The aim of this cross-sectional study is to propose an efficient strategy based on biomarkers adjunct with an interview/questionnaire covering risk factors for periodontitis for the identification of undiagnosed periodontitis by medical professionals. Active matrix metalloproteinase (aMMP)-8 levels in mouthrinse were analyzed by a point-of-care (PoC)/chairside lateral-flow immunotest, and salivary total MMP-8, total MMP-9 and calprotectin levels were analyzed by enzyme-linked immunosorbent assays (ELISAs) and active MMP-9 by gelatin zymography for 149 Greek patients. Patients underwent a full-mouth oral health examination for diagnosis according to the 2018 classification system of periodontal diseases. In addition, patient characteristics (risk factors: age, gender, education level, smoking and body mass index) were recorded. Receiver operating curve (ROC) analysis indicated better diagnostic precision to identify undiagnosed periodontitis for oral fluid biomarkers in adjunct with an interview/questionnaire compared with a plain questionnaire (i.e., risk factors): aMMP-8 AUC (95% confidence interval) = 0.834 (0.761-0.906), total MMP-8 = 0.800 (0.722-0.878), active MMP-9 = 0.787 (0.704-0.870), total MMP-9 = 0.773 (0.687-0.858) and calprotectin = 0.773 (0.687-0.858) vs. questionnaire = 0.764 (0.676-0.851). The findings of this study suggest that oral fluid biomarker analysis, such as a rapid aMMP-8 PoC immunotest, could be used as an adjunct to an interview/questionnaire to improve the precision of timely identification of asymptomatic, undiagnosed periodontitis patients by medical professionals. This strategy appears to be viable for referring patients to a dentist for diagnosis and treatment need assessment.

Proteolytic enzymes, such as matrix metalloproteinases (MMP), are associated to the progression of several cancers. They degrade extracellular components, which helps tumors to expand and cancer cells to escape from the primary site. Of all MMPs, gelatinases (MMP-2 and -9) and membrane type-1 matrix metalloproteinase (MT1-MMP, MMP-14), in particular, are often associated to more aggressive types of head and neck carcinomas as well as to a poorer outcome in patient survival. Although therapies during the last decades have advanced, the mortality of the disease is still rather high and adjuvant therapies are searched for continuously. MMP-9 and MT1-MMP are also involved in neo-angiogenesis, which is necessary for tumor expansion. For this reason, we have identified synthetic peptides-targeting gelatinases and MT1-MMP, and have also evaluated their anticancer effects in vitro and in vivo. Antigelatinolytic peptides effectively inhibited tongue-carcinoma cell invasion and reduced the growth of xenografted tumors. In tumor samples of mice that were treated with antigelatinolytic peptides, the micro-vessel density was significantly reduced. We also identified a novel MT1-MMP targeting peptide and demonstrated that it exerted anticancer effects against several malignant cell lines in vitro. The effects of MT1-MMP inhibition on tongue-squamous cell carcinomas were evaluated by using xenograft tumors, which it effectively inhibited. Tranexamic acid was also demonstrated to inhibit tongue-squamous cell carcinoma invasion, most probably due to its ability to prevent the plasmin-mediated activation of proMMP-9. Leukocyte β2 integrins are another interesting option when evaluating targets for the therapeutic intervention of inflammatory conditions or malignancies of hematopoietic origin, since β2 integrins are expressed mainly by leukocytes. We identified a novel technique for screening small-molecule libraries against β2 integrins, and by using this technique we identified a novel αMβ2 integrin-binding chemical (IMB-10). IMB-10 significantly enhances leukocyte adhesion and inhibits their motility. We also demonstrated that IMB-10 can be used to inhibit inflammation and lymphoma growth in vivo. Interestingly, IMB-10 also reduced leukocyte tumor infiltration and inhibited tumor invasion. Syövän paikallisessa leviämisessä ja etäpesäkkeiden muodostumisessa on erilaisilla proteiineja sekä muita solunulkoisia rakenneosia tuhoavilla entsyymeillä keskeinen rooli. Matriksin metalloproteinaasit (MMP) ovat ryhmä ehkä keskeisimpiä, ja eniten tutkittuja proteiineja hajottavia entsyymejä. MMP:t pystyvät tuhoamaan suurinta osaa solunulkoisentukiverkon ja solujen tyvikalvon rakenneosasista, osallistuvat syöpäkasvainten verisuonien uudismuodostukseen, sekä kykenevät aktivoimaan ja inaktivoimaan suurta joukkoa elimistön viestimolekyylejä. Etenkin MMP-2, -9 ja -14 on osoitettu vahvasti liittyvän pään ja kaulanalueensyöpien kasvuun sekä taudin ennusteeseen. Tästä syystä olemme kehittäneet 10-12 aminohapon ketjuja, peptidejä, jotka pystyvät estämään näitä entsyymejä. Osoitimme että nämä peptidit pystyivät estämään useiden eri syöpäsolujen liikettä solumaljalla, estivät hiiriin siirrettyjen ihmisen kielisyöpäsolujen kykyä muodostaa kasvaimia, sekä osin vähensivät näiden kasvainten verisuonenmuodostusta. Tulokset ovat lupaavia, ja peptidit saattavat tarjota tulevaisuudessa yhden mahdollisuuden parantaa suusyöpäpotilaiden ennustetta, jos kasvainten entsyymiprofiileihin kyetään keskittymään yksilöllisesti. Toinen tutkimuslinjamme oli kehittää menetelmä jolla löytää aineita jotka kykenevät estämään valkosolujen pinnalla esiintyviä tartunta- ja liikuntarakenteita, integriinejä. Valkosoluille tyypillisten beta-2-integriinien esto on mielenkiintoinen tutkimuskohde etenkin tulehduksellisissa sairauksissa ja verisyövissä, mutta tulehdusreaktion on osoitettu liittyvän olennaisesti myös kiinteinten syöpäkasvainten kasvuun. Osoitimme että kehittämällämme menetelmällä voidaan löytää kymmeniätuhansia keinotekoisia molekyylejä sisältävistä molekyylikirjastoista alfa-M-beta-2 -integriinejä estävä pienmolekyyli. Osoitimme myös että tämä uusi molekyyli pystyy estämään verisyöpäsolujen tarttumista soluviljelmissä, estämään kokeellista vatsakalvontulehdusta hiirimallissa, sekä estämään hiireen siirrettyjen ihmisen verisyöpäsolujen jakautumista ja kasvua. Mielenkiintoista oli että molekyyli esti jossain märin myös kielisyöpäkasvainten kasvua ja kudostuhoa hiiressä, vaikka kyseiset solut eivät tuota beta-2 integriinejä. Tämä osoittaa että tulehduksella voi olla merkitystä myös kielisyövän ennusteeseen. Löydetty pienmolekyyli on mielenkiintoinen kandidaatti etenkin tulehduksellisiin sairauksien sekä verisyöpien tulevaisuuden lääketutkimuksiin.
