Methods A training group was established to formulate the teaching and training program, teaching objectives, management requirements, training items, teaching conditions, training forms and assessment rules. The training program was carried out according to the implementation plan of Strengthening the Training and Standardized Management of Medical Care Worker in Beijing. Results Medical caregivers were satisfied with the contents of training and related theory teaching and practice provided during the period. The pass rate of the final assessment was 100%. Conclusion The training course for medical caregivers is reasonable and has achieved good results, which is a beneficial attempt to carry out standardized training of medical nurses in the future. (目的 探讨医疗护理员培训实践效果, 为更规范开展医疗护理员培训工作提供参考。方法 成立医疗护理员培训小组, 制定教学培训大纲及教学目标、管理要求、培训项目、教学条件、培训形式及考核等细则, 按照规范对11名护理员开展培训。结果 11名采用培训的护理员对培训内容、理论授课、操作培训、实践安排均表示满意, 均认同该培训模式, 学员考核通过率100. 00%。结论 通过成立规范的培训小组、设置合理的培训内容和课程, 有助于提升培训效果。)

Medical caregivers as auxiliary medical care personnel have become an indispensable labor supplement in nursing work. Given the public's rising awareness of health and rights protection, the quality of patients' nursing staff has also improved accordingly. In order to strengthen life care skills and professional quality, it's required to standardize professional training. However, there are still many problems in the training of medical nursing staff and content in China. This article analyzed the current employment conditions, management and training needs of medical caregivers, aiming to provide reference for further establishment of standardized training curriculums. This paper summarized the observation and nursing measures of a patient with acne treated by Chinese herbal medicine mask. Under the guidance of Traditional Chinese Medicine syndrome differentiation and pharmacological characteristics of the Chinese herbal mask, targeted nursing interventions were carried out to relieve the facial acne and negative emotion of the patient. This paper summarized the Traditional Chinese Medicine (TCM) nursing technology for a patient with pain due to herpes zoster. Under the guidance of TCM syndrome differentiation and nursing assessment, TCM nursing technology such as collapse stains, cupping, auricular acupoint sticking and pressing were carried out, working collaboratively with antiviral and analgesic therapy, to relieve the pain and other clinical symptoms of herpes zoster. This paper summarized the application effect of Traditional Chinese Medicine(TCM) acupoint injection technique and related nursing management for a patient with chronic eczema and skin lesions. Nursing assessment and health education about injection method and precautions were carried out. Under the guidance of TCM syndrome differentiation and nursing theory, acupoint injection technique was adopted to relieve the pruritus symptoms and improve the recovery from skin lesions in patient with chronic eczema. In this paper, the effect observation and nursing measures of a case of herpes zoster patient treated by Chinese herbal medicine wet-hot compress technique were summarized. The wet-hot compress technique was conducted under the guidance of Traditional Chinese Medicine (TCM) syndrome differentiation and nursing theory, and corresponding nursing interventions such as nursing assessment, basic nursing and emotional care were carried out to relieve the pain and shorten the time of wound healing. Background: The purpose of this study is to construct models for predicting platinum resistance in high‐grade serous ovarian cancer (HGSOC) derived from quantitative spatial heterogeneity indicators obtained from 18F‐FDG PET/CT images. Methods: A retrospective study was conducted on patients diagnosed with HGSOC. Quantitative indicators of spatial heterogeneity were generated using conventional features and Haralick texture features from both CT and PET images. Three groups of predictive models (conventional, heterogeneity, and integrated) were built. Each group's optimal model was the one with the highest area under curve (AUC). Postoperative immunohistochemical staining for Ki‐67 and p53 was conducted. The correlation between the heterogeneity indicators and scores for Ki‐67 and p53 was assessed by Spearman's correlation coefficient (ρ). Results: A total of 286 patients (54.6 ± 9.3 years) were enrolled. And 107 spatial heterogeneity indicators were extracted. The optimal models for each group were obtained using the Gradient Boosting Machine (GBM) algorithm. There was an AUC of 0.790 (95% CI: 0.696, 0.885) in the conventional model for the validation set, and an AUC of 0.904 (95% CI: 0.842, 0.966) in the heterogeneity model for the validation set. The integrated model achieved the highest predictive performance, with an AUC value of 0.928 (95% CI: 0.872, 0.984) for the validation set. Spearman's correlation showed that HU_Kurtosis had the strongest correlation with p53 scores with ρ = 0.718, while cluster site entropy had the strongest correlation with Ki‐67 scores with ρ = 0.753. Conclusions: Adding quantitative spatial heterogeneity indicators derived from PET/CT images can improve the prediction of platinum resistance in patients with HGSOC. DNA and histone modifications have notable effects on gene expression.sup.1. Being the most prevalent internal modification in mRNA, the N.sup.6-methyladenosine (m.sup.6A) mRNA modification is as an important post-transcriptional mechanism of gene regulation.sup.2-4 and has crucial roles in various normal and pathological processes.sup.5-12. However, it is unclear how m.sup.6A is specifically and dynamically deposited in the transcriptome. Here we report that histone H3 trimethylation at Lys36 (H3K36me3), a marker for transcription elongation, guides m.sup.6A deposition globally. We show that m.sup.6A modifications are enriched in the vicinity of H3K36me3 peaks, and are reduced globally when cellular H3K36me3 is depleted. Mechanistically, H3K36me3 is recognized and bound directly by METTL14, a crucial component of the m.sup.6A methyltransferase complex (MTC), which in turn facilitates the binding of the m.sup.6A MTC to adjacent RNA polymerase II, thereby delivering the m.sup.6A MTC to actively transcribed nascent RNAs to deposit m.sup.6A co-transcriptionally. In mouse embryonic stem cells, phenocopying METTL14 knockdown, H3K36me3 depletion also markedly reduces m.sup.6A abundance transcriptome-wide and in pluripotency transcripts, resulting in increased cell stemness. Collectively, our studies reveal the important roles of H3K36me3 and METTL14 in determining specific and dynamic deposition of m.sup.6A in mRNA, and uncover another layer of gene expression regulation that involves crosstalk between histone modification and RNA methylation.
